    Absolute and relative reliability of pain sensitivity and functional outcomes of the affected shoulder among women with pain after breast cancer treatment

    Objective Breast cancer survivors (BCS) are often characterized by decreased pressure pain thresholds (PPT), range of motion (ROM) and strength in and around the shoulder affected by the treatment. This intra-rater reliability study was to establish the relative and absolute reliability of PPT’s, active ROM and maximal isokinetic muscle strength (MIMS) of the affected shoulder in BCS with persistent pain after treatment. Methods Twenty-one BCS participated in the study. The PPTs of 17 locations and pain intensity were assessed using a pressure algometer and a numeric rating scale. The ROM was measured using a universal goniometer and MIMS was measured using an isokinetic dynamometer. Relative reliability was estimated using intra class correlation coefficient (ICC), and absolute reliability using standard error of measurement (SEM). Minimum detectable change (MDC) was calculated from SEM. Results The ICCs for PPTs ranged from 0.88–0.97, with SEM values ranging from 12.0 to 28.2 kPa and MDC ranging from 33.2 to 78.2 kPa. The ICCs for ROM ranged from 0.66–0.97, with SEM values ranging from 3.0 to 7.5˚ and MDC ranging from 8.4 to 20.8˚. Finally, ICCs for MIMS ranged from 0.62–0.92, with SEM values ranging from 0.03 to 0.07 Nm/Kg FFM and MDC ranging from 0.09 to 0.19 Nm/kg FFM. Conclusion The results of this study indicate that PPTs, ROM and MIMS can be measured reliably on the affected shoulder in BCS with pain after treatment.     The adequacy of alfalfa crops as an agri-environmental scheme: A review of agronomic benefits and effects on biodiversitye

    The agriculture intensification required to provide enough food commodities for humans has negative impacts on the environment. To reduce these drawbacks Agri-Environmental Schemes (AES) have been implemented in Europe since the last decade of the twentieth century. One of the measures included in these schemes was the introduction of alfalfas in crop rotation systems. In order to evaluate their suitability as an AES, we synthesize knowledge on the agronomical benefits of alfalfa cultivations, as well as on the relationships of alfalfa crops with biodiversity at three taxonomic levels: vascular plants, arthropods, and vertebrates. Based on the articles reviewed, alfalfas can help restoring native grassland communities due to the nutrient enrichment they provide. Moreover, this legume crop usually ensures food resources not only for invertebrates, but also for vertebrates. Alfalfas tend to harbour a stable arthropod community that benefits surrounding crops by improving ecological processes (pollination, pest predation) and reducing the need to use agrochemicals. At the same time, arthropod abundance attracts vertebrates, especially birds, which supports the role of alfalfa as a conservation tool to favour endangered farmland birds. Additionally, alfalfa crops may function as a reservoir habitat for voles, which are a preferred food resource for farmland-foraging raptors, many of them endangered. On the other hand, birds nesting in alfalfa crops may suffer higher nest destruction rates due to frequent cuts, and voles using alfalfa crops may also create agricultural damage in surrounding crops in certain cases. The review also highlights that negative ecological relationships of alfalfa crops with biodiversity can be minimized, and positive effects may also be maximized under the appropriate management of this crop. Examples of such management actions include cut delays or higher cuts to avoid nest destruction, the use of grazing cattle to reduce the impacts of rodent pests, reduced frequency of cutting or maintaining unharvested strips in the field to maximize positive effects on predatory arthropods and vole availability to raptors.     Taphonomy as a methodological framework for understanding surface funerary deposits: the human burial at the cave of El Espinoso (Ribadedeva, Asturias)

    RESUMEN: Durante la Edad del Bronce en el norte de la Península Ibérica algunos grupos humanos inhumaban a sus congéneres sobre la superficie del suelo de cuevas de difícil acceso. Esta tradición cultural, muy arraigada en el País Vasco y en Cantabria, se extiende hasta la región oriental de Asturias. Este trabajo se centra en el estudio tafonómico de los restos humanos encontrados en 1993 en la cueva de El Espinoso (Ribadedeva, Asturias). La cueva fue utilizada como lugar de inhumación de un número mínimo de veinte individuos, de ambos sexos y diferentes edades. Este yacimiento constituye el único enterramiento múltiple en cueva de la Edad del Bronce documentado hasta ahora en Asturias, sumándose así a otros yacimientos de la Prehistoria Reciente de esta región, como son El Toral III, La Llana y Fuentenegroso y consolidando esta tradición funeraria a lo largo de dos mil años en el Oriente de Asturias. La tafonomía ha ofrecido un marco teórico y metodológico preciso para estudiar este tipo de yacimientos en superficie, afectados por complejos procesos post-deposicionales.ABSTRACT: During the Bronze Age the human groups of Cantabria buried their dead on the surface of narrow caves. This cultural tradition, common to the Basque Country and Cantabria, extends to eastern Asturias. This work focuses on the taphonomical study of the human remains found in 1993 in the cave of El Espinoso, located in Ribadedeva (Asturias). The cavity was used as a burial place for a minimum of twenty individuals of both sexes and different ages. This site constitutes the only collective burial cave currently known from the Bronze Age in Asturias (other later prehistoric burial caves in the region ?El Toral III, La Llana, Fuentenegroso, etc.? have single burials). A funerary tradition of cave burial lasted over two thousand years in eastern Asturias.     A COVID-19 Rehabilitation Prospective Surveillance Model for Use by Physiotherapists

