    As I am giving my full attention to the Western omelet, home fries, and rye toast on my plate, I do not notice what is going on around me. I’m not drunk, but I’m not sober either. I’m with some friends I know well and some people I barely know, but they all seem nice enough to share a late night meal. I’m starving. I start to realize that more and more non-college students enter LDs, presumably local Gettysburg residents.     Aggressive behavior in men is seen as decisive, forceful, ambitious, and leader-like; it is often commended and even rewarded. Aggressive behavior in women is seen as hysterical, domineering, bitchy, and certainly not rewarded. Even assertive behavior in women is misunderstood to be aggressive. Both conscious and unconscious or implicit bias are at play. Although individual men are not responsible for these inequities, we all (men included) need to work to change our culture. Let’s agree that this double standard is unfair, and our workplaces need massive cultural change. This article is about how women can navigate through this double standard and still stand up for themselves.     Sentinel Lymph Node Biopsy Does Not Improve Disease-Specific Survival in Elderly Patients with Intermediate Thickness Melanoma

    Objective: To determine whether sentinel lymph node biopsy (SLNB) is associated with improved disease-specific survival among elderly patients with intermediate-thickness melanoma Design: Retrospective cohort study of prospectively-maintained tumor registry Setting: Single institution tertiary care center. P atients: Adults ≥ 70 years of age, who underwent surgical intervention for melanoma from 2000-2013. Main Outcome Measures: The primary outcomes were overall survival (OS) and disease-free survival (DFS). Other clinicopathologic variables measured included age, gender, anatomic site, histologic type, tumor thickness, presence of adverse features, receipt and result of SLNB, and receipt of completion lymph node dissection (CLND). Results: Ninety-one patients (mean age 80 years, 54% male) underwent wide excision of an intermediate-thickness melanoma. Forty-nine patients (54%) received a SLNB. Seven of these biopsies (14%) were positive, and five patients (71%) went on to receive CLND. Five-year OS was 41% in patients who did not receive SLNB and 52% in patients who did receive SLNB (Fig. 1A). However, 5-year DFS was 79% in patients who did not receive SLNB and 77% in patients who did receive SLNB (Fig. 1B). Conclusions: Among elderly patients with intermediate-thickness melanoma, patients who received SLNB had higher 5-year OS than those who did not receive SLNB. However, the 5-year DFS is similar between the two groups, which suggests that the OS differences are related to non-melanoma factors. Routine SLNB for intermediate-thickness melanoma patients may not significantly change the outcome for this age group, and clinical decision-making should consider individual patient comorbidities and goals of care

    Evaluation of Low-Pressure Cold Plasma for Disinfection for ISS Grown Produce and Metallic Instrumentation

    Cold plasma (CP) cleaning is a dry, non-thermal process, which can provide broad-spectrum antimicrobial activity yet reportedly causes little to no damage to the object being sanitized. Since cold plasma uses no liquids, it has the distinct advantage when used in microgravity of not having to separate liquids from the item being cleaned. This paper will present results on an effort to use low pressure CP to disinfect or sterilize materials for in space applications. Exposure times from 0 to 60 minutes and pressures ranging from 10 to 100 Pa were used to optimize plasma parameters to achieve acceptable kill rates for 3 bacteria, Bacillus cereus, E. coli and Bacillus pumulis SAFR-32 and one fungi, Aspergillus niger. These tests were done on produce and metal coupons to simulate medical equipment. Produce testing was not successful, with unacceptable kill rates and the produce being negatively impacted by exposure to the plasma. The plasma caused a 5 log reduction in the number of viable bacteria on metal coupon tests, which placed the number of viable bacteria below the detection limit. This is a very promising result showing that sterilization of medical equipment with cold plasma is feasible. Scanning Electron Microscope images were taken before and after exposure. The images after plasma exposure shows that the bacteria spores have been physically affected, as their size has gotten smaller and the appearance has changed

    Are Compact High-Velocity Clouds Extragalactic Objects?

