    NA60 results on thermal dimuons

    The NA60 experiment at the CERN SPS has measured muon pairs with unprecedented precision in 158A GeV In-In collisions. A strong excess of pairs above the known sources is observed in the whole mass region 0.2<M<2.6 GeV. The mass spectrum for M<1 GeV is consistent with a dominant contribution from pi+pi- -> rho -> mu+mu- annihilation. The associated rho spectral function shows a strong broadening, but essentially no shift in mass. For M>1 GeV, the excess is found to be prompt, not due to enhanced charm production, with pronounced differences to Drell-Yan pairs. The slope parameter Teff associated with the transverse momentum spectra rises with mass up to the rho, followed by a sudden decline above. The rise for M<1 GeV is consistent with radial flow of a hadronic emission source. The seeming absence of significant flow for M>1 GeV and its relation to parton-hadron duality is discussed in detail, suggesting a dominantly partonic emission source in this region. A comparison of the data to the present status of theoretical modeling is also contained. The accumulated empirical evidence, including also a Planck-like shape of the mass spectra at low pT and the lack of polarization, is consistent with a global interpretation of the excess dimuons as thermal radiation.     Virtual Reality after Surgery—A Method to Decrease Pain After Surgery in Pediatric Patients

    Background: Virtual Reality (VR) is used as an effective tool for distraction and as an adjunct for pain management. This study was conducted to compare VR to standard iPad use after surgery and examine its effect on pain score and opioid consumption. Methods: This was a randomized controlled study, with stratification by surgery type, age group (7-12yo, 13-18yo) and gender. Pain and anxiety were assessed with validated scales (STAI, FACES, VAS, FLACC) and outcomes were compared between each group. Results: 50 of the 106 enrolled patients used the VR device. After adjusting for age, gender, and STAI, patients had a decreased FLACC score while using the VR device compared to the iPad group (odds ratio 2.95, P =.021). The younger patients were found to have lower FLACC scores while using the VR device (odds ratio 1.15, p=0.044); this finding was most significant when patients used the VR device for 20-30 minutes (odds ratio 1.67, P =.0003). Additionally, after adjusting for treatment group, gender, and STAI, the younger patients had higher odds of withdrawal or exclusion from the study (odds ratio 1.18, P =.021). No significant difference in opioid consumption between the groups was found. Discussion: Virtual reality was well tolerated and more effective in decreasing pain during the immediate postoperative period than iPad use.     Phase II trial correlating standardized uptake value with pathologic complete response to pertuzumab and trastuzumab in breast cancer

    PURPOSE Predictive biomarkers to identify patients with human epidermal growth factor receptor 2 (HER2)–positive breast cancer who may benefit from targeted therapy alone are required. We hypothesized that early measurements of tumor maximum standardized uptake values corrected for lean body mass (SULmax) on [ 18 F] fluorodeoxyglucose positron emission tomography/computed tomography would predict pathologic complete response (pCR) to neoadjuvant pertuzumab and trastuzumab (PT). PATIENTS AND METHODS Patients with stage II/III, estrogen receptor–negative, HER2-positive breast cancer received four cycles of neoadjuvant PT. [ 18 F]Fluorodeoxyglucose positron emission tomography/computed tomography was performed at baseline and 15 days after PT initiation (C1D15). Eighty evaluable patients were required to test the null hypothesis that the area under the curve of percentage of change in SULmax by C1D15 predicting pCR is less than or equal to 0.65, with a one-sided type I error rate of 10%. RESULTS Eighty-eight women were enrolled (83 evaluable), and 85% (75 of 88) completed all four cycles of PT. pCR after PT alone was 34%. Receiver operating characteristic analysis yielded an area under the curve of 0.76 (90% CI, 0.67 to 0.85), which rejected the null hypothesis. Between patients who obtained pCR versus not, a significant difference in median percent reduction in SULmax by C1D15 was observed (63.8% v 33.5%; P, .001), an SULmax reduction greater than or equal to 40% was more prevalent (86% v 46%; P, .001; negative predictive value, 88%; positive predictive value, 49%), and a significant difference in median C1D15 SULmax (1.6 v 3.9; P, .001) and higher proportion of C1D15 SULmax less than or equal to 3 (93% v 38%; P, .001; negative predictive value, 94%; positive predictive value, 55%) were observed. CONCLUSION Early changes in SULmax predict response to four cycles of PT in estrogen receptor–negative, HER2-positive breast cancer. Once optimized, this quantitative imaging strategy may facilitate a more tailored approach to therapy in this setting

However, between patients who obtained pCR and who did not, a significant difference in median percent reduction in SULmax by C1D15 was observed (63.8% v 41.8%; P = .004) and SULmax reduction ≥ 40% was more prevalent (83% v 52%; P = .03; positive predictive value, 31%). Participants not obtaining a 40% reduction in SULmax by C1D15 were unlikely to obtain pCR (negative predictive value, 91%). CONCLUSION: Although the primary objective was not met, early changes in SULmax predict response to PT in estrogen receptor-negative and HER2-positive breast cancer. Once optimized, this quantitative imaging strategy may facilitate tailoring of therapy in this setting

    Factors Associated with Revision Surgery after Internal Fixation of Hip Fractures

    Background: Femoral neck fractures are associated with high rates of revision surgery after management with internal fixation. Using data from the Fixation using Alternative Implants for the Treatment of Hip fractures (FAITH) trial evaluating methods of internal fixation in patients with femoral neck fractures, we investigated associations between baseline and surgical factors and the need for revision surgery to promote healing, relieve pain, treat infection or improve function over 24 months postsurgery. Additionally, we investigated factors associated with (1) hardware removal and (2) implant exchange from cancellous screws (CS) or sliding hip screw (SHS) to total hip arthroplasty, hemiarthroplasty, or another internal fixation device. Methods: We identified 15 potential factors a priori that may be associated with revision surgery, 7 with hardware removal, and 14 with implant exchange. We used multivariable Cox proportional hazards analyses in our investigation.     The crimson speckled moth, Utetheisa pulchella (Linnaeus, 1758), is recorded in Brazilian Amazon for the first time. A moth (male) was collected with light trap in a multiple cropping area, in Mojuí dos Campos municipality, Pará State, Brazil. This record expands the area of occurrence and distribution of this rare species in the Neotropical Region. In addition, the occurrence of U. pulchella in the Amazon was unexpected, since this species is generally associated with open ecosystems and drier climates
