Diversidade de genótipos produtores de microcistinas em linhagens brasileiras de Microcystis (Cyanobacteria)
Microcystis Kützing ex Lemmermann is among the genera of cyanobacteria often associated to toxic blooms with the release of microcystins. A gene cluster codes for microcystin synthetases, which are involved in the biosynthesis of this toxin. The aim of the present study was to investigate the genetic diversity of the mcyB gene, specifically the B1 module, in Brazilian strains of Microcystis spp. and its microcystin variants. Broad genetic diversity was revealed in this region. From the phylogenetic analysis, three clusters were obtained that were not related to the geographic origin or morphospecies of the strains, nor with the variant of the microcystin produced. A group of strains that did not produce microcystins was found, despite the presence of the mcyB1 fragment. Eight microcystin isoforms were detected: MC-LR, [D-Asp³]-MC-LR, [Asp³]-MC-LR, MC-RR, [Dha7]-MC-LR, MC-LF, MC-LW and [D-Asp³, EtAdda5]-MC-LH, the latter of which is described for the first time in Brazil. Um agrupamento de genes codifica as sintetases de microscistinas, as quais estão envolvidas na biossÃntese desta toxina. O objetivo deste estudo foi investigar a diversidade genética do gene mcyB, especificamente o módulo B1, de linhagens brasileiras de Microcystis spp. e suas variantes de microcistinas. Uma ampla diversidade genética foi revelada nesta região. A partir da análise filogenética, três agrupamentos foram obtidos, os quais não se relacionaram com a origem geográfica ou com a morfoespécie das linhagens e nem tão pouco com a variante de microcistina produzida. Foi encontrado um grupo de linhagens que não produziu microcistinas, apesar da presença do fragmento mcyB1. Oito variantes de microcistinas foram detectadas: MC-LR, [D-Asp³]-MC-LR, [Asp³]-MC-LR, MC-RR, [Dha7]-MC-LR, MC-LF e MC-LW e [D-Asp³, EtAdda5]-MC-LH, sendo esta última descrita pela primeira vez no Brasil. Enterocolite Induzida por ProteÃnas Alimentares: Uma Etiologia Rara
Introdução: A enterocolite induzida por proteÃnas alimentares (EIPA) é uma forma de hipersensibilidade gastrointestinal alimentar não mediada por imunoglobulina E, sendo provavelmente mediada por células. A fisiopatologia permanece incerta. O leite de vaca e os cereais estão entre as causas mais comuns. O diagnóstico baseia-se na história clÃnica.
Descrição do caso: Descreve-se um caso raro de enterocolite à batata numa criança com antecedentes pessoais e familiares irrelevantes. O doente recusou várias vezes a introdução da primeira sopa (cenoura e batata) e sempre que a ingeria apresentava quadro de vómitos incoercÃveis com desidratação, mas sem necessidade de internamento. Os testes cutâneos por picada com extrato de batata foram negativos e a prova de provocação oral foi positiva.
Comentários: Neste contexto salienta-se a importância de reconhecer o quadro clÃnico de EIPA e estar alerta para a possibilidade de envolvimento de proteÃnas alimentares menos frequentes com o objetivo de fazer um diagnóstico precoce e evitar terapêuticas desadequadasinfo:eu-repo/semantics/publishedVersio
Diplopia, Convergent Strabismus, and Eye Abduction Palsy in a 12-Year-Old Boy with Autoimmune Thyroiditis
Pseudotumor cerebri (PTC) is defined by clinical criteria of increased intracranial pressure, elevated intracranial pressure with normal cerebrospinal fluid (CSF) composition, and exclusion of other causes such tumors, vascular abnormalities, or infections. The association of PTC with levothyroxine (LT4) has been reported. A 12-year-old boy has been followed up for autoimmune thyroiditis under LT4. Family history was irrelevant for endocrine or autoimmune diseases. A TSH level of 4.43 μUI/mL (0.39-3.10) motivated a LT4 adjustment from 75 to 88 μg/day. Five weeks later, he developed horizontal diplopia, convergent strabismus with left eye abduction palsy, and papilledema. Laboratorial evaluation revealed elevated free thyroxine level (1.05 ng/dL [0.65-1.01]) and low TSH, without other alterations. Lumbar puncture was performed and CSF opening pressure was 24 cm H2O with normal composition. Blood and CSF cultures were sterile. Brain MRI was normal. LT4 was temporarily discontinued and progressive improvement was observed, with a normal fundoscopy at day 10 and reversion of diplopia one month later. LT4 was restarted at lower dose and gradually titrated. The boy is currently asymptomatic. This case discloses the potential role of LT4 in inducing PTC. Despite its rarity and unclear association, PTC must be seen as a potential complication of LT4, after excluding all other intracranial hypertension causes.info:eu-repo/semantics/publishedVersio
Mental Retardation: Case Series of Dona Estefânia Hospital Child Development Centre
Segundo a DSM IV a Deficiência Mental (DM) define-se como o funcionamento intelectual
global inferior à média (QI < 70) associado a perturbações do comportamento adaptativo
com inÃcio antes dos 18 anos.
