Problematiche delle infezioni da Citomegalovirus in gravidanza Obiettivi: migliorare la sensibilità dell'ecografia nella diagnosi di infezione da CMV individuando un reperto ecografico cerebrale suggestivo di infezione fetale da Citomegalovirus a 20 settimane di gestazione. Metodi: tra febbraio 1989 e settembre 2009, 721 pazienti afferenti alla nostra Unità di Medicina Materno fetale per infezione primaria da CMV hanno eseguito amniocentesi e sono state sottoposte ad un esame neurosonografico transvaginale a 20-22 settimane di gestazione. Risultati: in 29 feti con infezione congenita sono state evidenziate anomalie ecografiche (17%), di cui in 22 casi a livello cerebrale. In 13 casi l'ecografia transvaginale ha permesso di identificare un alone ecogeno periventricolare a margini ben definiti ad un'epoca gestazionale media di 20.5 settimane (20-22 settimane). Di questi casi 12 pazienti hanno deciso di interrompere la gravidanza. L'unico neonato ha presentato alla nascita un'ipoacusia bilaterale. I riscontri autoptici ottenuti (7/12) hanno mostrato un'infezione citomegalica disseminata ed in 3 casi segni a livello cerebrale. Conclusioni: il limite ecografico della diagnosi di infezione fetale nei casi di infezione primaria da CMV è noto. In pazienti gravide alla 20 settimana di gestazione con infezione recente da Citomegalovirus, il riscontro di un alone ecogeno periventricolare risulta essere un precoce ed attendibile segno di infezione fetale e di possibile danno della sostanza bianca cerebrale. Occorrono però studi aggiuntivi per valutare la possibile manifestazione clinica di questa anomalia cerebrale nei neonati con infezione da CMV

Estratégias em negócios internacionais: fatores determinantes para o sucesso de uma trading company

Monografia apresentada para a obtenção do Grau de Bacharel em Administração, no Curso de Administração com Linha de Formação Específica em Comércio Exterior, da Universidade do Extremo Sul Catarinense, UNESC – UNESC.A inserção no mercado internacional é uma das alternativas para que as organizações consigam maximizar seus lucros e operações. Porém, muitas delas não possuem toda a estrutura e pessoal preparado para iniciar a importação ou exportação. Desta forma, buscam serviços terceirizados para realizar esta proposta: contratação de trading companies. Estas empresas auxiliam deste a negociação até o embarque das mercadorias, prestando serviços de qualidade. Isso posto, o presente estudo objetivou identificar os fatores determinantes para o sucesso em negócios internacionais de uma trading company. Metodologicamente, optou-se por uma abordagem qualitativa, e, quanto aos fins de investigação, caracterizou-se como uma pesquisa descritiva, e, quanto aos meios, enquadra-se como bibliográfica e um estudo multicaso, por meio de uma pesquisa de campo. A amostra foi composta por quatro empresas que atuam como trading companies no Sul de Santa Catarina. Os dados de origem primária foram coletados por meio de uma entrevista, com o apoio de um roteiro semiestruturado, junto aos sócios proprietários. Verificou-se que as trading companies possuem experiência e dispõem de diversos serviços, para obterem êxito em suas negociações internacionais. Transcriptional down-regulation of ccr5 in a subset of HIV+ controllers and their family members.

HIV +Elite and Viremic controllers (EC/VCs) are able to control virus infection, perhaps because of host genetic determinants. We identified 16% (21 of 131) EC/VCs with CD4 +T cells with resistance specific to R5-tropic HIV, reversed after introduction of ccr5. R5 resistance was not observed in macrophages and depended upon the method of T cell activation. CD4 +T cells of these EC/VCs had lower ccr2 and ccr5 RNA levels, reduced CCR2 and CCR5 cell-surface expression, and decreased levels of secreted chemokines. T cells had no changes in chemokine receptor mRNA half-life but instead had lower levels of active transcription of ccr2 and ccr5, despite having more accessible chromatin by ATAC-seq. Other nearby genes were also down-regulated, over a region of ~500 kb on chromosome 3p21. This same R5 resistance phenotype was observed in family members of an index VC, also associated with ccr2/ccr5 down-regulation, suggesting that the phenotype is heritable

A cluster randomized controlled trial of a modified vaccination clinical reminder for primary care providers

Objective: Adult vaccination rates in the United States fall short of national goals, and rates are particularly low for Black Americans. We tested a provider-focused vaccination uptake intervention: a modified electronic health record clinical reminder that bundled together three adult vaccination reminders, presented patient vaccination history, and included talking points for providers to address vaccine hesitancy. Method: Primary care teams at the Atlanta Veterans Affairs Medical Center, who saw 28,941 patients during this period, were randomly assigned to receive either the modified clinical reminder (N = 44 teams) or the status quo (N = 40 teams). Results: Uptake of influenza and other adult vaccinations was 1.6 percentage points higher in the intervention group, which was not statistically significant (CI = [-1.3, 4.4], p = 0.28). The intervention had similar effects on Black and White patients and did not reduce the disparity in vaccination rates between these groups. Conclusion: Provider-focused interventions are a promising way to address vaccine hesitancy, but they may need to be more intensive than a modified clinical reminder to have appreciable effects on vaccination uptake

Effects of Patient Aggression on Pediatric Healthcare Workers

Healthcare workers are injured at high rates, even those who work in pediatric settings. The source of these injuries may be overexertion, slips/trips/falls, or needle sticks, but one source of injuries that has not been studied as extensively is patient aggression. Our study looked at possible effects of experiencing “patient behavioral events” (or PBEs), which are defined as physical aggression toward an employee, whether or not there was an intention to harm. Surveys of employees at three children’s hospitals across the U.S. showed that increased frequency of PBEs is associated with decreased well-being and worse job/organization attitudes. One key finding from this study is that the same negative effects were shown when the frequency of witnessing or hearing about PBEs was higher, which suggests that one need not be the target of the aggression to experience negative effects. If this causal path holds in future research, it would mean that PBEs have ripple effects in the unit, beyond just the person who is targeted by the patient aggression. Ongoing data analyses will examine whether there are any mitigating factors that might reduce the harm caused by PBEs

The KLEVER Survey: spatially resolved metallicity maps and gradients in a sample of 1.2 < z < 2.5 lensed galaxies

We present near-infrared observations of 42 gravitationally lensed galaxies obtained in the framework of the KMOS Lensed Emission Lines and VElocity Review (KLEVER) Survey, a programme aimed at investigating the spatially resolved properties of the ionized gas in 1.2 3σ) ‘inverted’ gradients are also found, showing an anticorrelation between metallicity and star formation rate density on local scales, possibly suggesting recent episodes of pristine gas accretion or strong radial flows in place. Nevertheless, the individual metallicity maps are characterized by a variety of different morphologies, with flat radial gradients sometimes hiding non-axisymmetric variations on kpc scales, which are washed out by azimuthal averages, especially in interacting systems or in those undergoing local episodes of recent star formation
