Towards near 100% renewable power systems: Improving the role of distributed energy resources using optimization models
The envisioned near 100 % renewable Power Systems, crucial in attaining the sustainability goals aspired by society, will call for the active and multifaceted participation of all the actors involved in the energy systems.
Time-varying renewable energy systems (vRES), such as solar photovoltaic (PV) and wind, will play a decisive role in meeting the ambitious renewable targets. This is due to the large availability of natural resources and the rapid decrease in investment costs observed in the last two decades. In fact, most of the scenarios to achieve near 100% RES in Europe strongly rely on these two energy sources. However, the high temporal and spatial variability of the power generated by these technologies represents a challenge for preserving the high-security standards of supply, quality of service, and the robustness of current power systems, especially with the foreseen contributions from vRES.
With an emphasis on the vital role these renewable technologies play in this process, this work aims to develop new methods and tools that may assist different players in different stages of this transition. The three leading contributions are:
1. A Multiyear Expansion-Planning Optimization Method (MEPOM) to be used in the planning processes carried out by system operators and governmental entities.
2. An Optimal Design and Sizing of Hybrid Power Plants (OptHy) decision-support tool to be used in accessing investment decisions and other managing actions led by renewable power plant owners and investors.
3. A Decision-aid Algorithm for Market Participation and Optimal Bidding Strategy (OptiBID) that market agents may adopt to operate and value their renewable energy assets in the electricity markets
Evaluation of the neuroprotective effect of medicinal plants

traditional medicines throughout the centuries. The growing interest of the
scientific community in phytopharmaceuticals has resulted in a significant
number of research efforts towards in understanding their effect in the
potential treatment of neurodegenerative disorders. An approach to develop
new ways of treatment concerns facing the context of
neuroinflammation/oxidative stress on these group of diseases. This
regards to a pre-pathological condition characterized by a chronic and
systemic oxidative stress associated to a mild-subclinical chronic
inflammation which causes neuronal cell loss. Thus, by reverting and/or
attenuating this state, natural products can prevent and treat
neurodegenerative diseases through modulation of microglia, the cells from
brain innate system, inhibiting or reducing its inflammatory or oxidative
status.
The main goal of this work is to evaluate the pharmacological
potential of plants used in Traditional Chinese Medicine as Hemerocallis
citrina and Dendrobium nobile extracts, as well as Panax ginseng compounds
and alkaloids from Nelumbo nucifera, regarding to its anti-inflammatory and
antioxidant properties. For that, the scavenging activities on DPPH and NO
radicals as well as the anti-acetylcholinesterase activity were evaluated. In
silico methods were carried out to perform structure-activity relationships
of pure compounds. Moreover, using a BV2 murine microglia cell line, the
following parameters were evaluated: i) their citoxicity; ii) their antioxidant
potential evaluating their ability in reverting oxidative insults and iii) their
anti-inflammatory potential evaluating the capacity in decreasing proinflammatory
compounds after inflammatory stimulation (bacterial
lipopolysaccharide) . Finally, phytochemical analysis was performed for the
extracts in order to characterize them. Although Dendrobium nobile extract and Nelumbo nucifera alkaloids
have DPPH and NO radical-scavenging activity, as well as weak antiacetylcholinesterase
activity, their toxicity in the BV2 cells did not enable
their study in subsequent assays. Moreover, Panax ginseng compounds
have been shown to have antioxidant and anti-inflammatory properties,
mostly through indirect effects, probably through regulation of genetic
machinery. In addition, our studies and others indicated that, in most
cases, the sugar groups may not be necessary for the biological and
pharmaceutical activity. In the end, all planned assays were carried out with
extracts of Hemerocallis citrina. Its high content of phenolic and flavonoid
acids such as caffeoylquinic acids, quercetin and apigenin derivatives can
contribute to its antioxidant and anti-inflammatory activity. This last activity
seems to be related to a decrease in the expression of pro-inflammatory
cytokines and in the nitric oxide-producing enzyme, iNOS. Eventually, these
extracts may have a therapeutic potential in living organisms in reversing
neuroinflammation / oxidative stress and in the treatment of
neurodegenerative diseases.Tendo em conta o seu potencial terapêutico, os produtos naturais
têm sido usados na medicinal tradicional oriental ao longo dos tempos. O
crescente interesse pela comunidade científica no uso de fitofarmacêuticos
tem levado a um aumento significativo de investigação no que concerne a
elucidar o seu efeito terapêutico no potencial tratamento das doenças
neurodegenerativas. Uma abordagem amplamente abordada e recente
para o seu eventual tratamento consiste em confrontar a problemática da
neuroinflamação/stress oxidativo no contexto da neurodegeneração. Isso
diz respeito a uma condição pré-patológica caraterizada por um stress
oxidativo crónico e sistemático associado a um estado de inflamação
crónica que causa a perda neuronal. Assim, ao reverter e/ou atenuar este
estado, os produtos naturais podem prevenir e tratar as doenças
neurodegenerativas através da modulação das células de microglia, as
células do sistema imunitário do cérebro, inibindo ou reduzindo o seu
caráter inflamatório e pró-oxidante.
