Интонация как средство формальной дифференциации лексико-семантических вариантов многозначного слова в русском языке

The article treats the problem of the formal differentiation of a lexico-semantic variations of polysemantic word by means of intonation differences. The analysis of the Russian polysemants allows to define the formal differentiation of LSV as a regular systemic phenomenon, determined by the lan-guage position on the scale of lexicality-grammaticality. The extent and depth of dividing a word into grammatical and lexical parts determine the predominant usage of the formal differentiation means. In the unchangeable synsemantic word classes both intonation and stress are used. The stress independ-ence/dependence of the synsemantic parts of speech LSV depends on their functional and syntactic status which is due to their semantic difference.Настоящая статья посвящена проблеме формальной дифференциации ЛСВ многозначного слова посредством интонационных различий. Анализ полисеманов русского языка позволяет определить формальную дифференциацию ЛСВ как закономерное системное явление, детерминированное положением языка на шкале лексичности/грамматичности. Степень и глубина членения слова на лексическое и грамматическое определяют преимущественное использование средств формальной дифференциации. В неизменяемых служебных классах используются фонетические средства: интонационные и акцентные. Акцентная самостоятельность/несамостоятельность ЛСВ служебных частей речи зависит от их функционального и синтаксического статуса, обусловленного различиями в их семантике

Biochemical composition and quality of herring preserves with addition of bio-protective cultures

Herrings rich in vitamins B12, A, D, minerals, including calcium, potassium, magnesium, iodine, possess high levels of lysine, tyrosine, phenylalanine and tryptophan, as well as omega-3 unsaturated fatty acids, namely eicosapentanoic, docosahexoenoic and docosapentoenoic. To suppress the microbiological spoilage of fish preserves, it is promising to use bio-protective cultures that have minimal impact on the production process and product properties. Bacterial strains are able to exert a static effect on the microflora, which causes biodeterioration of food products. Microorganisms as part of bio-protective cultures are included in the fermentation process, so they can be attributed to ordinary food ingredients, so there is no need to put separate information on the packaging. The chemical composition of the frozen Atlantic and Pacific herring fillet, the amino acid composition of proteins, and the fatty acid composition of lipids were studied. The difference between Atlantic and Pacific herrings was detected, which consists in a significantly higher content of docosahexaenoic acid. The difference in the fractional composition of triacylglycerols for the Atlantic and Pacific herring was established. The microflora of SafePro B-2 biological product (Chr. Hansen GmbH), containing multiple strains of Lactobacillus sakei, and the viability of the culture in preserves fillings were studied. The experimental development of canned food with SafePro B-2 additives was carried out. Microbiological, organoleptic and biochemical indicators of the preserves quality during cold storage were studied. The influence of introduced culture on the dynamics of preserves curing period and their shelf life was established

The article analyses both generally-methodological and specifically-scientific principles of developing the typology of needs in a small or minority language. The results of the study show, that the identification need for a language may occur not only in the ethnic Karelians, but also in the natives of other regions living now in Karelia and aspiring to cultural integration.В статье анализируются общеметодологические и частнонаучные принципы выработки типологии потребностей в малом или миноритарном языке. Management of complex economic security of enterprises: empirical test in Russia

There is a new meaningful interpretation of the concept of "integrated economic security of industrial enterprise"; the problems of analyzing complex economic security are substantiated. Based on the study of the regulatory and legal and methodological support of the process of managing economic security in modern economic and political conditions, generalizations and analysis of existing practice in Russia and abroad, the main shortcomings in approaches to assessing the integrated economic security of industrial enterprises have been identified. The article examines the methods for assessing economic security: matrix, product, operational, dynamic one and methods of assessment based on the market value of the company. The parameters of the integrated economic security of industrial enterprise are classified: financial and economic security, information, technological, material and technical, personnel, epidemiological security, conceptual model for assessing financial and economic and integrated security of enterprise has been developed. The practical implementation of the proposed developments has been carried out. It is recommended to use the developed model of economic security management as enterprise resource planning (ERP) module of the enterprise system

Возможности мини- и неинвазивных методов термической деструкции в лечении больных агрессивным фиброматозом

