    Does bariatric surgery prior to total hip or knee arthroplasty reduce post-operative complications and improve clinical outcomes for obese patients? Systematic review and meta-analysis.

    AIMS: Our aim was to determine whether, based on the current literature, bariatric surgery prior to total hip (THA) or total knee arthroplasty (TKA) reduces the complication rates and improves the outcome following arthroplasty in obese patients. METHODS: A systematic literature search was undertaken of published and unpublished databases on the 5 November 2015. All papers reporting studies comparing obese patients who had undergone bariatric surgery prior to arthroplasty, or not, were included. Each study was assessed using the Downs and Black appraisal tool. A meta-analysis of risk ratios (RR) and 95% confidence intervals (CI) was performed to determine the incidence of complications including wound infection, deep vein thrombosis (DVT), pulmonary embolism (PE), revision surgery and mortality. RESULTS: From 156 potential studies, five were considered to be eligible for inclusion in the study. A total of 23 348 patients (657 who had undergone bariatric surgery, 22 691 who had not) were analysed. The evidence-base was moderate in quality. There was no statistically significant difference in outcomes such as superficial wound infection (relative risk (RR) 1.88; 95% confidence interval (CI) 0.95 to 0.37), deep wound infection (RR 1.04; 95% CI 0.65 to 1.66), DVT (RR 0.57; 95% CI 0.13 to 2.44), PE (RR 0.51; 95% CI 0.03 to 8.26), revision surgery (RR 1.24; 95% CI 0.75 to 2.05) or mortality (RR 1.25; 95% CI 0.16 to 9.89) between the two groups. CONCLUSION: For most peri-operative outcomes, bariatric surgery prior to THA or TKA does not significantly reduce the complication rates or improve the clinical outcome. This study questions the previous belief that bariatric surgery prior to arthroplasty may improve the clinical outcomes for patients who are obese or morbidly obese.     Computational study of the hydrodefluorination of fluoroarenes at [Ru(NHC)(PR<sub>3</sub>)<sub style="vertical-align: sub;">2</sub>(CO)(H)<sub style="vertical-align: sub;">2</sub>]: predicted scope and regioselectivities

    Density functional theory calculations have been employed to investigate the scope and selectivity of the hydrodefluorination (HDF) of fluoroarenes, C6F6-nHn (n = 0-5), at catalysts of the type [Ru(NHC)(PR3)(2)(CO)(H)(2)]. Based on our previous study (Angew. Chem., Int. Ed., 2011, 50, 2783) two mechanisms featuring the nucleophilic attack of a hydride ligand at a fluoroarene substrate were considered: (i) a concerted process with Ru-H/C-F exchange occurring in one step; and (ii) a stepwise pathway in which the rate-determining transition state involves formation of HF and a Ru-sigma-fluoroaryl complex. The nature of the metal coordination environment and, in particular, the NHC ligand was found to play an important role in both promoting the HDF reaction and determining the regioselectivity of this process. Thus for the reaction of C6F5H, the full experimental system (NHC = IMes, R = Ph) promotes HDF through (i) more facile initial PR3/fluoroarene substitution and (ii) the ability of the NHC N-aryl substituents to stabilise the key C-F bond breaking transition state through F center dot center dot center dot HC interactions. This latter effect is maximised along the lower energy stepwise pathway when an ortho-H substituent is present and this accounts for the ortho-selectivity seen in the reaction of C6F5H to give 1,2,3,4-C6F4H2. Computed C-F bond dissociation energies (BDEs) for C6F6-nHn substrates show a general increase with larger n and are most sensitive to the number of ortho-F substituents present. However, HDF is always computed to remain significantly exothermic when a silane such as Me3SiH is included as terminal reductant. Computed barriers to HDF also generally increase with greater n, and for the concerted pathway a good correlation between C-F BDE and barrier height is seen. The two mechanisms were found to have complementary regioselectivities. For the concerted pathway the reaction is directed to sites with two ortho-F substituents, as these have the weakest C-F bonds. In contrast, reaction along the stepwise pathway is directed to sites with only one ortho-F substituent, due to difficulties in accommodating ortho-F substituents in the C-F bond cleavage transition state. Calculations predict that 1,2,3,5-C6F4H2 and 1,2,3,4-C6F4H2 are viable candidates for HDF at [Ru(IMes)(PPh3)(2)(CO)(H)(2)] and that this would proceed selectively to give 1,2,4-C6F3H3 and 1,2,3-C6F3H3, respectively.</p