Este trabalho tem como principal objetivo avaliar o potencial
farmacológico de extratos de plantas usadas na Medicina Tradicional
Chinesa: Hemerocallis citrina e Dendrobium nobile, assim como compostos
puros de Panax ginseng e alcaloides de Nelumbo nucifera, no que diz
respeito às suas propriedades anti-inflamatórias e antioxidantes. Para isso,
foi avaliada a sua capacidade de capturar radicais DPPH e de óxido nítrico,
assim como a sua atividade anti-acetilcolinesterase. Adicionalmente, foram
efetuados métodos in silico para realizar estudos de relação
estrutura/atividade de compostos puros. Em seguida, foi avaliada, na linha
celular BV2 (microglia murina): (i) a sua citoxicidade; (ii) o seu potencial
antioxidante (avaliado pela capacidade de citoproteção face a insultos
oxidativos) e (iii) o seu potencial anti-inflamatório (avaliado pela
quantificação de compostos pró-inflamatórios após o estímulo inflamatório (lipopolissacarídeo bacteriano)). Finalmente, os perfis fitoquímicos dos
extratos foram delineados com vista à sua caraterização.
Apesar de o extrato de Dendrobium nobile e os alcaloides de Nelumbo
nucifera terem demonstrado atividade quelante de radicais DPPH e NO,
assim como atividade anti-acetilcolinesterase (embora fraca), a sua elevada
toxicidade na linha celular BV2 levou-nos a abortar o seu estudo em ensaios
posteriores.
Por outro lado, os compostos de Panax ginseng revelaram ter
capacidade antioxidante e anti-inflamatória na sua maioria através de
efeitos indiretos, provavelmente por regulação da maquinaria genética. Os
nossos estudos e de outros autores indicaram, na maioria das vezes, que
os grupos açúcares poderão não ser de grande relevância para a atividade
biológica e farmacológica dos compostos.
Finalmente, todos os ensaios planeados foram efetuados com os
extratos de Hemerocallis citrina. Estes extratos apresentaram atividade
antioxidante e anti-inflamatória. O seu elevado conteúdo em ácidos
fenólicos e flavonoides como ácidos cafeoilquínicos e derivados de
quercetina e apigenina parecem contribuir para a sua atividade
antioxidante, evidenciada pelo efeito protetor na linha celular BV2 após
insulto oxidativo. Por outro lado, a atividade anti-inflamatória foi
evidenciada, também em células BV2, pelo decréscimo da expressão de
citocinas pró-inflamatórias e na enzima produtora de óxido nítrico, iNOS,
após indução de inflamação com o lipopolissacarídeo bacteriano. Os
resultados obtidos mostram que estes extratos poderão apresentar um
potencial terapêutico em organismos vivos no controlo e/ou reversão da
neuroinflamação/stress oxidativo e no tratamento das doenças
neurodegenerativas
A relação afetiva adulto-criança em creche e jardim de infância
O presente relatório de projeto de investigação reflete as aprendizagens realizadas nos contextos de creche e Jardim de infância, onde realizei os estágios, no âmbito do Mestrado em Educação Pré-Escolar, na Escola Superior de Educação do Instituto Politécnico de Setúbal. Neste projeto procurei compreender a forma como o afeto pode influenciar ou otimizar a relação entre o adulto e a criança, num contexto educativo. Este estudo teve por base a metodologia qualitativa e a abordagem investigação-ação. Foram utilizados diferentes procedimentos de recolha e análise de informação, nomeadamente a observação direta e observação participante, as notas de campo e a análise documental. Como questão de partida defini: “De que forma o afeto potencia a relação adulto-criança em Creche e Jardim de Infância?”