    Compact high-velocity clouds (CHVCs) are the most distant of the HVCs in the Local Group model and would have HI volume densities of order 0.0003/cm^3. Clouds with these volume densities and the observed neutral hydrogen column densities will be largely ionized, even if exposed only to the extragalactic ionizing radiation field. Here we examine the implications of this process for models of CHVCs. We have modeled the ionization structure of spherical clouds (with and without dark matter halos) for a large range of densities and sizes, appropriate to CHVCs over the range of suggested distances, exposed to the extragalactic ionizing photon flux. Constant-density cloud models in which the CHVCs are at Local Group distances have total (ionized plus neutral) gas masses roughly 20-30 times larger than the neutral gas masses, implying that the gas mass alone of the observed population of CHVCs is about 40 billion solar masses. With a realistic (10:1) dark matter to gas mass ratio, the total mass in such CHVCs is a significant fraction of the dynamical mass of the Local Group, and their line widths would exceed the observed FWHM. Models with dark matter halos fare even more poorly; they must lie within approximately 200 kpc of the Galaxy. We show that exponential neutral hydrogen column density profiles are a natural consequence of an external source of ionizing photons, and argue that these profiles cannot be used to derive model-independent distances to the CHVCs.     The Origin and Distribution of Cold Gas in the Halo of a Milky Way-Mass Galaxy

    We analyze an adaptive mesh refinement hydrodynamic cosmological simulation of a Milky Way-sized galaxy to study the cold gas in the halo. HI observations of the Milky Way and other nearby spirals have revealed the presence of such gas in the form of clouds and other extended structures, which indicates on-going accretion. We use a high-resolution simulation (136-272 pc throughout) to study the distribution of cold gas in the halo, compare it with observations, and examine its origin. The amount (10^8 Msun in HI), covering fraction, and spatial distribution of the cold halo gas around the simulated galaxy at z=0 are consistent with existing observations. At z=0 the HI mass accretion rate onto the disk is 0.2 Msun/yr. We track the histories of the 20 satellites that are detected in HI in the redshift interval 0.5>z>0 and find that most of them are losing gas, with a median mass loss rate per satellite of 3.1 x 10^{-3} Msun/yr. This stripped gas is a significant component of the HI gas seen in the simulation. In addition, we see filamentary material coming into the halo from the IGM at all redshifts. Most of this gas does not make it directly to the disk, but part of the gas in these structures is able to cool and form clouds. The metallicity of the gas allows us to distinguish between filamentary flows and satellite gas. We find that the former accounts for at least 25-75% of the cold gas in the halo seen at any redshift analyzed here.     A Pilot Study of Neuroplasticity Based Cognitive Remediation in Early Onset Psychosis

    Introduction – Neuroplasticity based auditory and visual training programs appear to improve neurocognitive function in adults with schizophrenia, but use in younger individuals has not been determined. We hypothesized that adolescents might play more often and respond better than adults to training using a game-like laptop in their home environment. Methods -- Youth 10-19 years with Early Onset Psychosis (EOP) were provided a laptop and randomly assigned to play games to enhance basic auditory, visual and social processing neuroplasticity games (NPG) or assigned to control games with cognitive components, such as Sudoku or hangman or (CG). All received neurocognitive assessments at baseline, intervention completion and 4 months post treatment. Results — 12 youth (15.5 +3.2 yrs) were assigned to NPG and 10 participants (16.2 +2.1 years) were assigned to CG. More NPG than CG participants completed the prescribed hours of game play (block 1 - 92% vs. 70% over the first 40 hours), with both groups engaged less over time. Although most neurocognitive functions did not change, the NPG group did show improvements in WRAML Visual Learning, WISC Digit Span Forward, Spatial Span Backwards and CPT omission errors. Surprisingly, satisfaction was lower for NPG than CG. Conclusions — Groups were well matched for baseline illness characteristics. On the global measures of cognition, both EOP groups showed improvement over time but those improvements were generally greater in the CG than in the NPG group, with potentially significant differences favoring the CG evident in the neurocognitive composite score (p=0.072) and BRIEF metacognition (p=.117).