The purpose of the study was to characterize various methods of local physical destruction used in the treatment of patients with aggressive fibromatosis and demonstrate international experience of their use.Material and Methods. literature search was conducted in the electronic databases Rsci (Russian science citation index) and NcBi (National centre for Biotechnology information) in the interval time between 2002 and 2022. The review was devoted to the thermal ablation modalities (radiofrequency, microwave ablation, cryodestruction, high-intensity focused ultrasound ablations) used in therapy of patients with desmoid fibromas of various locations, as well as their safety and efficacy.Results. The review presents the current data on safety and efficacy of minimally-invasive and non-invasive methods of hypo- and hyperthermic local destruction in patients with aggressive fibromatosis, gives the characteristics of exposure and biological effects when performing radiofrequency and microwave ablation, cryodestruction, high-intensity focused ultrasound therapy, describes the main limitations of the methods, indications and contraindications for their application, as well as the methods of prevention of the development of desmoid fibromas.Conclusion. The described methods of local destruction are used in clinical practice mainly for palliative and symptomatic purposes, in case of persistent tumor growth or recurrence, ineffectiveness of other treatment options or contraindications to them due to concomitant somatic pathology or functional status of the patient. Inclusion of these methods in the treatment plan of patients with aggressive fibromatosis can contribute to the reduction of pain syndrome, improvement of functional status of patients as well as long survival with no evidence of tumor progression.Цель исследования – характеристика различных методов локальной физической деструкции, применяемых в лечении больных агрессивным фиброматозом, и демонстрация международного опыта их применения.Материал и методы. Обзор отечественных и зарубежных научных источников, представленных в базах данных РИНЦ (Российский индекс научного цитирования) и NcBi (National center for Biotechnology information) в период с 2002 по 2022 г., посвященных особенностям методов термической аблации (таких как радиочастотная и микроволновая аблация, криодеструкция, высокоинтенсивная фокусированная ультразвуковая терапия), применяющихся в терапии пациентов с десмоидными фибромами различной локализации, их безопасности и эффективности.Результаты. В обзоре представлены современные данные по безопасности и эффективности применения мини- и неинвазивных методов гипо- и гипертермической локальной деструкции у больных агрессивным фиброматозом, даны характеристики воздействия и указаны биологические эффекты при выполнении радиочастотной и микроволновой аблации, криодеструкции, высокоинтенсивной фокусированной ультразвуковой терапии, описаны основные ограничения методов, показания и противопоказания к их применению, а также используемые в клинической практике способы профилактики развития осложнений.Заключение. Описанные способы локальной деструкции применяют в клинической практике в основном с паллиативной и симптоматической целью, при упорном росте или рецидивировании опухоли, неэффективности других вариантов лечения или наличии противопоказаний к ним со стороны сопутствующей соматической патологии или функционального статуса пациента. Включение рассмотренных методов в план лечения больных агрессивным фиброматозом может способствовать снижению болевого синдрома, улучшению функционального статуса пациентов, а также достижению длительного периода жизни без прогрессирования опухолевого процесса

Comparaitve morbidity analysis with urban and rural areas (accroding to the medical preventive examination)

The research is dedicated to the structural analysis and prevalence of diseases detected in large-scale medical preventive examinations (supplementary medical examination within the national health care project). The result of disease prevalence analysis revealed in supplementary medical examinations is compared with medical aid appealability. Rural and urban morbidity is compared and analyzed. The results show the substantial difference in structure and level of diseases revealed in medical preventive examinations and morbidity according to the medical aid appealability. It is established that the difference between the diseases revealed in supplementary medical examination in comparison with general and primary morbidity according to the medical aid appealability is more vividly shown among rural people. The received results show the suitability of medical preventive examinations for the constant working population monitoring and validate the necessity of managerial procedures development aimed at a higher accessibility and quality of medical aid for both urban and rural population.Данная работа посвящена анализу показателей структуры и уровня распространенности заболеваний, выявленных в ходе масштабных медицинских профилактических осмотров, (дополнительной диспансеризации в рамках национального проекта в сфере здравоохранения). Представлены результаты анализа показателей заболеваемости, выявленной в ходе дополнительной диспансеризации, в сравнении с показателями зарегистрированной заболеваемости по обращаемости. Проведено сопоставление данных показателей заболеваемости среди городского и сельского населения. Полученные результаты свидетельствуют о существенных различиях в структуре и уровне выявленных в ходе медицинских профилактических осмотров заболеваний и показателей заболеваемости, зарегистрированной по обращаемости. Установлено, что различия в уровне выявленных при проведении дополнительной диспансеризации заболеваний над общей и первичной заболеваемостью по обращаемости в большей степени проявились среди сельского населения. Полученные данные показывают целесообразность проведения медицинских профилактических осмотров с целью постоянного мониторинга здоровья работающего населения и обосновывают необходимость разработки организационных мер, направленных на повышение доступности и качества медицинской помощи как городскому, так и сельскому населению