A experiência vivenciada no decorrer dos estágios, bem como as intervenções realizadas encontram-se descritas ao longo deste relatório. Estas intervenções, contemplaram a leitura de histórias, realização de desenhos, conversas, entre outras. Para além destas, saliento que durante os momentos de estágio existiu o cuidado de procurar responder às necessidades e interesses das crianças, sendo este aspeto descrito nas intervenções realizadas. Foram momentos de aprendizagem para mim enquanto futura educadora, sendo que também refletem possíveis melhorias a reconhecer na relação com as crianças.
Das intervenções realizadas pode-se inferir que a relação afetiva entre o adulto e a criança permitiu às crianças sentirem-se seguras e confiantes, o que as tornou mais disponíveis e motivadas para aprender, brincar e se relacionar com os outros.
Este projeto foi um momento primordial para a construção de conhecimentos teóricos e práticos, relativamente à relação afetiva, em contexto de educação de infância.This research project report reflects the learning in the context of daycare and kindergarten, where I carried out my internships as part of the Master's Degree in Preschool Education, at Escola Superior de Educação do Instituto Politécnico de Setúbal. In project I tried to understand how affection can influence or optimize the relationship between the adult and the child in an educational context. This study was based on the qualitative methodology and action research approach. Different procedures for information collection and analysis were used, namely direct and participant observation, field notes and document analysis. As a starting question I defined: "In what way does affection enhance the adult-child relationship in Daycare and Kindergarten?".
The experience lived during the internships, as well as the interventions carried out, are described throughout this report. These interventions included the reading of stories, drawing, dialogue, among others. In addition to these, I emphasize that during the internship, care was taken to try to respond to the needs and interests of the children, and this aspect is described in the interventions carried out. These were learning moments for me as a future educator, and they also reflect possible improvements to be recognized in the relationship with the children.
From the interventions made, it can be inferred that the affective relationship between the adult and the child allowed the children to feel safe and confident, which made them more available and motivated to learn, play and relate to others.
This project proved to be a primordial moment for the construction of theoretical and practical knowledge, regarding the affective relationship in the context of early childhood education
Sequencing batch airlift reactors (SBAR): a suitable technology for treatment and valorization of mineral oil wastewaters towards lipids production
Produced water (PW) and spent oil-based wastewaters are some of the largest mineral oil wastewaters produced. Due to the high toxicity of hydrocarbons, several countries set stringent discharge limits and its treatment is compulsory before discharge. In this work, biological treatment of mineral oil wastewaters coupled with the production of bacterial lipids is demonstrated in sequential batch airlift reactors (SBAR). Two SBAR (2 L working volume) were used for treatment of PW and lubricant-based wastewater (LW), inoculated with Alcanivorax borkumensis SK2 (SBARAb+PW) and Rhodococcus opacus B4 (SBARR.o+LW), respectively. A total petroleum hydrocarbon removal (TPH) efficiency up to 96% and 80% were achieved for SBARAb+PW and SBARR.o+LW, respectively. Intracellular lipids production in SBARAb+PW increased when lower TPH/N ratios and higher feast stage duration were applied (up to 0.74 g g-1 cell dry weight (CDW)), whereas in SBARR.o+LW higher lipids production was observed for higher TPH/N ratios (0.94 g g-1 in CDW). Triacylglycerols (TAG) were the main intracellular lipid accumulated in both SBARAb+PW and SBARR.o+LW operations, while wax ester (WE) production was only observed extracellularly in the SBARAb+PW.This work was supported by the Portuguese Foundation for Science

Modelação e otimização do funcionamento de uma central renovável virtual
Tese de mestrado integrado em Engenharia da Energia e Ambiente, apresentada à Universidade de Lisboa, através da Faculdade de Ciências, 2016O vento, a irradiação solar e a pluviosidade são recursos energéticos endógenos que possuem uma elevada variabilidade espacial e temporal e características de não controlabilidade e difícil previsão. Todas estas propriedades conferem variabilidade à energia elétrica obtida a partir destes recursos renováveis, o que introduz complexidade na gestão dos sistemas electroprodutores, bem como das redes elétricas que os servem.
A crescente penetração de fontes de energia renovável com elevada variabilidade no sistema electroprodutor acentua a necessidade de desenvolver modelos de simulação da produção renovável que, além de facilmente integráveis nos instrumentos de gestão do sistema electroprodutor, representem fielmente o comportamento das respetivas centrais e, quando aplicável, do resultado da agregação de várias dessas centrais.
O conceito de Central Renovável Virtual (CRV) surge, então, como uma possibilidade de tirar partido da sinergia entre várias Fontes de Energia Renovável (FER). A complementaridade natural da disponibilidade de alguns recursos permite diminuir as flutuações temporais da potência entregue à rede elétrica pelas centrais renováveis, quando agregados, bem como o estabelecimento de um perfil de produção diário da CRV que melhor se adapte ao perfil de consumos.
