DEGRADAÇÃO DE AGROTÓXICO NA CALDA EXCEDENTE PELO PROCESSO UV/H2O2

Engenharia Sanitária e Ambiental.O uso abusivo dos agrotóxicos no mundo todo vem trazendo uma preocupação com os problemas ambientais e sociais provocados por estes produtos. Os agrotóxicos apresentam uma ameaça ao meio ambiente e a saúde humana. O presente trabalho estudou o processo oxidativo avançado UV/H2O2 como uma alternativa de tratamento do agrotóxico Clorpirifós (CP), através da degradação do produto. Para isto foi realizado um experimento em escala laboratorial com reator de bancada e radiação UV através de lâmpada de mercúrio. Foi analisada a degradação do agrotóxico utilizando diferentes concentrações de peróxido de hidrogênio, 1500, 1750 e 2000 mg.L-1 respectivamente, durante três tempos de reação, 40, 60 e 80 minutos, para cada concentração. O pH se manteve ácido durante toda a reação. A concentração residual de peróxido apresentou um resultado diferente do esperado, sofrendo um aumento com o tempo de reação. A concentração do Clorpirifós apresentou diminuição significativa para um pré-tratamento, tendo comportamento muito similar para as concentrações de peróxido de hidrogênio de 1750 e 2000 mg.L-1, chegando a uma eficiência máxima de degradação de 32%. Nos primeiros 40 minutos de reação se obteve a queda de concentração do CP mais significativa. A reação de degradação desse agrotóxico pode ser explicada com uma reação cinética de pseudo-primeira ordem. As constantes obtidas para a cinética de pseudo-primeira ordem apresentam um comportamento diretamente proporcional em relação a concentração de agente oxidante, porém os resultados obtidos foram muito similares para todas as concentrações de H2O2 analisadas. É possível determinar que a concentração de peróxido de hidrogênio mais adequada testada nesse estudo foi de 1750 mg.L-1, devido aos resultados para esta concentração serem muito próximos aos da concentração máxima estudada. O tempo de reação mais adequado foi em torno de 40 minutos, otimizando o processo, diminuindo os custos e o tempo de reação
Hydropic leiomyoma of the uterus presenting as a giant abdominal mass
We describe a case of a 35-year-old woman with a
pedunculated uterine leiomyoma with diffuse hydropic
degeneration presenting as a giant abdominal mass. The
patient was admitted in the emergency department
because of diffuse abdominal bloating and discomfort.
Ultrasonography (US) showed a heterogeneous
abdominopelvic mass. Magnetic resonance imaging
(MRI) was performed to further characterise and revealed
a myometrial pedunculated tumour. Despite its marked
T2-signal heterogeneity and volume, there were no other
suspicious findings to suggest a malignant nature;
therefore, fertility-sparing myomectomy was performed.

AbstractIntroductionThe treatment of inflammatory bowel disease (IBD) has focussed on the management of symptoms but is becoming more resolute on changing the course of the disease and its complications in the long-term. In order to minimize the development of complications and to improve outcomes for these patients it is important to develop other strategies to manage IBD and to optimize current clinical practice.ObjectiveThis article reports the main consensus statements reached during the Portuguese National Meeting on improvement of disease control in IBD, on optimization of corticosteroid and immunosuppressive use in Crohn's disease and on best practice in topics of current interest in Crohn's disease.MethodsAn International Steering Committee selected the top 10 most important unanswered practical questions on the use of conventional therapy in Crohn's disease, to be debated and analysed in several National Meetings of different countries. In each country a National Steering Committee (NSC) was created to moderate a National Meeting during which several expert groups answered the selected questions in light of their clinical practice. Answers were classified according to the Oxford levels of evidence.ConsensusA general consensus was obtained, some of the conclusions were as follows. It is important to introduce conventional corticosteroids in moderate to severely active Crohn's disease of any localization with initial duration of treatment varying according to patient's response; the best option to prevent steroid-induced side effects is to avoid its prolonged or repetitive use and switching appropriate patients to immunosuppressive therapy. Initiation of immunomodulators early in the disease course should be considered for patients with a poor prognosis and optimal safety monitoring was discussed, with the need to reassess patients at appropriate timepoints, make corticosteroid-free remission a goal and treat beyond symptoms
Revisão da literatura e caracterização da população observada na consulta de reumatologia do Centro Hospitalar Cova da Beira, E. P. E.
