Grazie a ciò è possibile accedere da remoto alle informazioni sull'ambiente

    The article analyses the main features of the “reddito di cittadinanza”, a minimum income legislation recently introduced in Italy. It highlights the elements of continuity with the discipline previously in force. After investigating the adequacy of the typology of jobs that are offered through the new discipline and of the intricate system of penalty, the article assesses the mechanism of conditionality in the light of constitutional principles and new job-market trends. Finally, the article provides some suggestions about the opportunity to update the “scientific” paradigm on which labour literature have so far relied, thereby opening spaces to a universal and unconditional income scheme that exceeds the currently planned discipline

    AbstractIntroductionToday, the diagnosis of SA is usually not considered as the etiology for right lower quadrant pain in patient with history of appendectomy, resulting in delaying making the correct diagnosis and treatment. Obviously, other more common causes should be excluded first.DiscussionStump appendicitis (SA) was first described by Rose in 1945. The commonest presenting symptom are abdominal pain (right lower quadrant pain 59%, non specific abdominal pain 16%, and central abdominal pain radiating to the right lower quadrant 14%).Presentation of caseA 54-year-old appendectomized woman presented with recent history of abdominal pain with periumbilical pain radiating to the right side and in the right iliac fossa, in the absence of fever, vomiting or other symptoms.Elective colonoscopy revealed appendicular orifice clogged by big fecalith, with hyperemic surrounding mucosa and CT confirmed “stump appendicitis”. After 30 days of therapy with metronidazole 500mg/day and mesalazine 1000mg×2/day, the patient was submitted to surgery and appendectomy was performed, with a specimen of 24mm stump appendicitis.ConclusionSA may well be considered as one of the differential diagnoses of acute right lower abdominal pain even in patients with history of appendicectomy

    Basic income between workfare and labour changes: a labour law perspective

    L'elaborato analizza il tema del reddito di cittadinanza, nella sue diverse accezioni, indagandone le interferenze con la disciplina giulavoristica e previdenziale alla luce delle metamorfosi del lavoro. Dopo aver ricostruito la genealogia del concetto di lavoro e, con esso, dei sistemi di sicurezza sociale, esaminiamo de jure condito le misure di sostegno al reddito in Italia, con particolare accento sulla tutela contro la disoccupazione e sul reddito di cittadinanza, senza trascurare, nondimeno, le misure straordinarie messe in campo durante l'emergenza pandemica. Infine, chiedendoci quale significato assuma oggi il lavoro in un contesto caratterizzato da flessibilitĂ  e precarietĂ , analizzeremo dapprima le possibilitĂ  di riforma delle misure vigenti nel nostro ordinamento (dagli ammortizzatori sociali al reddito di cittadinanza), spingendoci poi de jure condendo verso la possibile declinazione di un reddito di base incondizionato, provando a ripensare alcune tradizionali categorie giuslavoristiche, a partire dalle nozioni di mansione, tempo di lavoro e retribuzione, chiedendoci se il lavoro possa avere un orizzonte di senso che vada oltre il fare in cambio di un salario.The paper analyses the issue of basic income, in its different meanings, investigating its implications for labour and social security regulations in the framework of labour metamorphosis. After reconstructing the genealogy of the concept of work and, with it, of social security systems, we examine de jure condito the income support measures in Italy, with particular emphasis on unemployment protection and citizenship income, without neglecting, however, the extraordinary measures put in place during the pandemic emergency. Finally, wondering what meaning work has today in a context characterised by flexibility and precariousness, we will first analyse the possibilities of reforming the measures currently in use in Italy (from social safety nets to citizenship income). We will then move towards the possible declination of an unconditional basic income, trying to rethink some of the classic categories of labour law, starting with the notions of jobs, working time and pay, asking ourselves whether work can have a horizon of meaning that goes beyond doing something in exchange for a wage

    Retrospective analysis of management of ingested foreign bodies and food impactions in emergency endoscopic setting in adults

    Background: Ingestion of foreign bodies and food impaction represent the second most common endoscopic emergency after bleeding. The aim of this paper is to report the management and the outcomes in 67 patients admitted for suspected ingestion of foreign body between December 2012 and December 2014. Methods: This retrospective study was conducted at Palermo University Hospitals, Italy, over a 2-year period. We reviewed patients’ database (age, sex, type of foreign body and its anatomical location, treatments, and outcomes as complications, success rates, and mortalities). Results: Foreign bodies were found in all of our 67 patients. Almost all were found in the stomach and lower esophagus (77 %). The types of foreign body were very different, but they were chiefly meat boluses, fishbones or cartilages, button battery and dental prostheses. In all patients it was possible to endoscopically remove the foreign body. Complications related to the endoscopic procedure were unfrequent (about 7 %) and have been treated conservatively. 5.9 % of patients had previous esophageal or laryngeal surgery, and 8.9 % had an underlying esophageal disease, such as a narrowing, dismotility or achalasia. Conclusion: Our experience with foreign bodies and food impaction emphasizes the importance of endoscopic approach and removal, simple and secure when performed by experienced hands and under conscious sedation in most cases. High success rates, lower incidence of minor complications, reduction of the need of surgery and reduced hospitalization time are the strengths of the endoscopic approach

    Alteration of Smell and Taste in Asymptomatic and Symptomatic COVID-19 Patients in Sicily, Italy.

    Objectives: Alteration of smell and taste has been reported in patients with coronavirus disease 2019 (COVID-19). The incidence and clinical-symptomatic manifestation of COVID-19 is different between northern and southern Italy. This study aims to evaluate the onset of alteration of smell and taste in asymptomatic and symptomatic patients in Sicily (extreme south of Italy). Methods: This prospective cross-sectional study was performed on asymptomatic and symptomatic COVID-19 patients tested for severe acute respiratory syndrome-coronavirus-2 (SARS-CoV-2) from May 1 to May 15, 2020. A questionnaire was used for evaluating the prevalence of smell and taste disorders in COVID-19 patients before performing nasopharyngeal swab. Results: Of the total 292 patients, 242 (83.2%) were negative for SARS-CoV-2 and 50 were positive (16.8%). Twenty-six of the 50 (52%) SARS-CoV-2 positive patients reported smell/taste disorders. Twenty-eight of the 50 (57.1%) SARS-CoV-2 positive patients were hospitalized (group A), and 22 (42.9%) were nonhospitalized (group B). The mean age in group A and group B was 45.4 ± 13.7 years and 57.0 ± 15.0, respectively ( P = .007). The symptoms reported by hospitalized patients were fever (71.4%), cough (64.2%), fatigue (82.1%), and dyspnea (100%), while in nonhospitalized patients, the most reported symptoms were sore throat (72.7%), rhinorrhea (77.2%), and altered smell (81.8%). Anosmia/hyposmia reported in group A and group B was 28.5% and 81.8%, respectively ( P = .001). Conclusion: These preliminary results indicate that the majority of SARS-Cov-2 positive patients in southern Italy did not require hospitalization and presented with milder symptoms or no symptoms and the alterations in smell and taste occurred