Assim, a presente dissertação tem como objetivo principal otimizar o funcionamento de uma CRV para que, mantendo o equilíbrio entre a potência produzida e a potência consumida, se maximize o lucro da mesma.The wind, sun and rainfall are renewable endogenous energy resources that are extremely variable, both in time and space, being difficult to control and predict their generation and gives rise to power fluctuations and increases the level of difficulty of the power system and energy networks operations.
The injection of increasing high levels of energy from variable renewable sources in the existing electric power system thus leads to an urging need for developing mathematical models specifically designed to easily integrate the existing power systems’ management tools. These models should represent, as faithfully as possible, the behaviour of the renewable power plants connected to the grid.
The concept of a Renewable Virtual Power Plant (RVPP) arises as an opportunity to exploit the synergies between multiple Renewable Energy Sources (RES). The natural complementarity of resource availability allows reducing the temporal fluctuations of the renewable power delivered to the grid, as well as the establishment of a daily CRV production profile best suiting the consumption profile.
Thus, the present work aims to optimize the operation of a CRV in order to, maintaining the balance between power production and power consumption, maximize de profit of the CRV
Influence of contact lens care solutions on the water content of hydrogel and silicone-hydrogel contact lenses
Purpose
The aim of this work was to study variations in water content (WC) of hydrogel and silicone-hydrogel contact lenses (CL) when preserved in different lens care systems.
Material and Method
Four silicone-hydrogel CL (Senofilcon A, Balafilcon A, Comfilcon A, Lotrafilcon B) and one conventional CL (Etafilcon A) and commercially available lens care solutions (Renu Multiplus™ and Biotrue (Bausch & Lomb), Optifree Puremoist and Aosept® Plus (CIBA Vision) were used.
Lenses were immersed in each lens care system during 12 hours and then RI was determined with the digital automated refractometer (CLR 12-70, Index Instruments, 174 Cambridge, UK). Water content was directly obtained with this instrument using RI values and the equation given in BSO ISO 10339:1997.
Measurements were performed before and after the lenses have been immersed in each care solution. Two CL of each combination (CL + solution) were tested, and five measurements per lens were obtained.
Results
The mean values of WC (in %) obtained for each lens material when CL were removed from their blisters (baseline value) was for: Etafilcon A (55.75 ± 0.52); Comfilcon A (45.84 ± 0.28); Senofilcon A (37.01 ± 0.98), Lotrafilcon B (25.53 ± 0.42) and Balafilcon A (34.61 ± 1.03).
Substantial changes and statistically significant were observed on these parameters when exposed to lens care systems depending on the CL material and the solution. Senofilcon A decreased the WC with all the solutions contrary
of lotrafilcon B where it was observed an increase in WC with all the solutions. Comfilcon A remained approximately with the same values of WC. Balafilcon A and Etafilcon A showed an irregular behavior increasing or decreasing depending on the solution.
Conclusions
The present study reveals that solutions induce changes in the CL properties studied showing the existence of interactions between CL material and lens care solution. These interactions can be essential when eye care practitioners choose the most appropriate lens care product for CL, especially when the patient present symptoms of discomfort or dryness.info:eu-repo/semantics/publishedVersio
Biological saline produced water treatment, a sustainable process towards lipids production
Oil and gas industry is responsible for the generation of large volumes of oil-contaminated
wastewaters, such as saline produced water (PW), that without proper treatment can cause
environmental contamination. Alcanivorax borkumensis SK2 is a biosurfactant producer
capable of degrading and converting hydrocarbons into bacterial lipids under nutrient limiting
conditions (e.g. nitrogen and/or oxygen). Recently, the industrial interest in biosurfactants and
bacterial lipids for biotechnological applications (e.g. bioremediation or biofuels production) is
increasing. Pursuing the interest of treating and simultaneously valorizing PW, a sequencing
batch airlift reactor (SBAR) strategy consisting of sequential cycles of feast and famine stages
was developed. A. borkumensis SK2 was used as bioreactor inoculum and the effect of cycle
duration, total petroleum hydrocarbon to nitrogen ratio (TPH/N) and dissolved oxygen (DO)
concentration (7-8 mg L-1 and 2-3 mg L-1 (famine and feast stage); and 7-8/1-2 mg L-1
(famine/feast stage)) were investigated. The system provided an efficient PW treatment,
achieving TPH removal efficiencies in a narrow range from 90 ± 2.1 to 96 ± 1.8 %. Intracellular
lipid production increased from 0.48 to 0.74 g g-1 of cellular dry weight (CDW) with the
application of higher feast stage duration and lower TPH/N ratios suggesting that nitrogen
availability is the most relevant factor to promote accumulation. Under 2-3 mg L-1 and 7-8/1-2
mg L-1 of oxygen, lipid accumulation dropped to 0.50 g g-1 of CDW. Intracellular lipid profile
changed according to the DO concentration. Triacylglycerols (TAG) and wax esters (WE) were
accumulated under maximum and limiting DO concentrations, while polyhydroxyalkanoates
(PHA) accumulation was triggered by the application of alternated DO conditions, indicating
that the type of intracellular compound can be selected by manipulating the oxygen
concentration in the SBAR. Extracellular lipid production (TAG and WE) was not significantly
affected by none of the operational conditions applied (0.06 g L-1). Moreover, the production of
a cell-bound and an extracellular glycolipid biosurfactant capable of reducing the medium
surface tension from 65 to approximately 41 mN m-1 was observed. The proposed strategy showed that biological PW treatment in a SBAR can be a sustainable process through the

Excess pneumonia and influenza hospitalizations associated with influenza epidemics in Portugal from 1998 to 2015
In Portugal there is no severe acute respiratory infection surveillance system in place.