    Application of analysis techniques for low frequency interior noise and vibration of commercial aircraft

    Finite element analysis (FEA), statistical energy analysis (SEA), and a power flow method (computer program PAIN) were used to assess low frequency interior noise associated with advanced propeller installations. FEA and SEA models were used to predict cabin noise and vibration and evaluate suppression concepts for structure-borne noise associated with the shaft rotational frequency and harmonics (less than 100 Hz). SEA and PAIN models were used to predict cabin noise and vibration and evaluate suppression concepts for airborne noise associated with engine radiated propeller tones. Both aft-mounted and wing-mounted propeller configurations were evaluated. Ground vibration test data from a 727 airplane modified to accept a propeller engine were used to compare with predictions for the aft-mounted propeller. Similar data from the 767 airplane was used for the wing-mounted comparisons

    Rotating and positive displacement pumps of various types were studied for pumping liquid fluorine for low-thrust, high-performance rocket engines. Included in the analysis were: centrifugal, pitot, Barske, Tesla, drag, gear, vane, axial piston, radial piston, diaphragm, and helirotor pump concepts. The centrifugal pump and the gear pump were selected and these were carried through detailed design and fabrication. Mechanical difficulties were encountered with the gear pump during the preliminary tests in Freon-12. Further testing and development was therefore limited to the centrifugal pump. Tests on the centrifugal pump were conducted in Freon-12 to determine the hydrodynamic performance and in liquid fluorine to demonstrate chemical compatibility

    A comparison of UK equity and property duration

    This paper considers the duration of property and equity. A general formula for duration of asset classes is derived. It is shown that calculations which assume, usually implicitly, that the flow-through of inflation to cash flow is zero, produce misleadingly high durations for property and equities. These are typically in the range 15 to 25 years. Simulations using the formulae show that property has some bond-like characteristics. The results indicate that, for realistic flow-through rates, equities have a higher duration than property. The flow-through rate is the most important variable in the estimation of equities. Using historical data, equity duration is estimated at 8.65 years and property’s at 3.15 years. These are substantially lower than those commonly cited. If these values can be substantiated, and if higher values are used in practice, portfolio immunisation strategies may need to be reconsidered

    ALMA and VLA Observations of the HD 141569 System

    We present VLA 9 mm (33 GHz) observations of the HD 141569 system from semester 16A. The observations achieve a resolution of 0.25 arcsec (28\sim28 au) and a sensitivity of 4.7 μJy beam14.7~\mu \rm Jy~beam^{-1}. We find (1) a 52±5 μ52\pm 5~\muJy point source at the location of HD 141569A that shows potential variability, (2) the detected flux is contained within the SED-inferred central clearing of the disc meaning the spectral index of the dust disc is steeper than previously inferred, and (3) the M dwarf companions are also detected and variable. Previous lower-resolution VLA observations (semester 14A) found a higher flux density, interpreted as solely dust emission. When combined with ALMA observations, the VLA 14A observations suggested the spectral index and grain size distribution of HD 141569's disc was shallow and an outlier among debris systems. Using archival ALMA observations of HD 141569 at 0.87 mm and 2.9 mm we find a dust spectral index of αmm=1.81±0.20\alpha_{\rm mm} = 1.81\pm 0.20. The VLA 16A flux corresponds to a brightness temperature of 5×106\sim5\times10^{6} K, suggesting strong non-disc emission is affecting the inferred grain properties. The VLA 16A flux density of the M2V companion HD 141569B is 149±9 μ149\pm9~\muJy, corresponding to a brightness temperature of 2×108\sim2\times10^{8} K and suggesting significant stellar variability when compared to the VLA14A observations, which are smaller by a factor of 6\sim6.Comment: Accepted for publication in MNRAS, 8 pages, 6 figure
