    Sporadic occurrence of hemorrhagic colitis associated with Escherichia coli O157:H7 in Newfoundland

    During a 9-month period in 1984, 113 fecal samples obtained from 92 patients with diarrheal illness were cultured for Escherichia coli serotype O157:H7 to determine the occurrence of this agent in diarrheal illness in Newfoundland. E. coli O157:H7 was isolated in almost pure culture from 12 stool specimens obtained from 7 (15%) of the 47 patients who had grossly bloody diarrhea; none of the 12 yielded any of the usual enteric pathogens. The agent was not isolated from the stool specimens obtained from the remaining 45 patients, who did not have bloody diarrhea. All seven patients whose specimens were positive for E. coli O157:H7 had clinical manifestations typical of hemorrhagic colitis, but the syndrome was clinically suspected and a specific test requested in only two cases. The seven cases were not clustered geographically or temporally, and no common exposure was identified. To determine whether hamburger meat was the source of E. coli O157:H7, 66 samples obtained from various retail outlets were tested; none were found to be positive. Hemorrhagic colitis may be a common disease, and E. coli O157:H7 should be considered as an agent in bloody diarrheal illness

    Statistical Description of Hydrodynamic Processes in Ionic Melts with taking into account Polarization Effects

    Statistical description of hydrodynamic processes for ionic melts is proposed with taking into account polarization effects caused by the deformation of external ionic shells. This description is carried out by means of the Zubarev nonequilibrium statistical operator method, appropriate for investigations of both strong and weak nonequilibrium processes. The nonequilibrium statistical operator and the generalized hydrodynamic equations that take into account polarization processes are received for ionic-polarization model of ionic molten salts when the nonequilibrium averaged values of densities of ions number, their momentum, dipole momentum and total energy are chosen for the reduced description parameters.     Power, norms and institutional change in the European Union: the protection of the free movement of goods

    How do institutions of the European Union change? Using an institutionalist approach, this article highlights the interplay between power, cognitive limits, and the normative order that underpins institutional settings and assesses their impact upon the process of institutional change. Empirical evidence from recent attempts to reinforce the protection of the free movement of goods in the EU suggests that, under conditions of uncertainty, actors with ambiguous preferences assess attempts at institutional change on the basis of the historically defined normative order which holds a given institutional structure together. Hence, path dependent and incremental change occurs even when more ambitious and functionally superior proposals are on offer

    On the upper bound of the electronic kinetic energy in terms of density functionals

    We propose a simple density functional expression for the upper bound of the kinetic energy for electronic systems. Such a functional is valid in the limit of slowly varying density, its validity outside this regime is discussed by making a comparison with upper bounds obtained in previous work.     Deciding to Enrol in a Cancer Trial: A Systematic Review of Qualitative Studies.

    Background:Clinical trials are essential for the advancement of cancer treatments; however, participation by patients is suboptimal. Currently, there is a lack of synthesized qualitative review evidence on the patient experience of trial entry from which to further develop decision support. The aim of this review is to synthesise literature reporting experiences of participants when deciding to enrol in a cancer clinical trial in order to inform practice. Methods:A systematic review and meta-synthesis of qualitative studies were conducted to describe the experiences of adult cancer patients who decided to enrol in a clinical trial of an anti-cancer treatment. Results:Forty studies met eligibility criteria for inclusion. Three themes were identified representing the overarching domains of experience when deciding to enrol in a cancer trial: 1) need for trial information; (2) trepidation towards participation; and (3) justifying the decision. The process of deciding to enrol in a clinical trial is one marked by uncertainty, emotional distress and driven by the search for a cure. Conclusion:Findings from this review show that decision support modelled by shared decision-making and the quality of a shared decision needs to be accompanied by tailored or personalised psychosocial and supportive care. Although the decision process bears similarities to theoretical processes outlined in decision-making frameworks, there are a lack of supportive interventions for cancer patients that are adapted to the clinical trial context. Theory-based interventions are urgently required to support the specific needs of patients deciding whether to participate in cancer trials

    Implementation Strategies for Interventions Aiming to Increase Participation in Mail-Out Bowel Cancer Screening Programs: A Realist Review.