Estimation of influenza burden has been accessed using hospital discharge database that covers the mainland Portuguese population.

Programa Nacional de Vigilância da Gripe: relatório da época 2017/2018
A vigilância da gripe em Portugal através do Programa
Nacional de Vigilância da Gripe permite
monitorizar, descrever a atividade gripal, detetar
e identificar os vírus da gripe em circulação. O
programa integra as componentes de vigilância
clínica e laboratorial. A componente clínica possibilita
o cálculo de taxas de incidência permitindo
descrever a intensidade e evolução da epidemia
no tempo. A componente virológica e laboratorial
tem por base o diagnóstico laboratorial do vírus
da gripe o que permite identificar e caraterizar os
vírus da gripe em circulação em cada inverno.
Durante o inverno 2017/2018, foi observada uma
atividade gripal de intensidade baixa a moderada.
O período epidémico ocorreu entre a semana
52/2017 (dezembro) e a semana 7/2018 (fevereiro)
e o valor mais elevado da taxa de incidência semanal
de síndrome gripal (78,8/105) ocorreu em
dezembro, na semana 52/2017.
O vírus da gripe foi detetado em co-circulação com
os outros vírus respiratórios. O vírus da gripe do
tipo B/Yamagata foi o predominante, tal como o
observado nos restantes países europeus. Em Portugal
foi identificado em 57,4% dos casos de gripe
confirmados laboratorialmente. Foram também detetados
em circulação, com menor frequência, os
vírus da gripe A(H1)pdm09, A(H3) e B/Victoria.
A caracterização genética demonstrou
que os vírus B/Yamagata pertenciam ao
clade 3 (representado pela estirpe vacinal
B/Phuket/30173/2013) e os vírus B/Victoria pertenciam
ao clade 1A (representados pela estirpe
vacinal B/Brisbane/60/2008, presente na vacina
trivalente 2017/2018). Cinco dos vírus B/Victoria
pertenciam ao novo subgrupo, que se distingue
da estirpe vacinal de 2017/2018, apresentando
uma deleção dos aminoácidos 162-163
no gene da hemaglutinina. Este novo grupo surgiu
em 2015/2016 e atualmente foi já detetado a
nível mundial. Os vírus da gripe A(H3) continuam
a apresentar uma grande diversidade genética.
Na época 2017/2018 os vírus deste subtipo
agruparam-se em 2 subgrupos genéticos:
3C.2a1b (A/England/74560298/2017) e 3C.2a2
(A/Norway/4465/2016). Os vírus A(H1)pdm09
pertenciam ao clade 6B.1, semelhantes à estirpe
vacinal A/Michigan/45/2015.
A avaliação da resistência aos antivirais inibidores
da neuraminidase, revelou uma susceptibilidade
normal dos vírus influenza A e B ao oseltamivir
e zanamivir, com excepção de um vírus do tipo
B/Yamagata que apresentou uma redução da
susceptibilidade a ambos os inibidores da neuraminidase.
Os vírus da gripe com diminuição da
susceptibilidade aos antivirais foram detetados
esporadicamente a nível europeu e mundial.
