    Using Deep Learning and 360 Video to Detect Eating Behavior for User Assistance Systems

    The rising prevalence of non-communicable diseases calls for more sophisticated approaches to support individuals in engaging in healthy lifestyle behaviors, particularly in terms of their dietary intake. Building on recent advances in information technology, user assistance systems hold the potential of combining active and passive data collection methods to monitor dietary intake and, subsequently, to support individuals in making better decisions about their diet. In this paper, we review the state-of-the-art in active and passive dietary monitoring along with the issues being faced. Building on this groundwork, we propose a research framework for user assistance systems that combine active and passive methods with three distinct levels of assistance. Finally, we outline a proof-of-concept study using video obtained from a 360-degree camera to automatically detect eating behavior from video data as a source of passive dietary monitoring for decision support

    Contemporary NSTEMI management: the role of the hospitalist.

    Non-ST-segment elevation myocardial infarction (NSTEMI) is defined as elevated cardiac biomarkers of necrosis in the absence of persistent ST-segment elevation in the setting of anginal symptoms or other acute event. It carries a poorer prognosis than most ST-segment elevation events, owing to the typical comorbidity burden of the older NSTEMI patients as well as diverse etiologies that add complexity to therapeutic decision-making. It may result from an acute atherothrombotic event (\u27Type 1\u27) or as the result of other causes of mismatch of myocardial oxygen supply and demand (\u27Type 2\u27). Regardless of type and other clinical factors, the hospital medicine specialist is increasingly responsible for managing or coordinating the care of these patients. Following published guidelines for risk stratification and basing anti-anginal, anticoagulant, antiplatelet, other pharmacologic therapies, and overall management approach on that individualized patient risk assessment can be expected to result in better short- and long-term clinical outcomes, including near-term readmission and recurrent events. We present here a review of the evidence basis and expert commentary to assist the hospitalist in achieving those improved outcomes in NSTEMI. Given that the Society for Hospital Medicine cites care of patients with acute coronary syndrome as a core competency for hospitalists, it is essential that those specialists stay current on optimal NSTEMI care

    Multi-epoch sub-arcsecond [Fe II] spectroimaging of the DG Tau outflows with NIFS-I. First data epoch

    Investigating the outflows emanating from young stellar objects (YSOs) on sub-arcsecond scales provides important clues to the nature of the underlying accretion-ejection process occurring near the central protostar. We have investigated the structures and kinematics of the outflows driven by the YSO DG Tauri, using the Near-infrared Integral Field Spectrograph (NIFS) on Gemini North. The blueshifted outflow shows two distinct components in [Fe II] 1.644 μm emission, which are separated using multicomponent line fitting. Jet parameters are calculated for the high-velocity component. A stationary recollimation shock is observed, in agreement with previous X-ray and far-ultraviolet observations. The presence of this shock indicates that the innermost streamlines of the high-velocity component are launched at a very small radius, 0.01-0.15 au, from the central star. The jet accelerates and expands downstream of the recollimation shock; the 'acceleration' is likely a sign of velocity variations in the jet. No evidence of rotation is found, and we compare this non-detection to previous counterclaims. Moving jet knots, likely the result of the jet velocity variations, are observed. One of these knots moves more slowly than previously observed knots, and the knot ejection interval appears to be non-periodic. An intermediate-velocity component surrounds this central jet, and is interpreted as the result of a turbulent mixing layer along the jet boundaries generated by lateral entrainment of material by the high-velocity jet. Lateral entrainment requires the presence of a magnetic field of strength a few mG or less at hundreds of au above the disc surface, which is argued to be a reasonable proposition. In H2 1-0 S(1) 2.1218 μm emission, a wide-angle, intermediate-velocity blueshifted outflow is observed. Both outflows are consistent with being launched by a magnetocentrifugal disc wind, although an X-wind origin for the high-velocity jet cannot be ruled out. The redshifted outflow of DG Tau takes on a bubble-shaped morphology, which will be discussed in a future paper

    Mapping access to health services as a strategy for planning: access to primary care for older people in regional Queensland

    Australia has seen a significant increase in people aged over 65 years accessing general practice services over the last decade. Although people aged 65 years and over comprise 14% of the total population, this age demographic accounts for the largest proportion of general practitioner (GP)–patient encounters. Access to general practice is important for older Australians as the burden of chronic disease increases with age. A geographic information system, ArcGIS, was used to assess geographic access to general practice for older people residing in the regional Queensland towns of Mackay, Townsville and Cairns. Geographic units with high proportions of over 65-year-old people were spatially analysed in relation to proximity to geomapped general practices with a 2-km buffer zone. Modelling of changes in access was performed with the strategic location of a new general practice where gaps existed. Geographic access to general practice for the older population was poorest in Cairns despite a high population density. Addition of a single, strategically placed general practice in Cairns markedly improved access. Socioeconomic analysis suggested that general practices were appropriately located in areas of greatest need. Geographic information systems provide a means to map population characteristics against service locations to assist in strategic development and location of future health services

    TLR9/MyD88 signaling is required for class switching to pathogenic IgG2a and 2b autoantibodies in SLE

    Loss of tolerance in systemic lupus erythematosus (SLE) leads to the generation of autoantibodies, which accumulate in end-organs where they induce disease. Here we show that immunoglobulin (Ig)G2a and 2b autoantibodies are the pathogenic isotypes by recruiting FcγRIV expressing macrophages. Class switching, but not development, of IgM anti-self B cells to these pathogenic subclasses requires the innate immune receptor Toll-like receptor (TLR)9 and MyD88 signaling. In their absence, switching of autoreactive B cells to the IgG2a and 2b subclasses is blocked, resulting in reduced pathology and mortality. In contrast, switching of anti-self B cells to IgG1 is not perturbed and generation of nonautoreactive IgG2a and 2b antibodies is not impaired in TLR9-deficient mice. Thus, the TLR9 pathway is a potential target for therapeutic intervention in SLE

    Mapping CO Gas in the GG Tauri A Triple System with 50 AU Spatial Resolution

    We aim to unveil the observational imprint of physical mechanisms that govern planetary formation in the young, multiple system GG Tau A. We present ALMA observations of 12^{12}CO and 13^{13}CO 3-2 and 0.9 mm continuum emission with 0.35" resolution. The 12^{12}CO 3-2 emission, found within the cavity of the circumternary dust ring (at radius <180< 180 AU) where no 13^{13}CO emission is detected, confirms the presence of CO gas near the circumstellar disk of GG Tau Aa. The outer disk and the recently detected hot spot lying at the outer edge of the dust ring are mapped both in 12^{12}CO and 13^{13}CO. The gas emission in the outer disk can be radially decomposed as a series of slightly overlapping Gaussian rings, suggesting the presence of unresolved gaps or dips. The dip closest to the disk center lies at a radius very close to the hot spot location at ∼250−260\sim250-260~AU. The CO excitation conditions indicate that the outer disk remains in the shadow of the ring. The hot spot probably results from local heating processes. The two latter points reinforce the hypothesis that the hot spot is created by an embedded proto-planet shepherding the outer disk.Comment: 8 pages, 4 figures. Accepted by Ap