Procurou-se caracterizar retrospectivamente a população de crianças com DM observadas
no Centro de Desenvolvimento do Hospital de Dona Estefânia (CDHDE), entre Janeiro
2005 e Junho 2007.
Foram avaliados os dados epidemiológicos, gravidade, etiologia, co-morbilidade e intervenção proposta.
Do total de 232 processos clÃnicos observados, 185 apresentavam DM.
Classificaram-se em DM ligeira 112 (61%), DM moderada 54 (29%), DM grave 17 (9%) e
profunda 2 (1%). Foram definidas etiologias em 86 crianças (46%) sendo a taxa de diagnóstico mais elevada na DM de maior gravidade. Observou-se uma elevada variabilidade de etiologias: as mais frequentemente encontradas foram as doenças genéticas, prematuridade e patologia associada. Foi detectada co-morbilidade em 123 crianças (66%), sendo a mais frequente as do foro oftalmológico (57 crianças, 46%). Foram propostas e sinalizadas para apoio a totalidade das crianças com DM, 47% em intervenção precoce e 58% em educação especial, das quais 5% usufruiram, por curto perÃodo, do apoio simultaneo de educadora de Intervenção Precoce e de docente do Ensino Especial, durante o perÃodo inicial de integração em jardim de infância. Observou-se um predomÃnio do sexo masculino.
Foi efectuada caracterização clÃnica e funcional das crianças seguidas no CDHDE com o diagnóstico de DM e encontraram-se semelhanças entre os dados presentes e os descritos na literatura. Contudo alguns dados diferem de outras casuÃsticas decorrente, muito provavelmente decorrente da heterogeneidade da população estudada, quer do ponto de vista etiológico, quer no referente aos grupos etários, provavelmente condicionada, pela polÃtica assistencial
Weathering indices as climate proxies. A step forward based on Congo and SW African river muds
Despite the influence of other geological and geomorphological factors, chemical weathering at the Earth's surface is strongly controlled by climate. Thus, a measure of weathering intensity determined from soils or sediments should provide information about the climatic conditions associated with their formation. Available geochemical and mineralogical data on modern fluvial and marine muds from different regions of southern Africa and its Atlantic continental margin are used to review the links between sediment composition and climatic properties together with the possible causes of variance. Although river muds may not be generated exclusively in a single sedimentary cycle and erosion and weathering processes do not necessarily take place in a spatially homogeneous way, significant relationships between mineralogical and geochemical signatures of river mud and rainfall in the corresponding catchment area were recognised. Our study shows that the composition of clay is strongly influenced by climatically-driven weathering, whilst coarser mud fractions tend to be more affected by provenance, grain size, hydraulic sorting, and recycling. In the marine environment the climatic signal may be lost even in clay, because of hydraulic fractionation, authigenic mineral growth and mixing with foreign particles. Given the ubiquitous character of fluvial muds, and the easy and non-expensive methods available for separating and analysing clay fractions, their geochemical fingerprints represent a most precious source of information concerning climate. Any geochemical parameter used as a regional proxy of climate, however, still requires that the diversity of geological, geomorphological, and biological factors that affect its value are cautiously considered
Antimicrobial activity of essential oils of Lamiaceae aromatic spices towards sheep mastitis-causing Staphylococcus aureus and Staphylococcus epidermidis
Mastitis in ewes is responsible for massive economic losses. Antibiotics are routinely used for mastitis control but its excessive use leads to development of antibiotic resistance with deleterious outcomes both for animal and public health. Essential oils (EOs) show antibacterial proprieties and no resistance has been reported after prolonged exposure; however their efficacy depends on their chemical composition.
