    Retinoblastoma: A Three-Year-Study at a Brazilian Medical School Hospital

    OBJECTIVE: To present the characteristics and treatment outcomes of patients with retinoblastoma. METHODS: A retrospective case series was conducted to review the records of all new patients diagnosed with retinoblastoma between 2003 and 2005. Eyes with early disease, or advanced disease with potential vision were treated with chemotherapy (carboplatin and etoposide) in conjunction with early local therapy (laser or cryo). Radiotherapy was used in cases where the disease did not respond to the above protocols or in recurrent cases. Eyes in the late stage of disease with no potential vision in the initial examination or eyes and where conservative treatment had failed were enucleated. RESULTS: In total, we reviewed 28 new cases of retinoblastoma, 15 of which were unilateral and 13 of which were bilateral (46%). These data correspond to a mean of 9.3 new cases per year (0.77 case/ month). The mean age at diagnosis was 33.8 months for unilateral cases, and 19.15 months for bilateral cases (p=0.015). Leucocoria was the major presenting symptom (75%). All but one patient with unilateral disease had the affected eye enucleated due to advanced disease (mean follow-up: 39.91 months). Among the 13 bilateral cases, 13 eyes (50%) were enucleated, 11 eyes (42.4%) were saved with chemotherapy in conjunction with local therapy and 2 eyes (7.6%) were saved using external beam radiotherapy (mean follow-up: 41.91 months). In unilateral and bilateral disease, pathology data revealed choroid involvement in 50% and 30%, respectively, and optic nerve invasion in 92% and 50%, respectively. CONCLUSION: In this population, retinoblastoma was diagnosed too late and most eyes were consequently enucleated. In cases with bilateral disease, half of the eyes were preserved

    O artigo objetiva analisar os objetos de pesquisa das dissertações do Programa de Mestrado de Enfermagem em Saúde Pública do MISP da EERP-USP e suas relações com os problemas oriundos das práticas de saúde. Estas, foram classificadas em seis áreas temáticas: Organização da Assistência à Saúde, Promoção da Saúde, Processo Saúde-Doença, Nutrição-Alimentação, Prática Profissional e Imunologia. Destaca-se a contribuição de trabalhos que abordam a crise dos paradigmas na saúde, as relações de gênero e os movimentos sociais em saúde. Conclui-se que as temáticas retratam as realidades profissionais vivenciadas pelos alunos com propostas de intervenção nos micro-espaços das práticas de saúde.Se analizan los objetos de investigación de tesis de maestría en Enfermería en Salud Pública del MISP de EERP, y sus relaciones con problemas originados de las prácticas de salud. Se concluyó, que las temáticas reflejan realidades profesionales de los alumnos con propuestas de intervención en el micro-espacio de las prácticas de salud.The purpose of this study was to analyze the research objects of the theses produced in the Master Program on Public Health offered by the Department of Maternal-Infant Nursing and Public Health ¾ University of São Paulo at Ribeirão Preto College of Nursing and their relationship with the problems originated from health care practice. These practices were classified within six major areas: Health Care Organization, Health Promotion, Health-Disease Process, Nutrition-Feeding, Professional Practice and Immunology. The contribution of works focussing on the crisis of health paradigms, gender relations and Social Movements in Health were emphasized. Authors concluded that the themes are closely related to the students' professional context, enabling them to present suggestions regarding interventions in health practices

    Diagnóstico oportuno da tuberculose nos serviços de saúde de diversas regiões do Brasil

    OBJETIVO: Analizar el primer contacto del paciente con los servicios de salud para el diagnóstico oportuno de la tuberculosis (TB) en diferentes regiones de Brasil. MÉTODO: Estudio transversal en seis ciudades en el sudeste, sur y noreste. La recolección de datos utilizó fuentes secundarias y entrevistas con los pacientes. Los datos fueron analizados utilizando técnicas descriptivas y el análisis factorial de correspondencias múltiples. RESULTADOS: La Atención Primaria obtuve menor proporción de diagnósticos y mayor tiempo. Los servicios asociados con el diagnóstico en la primera visita fueron los servicios especializados y los Programas de Control de la Tuberculosis, que ofrecen consultas y exámenes in situ. CONCLUSIÓN: Es necesaria la organización del trabajo integrado entre los equipos de los diferentes servicios para las acciones de control. En la Atención Primaria, es necesario también observar el grado de desarrollo y sostenibilidad en la aplicación de estas medidas a la práctica diaria de los servicios.OBJETIVO: analisar o primeiro contato do doente com os serviços de saúde para o diagnóstico oportuno da tuberculose (TB), em diferentes regiões do Brasil. RESULTADOS: a Atenção Primária à Saúde apresentou maior tempo e menor proporção de diagnósticos. Os serviços associados ao diagnóstico, na primeira consulta, foram os serviços especializados e os Programas de Controle da TB, que oferecem consulta e exames no próprio local. CONCLUSÃO: faz-se necessária a organização do trabalho de forma integrada, entre as equipes dos diferentes serviços, para as ações de controle da TB. Na Atenção Primária à Saúde, é preciso ainda observar o grau de incorporação e sustentabilidade na execução dessas ações à prática diária dos serviços.OBJECTIVE: To analyze the infected person's first contact with the health services, for the early diagnosis of tuberculosis (TB) in different regions of Brazil. METHOD: a cross-sectional study undertaken in 6 municipalities in the South-east, South and North-east regions of Brazil. Data collection involved secondary sources and interviews with the patients. The data was analyzed using descriptive techniques and multiple correspondence factor analysis. RESULTS: Primary Health Care (PHC) presented the longest time to diagnosis and the lowest proportion of diagnoses. The services associated with diagnosis in the first consultation were the specialized services and the Tuberculosis Control Programs, which offer consultations and tests on-site. CONCLUSION: For the control of TB, it is necessary to organize the work in an integrated way between the different services' teams. In Primary Health Care, it is also necessary to observe the extent of incorporation and sustainability in the implementation of these actions into the health services' daily practice

