    Infecciones Vectoriales: Manifestaciones Clínicas Y Diagnóstico Diferencial.

    According to the World Health Organization, vector infections are the utmost importance for public health because these diseases are infectious and spread rapidly through insects, snails, among others. Worldwide, it has been detected that these spreads of vector infections occur in countries with tropical zones causing diseases such as Malaria, Zika, Chikungunya, Dengue. The objective of this article was to analyze vector infections, clinical manifestations and differential diagnosis. The established methodology was of the retrospective type with a documentary design, through a systematic review, of scientific journals with scientific databases such as Scielo, Science direct, Redalyc, thus choosing all the available documents on vector infections. It was evidenced that the highest prevalence of a vector infection was Zika with 90% in Brazil, in relation to the clinical manifestations, they present fever, muscle aches, general malaise and to make a differential diagnosis, the different symptoms must be taken into account. presented by people infected by various factors. A nivel mundial se ha detectado que estas propagaciones de infecciones vectoriales se dan en países con zonas tropicales causando enfermedades como la Malaria, Zika, Chikungunya, Dengue. El objetivo de este artículo fue analizar las infecciones vectoriales, manifestaciones clínicas y diagnóstico diferencial. La metodología establecida fue del tipo retrospectiva con un diseño documental, a través de una revisión sistemática, de revistas científicas con bases de datos científicas como Scielo, Science direct, Redalyc, así escogiéndose todos los documentos disponibles sobre infecciones vectoriales. Se evidenció que la mayor prevalencia de una infección vector fue la del Zika con el 90% en Brasil, en relación con las manifestaciones clínicas, presentan fiebre, dolores musculares, malestar general y para hacer un diagnóstico diferencial hay que tener en cuenta los diferentes síntomas que presentan las personas infectadas por diversos factores. Se concluyó que, con respecto a la prevalencia de las infecciones vectoriales en países con zonas tropicales, las enfermedades transmitidas por vectores continúan contribuyendo significativamente a la carga mundial de morbilidad y provocan epidemias que perturban la seguridad sanitaria causando impactos socioeconómicos amplios en todo el mundo

    Recommendations from the International Consensus Conference on Anemia Management in Surgical Patients (ICCAMS)

    Background: Perioperative anemia has been associated with increased risk of red blood cell transfusion and increased morbidity and mortality following surgery. The optimal approach to the diagnosis and management of perioperative anemia is not fully established. Objective: To develop consensus recommendations for anemia management in surgical patients. Methods: An international expert panel reviewed the current evidence and developed recommendations using modified RAND Delphi methodology. Results: The panel recommends that all patients be screened for anemia prior to surgery. Appropriate therapy for anemia should be guided by an accurate diagnosis of the etiology. The need to proceed with surgery in some patients with anemia is expected to persist. However, early identification and effective treatment of anemia has the potential to reduce the risks associated with surgery and improve clinical outcomes. As with preoperative anemia, postoperative anemia should be treated in the perioperative period. Conclusions: Early identification and effective treatment of anemia has the potential to improve clinical outcomes in surgical patients

    Probabilistic Naming of Functions in Stripped Binaries

    Debugging symbols in binary executables carry the names of functions and global variables. When present, they greatly simplify the process of reverse engineering, but they are almost always removed (stripped) for deployment. We present the design and implementation of punstrip, a tool which combines a probabilistic fingerprint of binary code based on high-level features with a probabilistic graphical model to learn the relationship between function names and program structure. As there are many naming conventions and developer styles, functions from different applications do not necessarily have the exact same name, even if they implement the exact same functionality. We therefore evaluate punstrip across three levels of name matching: exact; an approach based on natural language processing of name components; and using Symbol2Vec, a new embedding of function names based on random walks of function call graphs. We show that our approach is able to recognize functions compiled across different compilers and optimization levels and then demonstrate that punstrip can predict semantically similar function names based on code structure. We evaluate our approach over open source C binaries from the Debian Linux distribution and compare against the state of the art

    Distinct and shared functions of ALS-associated proteins TDP-43, FUS and TAF15 revealed by multisystem analyses

    The RNA-binding protein (RBP) TAF15 is implicated in amyotrophic lateral sclerosis (ALS). To compare TAF15 function to that of two ALS-associated RBPs, FUS and TDP-43, we integrate CLIP-seq and RNA Bind-N-Seq technologies, and show that TAF15 binds to ∼4,900 RNAs enriched for GGUA motifs in adult mouse brains. TAF15 and FUS exhibit similar binding patterns in introns, are enriched in 3′ untranslated regions and alter genes distinct from TDP-43. However, unlike FUS and TDP-43, TAF15 has a minimal role in alternative splicing. In human neural progenitors, TAF15 and FUS affect turnover of their RNA targets. In human stem cell-derived motor neurons, the RNA profile associated with concomitant loss of both TAF15 and FUS resembles that observed in the presence of the ALS-associated mutation FUS R521G, but contrasts with late-stage sporadic ALS patients.     Equity, diversity, and inclusion at the Global Alliance for Genomics and Health