Introdução: A Esclerose Sistémica é uma doença do tecido conjuntivo caracterizada por lesões vasculares e microvasculares generalizadas e pelo aumento da deposição de componentes da matriz na pele e nos órgãos internos, particularmente nos pulmões, coração, rim e tracto gastro-intestinal. Este estudo visa caracterizar uma amostra de pacientes com esta patologia que, embora rara, apresenta uma elevada morbilidade e mortalidade. Objectivo: Proponho-me a fazer um estudo da população seguida na Consulta de Reumatologia do Centro Hospitalar Cova da Beira com esta patologia, procurando avaliar as características epidemiológicas da amostragem; as etiologias passíveis de ligação com esta doença; o tempo até ao diagnóstico; o padrão de auto-anticorpos; as manifestações cutâneas, vasculares e das diferentes apresentações sistémicas desta doença; avaliação da patologia de órgão alvo de acordo com os meios complementares de diagnóstico recomendados e, finalmente, aplicação de escalas de severidade e actividade da doença, assim como questionários para a avaliação da qualidade de vida e incapacidade nestes pacientes. Métodos: Foram identificados oito pacientes com Esclerose Sistémica (ES) na Consulta de Reumatologia do CHCB. Foi então elaborado um questionário com vista à caracterização das manifestações clínicas, assim como aplicados os questionários padronizados SF-36 e HAQ. Foram então revistos os Exames Complementares de Diagnóstico nos respectivos processos clínicos. Após a colheita de todos os dados foi realizado um tratamento de forma descritiva, justificável pela reduzida amostragem do estudo.
Resultados: A amostragem é constituída por 8 doentes (6 com ES Limitada e 2 com ES Difusa), sendo que 7 destes são do sexo feminino. A média de idades é de 63,62 anos (55-77 anos) e a média de anos de evolução da doença nesta população é de 16,25 anos (8-34 anos). O tempo médio até ao diagnóstico foi de 12,37 anos. As manifestações clínicas mais comuns são as cutâneas e as vasculares e são também as de aparecimento mais precoce. As manifestações clínicas indicadoras de atingimento de órgão são escassas a nível pulmonar, cardíaco e renal para toda a amostra, comparativamente ao nível de envolvimento detectado nos exames complementares de diagnóstico. Os pacientes com ES difusa apresentam mais manifestações sistémicas que os pacientes com ES limitada. As manifestações vasculares e pulmonares são as que apresentam graus mais elevados de severidade da doença. Sete pacientes apresentam doença activa segundo a aplicação do índice de actividade da doença. Os índices de incapacidade avaliados através do HAQ foram mais elevados para a ESD, com maiores dificuldades ao nível das actividades da vida diária. As médias dos componentes físicos e mentais do SF-36 estiveram abaixo dos controlos saudáveis. Discussão: A reduzida amostragem, embora se encontre dentro dos valores de prevalência descritos para esta patologia, é uma das principais limitações à discussão do estudo. O tempo até ao diagnóstico é muito longo, o que confirma as dificuldades diagnósticas nesta patologia. Os clínicos devem estar atentos às manifestações iniciais, que podem ser frustres, atrasando em muito a detecção precoce e tratamento atempado do atingimento de órgão alvo. Conclusões: É necessária uma avaliação multifacetada e sistemática nesta patologia, para que a detecção mais precoce desta doença e do atingimento de cada órgão alvo seja cada vez mais uma realidade.Introduction: Systemic Sclerosis (SS) is a generalized disorder of the conjunctive tissue characterized
by widespread vascular and micro vascular lesions and by the increased deposition of matrix
components in the skin and internal organs, particularly the lungs, heart, kidney and gut. This study
wills to evaluate a sample of patients with this disease that, despite its rarity, presents with a high
morbidity and mortality.
Goal: I propose to do a populational study of patients with this disease followed in the Rheumatology
Consult in CHCB, searching to evaluate the epidemiological features of the sample; the possible
etiology of the disease; the time till diagnosis; the antibody pattern; the cutaneous, vascular and
systemic presentations of this disease; evaluation of the target organ pathology accordingly with the
recommended complementary exams and, finally, the application of severity and activity indices, and
also questionnaires for evaluation of the quality of life and disability in these patients.
Methods: Eight patients were identified with SS followed in the Rheumatology Consult of CHCB. A
questionnaire was then elaborated to characterize the clinical symptoms. Padronized questionnaires
SF-36 and HAQ were also applied. The complementary exams were then reviewed in the respective
clinical files. After the collection of all data, a descriptive treatment was applied, justifiable by the
reduced sample of the study.
Results: The sampling constitutes of eight patients (6 with limited SS and 2 with diffuse SS), in witch 7
are females. The age average is 63,62 years (55-77 years) and the medium of years of disease
evolution in this population is 16,25 years (8-34 years). The median time until the diagnosis was 12,37
years. The cutaneous and vascular clinical manifestations are the earlier and most common. The
clinical manifestations indicative of organ involvement are scarce at the pulmonary, cardiac and renal
levels in comparison to the involvement detected in the complementary exams. Patients with the
diffuse form of the disease present more systemic manifestations than the patients with the limited
form. Vascular and pulmonary manifestations are the ones that present the higher index of disease
severity. Seven patients present active disease according to the application of the activity criteria. The
disability indices evaluated through HAQ were higher for the diffuse form, with greater difficulties at the level of daily activities. The averages of the physical and mental components of SF-36 were below the
healthy subjects.