In this study EOs chemical composition from four autochthonous aromatic herbs, from Alentejo region, southern Portugal, Calamintha nepeta subsp. nepeta, Lavandula stoechas subsp. luisieri, Rosmarinus oficinalis and Thymus mastichina was accessed. EOs of R. officinalis showed predominance in monoterpene hydrocarbons (63%) whereas EOs of T. mastichina, L. luisieri and C. nepeta were rich in oxygenated monoterpenes (71-95%).
The antimicrobial activity of selected EOs was investigated towards Staphylococcus aureus (n= 24) and Staphylococcus epidermidis isolates (n= 24) from ovine mastitic milk origin. Results of disk diffusion assay revealed that C. nepeta, L. stoechas and T. mastichina EOs are highly active against both S. aureus and S. epidermidis strains, whereas R. officinalis EO is highly active against S. aureus strains but inactive against several S. epidermidis isolates. EOs concentration causing bacterial growth inhibition ranged from 500 to 4,000 µg mL-1 in liquid microassays
Impact of diet and nutraceutical supplementation on inflammation in elderly people. Results from the RISTOMED study, an open-label randomized control trial.
BACKGROUND & AIMS: Eating habits may influence the life span and the quality of ageing process by modulating inflammation. The RISTOMED project was developed to provide a personalized and balanced diet, enriched with or without nutraceutical compounds, to decrease and prevent inflammageing, oxidative stress and gut microbiota alteration in healthy elderly people. This paper focused on the effect on inflammation and metabolism markers after 56 days of RISTOMED diet alone or supplementation with three nutraceutical compounds.
METHODS:A cohort of 125 healthy elderly subjects was recruited and randomized into 4 arms (Arm A, RISTOMED diet; Arm B, RISTOMED diet plus VSL#3 probiotic blend; Arm C, RISTOMED diet plus AISA d-Limonene; Arm D, RISTOMED diet plus Argan oil). Inflammatory and metabolism parameters as well as the ratio between Clostridium cluster IV and Bifidobacteria (CL/B) were collected before and after 56 days of dietary intervention, and their evolution compared among the arms. Moreover, participants were subdivided according to their baseline inflammatory parameters (erythrocytes sedimentation rate (ESR), C-Reactive Protein, fibrinogen, Tumor Necrosis Factor-alfa (TNF-α), and Interleukin 6) in two clusters with low or medium-high level of inflammation. The evolution of the measured parameters was then examined separately in each cluster.
RESULTS:Overall, RISTOMED diet alone or with each nutraceutical supplementation significantly decreased ESR. RISTOMED diet supplemented with d-Limonene resulted in a decrease in fibrinogen, glucose, insulin levels and HOMA-IR. The most beneficial effects were observed in subjects with a medium-high inflammatory status who received RISTOMED diet with AISA d-Limonene supplementation. Moreover, RISTOMED diet associated with VSL#3 probiotic blend induced a decrease in the CL/B ratio.
CONCLUSIONS:Overall, this study emphasizes the beneficial anti-inflammageing effect of RISTOMED diet supplemented with nutraceuticals to control the inflammatory status of elderly individuals
Niños como cuidadores: una investigación sobre la perspectiva de los enfermeros en la Región Autónoma de Madeira
Enquadramento: Não há registos, em Portugal, sobre crianças como cuidadoras. A literatura internacional realça que esta é uma
área importante de intervenção dos profissionais de saúde.
Objetivos: Analisar a perspetiva dos enfermeiros dos cuidados de saúde primários da Madeira sobre a problemática das crianças
como cuidadoras e conhecer as áreas de intervenção de enfermagem junto destas.
Metodologia: Estudo quantitativo, descritivo, com aplicação de um questionário a uma amostra de 157 enfermeiros da Região
Autónoma da Madeira
Resultados: Dos enfermeiros, 23% refere que já contactou com crianças cuidadoras, crianças que cuidam dos avós (29%), dos
pais com doença psiquiátrica (25%) e dos pais com doença crónica (21%). Foram identificadas como áreas de intervenção de
Enfermagem o ensino, treino de competências, suporte individual e aconselhamento.
