Effect of real-time computer-aided polyp detection system (ENDO-AID) on adenoma detection in endoscopists-in-training: a randomized trial
Background
The effect of computer-aided polyp detection (CADe) on adenoma detection rate (ADR) among endoscopists-in-training remains unknown.
Methods
We performed a single-blind, parallel-group, randomized controlled trial in Hong Kong between April 2021 and July 2022 (NCT04838951). Eligible subjects undergoing screening/surveillance/diagnostic colonoscopies were randomized 1:1 to receive colonoscopies with CADe (ENDO-AID(OIP-1), Olympus Co., Japan) or not (control) during withdrawal. Procedures were performed by endoscopists-in-training with <500 procedures and <3 years’ experience. Randomization was stratified by patient age, sex, and endoscopist experience (beginner vs intermediate-level, <200 vs 200-500 procedures). Image enhancement and distal attachment devices were disallowed. Subjects with incomplete colonoscopies or inadequate bowel preparation were excluded. Treatment allocation was blinded to outcome assessors. The primary outcome was ADR. Secondary outcomes were ADR for different adenoma sizes and locations, mean number of adenomas, and non-neoplastic resection rate.
Results
386 and 380 subjects were randomized to CADe and control groups, respectively. The overall ADR was significantly higher in CADe than control group (57.5% vs 44.5%, adjusted relative risk 1.41, 95%CI 1.17-1.72, p<0.001). The ADRs for <5mm (40.4% vs 25.0%) and 5-10mm adenomas (36.8% vs 29.2%) were higher in CADe group. The ADRs were higher in CADe group in both right (42.0% vs 30.8%) and left colon (34.5% vs 27.6%), but there was no significant difference in advanced ADR. The ADRs were higher in CADe group among beginners (60.0% vs 41.9%) and intermediate-level endoscopists (56.5% vs 45.5%). Mean number of adenomas (1.48 vs 0.86) and non-neoplastic resection rate were higher in CADe group (52.1% vs 35.0%).
Conclusions
Among endoscopists-in-training, the use of CADe during colonoscopies was associated with increased overall ADR. (ClinicalTrials.gov: NCT04838951
Association of Co-Exposure of Antenatal Steroid and Prophylactic Indomethacin with Spontaneous Intestinal Perforation
Objective: To evaluate the association of a combined exposure to antenatal steroids and prophylactic indomethacin with the outcome of spontaneous intestinal perforation (SIP) among neonates born at \u3c26 weeks of gestation or \u3c750 g birth weight. Study design: We conducted a retrospective study of preterm infants admitted to Canadian Neonatal Network units between 2010 and 2018. Infants were classified into 2 groups based on receipt of antenatal steroids; the latter subgrouped as recent (≤7 days before birth) or latent (\u3e7 days before birth) exposures. The co-exposure was prophylactic indomethacin. The primary outcome was SIP. Multivariable logistic regression analysis was used to calculate aORs. Results: Among 4720 eligible infants, 4121 (87%) received antenatal steroids and 1045 (22.1%) received prophylactic indomethacin. Among infants exposed to antenatal steroids, those who received prophylactic indomethacin had higher odds of SIP (aOR 1.61, 95% CI 1.14-2.28) compared with no prophylactic indomethacin. Subgroup analyses revealed recent antenatal steroids exposure with prophylactic indomethacin had higher odds of SIP (aOR 1.67, 95% CI 1.15-2.43), but latent antenatal steroids exposure with prophylactic indomethacin did not (aOR 1.24, 95% CI 0.48-3.21), compared with the respective groups with no prophylactic indomethacin. Among those not exposed to antenatal steroids, mortality was lower among those who received prophylactic indomethacin (aOR 0.45, 95% CI 0.28-0.73) compared with no prophylactic indomethacin. Conclusions: In preterm neonates of \u3c26 weeks of gestation or birth weight \u3c750 g, co-exposure of antenatal steroids and prophylactic indomethacin was associated with SIP, especially if antenatal steroids was received within 7 days before birth. Among those unexposed to antenatal steroids, prophylactic indomethacin was associated with lower odds of mortality
Formation of the in Two-Photon Collisions at LEP
The two-photon width of the meson has been
measured with the L3 detector at LEP. The is studied in the decay
modes , KK, KK,
KK, , , and
using an integrated luminosity of 140 pb at GeV and
of 52 pb at GeV. The result is
(BR) keV. The dependence of the cross section is studied for
GeV. It is found to be better described by a Vector Meson
Dominance model form factor with a J-pole than with a -pole. In addition,
a signal of events is observed at the mass. Upper limits
for the two-photon widths of the , , and are also
given
Direct Observation of Longitudinally Polarised W Bosons
The three different helicity states of W bosons, produced in the reaction
e+e- -> W+W- -> l nu q q~ are studied using leptonic and hadronic W decays at
sqrt{s}=183GeV and 189GeV. The W polarisation is also measured as a function of
the scattering angle between the W- and the direction of the e- beam. The
analysis demonstrates that W bosons are produced with all three helicities, the
longitudinal and the two transverse states. Combining the results from the two
center-of-mass energies and with leptonic and hadronic W decays, the fraction
of longitudinally polarised W bosons is measured to be 0.261 +/- 0.051(stat.)
