Investigating Veterans’ Pre-, Peri-, and Post-Deployment Experiences as Potential Risk Factors for Problem Gambling
Background and aims Gambling disorder and its comorbid diagnoses are observed at higher rates in military veterans than in the general population. A significant research gap exists regarding the relationships of veterans’ life and service experiences to problematic gambling. The present study explored pre-, peri-, and post-deployment factors associated with problem gambling in veterans. Methods Veterans of Operation Iraqi Freedom, Operation Enduring Freedom, and Operation New Dawn (n = 738; 463 males, and 275 females) completed questionnaires via structured telephone interview. We conducted bivariate and multinomial logistic regression analyses exploring associations among problem-gambling severity and socio-demographic variables, psychiatric comorbidities, and 10 scales of the Deployment Risk and Resilience Inventory measuring experiences pre-, peri-, and post-deployment. Results Approximately 4.2% of veterans indicated at-risk or probable pathological gambling (ARPG) post-deployment (two or more DSM-IV criteria for pathological gambling). Bivariate analyses found more severe gambling in males, higher frequencies of post-traumatic stress disorder, substance dependence, traumatic brain injury, panic disorder, and depression in veterans with ARPG, and higher general harassment during deployment, and lower social support and more stressful life events post-deployment in those with ARPG. In multivariable models, both post-deployment factors remained significantly associated with ARPG. Discussion The study suggests that problem gambling among veterans is related to service experiences, and particularly to life experiences post-deployment. Conclusions Adverse service and life experiences and lack of social support may contribute to the risk of problem gambling in military veterans. Investigation of how Veterans Affairs clinical settings may serve veterans following deployment to prevent behavioral addictions is warranted
Improving the outcomes of carotid endarterectomy: Results of a statewide quality improvement project
AbstractObjective: The purpose of this study was to establish the statewide outcomes for carotid endarterectomy (CEA) and to facilitate improvement in outcomes through feedback, peer discussion, and ongoing process and outcome measurement. Methods: The Medicare Part A claims files were used to identify all Medicare patients undergoing CEA in Iowa during two 12-month time periods (January 1994–December 1994 and June 1995–May 1996). Medical record abstraction was used to obtain surgical indications, perioperative care process, and outcome information. Confidential reports were provided to each hospital (N = 30) where the procedure was performed. Surgeons performing the procedure (N = 79) were invited to meetings to discuss care process variation and outcomes. Voluntary participation was solicited in a standardized program of ongoing hospital-based data collection of CEA process and outcome data. Results: The statewide combined stroke or mortality rate decreased from 7.8% in 1994 to 4.0% in the 1995 to 1996 time period (P <.001). Fourteen hospitals, accounting for 74% of the statewide cases, participated in ongoing data collection. The combined stroke or mortality rate in these hospitals decreased significantly (P <.05) over time from 6.5% (1994) to 3.7% (1995-1996) to 1.8% (June 1997–May 1998). The use of intraoperative assessment of the operative site (20% in 1994, 46% in 1997-1998) and patch angioplasty (14% in 1994, 30% in 1997-1998) increased significantly during this time in the participating hospitals. Conclusions: Confidential feedback of outcome and process data for CEA may lead to change in perioperative care processes and improved outcomes. Standardized community-based outcome analysis should become routine for CEA to ensure that optimum results are being achieved. (J Vasc Surg 2000;31:918-26.
