    Anatomic Two-dimensional and Three-dimensional Models for Cyclic Fatigue Testing of Endodontic Instruments

    Introduction: In this study, new devices were developed and evaluated for cyclic fatigue testing (CTF) of endodontic instruments in two-dimensional (2D) and three-dimensional (3D) replicas of the internal anatomy of a mesial root of a mandibular molar. Methods and Materials: The trajectories of both curved mesial canals of the same root were outlined from computed tomographic scans and exported to a computer assisted drawing (CAD) software. In the CAD program, the canals were virtually enlarged to a size 25/0.06. The CTF devices were then prototyped in stainless steel. The 2D models represented the bucco-lingual (BL) and mesio-distal (MD) views of the canals, while the original trajectory was kept in the 3D model. Vortex Blue 25/0.06 instruments were tested for fatigue in the six canals (n=12). The number of cycles to failure (NCF) and fragment length (mm) were recorded. Data was statistically analyzed (ANOVA and post-hoc Games-Howell test) with the level of significance set at 5%. Results: The mean NCF was significantly different among all the canals (P<0.05). The lifespan of the files was greater in the 2D-BL models, followed by the 2D-MD and 3D. The mean fragment length was also different among the tested models (P<0.05). Conclusions: The tested 2D and 3D representations of the same canal trajectory resulted in significant differences both in the amount of stress (seen from NCF) and localization of the maximum stress (seen from fragment length). Further investigations are required to better understand the effects of different 3D curvatures on the cyclic fatigue of endodontic instruments.Keywords: Cyclic Fatigue; Dental Instruments; Endodontics; Nickel-Titaniu

    Mechanical properties of the NiTi Memoria Leaf Spring Activated Expander (NiTi MLSAE) for maxillary transverse discrepancy correction : an in-vitro study

    To determine the mechanical properties of the NiTi Memoria® Leaf Spring Activated Expander (NiTi MLSAE) in two forms, unaltered (unbent) and altered (bent) to mimic clinical use. This in-vitro pilot study was conducted using eight NiTi MLSAE expanders (American Tooth Industries, Oxnard, California) representing four force magnitudes: 10mm 500g, 10mm 900g, 6mm 450g and 6mm 900g models. Two experiments were performed: the first tested the expanders in their unbent form and the second tested them after they were bent by one experienced technician. All expanders were adapted to a standard three dimensional printed maxillary study model. A Dillion Quantrol 500N (110lbf) load cell and a custom-made fixturing apparatus was used to determine the amount of expansive forces delivered. Prior to testing, the ligation compressing the NiTi MLSAE leaves was cut to allow the appliances to expand to their original form. Emperor? (force) Software was used to measure the expansion forces. The average expansion forces generated by the expanders were: unaltered = 897.4g (8.8N) and bent = 877.0g (8.6N) for the 10mm 900g model, unaltered = 489.5g (4.8N) and bent = 479.3g (4.7N) for the 10mm 500g model, unaltered = 458.9g (4.5N) and bent = 438.5g (4.3N) for the 6mm 450g model, and unaltered = 805.6g (7.9N) and bent = 785.2g (7.7N) for the 6mm 900g model. Regardless of whether the expander was straight or bent, the forces generated by the 10mm 900g, 10mm 500g and 6mm 450g Ni-Ti MLSAEs correlated with the benchmark study conducted by the manufacturer. However, the forces generated by the 6mm 900g Ni-Ti MLSAE were less than the data published by the manufacturer. Binding was observed when the expanders were manipulated to mimic clinical use, and this may account for the reported lower expansion force

    Towards liver-directed gene therapy: Retrovirus-mediated gene transfer into human hepatocytes

    Liver-directed gene therapy is being considered in the treatment of inherited metabolic diseases. One approach we are considering is the transplantation of autologous hepatocytes that have been genetically modified with recombinant retroviruses ex vivo. We describe, in this report, techniques for isolating human hepatocytes and efficiently transducing recombinant genes into primary cultures. Hepatocytes were isolated from tissue of four different donors, plated in primary culture, and exposed to recombinant retroviruses expressing either the LacZ reporter gene or the cDNA for rabbit LDL receptor. The efficiency of gene transfer under optimal conditions, as determined by Southern blot analysis, varied from a maximum of one proviral copy per cell to a minimum of 0.1 proviral copy per cell. Cytochemical assays were used to detect expression of the recombinant derived proteins, E. coli β-galactosidase and rabbit LDL receptor. Hepatocytes transduced with the LDL receptor gene expressed levels of receptor protein that exceeded the normal endogenous levels.     Development of a Gripping Fixture for Micro-Tensile Testing of Bonded Ceramic Dumbbells