    Background: Bowel cancer is the third most commonly diagnosed cancer and the third most common cause of cancer-related death, with 1,849,518 new cases of bowel diagnosed and 880,792 deaths reported globally in 2018 alone. Survival can be improved through early detection via national mail-out bowel cancer screening programs; however, participation remains low in many countries. Behavior change is therefore required to increase participation. This realist review aims to (a) identify the behavior change techniques (BCTs) used in each intervention, (b) understand the mechanisms of action (MoAs) responsible for the BCT effectiveness, and (c) apply a behavior change model to inform how MoAs can be combined to increase screening participation. Methods: We systematically reviewed the literature for interventions aiming to increase participation in mail-out bowel cancer screening. We used a four-stage realist synthesis approach whereby (1) interventions were extracted from each study; (2) BCTs applied in each intervention were identified and coded using the BCT Taxonomy-v1; (3) the Theory and Techniques Tool was used to link BCTs to their MoA; and (4) BCTs and MoAs were categorized according to their effectiveness and what Health Action Process Approach (HAPA) stage of change they would affect. Results: We identified 68 intervention trials using 26 unique BCTs and 13 MoAs to increase participation. Sixteen BCTs and 10 MoAs were identified within the interventions that successfully increased participation rates. Interventions targeting both stages of the HAPA model had a higher success rate (80%) than those targeting one stage of change (51%). When targeting only one stage, interventions targeting the volitional stage had a higher success rate (71%) than interventions targeting only the motivational stage of change (26%). Conclusion: Importantly, this review identified a suite of BCTs and MoAs effective for increasing participation in mail-out bowel cancer screening programs. With increased participation in bowel cancer screening leading to improved survival, our findings are key to informing the improvement of policy and interventions that aim to increase screening using specific strategies at key stages of health decision-making

    The Communist Party of Greece (KKE) and the Coalition of the Radical Left (SYRIZA) made remarkable ‘turns towards civil society’ over the last decade. It is argued that this was primarily a response aimed at strengthening their social legitimacy, which had reached its lowest point in the early 1990s. Differences in the way the two parties attempted to stabilise and engage their membership and re-establish links to trade unions and new social movements can be attributed to their distinct ideological and organisational legacies. Despite those differences, their respective linkage strategies were both successful until the game-changing 2012 Greek national elections, which brought about the remarkable rise of SYRIZA and the electoral demise of the KKE

    Two-particle photoemission from strongly correlated systems: A dynamical-mean field approach

    We study theoretically the simultaneous, photo-induced two-particle excitations of strongly correlated systems on the basis of the Hubbard model. Under certain conditions specified in this work, the corre- sponding transition probability is related to the two-particle spectral function which we calculate using three different methods: the dynamical-mean field theory combined with quantum Monte Carlo (DMFT- QMC) technique, the first order perturbation theory and the ladder approximations. The results are analyzed and compared for systems at the verge of the metal-insulator transitions. The dependencies on the electronic correlation strength and on doping are explored. In addition, the account for the orbital degeneracy allows an insight into the influence of interband correlations on the two particle excitations.     Based on a retrospective case-control study we evaluated the score system adopted by the Ministry of Health of Brazil (Ministério da Saúde - MS), to diagnose pulmonary tuberculosis (PTB) in childhood. This system is independent of bacteriological or histopathological data to define a very likely (>; or = 40 points), possible (30-35 points) or unlikely (;10mm (OR = 8.23). The best cut-off point to the diagnosis of PTB was 30 points, where the score system was more accurate, with sensitivity of 88.9% and specificity of 86.5%.Avaliou-se o sistema de pontuação adotado pelo Ministério da Saúde do Brasil para o diagnóstico de tuberculose pulmonar (TP) na infância através de estudo caso-controle retrospectivo.     Thomas-Fermi Method For Particles Obeying Generalized Exclusion Statistics

    We use the Thomas-Fermi method to examine the thermodynamics of particles obeying Haldane exclusion statistics. Specifically, we study Calogero-Sutherland particles placed in a given external potential in one dimension. For the case of a simple harmonic potential (constant density of states), we obtain the exact one-particle spatial density and a {\it closed} form for the equation of state at finite temperature, which are both new results. We then solve the problem of particles in a x2/3 x^{2/3} ~ potential (linear density of states) and show that Bose-Einstein condensation does not occur for any statistics other than bosons.Comment: 10 pages (TeX), 2 figures available upon reques