The CREB coactivator TORC2 functions as a calcium- and cAMP-sensitive coincidence detector
Elevations in circulating glucose and gut hormones during feeding promote pancreatic islet cell viability in part via the calcium- and cAMP-dependent activation of the transcription factor CREB. Here, we describe a signaling module that mediates the synergistic effects of these pathways on cellular gene expression by stimulating the dephosphorylation and nuclear entry of TORC2, a CREB coactivator. This module consists of the calcium-regulated phosphatase calcineurin and the Ser/Thr kinase SIK2, both of which associate with TORC2. Under resting conditions, TORC2 is sequestered in the cytoplasm via a phosphorylation-dependent interaction with 14-3-3 proteins. Triggering of the calcium and cAMP second messenger pathways by glucose and gut hormones disrupts TORC2:14-3-3 complexes via complementary effects on TORC2 dephosphorylation; calcium influx increases calcineurin activity, whereas cAMP inhibits SIK2 kinase activity. Our results illustrate how a phosphatase/kinase module connects two signaling pathways in response to nutrient and hormonal cues
Liger for Next Generation Keck AO: Filter Wheel and Pupil Design
Liger is a next-generation near-infrared imager and integral field
spectrograph (IFS) for the W.M. Keck Observatory designed to take advantage of
the Keck All-Sky Precision Adaptive Optics (KAPA) upgrade. Liger will operate
at spectral resolving powers between R4,000 - 10,000 over a wavelength
range of 0.8-2.4m. Liger takes advantage of a sequential imager and
spectrograph design that allows for simultaneous observations between the two
channels using the same filter wheel and cold pupil stop. We present the design
for the filter wheels and pupil mask and their location and tolerances in the
optical design. The filter mechanism is a multi-wheel design drawing from the
heritage of the current Keck/OSIRIS imager single wheel design. The Liger
multi-wheel configuration is designed to allow future upgrades to the number
and range of filters throughout the life of the instrument. The pupil mechanism
is designed to be similarly upgradeable with the option to add multiple pupil
mask options. A smaller wheel mechanism allows the user to select the desired
pupil mask with open slots being designed in for future upgrade capabilities.
An ideal pupil would match the shape of the image formed of the primary and
would track its rotation. For different pupil shapes without tracking we model
the additional exposure time needed to achieve the same signal to noise of an
ideal pupil and determine that a set of fixed masks of different shapes

Liger for Next Generation Keck Adaptive Optics: Opto-Mechanical Dewar for Imaging Camera and Slicer
Liger is a next generation adaptive optics (AO) fed integral field
spectrograph (IFS) and imager for the W. M. Keck Observatory. This new
instrument is being designed to take advantage of the upgraded AO system
provided by Keck All-Sky Precision Adaptive-optics (KAPA). Liger will provide
higher spectral resolving power (R4,000-10,000), wider wavelength
coverage (0.8-2.4 m), and larger fields of view than any current
IFS. We present the design and analysis for a custom-made dewar chamber for
characterizing the Liger opto-mechanical system. This dewar chamber is designed
to test and assemble the Liger imaging camera and slicer IFS components while
being adaptable for future experiments. The vacuum chamber will operate below
Torr with a cold shield that will be kept below 90 K. The dewar test
chamber will be mounted to an optical vibration isolation platform and further
isolated from the cryogenic and vacuum systems with bellows. The cold head and
vacuums will be mounted to a custom cart that will also house the electronics
and computer that interface with the experiment. This test chamber will provide
an efficient means of calibrating and characterizing the Liger instrument and

Targeting effector memory T cells with alefacept in new onset type 1 diabetes: 12 month results from the T1DAL study
Background Type 1 diabetes (T1D) results from autoimmune targeting of the pancreatic beta cells, likely mediated by effector memory T cells (Tems). CD2, a T cell surface protein highly expressed on Tems, is targeted by the fusion protein alefacept, depleting Tems and central memory T cells (Tcms). We hypothesized that alefacept would arrest autoimmunity and preserve residual beta cells in newly diagnosed T1D. Methods The T1DAL study is a phase II, double-blind, placebo-controlled trial that randomised T1D patients 12-35 years old within 100 days of diagnosis, 33 to alefacept (two 12-week courses of 15 mg IM per week, separated by a 12-week pause) and 16 to placebo, at 14 US sites. The primary endpoint was the change from baseline in mean 2-hour C-peptide area under the curve (AUC) at 12 months. This trial is registered with ClinicalTrials.gov, number NCT00965458. Findings The mean 2-hour C-peptide AUC at 12 months increased by 0.015 nmol/L (95% CI -0.080 to 0.110 nmol/L) in the alefacept group and decreased by 0.115 nmol/L (95% CI -0.278 to 0.047) in the placebo group, which was not significant (p=0.065). However, key secondary endpoints were met: the mean 4-hour C-peptide AUC was significantly higher (p=0.019), and daily insulin use and the rate of hypoglycemic events were significantly lower (p=0.02 and p<0.001, respectively) at 12 months in the alefacept vs. placebo groups. Safety and tolerability were comparable between groups. There was targeted depletion of Tems and Tcms, with sparing of naïve and regulatory T cells (Tregs). Interpretation At 12 months, alefacept preserved the 4-hour C-peptide AUC, lowered insulin use, and reduced hypoglycemic events, suggesting a signal of efficacy. Depletion of memory T cells with sparing of Tregs may be a useful strategy to preserve beta cell function in new-onset T1D

Bibliometric analysis of academic journal recommendations and requirements for surgical and anesthesiologic adverse events reporting.
