    Emergency preparedness and management at the University of L’aquila (Central Italy) and the role of students’ associations in the April 6th 2009 earthquake

    none3noOn April 6th 2009 an earthquake of Mw=6.3 hit the historical downtown of L’Aquila and its hinterland causing more than 300 fatalities and severe damage to private and public buildings. At the time, the University of L’Aquila represented a major source of employment and income for the city. The earthquake impacted both the facilities and the administrative, financial and patrimonial activities of the university, bringing into the open the tendency – widespread in Italy – to rely on adaptive tactics rather than on strategic pre-disaster plans. This paper investigates the university’s emergency preparedness and response capability and the strategies adopted to restore the education activities as well as avoid students migration to other universities. In addition, emphasis is placed on the role played by Student Associations in pre and post-disaster phases, and how students perceived the activities performed by these associations. Methods: To achieve this goal, it was undertaken: i) qualitative evaluation to assess the impact of earthquake on services and facilities of the university, the emergency preparedness and the measures adopted to face the emergency, ii) survey on the role played by Student Associations, both in emergency preparedness and response, according to students’ perception; iii) quantitative analysis to measure changes in the enrollment trend after the earthquake, and how university policies could curb students’ migration. Results: The policies adopted by the University allowed to diminish students’ migration; however, the measures taken by the university were based on an ad hoc plan as no emergency and continuity plans were prepared in advance. Similarly Student Associations got involved more in restoration activities than in emergency preparedness and risk awareness promotion. Discussion: Greater awareness and involvement are essential at each level (administrators, faculties, students) to plan in advance for an adverse scenario and to make important steps forward in understanding and embracing a culture of safety.     Defining the Characteristics of Story Production of Autistic Children: A Multilevel Analysis

    Several studies suggest that a valuable tool to examine linguistic skills in communication disorders is offered by procedures of narrative discourse assessment. Following this line of research, we present an exploratory study aimed to investigate storytelling abilities of autistic children to better define the characteristics of their story production. Participants included 41 autistic children and 41 children with typical development aged between 7.02 and 11.03 years matched on age, gender, level of formal education, intelligence quotient, working memory, attention skills, theory of mind, and phonological short-term memory. Narrative production was assessed by analysing the language samples obtained through the “Nest Story” description task. A multilevel analysis including micro- and macro-linguistic variables was adopted for narrative assessment. Group differences emerged on both micro- and macro-linguistic dimensions: autistic children produced narratives with more phonological errors and semantic paraphasias (microlinguistic variables) as well as more errors of global coherence and a fewer number of visible events and inferred events (macrolinguistic variables) than the control group.This study shows that even autistic children with adequate cognitive skills display several limitations in their narrative competence and that such weaknesses affect both micro- and macrolinguistic aspects of story production

    Time and Narrative: An Investigation of Storytelling Abilities in Children With Autism Spectrum Disorder

    This study analyzed the relation between mental time travel (MTT) and the ability to produce a storytelling focusing on global coherence, which is one of the most notable characteristics of narrative discourse. As global coherence is strictly tied to the temporal sequence of the events narrated in a story, we hypothesized that the construction of coherent narratives would rely on the ability to mentally navigate in time. To test such a hypothesis, we investigated the relation between one component of MTT—namely, episodic future thinking (EFT)—and narrative production skills by comparing the narratives uttered by 66 children with high-functioning autism spectrum disorder (ASD) with those produced by 66 children with typical development. EFT was assessed by administering a task with minimal narrative demands, whereas storytelling production skills were assessed by administering two narrative production tasks that required children to generate future or past episodes with respect to the target stimuli. The results showed that EFT skills were impaired only in a subgroup of children with ASD and that such subgroup performed significantly worse on the narrative production task than ASD participants with high EFT skills and participants with typical development. The practical and theoretical implications of these findings are discussed

    The Italian version of the Juvenile Arthritis Multidimensional Assessment Report (JAMAR)

    The Juvenile Arthritis Multidimensional Assessment Report (JAMAR) is a new parent/patient reported outcome measure that enables a thorough assessment of the disease status in children with juvenile idiopathic arthritis (JIA). We report the results of the cross-cultural adaptation and validation of the parent and patient versions of the JAMAR in the Italian language.The reading comprehension of the questionnaire was tested in 10 JIA parents and patients. Each participating centre was asked to collect demographic, clinical data and the JAMAR in 100 consecutive JIA patients or all consecutive patients seen in a 6-month period and to administer the JAMAR to 100 healthy children and their parents.The statistical validation phase explored descriptive statistics and the psychometric issues of the JAMAR: the 3 Likert assumptions, floor/ceiling effects, internal consistency, Cronbach's alpha, interscale correlations, test-retest reliability, and construct validity (convergent and discriminant validity).A total of 1296 JIA patients (7.2% systemic, 59.5% oligoarticular, 21.4% RF negative polyarthritis, 11.9% other categories) and 100 healthy children, were enrolled in 18 centres. The JAMAR components discriminated well healthy subjects from JIA patients except for the Health Related Quality of Life (HRQoL) Psychosocial Health (PsH) subscales. All JAMAR components revealed good psychometric performances.In conclusion, the Italian version of the JAMAR is a valid tool for the assessment of children with JIA and is suitable for use both in routine clinical practice and clinical research