    A lack of diversity in genomics for health continues to hinder equitable leadership and access to precision medicine approaches for underrepresented populations. To avoid perpetuating biases within the genomics workforce and genomic data collection practices, equity, diversity, and inclusion (EDI) must be addressed. This paper documents the journey taken by the Global Alliance for Genomics and Health (a genomics-based standard-setting and policy-framing organization) to create a more equitable, diverse, and inclusive environment for its standards and members. Initial steps include the creation of two groups: the Equity, Diversity, and Inclusion Advisory Group and the Regulatory and Ethics Diversity Group. Following a framework that we call "Reflected in our Teams, Reflected in our Standards," both groups address EDI at different stages in their policy development process.     Association Between Race/Ethnicity and COVID-19 Outcomes in Systemic Lupus Erythematosus Patients From the United States: Data From the COVID-19 Global Rheumatology Alliance

    OBJECTIVE: To determine the association between race/ethnicity and COVID-19 outcomes in individuals with systemic lupus erythematosus (SLE). METHODS: Individuals with SLE from the US with data entered into the COVID-19 Global Rheumatology Alliance registry between March 24, 2020 and August 27, 2021 were included. Variables included age, sex, race, and ethnicity (White, Black, Hispanic, other), comorbidities, disease activity, pandemic time period, glucocorticoid dose, antimalarials, and immunosuppressive drug use. The ordinal outcome categories were: not hospitalized, hospitalized with no oxygenation, hospitalized with any ventilation or oxygenation, and death. We constructed ordinal logistic regression models evaluating the relationship between race/ethnicity and COVID-19 severity, adjusting for possible confounders. RESULTS: We included 523 patients; 473 (90.4%) were female and the mean ± SD age was 46.6 ± 14.0 years. A total of 358 patients (74.6%) were not hospitalized; 40 patients (8.3%) were hospitalized without oxygen, 64 patients (13.3%) were hospitalized with any oxygenation, and 18 (3.8%) died. In a multivariable model, Black (odds ratio [OR] 2.73 [95% confidence interval (95% CI) 1.36–5.53]) and Hispanic (OR 2.76 [95% CI 1.34–5.69]) individuals had higher odds of more severe outcomes than White individuals. CONCLUSION: Black and Hispanic individuals with SLE experienced more severe COVID-19 outcomes, which is consistent with findings in the US general population. These results likely reflect socioeconomic and health disparities and suggest that more aggressive efforts are needed to prevent and treat infection in this population

    Remarkable fly (Diptera) diversity in a patch of Costa Rican cloud forest : Why inventory is a vital science

    Study of all flies (Diptera) collected for one year from a four-hectare (150 x 266 meter) patch of cloud forest at 1,600 meters above sea level at Zurqui de Moravia, San Jose Province, Costa Rica (hereafter referred to as Zurqui), revealed an astounding 4,332 species. This amounts to more than half the number of named species of flies for all of Central America. Specimens were collected with two Malaise traps running continuously and with a wide array of supplementary collecting methods for three days of each month. All morphospecies from all 73 families recorded were fully curated by technicians before submission to an international team of 59 taxonomic experts for identification. Overall, a Malaise trap on the forest edge captured 1,988 species or 51% of all collected dipteran taxa (other than of Phoridae, subsampled only from this and one other Malaise trap). A Malaise trap in the forest sampled 906 species. Of other sampling methods, the combination of four other Malaise traps and an intercept trap, aerial/hand collecting, 10 emergence traps, and four CDC light traps added the greatest number of species to our inventory. This complement of sampling methods was an effective combination for retrieving substantial numbers of species of Diptera. Comparison of select sampling methods (considering 3,487 species of non-phorid Diptera) provided further details regarding how many species were sampled by various methods. Comparison of species numbers from each of two permanent Malaise traps from Zurqui with those of single Malaise traps at each of Tapanti and Las Alturas, 40 and 180 km distant from Zurqui respectively, suggested significant species turnover. Comparison of the greater number of species collected in all traps from Zurqui did not markedly change the degree of similarity between the three sites, although the actual number of species shared did increase. Comparisons of the total number of named and unnamed species of Diptera from four hectares at Zurqui is equivalent to 51% of all flies named from Central America, greater than all the named fly fauna of Colombia, equivalent to 14% of named Neotropical species and equal to about 2.7% of all named Diptera worldwide. Clearly the number of species of Diptera in tropical regions has been severely underestimated and the actual number may surpass the number of species of Coleoptera. Various published extrapolations from limited data to estimate total numbers of species of larger taxonomic categories (e.g., Hexapoda, Arthropoda, Eukaryota, etc.) are highly questionable, and certainly will remain uncertain until we have more exhaustive surveys of all and diverse taxa (like Diptera) from multiple tropical sites. Morphological characterization of species in inventories provides identifications placed in the context of taxonomy, phylogeny, form, and ecology. DNA barcoding species is a valuable tool to estimate species numbers but used alone fails to provide a broader context for the species identified.Peer reviewe
