The goal of this article is to present the problem of the sale of products with random content. Even though such products have been available on the market for many years it was not until recently that they began to raise doubts and controversies. The reason for this change lies mainly in the reactions to the solutions implemented in video games by their producers. More and more often they offer the players so-called loot boxes, i.e. products with randomly generated content which can be used within the game. The phenomenon grew to such a scale that in some countries governments and authorities supervising the gambling sector took interest in it. Similar interest is currently being generated in various fields of science, especially psychology. In this article, basic concepts needed to properly understand this phenomenon are defined. The producers’ motives for the ‘random content’ model of distributionand the reasons behind the legislators’ interest in it are also presented. The work also discusses the similarities between goods with random content and gambling. Presented are both psychological and legal aspects (with regards to laws regulating the gambling industryin selected countries). Reflections on this matter are based mostly on the review of current literature. Podobne zainteresowanie zagadnienie to wywołuje obecnie w różnych dziedzinach nauki, zwłaszcza psychologii. W niniejszym artykule zdefiniowane zostały pojęcia niezbędne do prawidłowego zrozumienia wskazanego zjawiska.
Top-down machine learning of coarse-grained protein force-fields
Developing accurate and efficient coarse-grained representations of proteins
is crucial for understanding their folding, function, and interactions over
extended timescales. Our methodology involves simulating proteins with
molecular dynamics and utilizing the resulting trajectories to train a neural
network potential through differentiable trajectory reweighting. Remarkably,
this method requires only the native conformation of proteins, eliminating the
need for labeled data derived from extensive simulations or memory-intensive
end-to-end differentiable simulations. Once trained, the model can be employed
to run parallel molecular dynamics simulations and sample folding events for
proteins both within and beyond the training distribution, showcasing its
extrapolation capabilities. By applying Markov State Models, native-like
conformations of the simulated proteins can be predicted from the
coarse-grained simulations. Owing to its theoretical transferability and
ability to use solely experimental static structures as training data, we
anticipate that this approach will prove advantageous for developing new
protein force fields and further advancing the study of protein dynamics,
folding, and interactions
Age dependent treatment response to the Carriere ® Motion 3D ™ appliance for the correction of Class II malocclusion
Introduction and Purpose
Class II malocclusion is a common anomaly. Both adults and adolescents with Class II malocclusion can be treated. One of the methods for correction is Carriere ® Motion 3D ™ appliance (CMA).
The aim was to evaluate and compare the rate of tooth movement depending on age during treatment with Carriere ® Motion 3D ™ appliance (CMA).
Material and method
The retrospective study investigated medical records of 28 adult and 27 adolescent subjects who were treated with the Carriere ® Motion 3D ™ distalizer to correct Class II malocclusion. Time (in days) needed to achieve 1 mm correction was calculated for each patient, based on the quotient of days needed for Class II correction and the distance required to achieve Class I canine relationship. The difference between groups was analyzed using the Mann-Whitney-U test.
Results
Phase I treatment with CMA lasted on average 5,67 ± 2,01 months for adults and 4,67 ± 1,48 months for teenagers. The mean time to achieve 1 mm correction was 36,77 ± 11,01 days in the adult group and 36,59 ± 26,17 days in the adolescent group. The results showed no statistically significant difference between the two groups.
Conclusions
CMA can be applied to efficiently treat Class II malocclusion in adolescent as well as adult patients. No difference in age related tooth movement using the CMA was found
Synchronous gastric and rectal tumour - case description
Postęp w metodach diagnostycznych przyczynił się do zwiększenia wykrywalności guzów synchronicznych
przewodu pokarmowego.
Autorzy przedstawiają przypadek 50-letniego chorego z synchronicznym guzem żołądka i odbytnicy, u którego jednocześnie wykonano resekcję żołądka metodą Reichel-Polya, przednią resekcję odbytnicy i usunięto 2 ogniska przerzutowe w obu płatach wątroby. The continuous progress in cancer diagnosis has increased the incidence of synchronous malignancy detection
within the digestive system. According to the literature, the frequency of their occurrence ranges from
5 to 13% and almost doubles in the patients with a reported oncogenic pedigree. There is still no properly
defined treatment algorithm in this group of patients. We present the case of a 50-year-old male patient with
synchronous gastric and rectal cancers, in which B II gastrectomy and an anterior rectal resection together
with an excision of two metastases of both lobes of the liver were performed simultaneously
An investigation of structural stability in protein-ligand complexes reveals the balance between order and disorder
The predominant view in structure-based drug design is that small-molecule ligands, once bound to their target structures, display a well-defined binding mode. However, structural stability (robustness) is not necessary for thermodynamic stability (binding affinity). In fact, it entails an entropic penalty that counters complex formation. Surprisingly, little is known about the causes, consequences and real degree of robustness of protein-ligand complexes. Since hydrogen bonds have been described as essential for structural stability, here we investigate 469 such interactions across two diverse structure sets, comprising of 79 drug-like and 27 fragment ligands, respectively. Completely constricted protein-ligand complexes are rare and may fulfill a functional role. Most complexes balance order and disorder by combining a single anchoring point with looser regions. 25% do not contain any robust hydrogen bond and may form loose structures. Structural stability analysis reveals a hidden layer of complexity in protein-ligand complexes that should be considered in ligand design
What might cause pain in the thyroid gland? Report of a patient with subacute thyroiditis of atypical presentation
Podostre ziarniniakowe zapalenie tarczycy, zwane inaczej chorobą de Quervaina lub bolesnym podostrym zapaleniem tarczycy, jest najczęstszą
patologią gruczołu tarczowego odpowiedzialną za występowanie bolesności okolicy szyi. Inne przyczyny bólu w tej lokalizacji
muszą zostać uwzględnione w diagnostyce różnicowej, zwłaszcza jeśli objawy występujące u pacjenta są niejednoznaczne bądź mylące.