    Disease status, reasons for discontinuation and adverse events in 1038 Italian children with juvenile idiopathic arthritis treated with etanercept

    Background: Data from routine clinical practice are needed to further define the efficacy and safety of biologic medications in children with juvenile idiopathic arthritis (JIA). The aim of this analysis was to investigate the disease status, reasons for discontinuation and adverse events in Italian JIA patients treated with etanercept (ETN). Methods: In 2013, all centers of the Italian Pediatric Rheumatology Study Group were asked to make a census of patients given ETN after January 2000. Patients were classified in three groups: group 1 = patients still taking ETN; group 2 = patients discontinued from ETN for any reasons; group 3 = patients lost to follow-up while receiving ETN. All three groups received a retrospective assessment; patients in group 1 also underwent a cross-sectional assessment. Results: 1038 patients were enrolled by 23 centers: 422 (40.7%) were in group 1, 462 (44.5%) in group 2, and 154 (14.8%) in group 3. Median duration of ETN therapy was 2.5 years. At cross-sectional assessment, 41.8% to 48.6% of patients in group 1 met formal criteria for inactive disease, whereas 52.4% of patients in group 2 and 55.8% of patients in group 3 were judged in clinical remission by their caring physician at last visit. A relatively greater proportion of patients with systemic arthritis were discontinued or lost to follow-up. Parent evaluations at cross-sectional visit in group 1 showed that 52.4% of patients had normal physical function, very few had impairment in quality of life, 51.2% had no pain, 76% had no morning stiffness, and 82.7% of parents were satisfied with their child's illness outcome. Clinically significant adverse events were reported for 27.8% of patients and ETN was discontinued for side effects in 9.5%. The most common adverse events were new onset or recurrent uveitis (10.2%), infections (6.6%), injection site reactions (4.4%), and neuropsychiatric (3.1%), gastrointestinal (2.4%), and hematological disorders (2.1%). Ten patients developed an inflammatory bowel disease and 2 had a malignancy. One patient died of a fulminant streptococcal sepsis. Conclusions: Around half of the patients achieved complete disease quiescence under treatment with ETN. The medication was overall well tolerated, as only one quarter of patients experienced clinically significant adverse events and less than 10% had treatment discontinued for toxicity

    Mental Time Travel and language evolution: a narrative account of the origins of human communication

    In this paper we propose a narrative account for the origin of language. Such a proposal is based on two assumptions. The first is conceptual and concerns the idea that the distinctive feature of human language (what sets it apart from other forms of animal communication) has to be traced to its inherently narrative character. The second assumption is methodological and connected to the idea that the study of language origin is closely related to the analysis of the cognitive systems at the base of narrative. Research on narrative abilities of subjects with Autism Spectrum Disorder has shown that storytelling requires the capability to link events causally connected to one another, and especially events which are remote from one another on the temporal axis of a story. Based on this research, we hypothesize that an important cognitive device involved in narrative is Mental Time Travel (MTT), that is, the system that allows humans to project themselves into the past and future. We show that such a system is present (to a greater or lesser extent) even in non-human animals. By virtue of this, we argue that MTT is independent of language and that it may be considered a cognitive precursor for the origin of language. Specifically, we propose that MTT allowed our ancestors to develop a form of pantomimic communication that might be considered as the foundation of the narrative origin of language

    Individuals with Autism spectrum disorders (ASD) have difficulties in the recollection of past experiences (Episodic Memory). Accumulating evidence suggests that they might have also difficulties in the ability to imagine potential future scenarios (Episodic Future Thinking, EFT) and in narrative generation skills. This investigation aimed to determine 1) whether impairments of EFT can be identified in a large cohort of children with high functioning ASD using a task with minimal narrative demands; and 2) if such impairments are related to the ability to generate a narrative's scenario. 77 children with high-functioning ASD and 77 children with typical development were recruited for the study. The two groups were balanced for age, level of formal education, and IQ. EFT was assessed by administering a task with minimal narrative demands, whereas narrative generation skills were assessed with three tasks requiring children to generate past, middle or future episodes in a narrative discourse. With respect to control participants, a subgroup of children with ASD had impaired EFT skills and also showed significant impairments in the ability to generate adequate narratives. On the contrary, participants with spared EFT had normal performance on the narrative generation task. Interestingly, EFT skills predicted narrative generation abilities in both groups. The results of this study support the hypothesis that EFT may be impaired in some but not all children with ASD and of a relation between difficulties with EFT and impairments in the process of narrative generation. The assessment of EFT should employ tasks that do not require narrative production, as children with impaired EFT may also have reduced narrative skills

    Self-projection in middle childhood: a study on the relationship between theory of mind and episodic future thinking

    Growing evidence suggests that theory of mind (ToM) and episodic future thinking (EFT) are closely related at both brain and functional level. This study explored the relationship between ToM and EFT in 96 Italian-speaking children with typical development aged between 8 and 10.11 using a behavioral design. ToM was assessed through an emotional facial expression recognition task. EFT was assessed with a task where participants were required to project themselves forward in time by anticipating future states of the self; this resulted in two scores: a nonverbal measure and a verbal explanation measure. Results showed that the participants’ performance on the task assessing ToM correlated with and predicted the nonverbal measure of the EFT task. These findings are discussed in the light of theories suggesting that each of these abilities is governed by a common system devoted to self-projectio