A role for rehabilitation professionals from all disciplines in addressing COVID-19 sequelae is recognised, but it is essential that patient assessment be systematic if health complications are to be identified and treated and, if possible, prevented. The aim is to present a COVID-19 prospective surveillance model based on sensitive and easily used assessment tools, which is urgently required. Following the Oxford Centre for Evidence-Based Medicine Level of Evidence Tool, an expert team in cardiorespiratory, neuromuscular and mental health worked via telemeetings to establish a model that provides guidelines to rehabilitation professionals working with patients who require rehabilitation after suffering from COVID-19. A COVID-19 prospective surveillance model is proposed for use by rehabilitation professionals and includes both face-to-face and telematic monitoring components.     Pain sensitivity and shoulder function among breast cancer survivors compared to matched controls: a case-control study

    Objective Persistent pain and loss of shoulder function are common adverse effects to breast cancer treatment, but the extent of these issues in comparison with healthy controls is unclear for survivors beyond 1.5 years after treatment. The purpose of this study was to benchmark differences in pressure pain thresholds (PPT), maximal isokinetic muscle strength (MIMS), and active range of motion (ROM) of females with persistent pain ≥1.5 years after breast cancer treatment (BCS) compared with pain-free matched controls (CON), and examine the presence of movement-evoked pain (MEP) during assessment of MIMS. Methods The PPTs of 18 locations were assessed using a pressure algometer and a numeric rating scale was used to assess intensity of MEP. Active ROM and MIMS were measured using a universal goniometer and an isokinetic dynamometer, respectively. Results A two-way analysis of variance revealed that PPTs across all locations,MIMS for horizontal shoulder extension/flexion and shoulder adduction, active ROM for shoulder flexion, horizontal shoulder extension, shoulder abduction, and external shoulder rotation were significantly lower for BCS compared with CON (P < 0.05). MEP was significantly higher for BCS and MEP intensity had a significant, negative correlation with PPTs (P < 0.01). Discussion/conclusion BCS with persistent pain ≥1.5 years after treatment demonstrates widespread reductions in PPTs and movement-specific reductions inMIMS and active ROMof the affected shoulder, along with MEP during physical performance assessment. Implications for cancer survivors BCS with persistent pain ≥1.5 years after treatment shows signs of central sensitization andmay benefit from individualized rehabilitation.Danish Cancer Association R204-A1246

    Palaeogenomics of Upper Palaeolithic to Neolithic European hunter-gatherers

    Modern humans have populated Europe for more than 45,000 years1,2. Our knowledge of the genetic relatedness and structure of ancient hunter-gatherers is however limited, owing to the scarceness and poor molecular preservation of human remains from that period3. Here we analyse 356 ancient hunter-gatherer genomes, including new genomic data for 116 individuals from 14 countries in western and central Eurasia, spanning between 35,000 and 5,000 years ago. We identify a genetic ancestry profile in individuals associated with Upper Palaeolithic Gravettian assemblages from western Europe that is distinct from contemporaneous groups related to this archaeological culture in central and southern Europe4, but resembles that of preceding individuals associated with the Aurignacian culture. This ancestry profile survived during the Last Glacial Maximum (25,000 to 19,000 years ago) in human populations from southwestern Europe associated with the Solutrean culture, and with the following Magdalenian culture that re-expanded northeastward after the Last Glacial Maximum. Conversely, we reveal a genetic turnover in southern Europe suggesting a local replacement of human groups around the time of the Last Glacial Maximum, accompanied by a north-to-south dispersal of populations associated with the Epigravettian culture. From at least 14,000 years ago, an ancestry related to this culture spread from the south across the rest of Europe, largely replacing the Magdalenian-associated gene pool.     Structuring domestic space in the Lower Magdalenian: an analysis of the fauna from Level 115 of El Mirón Cave, Cantabria

    Documenting the intentional structuring of space by hunter-gatherers can be challenging, especially in complex cave contexts. One approach is the spatial analysis of discard patterns. Here, the authors consider the spatial distribution of faunal remains from the Lower Magdalenian Level 115 in El Mirón Cave, Cantabria, to assess a possible structuring function for an unusual alignment of rocks.     The study of the economic behaviour of the last hunter-gatherers of the Cantabrian Mesolithic, mainly dominated by the rich assemblages related to marine exploitation, has limited evidences of terrestrial mammals consumption, which implies a break with the general trend observed during the Upper Palaeolithic. The reasons behind this change and its implication in the demography of the region are assessed here with the detailed archaezoological and taphonomical analysis of the macromammals of Mazaculos II Cave (Ribadedeva, Asturias), a shell-midden that houses one of the most important fossil deposits of this period. In addition the first signs of domestication in Western Cantabria are presented.El estudio del comportamiento económico desarrollado por los últimos grupos de cazadores-recolectores del Mesolítico Cantábrico, fundamentalmente dominado por el abundante registro de la explotación del medio marino, cuenta con reducidas evidencias del consumo de mamíferos terrestres, en lo que supone una ruptura con la tendencia observada durante el Paleolítico Superior.