Autorzy przedstawili przypadek 39-letniej kobiety, u której rozpoznano podostre ziarniakowe zapalenie tarczycy, którego obraz kliniczny,
Mimo zastosowanego leczenia objawy u pacjentki stopniowo narastały, a ból zaczął znacząco
upośledzać jej funkcjonowanie. Ostatecznie pacjentka przebyła zabieg całkowitego wycięcia tarczycy. W rezultacie objawy ustąpiły, jednak
obraz makroskopowy preparatu operacyjnego gruczołu tarczowego nasuwał podejrzenie procesu nowotworowego. Niemniej jednak
badanie histopatologiczne pobranego materiału potwierdziło wstępne rozpoznanie choroby de Quervaina, zatem poprzednie wyniki
badań nie pozostawały w zgodzie z tą diagnozą.
Przypadek ten potwierdza możliwość atypowego przebiegu podostrego ziarniniakowego zapalenia tarczycy. Dodatkowo wskazuje na
możliwość rozważenia leczenia operacyjnego u pacjentów z ciężkim, przetrwałym, upośledzającym funkcjonowanie bólem gruczołu
tarczowego w przebiegu podostrego ziarniniakowego zapalenia tarczycy. (Pol J Endocrinol 2012; 63 (2): 138–142)Subacute granulomatous thyroiditis (SAT), also known as de Quervain’s thyroiditis or painful subacute thyroiditis, is the commonest
thyroid condition responsible for neck tenderness. Other causes of pain in the thyroid gland should be taken into consideration during
differential diagnosis, especially when a patient presents with misleading or equivocal signs and symptoms.
We report the case of a 39 year-old woman diagnosed as having SAT whose clinical, biochemical and radiological presentation varied significantly
from the common SAT manifestation. A tentative diagnosis of SAT was made based on the presented symptoms, ultrasonography
and fine-needle biopsy results. However, biochemical analysis suggested neither inflammatory process nor the presence of thyrotoxicosis.
Moreover, technetium scan of the thyroid revealed normal uptake of the isotope and there was neither clinical nor ultasonographic
response for corticosteroids. The patient’s symptoms, despite being prescribed typical treatment, gradually deteriorated and the pain became
increasingly debilitating. Eventually, the patient underwent total thyroidectomy. As a result, she has become free of symptoms, but the
macroscopic picture of thyroid gland, noted during the operation, gave a suspicion of neoplastic process. Nevertheless, histological study
of flow samples confirmed the tentative diagnosis of de Quervain’s thyroiditis, despite all previous findings that were not suggestive of it.
This report confirms the likelihood that SAT can present atypically. Additionally, it indicates that surgical treatment may be considered in
patients with severe, debilitating, persistent thyroid gland pain connected with SAT clinical course. (Pol J Endocrinol 2012; 63 (2): 138–142
Machine Learning Small Molecule Properties in Drug Discovery
Machine learning (ML) is a promising approach for predicting small molecule
properties in drug discovery. Here, we provide a comprehensive overview of
various ML methods introduced for this purpose in recent years. We review a
wide range of properties, including binding affinities, solubility, and ADMET
(Absorption, Distribution, Metabolism, Excretion, and Toxicity). We discuss
existing popular datasets and molecular descriptors and embeddings, such as
chemical fingerprints and graph-based neural networks. We highlight also
challenges of predicting and optimizing multiple properties during hit-to-lead
and lead optimization stages of drug discovery and explore briefly possible
multi-objective optimization techniques that can be used to balance diverse
properties while optimizing lead candidates. Finally, techniques to provide an
understanding of model predictions, especially for critical decision-making in
drug discovery are assessed. Overall, this review provides insights into the
landscape of ML models for small molecule property predictions in drug
discovery. So far, there are multiple diverse approaches, but their
performances are often comparable. Neural networks, while more flexible, do not
always outperform simpler models. This shows that the availability of
high-quality training data remains crucial for training accurate models and
there is a need for standardized benchmarks, additional performance metrics,
and best practices to enable richer comparisons between the different
techniques and models that can shed a better light on the differences between
the many techniques.Comment: 46 pages, 1 figur