Conclusão: O envelhecimento e as doenças psiquiátricas aumentam a probabilidade de uma criança assumir o papel de
cuidadora. Propomos a replicação desta investigação em Portugal continental assim como de estudos de natureza qualitativa,
explorando o papel da pessoa dependente e da criança cuidadora.Background: In Portugal, there are no records of children as
caregivers. The international literature highlights that this is an
important area of intervention for health professionals.
Objectives: To analyze the primary care nurses’ perspective about
young caregivers in the Autonomous Region of Madeira and identify
areas of intervention.
Methodology: A quantitative and descriptive study was conducted
based on a questionnaire applied to a sample of 157 nurses of the
Autonomous Region of Madeira.
Results: Among the participating nurses, 23% had already contacted
with young caregivers, 29% with young caregivers caring for their
grandparents, 25% with young caregivers caring for parents with
mental disorders, and 21% with young caregivers caring for parents
with chronic diseases. The following areas of nursing intervention were
identified, education, skills training, individual support and counselling.
Conclusion: The aging of the population and the high prevalence of
mental disorders increase the probability of children assuming the
role of caregivers. We recommend that this study be replicated in
mainland Portugal. Qualitative studies should also be conducted on

primaria de Madeira (Portugal) sobre la problemática de los
niños como cuidadores y conocer las áreas de intervención de
la enfermerÃa.
MetodologÃa: Se trata de un estudio cuantitativo de perfil
descriptivo para el cual se autoadministró un cuestionario a una
muestra ocasional de 157 enfermeros de Madeira.
Resultados: El 23 % de los enfermeros indica que ya tuvo contacto
con niños cuidadores. Las situaciones identificadas fueron: niños
que cuidan de los abuelos (29 %); niños que cuidan de los padres
con enfermedad psiquiátrica (25 %), y niños que cuidan de los
padres con enfermedad crónica (21 %). Las áreas de intervención
de la enfermerÃa identificadas fueron: enseñanza, práctica de
competencias, apoyo individual y asesor.
Conclusión: El envejecimiento y las enfermedades psiquiátricas
aumentan la probabilidad de que los niños asuman el papel de
cuidadores. Se propone realizar esta misma investigación en
Portugal Continental, asà como estudios cualitativos para explorar

Child Neglect and Maltreatment and Childhood-to-Adulthood Cognition and Mental Health in a Prospective Birth Cohort
OBJECTIVE: Life-long adverse effects of childhood maltreatment on mental health are well established, but effects on child-to-adulthood cognition and related educational attainment have yet to be examined in the general population. We aimed to establish whether different forms of child maltreatment are associated with poorer cognition and educational qualifications in childhood/adolescence and whether associations persist to midlife, parallel to associations for mental health. METHOD: Cognitive abilities at ages 7, 11, and 16 years (math, reading, and general intellectual ability) and 50 years (immediate/delayed memory, verbal fluency, processing speed) were assessed using standardized tests, and qualifications by age 42 were self-reported. Information on childhood maltreatment (neglect and abuse: sexual, physical, psychological, witnessed), cognition, and mental health was available for 8,928 participants in the 1958 British Birth Cohort. RESULTS: We found a strong association of child neglect with cognitive deficits from childhood to adulthood. To illustrate, the most neglected 6% of the population (score ≥4) had a 0.60 (95% CI = 0.56-0.68) SD lower cognitive score at age 16 and a 0.28 (95% CI = 0.20-0.36) SD deficit at age 50 years relative to the non-neglected participants (score = 0) after adjustment for confounding factors and mental health, and they also had increased risk of poor qualifications (i.e., none/low versus degree-level). Childhood neglect and all forms of abuse were associated with poorer child-to-adulthood mental health, but abuse was mostly unrelated to cognitive abilities. CONCLUSION: The study provides novel data that child neglect is associated with cognitive deficits in childhood/adolescence and decades later in adulthood, independent of mental health, and highlights the lifelong burden of child neglect on cognitive abilities and mental health