+/- 0.016(syst.) in agreement with the expectation from the Standard Model
Search for Scalar Leptons in e+e- collisions at \sqrt{s}=189 GeV
We report the result of a search for scalar leptons in e+e- collisions at 189
GeV centre-of-mass energy at LEP. No evidence for such particles is found in a
data sample of 176 pb^{-1}. Improved upper limits are set on the production
cross sections for these new particles. New exclusion contours in the parameter
space of the Minimal Supersymmetric Standard Model are derived, as well as new
lower limits on the masses of these supersymmetric particles. Under the
assumptions of common gaugino and scalar masses at the GUT scale, we set an
absolute lower limit on the mass of the lightest scalar electron of 65.5 Ge
Study of Z Boson Pair Production in e+e- Collisions at LEP at \sqrt{s}=189 GeV
The pair production of Z bosons is studied using the data collected by the L3
detector at LEP in 1998 in e+e- collisions at a centre-of-mass energy of 189
GeV. All the visible final states are considered and the cross section of this
process is measured to be 0.74 +0.15 -0.14 (stat.) +/- 0.04 (syst.) pb. Final
states containing b quarks are enhanced by a dedicated selection and their
production cross section is found to be 0.18 +0.09 -0.07 (stat.) +/- 0.02
(syst.) pb. Both results are in agreement with the Standard Model predictions.
Limits on anomalous couplings between neutral gauge bosons are derived from
these measurements
K0s K0s Final State in Two-Photon Collisions and Implications for Glueballs
The K0s K0s final state in two-photon collisions is studied with the L3
detector at LEP. The mass spectrum is dominated by the formation of the
f_2'(1525) tensor meson in the helicity-two state with a two-photon width times
the branching ratio into K Kbar of 76 +- 6 +- 11 eV. A clear signal for the
formation of the f_J(1710) is observed and it is found to be dominated by the
spin-two helicity-two state. No resonance is observed in the mass region around
2.2 GeV and an upper limit of 1.4 eV at 95% C.L. is derived for the two-photon
width times the branching ratio into K0s K0s for the glueball candidate
xi(2230)
Study of Z Boson Pair Production in e^+e^- Interactions at \sqrt{s}=192 - 202 GeV
The cross section for the production of Z boson pairs is measured using the
data collected by the L3 detector at LEP in 1999 in e^+e^- collisions at
centre-of-mass energies ranging from 192 GeV up to 202 GeV. Events in all the
visible final states are selected, measuring the cross section of this process.
The special case of final states containing b quarks is also investigated. All
results are in agreement with the Standard Model predictions
Search for a Higgs Boson Decaying into Two Photons at LEP
A Higgs particle produced in association with a Z boson and decaying into two
photons is searched for in the data collected by the L3 experiment at LEP. All
possible decay modes of the Z boson are investigated. No signal is observed in
447.5 pb^-1 of data recorded at centre-of-mass energies up to 209 GeV. Limits
on the branching fraction of the Higgs boson decay into two photons as a
function of the Higgs mass are derived. A lower limit on the mass of a
fermiophobic Higgs boson is set at 105.4 GeV at 95% confidence level
Mock data sets for the Eboss and DESI Lyman- forest surveys
{We present a publicly-available code to generate mock Lyman- (\lya)
forest data sets. The code is based on the Fluctuating Gunn-Peterson
Approximation (FGPA) applied to Gaussian random fields and on the use of fast
Fourier transforms (FFT). The output includes spectra of lya transmitted flux
fraction, , a quasar catalog, and a catalog of high-column-density systems.
While these three elements have realistic correlations, additional code is then
used to generate realistic quasar spectra, to add absorption by
high-column-density systems and metals, and to simulate instrumental effects.
Redshift space distortions (RSD) are implemented by including the large-scale
velocity-gradient field in the FGPA resulting in a correlation function of
that can be accurately predicted. One hundred realizations have been produced
over the 14,000 deg Dark Energy Spectroscopy Instrument (DESI) survey
footprint with 100 quasars per deg, and they are being used for the
Extended Baryon Oscillation Survey (eBOSS) and DESI surveys. The analysis of
these realizations shows that the correlation of follows the prediction
within the accuracy of eBOSS survey. The most time-consuming part of the
production occurs before application of the FGPA, and the existing pre-FGPA
forests can be used to easily produce new mock sets with modified
redshift-dependent bias parameters or observational conditions.Comment: to be submitted ot JCA
