    The "Smart Ring" Experience in l'Aquila (Italy): Integrating Smart Mobility Public Services with Air Quality Indexes

    This work presents the "City Dynamics and Smart Environment" activities of the Smart Ring project, a model for the smart city, based on the integration of sustainable urban transport services and environmental monitoring over a 4–5-km circular path, the "Smart Ring", around the historical center of l'Aquila (Italy). We describe our pilot experience performed during an experimental on-demand public service electric bus, "SmartBus", which was equipped with a multi-parametric air quality low-cost gas electrochemical sensor platform, "NASUS IV". For five days (28–29 August 2014 and 1–3 September 2014), the sensor platform was installed inside the SmartBus and measured air quality gas compounds (nitrogen dioxide, carbon oxide, sulfur dioxide, hydrogen sulfide) during the service. Data were collected and analyzed on the bases of an air quality index, which provided qualitative insights on the air status potentially experienced by the users. The results obtained are in agreement with the synoptic meteorological conditions, the urban background air quality reference measurements and the potential traffic flow variations. Furthermore, they indicated that the air quality status was influenced by the gas component NO 2 , followed by H 2 S, SO 2 and CO. We discuss the features of our campaign, and we highlight the potential, limitations and key factors to consider for future project designs

    Comparison of source apportionment approaches and analysis of non-linearity in a real case model application

    Abstract. The response of particulate matter (PM) concentrations to emission reductions was analysed by assessing the results obtained with two different source apportionment approaches. The brute force (BF) method source impacts, computed at various emission reduction levels using two chemical transport models (CAMx and FARM), were compared with the contributions obtained with the tagged species (TS) approach (CAMx with the PSAT module). The study focused on the main sources of secondary inorganic aerosol precursors in the Po Valley (northern Italy): agriculture, road transport, industry and residential combustion. The interaction terms between different sources obtained from a factor decomposition analysis were used as indicators of non-linear PM10 concentration responses to individual source emission reductions. Moreover, such interaction terms were analysed in light of the free ammonia / total nitrate gas ratio to determine the relationships between the chemical regime and the non-linearity at selected sites. The impacts of the different sources were not proportional to the emission reductions, and such non-linearity was most relevant for 100 % emission reduction levels compared with smaller reduction levels (50 % and 20 %). Such differences between emission reduction levels were connected to the extent to which they modify the chemical regime in the base case. Non-linearity was mainly associated with agriculture and the interaction of this source with road transport and, to a lesser extent, with industry. Actually, the mass concentrations of PM10 allocated to agriculture by the TS and BF approaches were significantly different when a 100 % emission reduction was applied. However, in many situations the non-linearity in PM10 annual average source allocation was negligible, and the TS and BF approaches provided comparable results. PM mass concentrations attributed to the same sources by TS and BF were highly comparable in terms of spatial patterns and quantification of the source allocation for industry, transport and residential combustion. The conclusions obtained in this study for PM10 are also applicable to PM2.5

    A Case of the Co-Existence of Subcorneal Pustular Dermatosis and Pyoderma Gangrenosum and a Review of the Literature

    BACKGROUND: Subcorneal pustular dermatosis, also known as Sneddon-Wilkinson disease, can be classified as one of the neutrophilic dermatoses together with pyoderma gangrenosum. The development of both SPD and PG in the same patient has rarely been reported and may be a strong indicator of IgA dysglobulinemiaCASE REPORT: We report the case of a 34-year-old woman with a 2-year history of relapsing pustular eruptions mainly affecting the abdomen, gluteus region, elbows, and the extremities. Four years after the onset of subcorneal pustular dermatosis (SPD), she developed pyoderma gangrenosum (PG) on her right hand. In literature, the coexistence of SPD and PG in the same patient has already been described. This co-occurrence might indicate a certain predisposition for immune dysregulation.CONCLUSION: Although the two NDs are often associated with systemic diseases, these patients should be followed up for any malignancy because of the strong association between these disorders

    Psoriasis Features in Patients with Inflammatory Bowel Disease

    BACKGROUND: Psoriasis and inflammatory bowel diseases (IBD) share common pathways based on immune dysregulation with an important role of tumour necrosis factor-α and Th17 cells, as well as the genetic background. Several studies showed an increased prevalence of psoriasis in IBD patients. However, data regarding psoriasis features in IBD patients are still lacking. AIM: We aimed to conduct an observational study to assess psoriasis clinical features and its severity in a group of patients with IBD. METHODS: Dermatological assessment was performed consecutively in 200 IBD patients (123 with CD and 77 with UC) attending the IBD Care Centre of Gastroenterology at the University of Naples Federico II from 2015 to 2016. RESULTS: A group of 32 from 200 IBD patients (16%) had a familiar history positive for psoriasis, whereas, medical history and dermatologic examination revealed that 18 (9%) IBD patients were affected by psoriasis: 11 out of these 18 subjects (61.2%) had CD, and 7 had UC (38.2%); no significant differences were found between CD and UC groups. Concerning psoriasis severity, the mean psoriasis area severity index score was 3.7. CONCLUSION: This one-year retrospective study showed that psoriasis and IBD both require the use of immunosuppressive drugs so; we can count on a better treatment outcome for both diseases

    Cerebrovascular reactivity in multiple sclerosis is restored with reduced inflammation during immunomodulation

