Therapeutic Response After Immunosuppressive Drug Prescription in Non-infectious Uveitis: A Survival Analysis
Immunosuppressive drugs; Response to therapyMedicaments immunosupressors; Resposta al tractamentMedicamentos inmunosupresores; Respuesta al tratamientoIntroduction
To identify factors affecting the response rate to immunosuppressive drugs (ISDs) in patients with non-infectious uveitis (NIU).
Methods
This longitudinal retrospective cohort study included patients from the Hospital Clinico San Carlos Uveitis Clinic diagnosed with NIU from 1992 to 2016. Subjects were followed up from ISD prescription until the achievement of good therapeutic response (GTR), ISD treatment change, or up to 12 months. GTR was defined as the complete resolution of the eye inflammatory manifestations with a corticosteroid dose ≤ 10 or ≤ 5 mg per day of prednisone or equivalent (GTR10 and GTR5, respectively) maintained for at least 28 days. Kaplan–Meier curves were estimated for GTR. Demographic, clinical, and treatment-related factors were analyzed using Cox robust regression.
Results
A total of 73 patients (100 episodes of ISD prescription) were analyzed. In 44 and 41 episodes, GTR10 and GTR5 were achieved, respectively. A lower hazard for both GTRs was associated with uveitic macular edema at prescription and with a higher “highest oral corticosteroid dose prescribed in the year before ISD prescription”. GTR10 was higher if cyclosporine was prescribed (compared to other ISDs), and if a higher number of ISDs had been previously prescribed. GTR5 hazard was lower for patients with posterior uveitis or if the ISDs were prescribed before 2008, and higher if periocular corticosteroids had been administered before ISD prescription, or if the duration of the posterior segment activity was shorter.
Conclusions
Factors associated with GTR to ISDs may help to identify patients with NIUs who could benefit from a thorough follow-up.This work was supported by the Instituto de Salud Carlos III, Ministry of Health, Madrid, Spain [ICI19/00020; PI20/01221; RD21/002/0001].
mercado para posicionar la empresa y que el impacto en la sociedad sí es significativo.According to Casas (2020) poverty and inequality are attributes which are closely linked;
since the existence of poverty is a factor of the existence of inequality. In Peru low-income
people have fewer possibilities of overcoming the poverty gaps, since they do not have
inequality of opportunities. This becomes more tragic in the case of women living in poverty or
extreme poverty with children under three years of age; since, having the family burden, the few
job opportunities that she could obtain to generate income are subtracted.
This situation generates two big problems: First, the children of the mothers will not have
access to quality education, decent health services and a balanced diet; and, the mother will not
be able to develop personally or meet her own goals, generating personal frustration.
Based on the above, this thesis proposes to increase the formal job offer by creating a
company that can be installed in an area close to the mothers' homes; whose job profiles have
operational activities that do not require further instruction; and that provides facilities for the
care of their minor children.
For this reason, the creation of the company Paykuna, a Quechua word used to refer to
women in the third person, is being considered; which has as its core business the breeding,
transformation and commercialization of guinea pigs in various presentations (whole guinea pig,
chopped guinea pig, breaded guinea pig and seasoned guinea pig), in the district of Pucusana -
Lima.
Additionally, the proposal is aligned with Human Development Goals (SDGs) 8 and 10
since it impacts the generation of formal employment for mothers in poverty and increases
income and access to health services.
Finally, in order to define the business model, differents hypotheses have been
formulated which result in the desirability of the presentations; with an approximate demand of
4,621,231 people in Metropolitan Lima. Likewise, at the feasibility level, an IRR of 68.17%,
WACC of 10.878% has been identified; a net present value (FCE) of S/3'023,738.3; and a
current value (FCF) of S/3'052,732.4. On the other hand, the result of the project shows a social
NPV of S/ 3.3 MM equivalent to USD 885,635, therefore it can be determined that there is a
market to position the company and that the impact on society is significant
High relative abundance of firmicutes and increased TNF-α levels correlate with obesity in children
Objective. To investigate the correlation among pro- or anti-inflammatory cytokines and the two main gut microbiota phyla in obese children. Materials and methods. Anthropometric data were obtained from 890 children under 14 years old to determine the degree of obesity. Serum cytokine concentration was measured by ELISA. Relative abundance of gut microbiota in feces was evaluated by quantitative RealTime PCR assays. Results. Anthropometric and biochemical parameters were statistically higher in overweigth/ obese children (OW/O) than in lean (NW), Increased TNF-α levels were found in obese children that also have a high relative abundance of Firmicutes. Conclusions. Obese children have a high relative abundance of Firmicutes that correlates with increased levels of TNF-α. This is the first study that shows a relation between Firmicute abundance and TNF-α serum concentration in obese children

Multiomics integrative analysis identifies APOE allele-specific blood biomarkers associated to Alzheimer's disease etiopathogenesis
Alzheimer’s disease (AD) is the most common form of dementia, currently affecting 35 million people worldwide. Apolipoprotein E (APOE) ε4 allele is the major risk factor for sporadic, late-onset AD (LOAD), which comprises over 95% of AD cases, increasing the risk of AD 4-12 fold. Despite this, the role of APOE in AD pathogenesis is still a mystery. Aiming for a better understanding of APOE-specific effects, the ADAPTED consortium analysed and integrated publicly available data of multiple OMICS technologies from both plasma and brain stratified by APOE haplotype (APOE2, APOE3 and APOE4). Combining genome-wide association studies (GWAS) with differential mRNA and protein expression analyses and single-nuclei transcriptomics, we identified genes and pathways contributing to AD in both APOE dependent and independent fashion. Interestingly, we characterised a set of biomarkers showing plasma and brain consistent protein profiles and opposite trends in APOE2 and APOE4 AD cases that could constitute screening tools for a disease that lacks specific blood biomarkers. Beside the identification of APOE-specific signatures, our findings advocate that this novel approach, based on the concordance across OMIC layers and tissues, is an effective strategy for overcoming the limitations of often underpowered single-OMICS studies