    Mortality from gastrointestinal congenital anomalies at 264 hospitals in 74 low-income, middle-income, and high-income countries: a multicentre, international, prospective cohort study

    Summary Background Congenital anomalies are the fifth leading cause of mortality in children younger than 5 years globally. Many gastrointestinal congenital anomalies are fatal without timely access to neonatal surgical care, but few studies have been done on these conditions in low-income and middle-income countries (LMICs). We compared outcomes of the seven most common gastrointestinal congenital anomalies in low-income, middle-income, and high-income countries globally, and identified factors associated with mortality. Methods We did a multicentre, international prospective cohort study of patients younger than 16 years, presenting to hospital for the first time with oesophageal atresia, congenital diaphragmatic hernia, intestinal atresia, gastroschisis, exomphalos, anorectal malformation, and Hirschsprung’s disease. Recruitment was of consecutive patients for a minimum of 1 month between October, 2018, and April, 2019. We collected data on patient demographics, clinical status, interventions, and outcomes using the REDCap platform. Patients were followed up for 30 days after primary intervention, or 30 days after admission if they did not receive an intervention. The primary outcome was all-cause, in-hospital mortality for all conditions combined and each condition individually, stratified by country income status. We did a complete case analysis. Findings We included 3849 patients with 3975 study conditions (560 with oesophageal atresia, 448 with congenital diaphragmatic hernia, 681 with intestinal atresia, 453 with gastroschisis, 325 with exomphalos, 991 with anorectal malformation, and 517 with Hirschsprung’s disease) from 264 hospitals (89 in high-income countries, 166 in middleincome countries, and nine in low-income countries) in 74 countries. Of the 3849 patients, 2231 (58·0%) were male. Median gestational age at birth was 38 weeks (IQR 36–39) and median bodyweight at presentation was 2·8 kg (2·3–3·3). Mortality among all patients was 37 (39·8%) of 93 in low-income countries, 583 (20·4%) of 2860 in middle-income countries, and 50 (5·6%) of 896 in high-income countries (p<0·0001 between all country income groups). Gastroschisis had the greatest difference in mortality between country income strata (nine [90·0%] of ten in lowincome countries, 97 [31·9%] of 304 in middle-income countries, and two [1·4%] of 139 in high-income countries; p≤0·0001 between all country income groups). Factors significantly associated with higher mortality for all patients combined included country income status (low-income vs high-income countries, risk ratio 2·78 [95% CI 1·88–4·11], p<0·0001; middle-income vs high-income countries, 2·11 [1·59–2·79], p<0·0001), sepsis at presentation (1·20 [1·04–1·40], p=0·016), higher American Society of Anesthesiologists (ASA) score at primary intervention (ASA 4–5 vs ASA 1–2, 1·82 [1·40–2·35], p<0·0001; ASA 3 vs ASA 1–2, 1·58, [1·30–1·92], p<0·0001]), surgical safety checklist not used (1·39 [1·02–1·90], p=0·035), and ventilation or parenteral nutrition unavailable when needed (ventilation 1·96, [1·41–2·71], p=0·0001; parenteral nutrition 1·35, [1·05–1·74], p=0·018). Administration of parenteral nutrition (0·61, [0·47–0·79], p=0·0002) and use of a peripherally inserted central catheter (0·65 [0·50–0·86], p=0·0024) or percutaneous central line (0·69 [0·48–1·00], p=0·049) were associated with lower mortality. Interpretation Unacceptable differences in mortality exist for gastrointestinal congenital anomalies between lowincome, middle-income, and high-income countries. Improving access to quality neonatal surgical care in LMICs will be vital to achieve Sustainable Development Goal 3.2 of ending preventable deaths in neonates and children younger than 5 years by 2030

    Evaluation of Adrenal Reserve in Children with Acute Lymphocytic Leukemia Treated with Prednisone or Dexamethasone

    Background/Aims: The purpose of this study was to compare adrenal gland reserve in acute lymphocytic leukemia (ALL) patients 8 weeks after treatment with either prednisone (PRED) or dexamethasone (DEX) during the induction phase of therapy. Methods: A double-blind comparative study of patients treated with PRED and DEX was performed. Sixteen patients received PRED (40 mg/m(2)/day) and 13 patients received DEX (6 mg/m(2)/day), both for 28 days. A low-dose adrenocorticotropic hormone test (1.0 mu g/m(2), IV) was performed before and weekly for 8 weeks after abrupt cessation of glucocorticoid therapy. Sixteen children without ALL were used as controls to determine the cutoff peak cortisol level (14.2 mu g/dl). Results: Both groups (PRED and DEX) displayed similar mean peak cortisol levels before treatment and during the 8 weeks of evaluation (p = 0.652). No relationship was observed between the incidence of infection/stress and peak cortisol level within each group, nor was there a difference in the frequency of infection/stress between groups (p = 0.359). Although the patients presented variations in peak cortisol during the study period, no signs or symptoms of adrenal insufficiency were observed. Conclusion: Patients who received PRED or DEX for 4 weeks showed similar adrenal reserves and infection rates for 8 weeks after abruptly stopping glucocorticoid therapy, suggesting that DEX, which is a better antileukemic drug than PRED, has similar adrenal suppression and recovery rates.