    Cerebrovascular reactivity (CVR) reflects the capacity of the brain’s vasculature to increase blood flow following a vasodilatory stimulus. Reactivity is an essential property of the brain’s blood vessels that maintains nutrient supplies in the face of changing demand. In Multiple Sclerosis (MS), CVR may be diminished with brain inflammation and this may contribute to neurodegeneration. We test the hypothesis that CVR is altered with MS neuroinflammation and that it is restored when inflammation is reduced. Using a breath-hold task during functional Magnetic Resonance Imaging (MRI), we mapped grey matter and white matter CVRs (CVRGM and CVRWM, respectively) in 23 young MS patients, eligible for disease modifying therapy, before and during Interferon beta treatment. Inflammatory activity was inferred from the presence of Gadolinium enhancing lesions at MRI. Eighteen age and gender-matched healthy controls (HC) were also assessed. Enhancing lesions were observed in 12 patients at the start of the study and in 3 patients during treatment. Patients had lower pre-treatment CVRGM (p = 0.04) and CVRWM (p = 0.02) compared to HC. In patients, a lower pre-treatment CVRGM was associated with a lower GM volume (r = 0.60, p = 0.003). On-treatment, there was an increase in CVRGM (p = 0.02) and CVRWM (p = 0.03) that negatively correlated with pre-treatment CVR (GM: r = − 0.58, p = 0.005; WM: r = − 0.60, p = 0.003). CVR increased when enhancing lesions reduced in number (GM: r = − 0.48, p = 0.02, WM: r = − 0.62, p = 0.003). Resolution of inflammation may restore altered cerebrovascular function limiting neurodegeneration in MS. Imaging of cerebrovascular function may thereby inform tissue physiology and improve treatment monitoring

    At the intersection of cultural and natural heritage: Distribution and conservation of the type localities of Italian endemic vascular plants

    We conducted a GIS spatial analysis with the aim of providing the first quantitative large-scale overview of the distribution patterns of 1536 type localities (loci classici) of 1216 Italian endemic vascular plants and their relationship with a set of descriptive variables. Whereas some variables were used to model the presence-absence distribution patterns of the type localities for the whole set of endemics as well as for the subset of narrow endemics, others (e.g., presence inside or outside protected areas and Italian Important Plant Areas) were considered with the purpose of assessing potential assets or risks for conservation. The largest number of type localities was found within the Mediterranean biogeographic region (1134), followed by the Alpine region (306) and Continental region (96). A total of 670 locations are located on islands, whereas 866 are located on the Italian mainland (139 and 124 in the case of narrow endemics, respectively). A large number of type localities are located in mountainous areas and along the coastline, which can be seen as a potential risk for conservation. On the contrary, we detected a positive correlation with the distance from roads, which might be considered to be an asset. Importantly, 1030 type localities fall inside protected areas, whereas 506 localities fall outside protected areas, with 259 of these unprotected localities on islands. We propose considering the results of the analysis of the distribution of type localities of Italian endemics to be a strategic tool for conservation planning and resource management. Application of plant micro-reserves and integration of diverse legislation tools are suggested to strengthen efforts and increase conservation success

    Inter-society consensus for the use of inhaled corticosteroids in infants, children and adolescents with airway diseases

    Background: In 2019, a multidisciplinary panel of experts from eight Italian scientific paediatric societies developed a consensus document for the use of inhaled corticosteroids in the management and prevention of the most common paediatric airways disorders. The aim is to provide healthcare providers with a multidisciplinary document including indications useful in the clinical practice. The consensus document was intended to be addressed to paediatricians who work in the Paediatric Divisions, the Primary Care Services and the Emergency Departments, as well as to Residents or PhD students, paediatric nurses and specialists or consultants in paediatric pulmonology, allergy, infectious diseases, and ear, nose, and throat medicine. Methods: Clinical questions identifying Population, Intervention(s), Comparison and Outcome(s) were addressed by methodologists and a general agreement on the topics and the strength of the recommendations (according to the GRADE system) was obtained following the Delphi method. The literature selection included secondary sources such as evidence-based guidelines and systematic reviews and was integrated with primary studies subsequently published. Results: The expert panel provided a number of recommendations on the use of inhaled corticosteroids in preschool wheezing, bronchial asthma, allergic and non-allergic rhinitis, acute and chronic rhinosinusitis, adenoid hypertrophy, laryngitis and laryngospasm. Conclusions: We provided a multidisciplinary update on the current recommendations for the management and prevention of the most common paediatric airways disorders requiring inhaled corticosteroids, in order to share useful indications, identify gaps in knowledge and drive future research

    Pediatric tuberculosis in Italian children: Epidemiological and clinical data from the Italian register of pediatric tuberculosis

    Tuberculosis (TB) is one of the leading causes of death worldwide. Over the last decades, TB has also emerged in the pediatric population. Epidemiologic data of childhood TB are still limited and there is an urgent need of more data on very large cohorts. A multicenter study was conducted in 27 pediatric hospitals, pediatric wards, and public health centers in Italy using a standardized form, covering the period of time between 1 January 2010 and 31 December 2012. Children with active TB, latent TB, and those recently exposed to TB or recently adopted/immigrated from a high TB incidence country were enrolled. Overall, 4234 children were included; 554 (13.1%) children had active TB, 594 (14.0%) latent TB and 3086 (72.9%) were uninfected. Among children with active TB, 481 (86.8%) patients had pulmonary TB. The treatment of active TB cases was known for 96.4% (n = 534) of the cases. Overall, 210 (39.3%) out of these 534 children were treated with three and 216 (40.4%) with four first-line drugs. Second-line drugs where used in 87 (16.3%) children with active TB. Drug-resistant strains of Mycobacterium tuberculosis were reported in 39 (7%) children. Improving the surveillance of childhood TB is important for public health care workers and pediatricians. A non-negligible proportion of children had drug-resistant TB and was treated with second-line drugs, most of which are off-label in the pediatric age. Future efforts should concentrate on improving active surveillance, diagnostic tools, and the availability of antitubercular pediatric formulations, also in low-endemic countries