A percentagem mais elevada de casos de gripe
foi verificada nos indivíduos com idade inferior a
15 anos, no entanto devido ao número mais reduzido
de casos estudados nesta faixa etária esta
informação deverá ser analisada com cuidado. A
nível hospitalar, a maior percentagem de casos
de gripe foi também detetada no grupo das crianças
entre os 5 e os 14 anos. Foi nos doentes com
idade superior a 65 anos que se verificou a mais
elevada taxa de internamento hospitalar e em unidades
de cuidados intensivos. A febre, as cefaleias e a tosse foram os sintomas
mais frequentemente associados a casos de gripe
confirmados laboratorialmente.
Foram estudados os grupos de risco para a maior
gravidade da infeção pelo vírus da gripe: doentes
crónicos e as mulheres grávidas. Foi nos doentes
com obesidade, diabetes e com doença cardiovascular
que se observou a maior proporção de casos
de gripe confirmada laboratorialmente, seguindose
dos indivíduos com doença respiratória crónica
e imunodeficiência congénita ou adquirida. A proporção
de casos de infeção pelo vírus da gripe foi
similar nas mulheres grávidas e nas mulheres do
mesmo grupo etário não grávidas.
A vacina como a principal forma de prevenção da
gripe é fortemente recomendada para pessoas
com idade igual ou superior a 60 anos, doentes
crónicos e imunodeprimidos, grávidas e profissionais
de saúde. A vacinação foi reportada em
17,1% dos casos notificados, valor idêntico ao observado
na época 2016/2017. A deteção do vírus
da gripe ocorreu em 33,6% dos casos vacinados e
sujeitos a diagnóstico laboratorial. A confirmação
de gripe em indivíduos vacinados poderá estar
relacionada com uma moderada efetividade da
vacina antigripal.
A pesquisa de outros vírus respiratórios nos casos
de SG negativos para o vírus da gripe, veio revelar
a circulação e o envolvimento de outros agentes
virais respiratórios em casos de SG. Os vírus respiratórios
foram detetados durante todo o período
de vigilância da gripe, entre a semana 38/2017 e
a semana 7/2018. O rinovírus e o coronavírus humano
foram os mais frequentemente detetados. A
deteção de vírus respiratórios atingiu proporções
mais elevadas nas crianças até aos 4 anos e entre
os 5-14 anos de idade. As infeções por dois ou
mais agentes virais foram detetadas com baixa
frequência.
A Rede Portuguesa de Laboratórios para o Diagnóstico
da Gripe, efetuou o diagnóstico da gripe
em 13885 casos de infeção respiratória tendo sido
o vírus influenza detetado em 3722 destes casos.
Em 55% dos casos de gripe foi detetado o vírus influenza
B. Os vírus da gripe A(H3), A(H1)pdm09 e
A (não subtipados) foram detetados em 14%, 12%
e 19% dos casos de gripe, respetivamente. Em
1909 casos de infeção respiratória foram identificados
outros vírus respiratórios sendo o RSV, os
picornavírus (hRV, hEV e picornavírus) e os hMPV
os mais frequentes e em co-circulação com o vírus
da gripe. A grande maioria destes casos foi identificada
nas crianças com idade inferior a 4 anos.
Durante a época de gripe 2017/2018 observou-se
um excesso de mortalidade por todas as causas
entre as semanas 52/2017 e 9/2018 de cerca de
3,714 óbitos (15 % superior em relação ao esperado).
Este excesso foi observado em ambos os
sexos, a partir dos 65 anos de idade, em especial
acima dos 85 anos. As regiões do Norte, Centro
e Lisboa e Vale do Tejo foram aquelas em que se
observaram excessos de mortalidade mais elevado.
Durante toda a época, estimaram-se cerca
de 3.700 atribuíveis à epidemia de gripe. Noutros
países europeus foi igualmente observado um excesso
de mortalidade por todas as causas, possivelmente
atribuível à epidemia de gripe e à vaga
de frio que se fez sentir na europa durante o mês
de fevereiro.
Na época 2017/2018 participaram na vigilância da
gripe em Unidades de Cuidados Intensivos (UCI) 33
UCI pertencentes a 24 hospitais. Durante a época
foram reportados 220 casos de gripe. Verificou-se um aumento apreciável da proporção
de casos de gripe admitidos em UCI entre as semanas
51 de 2017 e 01 de 2018, em que foi atingido o
valor máximo (7,6%). A partir daí o valor decresceu,
com algumas flutuações, aproximando-se progressivamente
da linha de base até à semana 17 (0%),
onde se manteve até ao final da época.
