    Some MIMO applications in cognitive radio networks

    In the last decade, the wireless communication technology has witnessed a rapid development, which led to a rapid growth in wireless applications and services. However, the radio spectrum resources scarcity resulting from using the traditional methods of fixed spectrum resources allocation has potential constraints on this wireless services rapid growth. Consequently, cognitive radio has been emerged as a possible solution for alleviating this spectrum scarcity problem by employing dynamic resource allocation strategies in order to utilize the available spectrum in a more efficient way so that finding opportunities for new wireless application services could be achieved. In cognitive radio networks, the radio spectrum resources utilization is improved by allowing unlicensed users, known as secondary users, to share the spectrum with licensed users, known as primary users, as long as this sharing do not induce harmful interference on the primary users, which completely entitled to utilize the spectrum. Motivated by MIMO techniques that have been used in practical systems as a means for high data rate transmission and a source for spatial diversity, and by its ease implementation with OFDM, different issues in multi-user MIMO (MU-MIMO) in both the uplink and downlink in the context of cognitive radio are studied in this thesis. More specifically, in the first thrust of this thesis, the spectrum spatial holes which could exist in an uplink MU-MIMO cell as a result of the possible free spatial dimensions resulted from the sparse activity of the primary users is studied; a modified sensing algorithm for these spectrum spatial holes that exploit both the block structure of the OFDM signals and the correlation of their activity states along time are proposed. The second thrust is concerned with cognitive radio relaying in the physical layer where the cognitive radio base station (CBS) relays the PU signal while transmitting its own signals to its SUs. We define secondary users with different priorities (different quality of service requirements); the different levels of priority for SUs are achieved by a newly proposed simple linear scheme based on zero forcing called Hierarchal Priority Zero Forcing scheme HPZF

    Association of Blood Levels of Vitamin D and Its Binding Protein with Clinical Phenotypes of Multiple Sclerosis.

    Low vitamin D levels may synergize with changing levels of the vitamin D binding protein (DBP) to precipitate in the development and clinical progression of multiple sclerosis (MS). In this study, this hypothesis was explored in groups of Kuwaiti healthy controls and patients with different clinical phenotypes of MS. Fasting serum concentrations of 25-hydroxyvitamin D [25(OH)D] and DBP were measured in 146 healthy controls and 195 patients with MS. The latter were classified according to the duration, type, and onset of the disease and the mode of treatment. Factors such as relapse/remitting, and the use of nutritional supplements were also considered. The DBP levels were significantly lower in the patients than in the controls. This was more evident in newly diagnosed drug-naïve patients than in those patients with more established MS. MS status and severity were negatively impacted by concurrently low levels of 25(OH)D and DBP. This was most clearly expressed in drug-naïve patients and in those with a disease in relapse. It was also established that the 25(OH)D level had a significant positive correlation with the duration of the disease. Lower levels of 25(OH)D and DBP appear to have a synergistic effect on MS status. This was most clearly demonstrated in patients who were newly diagnosed (drug-naïve) and in those patients who were in relapse.This research was funded by Kuwait University Research Administration; grant number MM 03/09 and the APC was funded by Suhail Al-Shammri

    Effectiveness of a national quality improvement programme to improve survival after emergency abdominal surgery (EPOCH): a stepped-wedge cluster-randomised trial

    Background: Emergency abdominal surgery is associated with poor patient outcomes. We studied the effectiveness of a national quality improvement (QI) programme to implement a care pathway to improve survival for these patients. Methods: We did a stepped-wedge cluster-randomised trial of patients aged 40 years or older undergoing emergency open major abdominal surgery. Eligible UK National Health Service (NHS) hospitals (those that had an emergency general surgical service, a substantial volume of emergency abdominal surgery cases, and contributed data to the National Emergency Laparotomy Audit) were organised into 15 geographical clusters and commenced the QI programme in a random order, based on a computer-generated random sequence, over an 85-week period with one geographical cluster commencing the intervention every 5 weeks from the second to the 16th time period. Patients were masked to the study group, but it was not possible to mask hospital staff or investigators. The primary outcome measure was mortality within 90 days of surgery. Analyses were done on an intention-to-treat basis. This study is registered with the ISRCTN registry, number ISRCTN80682973. Findings: Treatment took place between March 3, 2014, and Oct 19, 2015. 22 754 patients were assessed for elegibility. Of 15 873 eligible patients from 93 NHS hospitals, primary outcome data were analysed for 8482 patients in the usual care group and 7374 in the QI group. Eight patients in the usual care group and nine patients in the QI group were not included in the analysis because of missing primary outcome data. The primary outcome of 90-day mortality occurred in 1210 (16%) patients in the QI group compared with 1393 (16%) patients in the usual care group (HR 1·11, 0·96–1·28). Interpretation: No survival benefit was observed from this QI programme to implement a care pathway for patients undergoing emergency abdominal surgery. Future QI programmes should ensure that teams have both the time and resources needed to improve patient care.