BACKGROUND
Standards for reporting surgical adverse events vary widely within the scientific literature. Failure to adequately capture adverse events hinders efforts to measure the safety of healthcare delivery and improve the quality of care. The aim of the present study is to assess the prevalence and typology of perioperative adverse event reporting guidelines among surgery and anesthesiology journals.
MATERIALS AND METHODS
In November 2021, three independent reviewers queried journal lists from the SCImago Journal & Country Rank (SJR) portal (www.scimagojr.com), a bibliometric indicator database for surgery and anesthesiology academic journals. Journal characteristics were summarized using SCImago, a bibliometric indicator database extracted from Scopus journal data. Quartile 1 (Q1) was considered the top quartile and Q4 bottom quartile based on the journal impact factor. Journal author guidelines were collected to determine whether adverse event reporting recommendations were included and, if so, the preferred reporting procedures.
RESULTS
Of 1,409 journals queried, 655 (46.5%) recommended surgical adverse event reporting. Journals most likely to recommend adverse event reporting were: 1) by category surgery (59.1%), urology (53.3%), and anesthesia (52.3%); 2) in top SJR quartiles (i.e. more influential); 3) by region, based in Western Europe (49.8%), North America (49.3%), and the Middle East (48.3%).
CONCLUSIONS
Surgery and anesthesiology journals do not consistently require or provide recommendations on perioperative adverse event reporting. Journal guidelines regarding adverse event reporting should be standardized and are needed to improve the quality of surgical adverse event reporting with the ultimate goal of improving patient morbidity and mortality
Migration Patterns, Use of Stopover Areas, and Austral Summer Movements of Swainson\u27s Hawks
From 1995 to 1998, we tracked movements of adult Swainson’s Hawks (Buteo swainsoni), using satellite telemetry to characterize migration, important stopover areas, and movements in the austral summer. We tagged 46 hawks from July to September on their nesting grounds in seven U.S. states and two Canadian provinces. Swainson’s Hawks followed three basic routes south on a broad front, converged along the east coast of central Mexico, and followed a concentrated corridor to a communal area in central Argentina for the austral summer. North of 20°N, southward and northward tracks differed little for individuals from east of the continental divide but differed greatly (up to 1700 km) for individuals from west of the continental divide. Hawks left the breeding grounds mid-August to mid-October; departure dates did not differ by location, year, or sex. Southbound migration lasted 42 to 98 days, northbound migration 51 to 82 days. Southbound, 36% of the Swainson’s Hawks departed the nesting grounds nearly 3 weeks earlier than the other radio-marked hawks and made stopovers 9.0–26.0 days long in seven separate areas, mainly in the southern Great Plains, southern Arizona and New Mexico, and northcentral Mexico. The birds stayed in their nonbreeding range for 76 to 128 days. All used a core area in central Argentina within 23% of the 738 800-km2 austral summer range, where they frequently moved long distances (up to 1600 km). Conservation of Swainson’s Hawks must be an international effort that considers habitats used during nesting and non-nesting seasons, including migration stopovers
Sports-based intervention and the problem of youth offending: a diverse enough tool for a diverse society?
This paper discusses sports-based interventions (SBIs) and the problem of youth crime. It notes the positive role sport can play in changing to better the lives of young people. However, there is a lack of robust evidence to support the argument that participation in sporting activity can lead to a reduction in anti-social and offending behaviour. The paper discusses how through focusing on ‘individual needs’ and ‘pathways to work’, SBIs can become overly reductionist and mask broader structural class-, gender- and race-based inequalities that permeate through neoliberal nation-states and western criminal justice systems. It concludes that SBI advocates must seek to promote a less homogeneous idea of what an SBI is, as well as be more sensitive to the diverse needs of young people, particularly if they are to tackle the underlying structural inequalities that arguably create the social problem, that is youth crime in the first place