Os vírus tipo A e B circularam em proporções
semelhantes, tendo sido subtipadas 32,5% das
amostras.
Mais de metade dos doentes (56%) tinha idade superior
a 65 anos, 88% dos quais com doença crónica,
sendo a patologia cardiovascular reportada
em 44%.
A proporção de vacinados contra a gripe sazonal
foi de 14% dos doentes, menos de metade do observado
na época anterior.
Foi prescrito oseltamivir a 92% dos doentes, 74%
necessitaram de ventilação mecânica invasiva e
8% teve suporte de oxigenação por membrana extracorporal.
O diagnóstico de gripe foi confirmado no próprio
dia da admissão em UCI, em 52 % dos casos,
sendo já conhecido previamente à admissão, em
21% dos casos.
A duração da hospitalização foi inferior a 9 dias em
cerca de metade dos doentes com alta para o domicílio
e inferior a 8 dias para cerca de metade dos
óbitos.
A taxa de letalidade foi estimada em 26%, valor semelhante
ao das três épocas anteriores.
Este sistema de vigilância da gripe sazonal em UCI
poderá ser aperfeiçoado nas próximas épocas,
reduzindo a subnotificação e melhorando o preenchimento
dos campos necessários ao estudo da
doença.
A época de vigilância da gripe 2017/2018 foi em
muitas características comparável ao descrito na
maioria dos países europeus. A situação em Portugal
destacou-se pelo início do período epidémico
mais tardio relativamente ao inverno anterior, mas
de duração igualmente prolongada. De forma semelhante
aos restantes países europeus o vírus da
gripe predominante foi do tipo B/Yamagata associado
a uma intensidade da epidemia baixa a moderada.
Verificou-se um excesso de mortalidade
por todas as causas, essencialmente no grupo dos
indivíduos mais fragilizados e com idade acima dos
85 anos. O conhecimento das características da
epidemia da gripe, do seu desenvolvimento e dos
vírus da gripe circulantes são essenciais para a implementação
de medidas de prevenção e de controlo
da doença em cada inverno.The National Influenza Surveillance Program ensures
influenza epidemiological surveillance in Portugal,
integrating clinical and laboratory surveillance
components. Clinical component enables the calculation
of Influenza like illness (ILI) incidence
rates, to describe the intensity of influenza epidemic
and evolution over time. Virological component
is based on laboratory diagnosis of influenza
viruses including detection and characterization of
influenza in circulation during each winter.
During the 2017/2018 influenza season, the flu activity
was considered of low to moderate intensity
reaching the maximum value of 78,8 ILI cases
per 100 000 inhabitants in week 52/2017. The epidemic
period occurred between weeks 52/2017
(December) and 7/2018 (February).
Influenza was detected in co circulation with other
respiratory viruses. Influenza B/Yamagata viruses
predominate, like the observed in many other European
countries. In Portugal B/Yamagata was
identified in 57,4% of the laboratory confirmed
flu cases. The influenza A (H1)pdm09,A(H3) and
B/Victoria were also detected in lower numbers.
The influenza B/Yamagata viruses belonged to
clade 3, from a different lineage compared to
the 2017/2018 recommended vaccine strain. The
B/Victoria viruses belonged to clade 1A, although
5 of these viruses were from the new variant group
that presented 2 amino acid deletions in positions
162-164 of the hemagglutinin gene. Influenza A(H3)
viruses showed an increasing genetic diversity,
belonged to 2 distinct genetic groups: 3C.2a1 and
the majority belonging to the 3C.2a2, distinct from
the vaccine strain. The influenza A(H1)pdm09 were
similar to the vaccine strain A/Michigan/45/2015.
All influenza A and B viruses assessed for antiviral
susceptibility showed a normal inhibition for
oseltamivir and zanamivir, except one influenza
B/Yamagata virus that showed a reduced susceptibility
to both neuraminidase inhibitors.
The highest proportion of influenza cases was
observed in the age group of children under 15
years, however due to small number of cases notified
this data should be looked with caution. At
hospital level was also in children, between 4 and
15 years, that were detected a higher percentage
of flu cases. Was in the elderly ( 65) that was observed
the highest hospital admission rate and
even in intensive care units.
Fever, headache, cough were the most frequent
symptoms associated with influenza laboratory
confirmed cases. In the group of patients with
obesity, diabetes, and cardiovascular disease, that
showed higher percentage of influenza cases. In
pregnant women, wasobserved a similar proportion
of influenza cases compared to non-pregnant
women of the same age group.
The flu vaccination was reported in 17,1 % of the
ILI cases, a similar coverage than in previous season.