Discussion: The reduced sampling, although inside the prevalence values described for this
pathology, is one of the principal limitations to the study discussion. The time till diagnosis is very long,
which confirms the difficulties in the diagnostic of this pathology. The doctors should be aware to the
inicial complaints, which can be deceiveing, delaying the earlier detection and timed treatment of the
target organ involvement.
Conclusion: It is necessary a multifaceted and systematic analysis of this disease, so that earlier
detection of the symptoms and the target organ involvement represents reality even more

Clinical performance of an infliximab rapid quantification assay
Background: Therapeutic drug monitoring (TDM)-based algorithms can be used to guide infliximab (IFX) adjustments in inflammatory bowel disease (IBD) patients. This study aimed to explore a rapid IFX-quantification test from a clinical perspective. Methods: This manuscript describes a prospective cohort study involving 110 ulcerative colitis (UC) patients on the maintenance phase of IFX. IFX trough levels were quantified using a rapid quantification assay and a commonly-used reference kit. Results: Irrespective of the assay used to measure IFX, its through levels were statistically different between patients with and without endoscopic remission (Mayo endoscopic score = 0), as well as between patients stratified by their faecal calprotectin (FC) levels. Despite the fact that the two methods correlated well with each other [Spearman's rank correlation coefficient = 0.843, p < 0.001; intraclass correlation coefficients = 0.857, 95% confidence interval (CI): 0.791-0.903], there was a discernible systematic variation; values obtained with the reference kit were on average 2.62 units higher than those obtained with the rapid assay. Notwithstanding, 3 mu g/ml was shown to be an acceptable cut-off to assess endoscopic status and inflammatory burden levels using both assays. The percentage of patients that had a positive outcome when the IFX concentration measured by the rapid assay ranked above 3 mu g/ml was 88% both for a Mayo endoscopic score <= 1 and for an FC concentration <250 mu g/g. Conclusions: Based on this study, we concluded that using the rapid IFX assessment system with a 3 mu g/ml threshold is a reliable alternative to the time-consuming enzyme-linked immunosorbent assays in patients on the maintenance phase of IFX.Portuguese IBD Group (GEDII, Grupo de Estudo da Doenca Inflamatoria Intestinal)info:eu-repo/semantics/publishedVersio

Imigração e trabalho precário: reflexões acerca da chegada da população haitiana no oeste de Santa Catarina
O contexto econômico atual tem transformado o Brasil em importante destino da força de trabalho internacional que migra em busca de melhores condições de vida. Nesse sentido, este artigo pretende realizar reflexões quanto à s implicações do recente fluxo imigratório de haitianos para a região Oeste de Santa Catarina. A pesquisa aqui apresentada baseou-se no método qualitativo, em que foram utilizados como instrumentos para o levantamento de informações entrevistas semiestruturadas, observação participante e pesquisa documental. Os resultados indicam que os haitianos são motivados a imigrar para o Brasil pela perspectiva de conseguir trabalho formal e melhorar as condições de vida, tendo em vista o contexto de crise econômica e desemprego em seu país de origem. No entanto, a inserção desses trabalhadores se dá especialmente em postos de trabalho precários em função da baixa remuneração e dos riscos de adoecimento. A contratação de haitianos é feita por empresas e indústrias que buscam suprir a falta de força de trabalho local, no entanto, esta população começa a sofrer com o aumento do desemprego. Por fim, os resultados sinalizam para a necessidade de criar políticas públicas específicas para a população imigrante no país, tendo em vista a condição e vulnerabilidade dessa população no que diz respeito à exploração do trabalho
The Interaction between Tribolium castaneum and Mycotoxigenic Aspergillus flavus in Maize Flour
Tribolium castaneum is one of the most common insect pests of stored products. Its presence
makes cereals more susceptible to the spread of the fungi Aspergillus flavus, which may produce
mycotoxins. The aim of this work was to evaluate the influence of T. castaneum adults on the
development of a mycotoxigenic A. flavus strain in maize flour as well as the influence of this fungus
on the insects. Maize flour was exposed to T. castaneum, spores of A. flavus or to both. The results
revealed an interaction between T. castaneum and A. flavus as the flour exposed to both organisms
was totally colonized by the fungus whereas almost all the insects were killed. Aflatoxin B1 (AFB1)
revealed a significantly higher concentration in the flour inoculated with both organisms (18.8 g/kg),
being lower when exposed only to A. flavus, suggesting that the presence of insects may trigger
fungal development and enhance mycotoxin production. The ability of these organisms to thrive
under the same conditions and the chemical compounds they release makes the interaction between
them a subject of great importance to maintain the safety of stored maize. This is the first work