Модель прогнозирования и управления преждевременными родами

Background: It seems relevant to study the contribution of socio-demographic, somatic and obstetric-gynecological factors in the implementation of preterm birth. Aims: Assessment of the prognostic significance of socio-demographic, obstetric-gynecological and somatic factors in the prediction of preterm birth and associated adverse pregnancy outcomes with subsequent validation of the prognostic model. Materials and methods: Cohort study with a mixed cohort. A retrospective assessment of socio-demographic factors, harmful habits, obstetric and gynecological pathology, somatic diseases, course and outcomes of pregnancy was carried out with the assessment of the status of newborns in 1246 women with subsequent construction of a predictive model of preterm birth and adverse outcomes of pregnancy using Regression with Optimal Scaling and its prospective validation in 100 women. Results: The most significant predictors, that increase the chance of preterm birth and adverse pregnancy outcomes, were history of premature birth, irregular monitoring during pregnancy, history of pelvic inflammatory disease, smoking, obesity, the onset of sexual activity up to 16 years, cardiovascular and endocrine diseases. Intellectual job reduced the chance of preterm birth and adverse pregnancy outcomes This multivariate predictive model has a diagnostic value. The score of risk factors 25 points had a sensitivity of 73%, a specificity of 71%, the area under the ROC curve (AUC) 0.76 (good quality), p0.001. After stratification of high-risk groups by maternal and perinatal pathology the following list of diagnostic and therapeutic measures is introduced and actively implemented in antenatal clinics. To stratificate this model, we prospectively analyze the course and pregnancy outcomes of 100 women divided into 2 groups: group 1 ― 50 women with preterm delivery, group 2 ― 50 women with term delivery. A total score of 25 and above had 44% of women in group 1 and only 10% of women in group 2 (sensitivity 81.4%, specificity 61.6%, positive predictive value 44%, negative predictive value 90%, positive likelihood ratio 2.2 [1.53.0], negative likelihood ratio 0.3 [0.130.68]). Conclusions: We have proposed a model for predicting preterm birth and delivery and perinatal losses using the available characteristics of pregnant women from early pregnancy with moderate indicators of diagnostic value. Further validation of the model in the general population of pregnant women is required.Обоснование. Представляется актуальным изучение вклада социально-демографических, соматических и акушерско-гинекологических факторов в реализацию преждевременных родов. Цель исследования ― оценка значимости социально-демографических, акушерско-гинекологических и соматических факторов в прогнозировании преждевременных родов и ассоциированных с ними неблагоприятных исходов беременности с последующей валидизацией прогностической модели. Методы. Когортное исследование со смешанными группами участников. Проводилась ретроспективная оценка социально-демографических факторов, вредных привычек, соматических заболеваний, гинекологической патологии, течения и исходов беременности для новорожденных у 1246 женщин с целью построения модели прогнозирования преждевременных родов и неблагоприятных исходов беременности при помощи регрессионного анализа с оптимальным шкалированием и валидизацией модели у 100 женщин. Результаты. Наиболее значимыми предикторами, увеличивающими риск преждевременных родов и неблагоприятных исходов беременности, оказались преждевременные роды в анамнезе, нерегулярное наблюдение во время беременности, воспалительные заболевания органов малого таза в анамнезе, курение, ожирение, начало половой жизни до 16 лет, сердечно-сосудистые и эндокринные заболевания. Принадлежность к интеллектуальной работе, наоборот, уменьшала риск преждевременных родов и ассоциированных с ними неблагоприятных исходов. Данная прогностическая модель продемонстрировала диагностическую ценность. При подсчете 25 и более баллов модель имела чувствительность 73%, специфичность 71%, площадь под ROC-кривой (AUC) 0,76 (качество хорошее), р0,001. После стратификации групп высокого риска по материнской и перинатальной патологии в условиях женской консультации внедрен и активно проводится определенный перечень диагностических и лечебно-профилактических мероприятий. С целью стратификации модели проспективно проанализированы течение и исходы беременности у 100 женщин, разделенных на 2 группы по 50 человек, чья беременность закончилась преждевременными или своевременными родами. В группе 1 суммарный балл 25 имели 44% женщин, в группе 2 ― только 10% женщин (чувствительность 81,4%, специфичность 61,6%, положительная прогностическая значимость 44%, отрицательная прогностическая значимость 90%, положительное отношение правдоподобия 2,2 [1,53,0], отрицательное отношение правдоподобия 0,3 [0,130,68]). Заключение. Нами предложена модель прогнозирования преждевременных родов и перинатальных потерь с использованием доступных характеристик беременных женщин с ранних сроков беременности с умеренными показателями диагностической значимости. Требуется дальнейшая валидизация модели на большей популяции беременных женщин