Among these, 33,6 % was positive for influenza.
Confirmation of influenza in vaccinated
individuals may be related to moderate influenza
vaccine effectiveness in the general population
and individuals over 64 years old. The diagnosis of other respiratory viruses has
shown the circulation and involvement of other
respiratory virus in ILI cases. Respiratory viruses
were detected during entire winter between
weeks 38/2017 and 7/2018. The hRV, hCoV were
detected frequently in addition to influenza virus.
Respiratory viruses were more frequent in children
under 4 and between 5-14 years old.
The Portuguese Laboratory Network for Influenza
Diagnosis reported 13885 cases of respiratory infection
laboratory tested for influenza; from these
were confirmed 3722 influenza cases. Influenza B
was detected in 55,0% of the flu confirmed cases.
The influenza A(H3), A(H1)pdm09 were detected in
low numbers 14% and 12% of the cases, respectivly.
In 1909 cases were detected other respiratory
viruses being the RSV the predominant one, followed
by the picornavirus (hRV, hEV e picornavírus)
and the hMPV. The majority of these viruses were
detected in children under 4 years old.
During the 2017/2018 flu season the number of deaths
from "all causes" was above the confidence
upper limit to 95% from baseline between week
52/2017 and week 9/2018, corresponding to an
excess of 3.714 deaths. Excess mortality was observed
in both genders, above 65 years old, especially
above 85 years. Higher excess mortality
rates were observed at Centro, Norte and Lisboa
e Vale do Tejo regions. During the whole season
3.700 deaths associated with influenza epidemic
were estimated. This was observed during the influenza
epidemic period coincident with the period
of extreme low temperatures. Excess mortality
was also reported by other European countries.
During 2017/2018 were reported 220 influenza
confirmed cases in ICU, in higher number between
weeks 51/2017 and 01/2018. Influenza A and
B was identified in reported cases. More than a
half (56%) of the patients had more than 65 years
old and 88% had a chronic disease, being the
cardiovascular disease, the most frequent reported
in 44% of the cases. 14% of the patients were
vaccinated for influenza. The mortality rate was
26%, similar to the previous season. The influenza
surveillance in ICU could be improved in coming
seasons to reduce the sub-notifications and
to enhance the completeness of epidemiological
data collected for each patient.
The 2017/2018 influenza surveillance season was
in many features comparable to the observed in
most European countries. The situation in Portugal
was characterized by a low to moderate intensity
influenza activity and the prevalence of influenza
B/Yamagata in co circulation with A(H1)pdm09 and
A(H3) viruses. The Portuguese situation was characterized
by a later beginning of the season, with
a long epidemic period. Was observed an excess
mortality rate in the elderly aged 85 and above.
Knowledge of the influenza epidemic characteristics,
its development and circulating influenza viruses
are essential for the prevention and control of

Loss to follow-up in registries of rheumatic patients treated with biologics : a potential information bias in assessing pharmacovigilance and efficacy outcomes
© 2001-2021 Sociedade Portuguesa de Reumatologia.Background: The information associated with loss to follow-up (LFU) patients may affect real-world data evaluation of the use of biologics that is not being adequately captured in registries. Methods: We identified all patients(Pts) treated with biologics in our center who had no visits registered for more than 6 months, in the Rheumatic Diseases Portuguese Register, Reuma.pt. We retrieved baseline information from Reuma.pt and from the hospital electronic clinical record. We then performed a telephonic interview to characterize the reasons for LFU at our day care unit. For Pts unable to be contacted by telephone a letter of invitation to an appointment at the hospital was sent. Results: From a total of 794 Ptsregistered in Reuma.pt at our center with active biologic therapy 227 did not have any information registered in the last 6 months. Of this, 36 Pts were on biologic therapy prescribed by other departments and maintained follow-up in these departments. 102 Pts had suspended biologic administration by medical indication and this information was registered in the hospital electronic clinical records but not updated in Reuma.pt. For 89 Pts no information could be retrieved from either the hospital electronic clinical record or Reuma.pt and we classified these Pts as true LFU. 26 of these LFU Pts were being followed up in another Rheumatology center. 26 of the LFU Pts died. 11 Pts had an adverse effect. 4 Pts of the LFU were considering to be in remission. We were not able to contact 15 of the LFU pts. Conclusion: Identifying LFU Pts and clarifying the reason for the loss of data in a register contributes to a better knowledge on strategies to discontinue biologics in stable pts, to a better pharmacovigilance of adverse effects and to more efficiency in data capture by registries.