    It is proposed that both the adhesive interface geometry and the mismatch of elastic moduli influence the tensile strengths of dental bonds attaching restorative ceramics to dentin. Prior calculations indicate this to be due to peripheral interface stress singularities. A physical testing approach to examine and validate the theoretical conclusions utilizing a microtensile testing system is presented. Considering the choice of shear versus tensile and then macro versus micro tensile testing, reasons were identified for choosing micro tensile testing. Specimen dimensions and shapes were developed to optimize the adjustment of the interface geometries and information that could be obtained therein. Here a dumbbell structure is best suited to the testing needs. Dumbbell specimens were first fabricated using the ceramic press technique, and then mini-CNC machining. Specimens fabricated by each technique were examined, showing that the mini-CNC machining methodology was superior. Significant problems in instrumentation were overcome by the design and fabrication of two testing fixtures: 1. A collet based design with independent upper and lower mechanical grips for each end of the dumbbell, to be used in conjunction with a loading device; 2. A screw based clamping design similar to previous jigs, using two screw clamps on V-channels connected by sliding rods. Testing revealed that the collet-based design shows the most promise because of its distributed gripping load. Further tests that can evaluate the effectiveness of this device for microtensile testing are outlined

    Shear Bond Strength and Bracket Base Morphology of New and Rebonded Orthodontic Ceramic Brackets

    The objectives of this study were to (1) to evaluate the shear bond strength (SBS) of two ceramic brackets when new and when rebonded following various bracket base conditioning methods, and (2) to determine bond failure mode relative to bracket base morphology. 100 Symetri ClearTM (SC) and 100 Radiance Plus® (RP) ceramic brackets were bonded to bovine incisors and divided into five groups: one group served as controls and four had brackets rebonded following conditioning by: no surface treatment, sealant, sandblasting, and flame then steam. SBS, adhesive remnant index, and bracket base morphology were evaluated. SBS showed no statistical difference between new and rebonded with no surface treatment or sealant (SC brackets) and with sealant or flame and steam (RP brackets). When comparing SC to RP, SBS was higher with SC, no surface treatment, and sandblasted groups. All groups had varying amounts of adhesive left on the tooth, with the sandblasted group having the most. SEM analysis showed that sandblasting damaged the retention features of bracket bases. In conclusion, when rebonded, the SBS of SC brackets that had no surface treatment and both SC and RP brackets that had sealant showed no significant differences to new brackets.     Antibody-Mediated Rejection of Human Orthotopic Liver Allografts: A Study of Liver Transplantation Across ABO Blood Group Barriers

    A clinicopathologic analysis of liver transplantation across major ABO blood group barriers was carried out 1) to determine if antibody-mediated (humoral) rejection was a cause of graft failure and if humoral rejection can be identified, 2) to propose criteria for establishing the diagnosis, and 3) to describe the clinical and pathologic features of humoral rejection. A total of 51 (24 primary) ABO-incompatible (ABO-I) liver grafts were transplanted into 49 recipients. There was a 46% graft failure rate during the first 30 days for primary ABO-I grafts compared with an 11% graft failure rate for primary ABO compatible (ABO-C), crossmatch negative, age, sex and priority-matched control patients (P < 0.02). A similarly high early graft failure rate (60%) was seen for nonprimary ABO-I grafts during the first 30 days. Clinically, the patients experienced a relentless rise in serum transaminases, hepatic failure, and coagulopathy during the first weeks after transplant. Pathologic examination of ABO-I grafts that failed early demonstrated widespread areas of geographic hemorrhagic necrosis with diffuse intraorgan coagulation. Prominent arterial deposition of antibody and complement components was demonstrated by immunoflourescent staining. Elution studies confirmed the presence of tissue-bound, donor-specific isoagglutinins within the grafts. No such deposition was seen in control cases. These studies confirm that antibody mediated rejection of the liver occurs and allows for the development of criteria for establishing the diagnosis
