    Latar belakang : Salah satu upaya higiene sanitasi makanan yaitu dengan meningkatkan higiene perorangan pada penjamah makanan yang merupakan kunci keberhasilan dalam pengolahan makanan yang aman dan sehat. Higiene perorangan yang terlibat dalam pengolahan makanan akan dapat dicapai, apabila dalam diri pekerja tertanam pengertian tentang pentingnya menjaga kesehatan dan kebersihan diri. Kajian ini bertujuan untuk mengetahui profil kepatuhan higiene perorangan penjamah makanan di Instalasi Gizi dan Tata Boga RSPI Prof. Dr. Sulianti SarosoMetode : Kajian ini menggunakan desain deskriptif dengan metode potong lintang. Sampel meliputi petugas penjamah makanan di Instalasi Gizi dan Tata Boga - RSPI Prof. Dr. Sulianti Saroso periode tahun 2016.Hasil : Dari 19 penjamah makanan sebagian besar berjenis kelamin perempuan (52.6%), tingkat pendidikan sebagian besar memiliki latar belakang pendidikan setara SMA (78.9%, Kondisi kesehatan penjamah makanan sebagian besar pernah MCU 73.7% dengan hasil sehat bersyarat, dari observasi perilaku diketahui bahwa penjamah makanan berperilaku tidak higienis diantaranya yaitu tidak memakai sepatu tertutup sebanyak 96.5%, tidak memakai baju kerja sebanyak 87.5%, tidak memakai masker sebanyak 77.2%, tidak memakai penutup kepala sebanyak 71.9%, tidak memakai sarung tangan sebanyak 50%, tidak memakai celemek sebanyak 33.3%, bercakap cakap sebanyak 28.1% dan tidak menutup makanan yang matang sebanyak 25.9%, kepatuhan dalam hygiene perorangan sebagian besar 89.5% dengan kepatuhan kategori kurang.Kesimpulan : Penjamah makanan sebagian besardengan kepatuhan kategori kurang 89.5% dan 10.5% dengan kepatuhan kategori sedang

    Internal and External Perspectives on Quality of Healthcare Services at Sanglah General Hospital Denpasar

    Background and purpose: Customer satisfaction is a primary indicator of the quality of public healthcare services. This study investigated internal (hospital staff) and external (hospital clients) perspectives in order to gain insight into the quality of care at Sanglah General Hospital.Methods: The study used both qualitative and quantitative methodologies with 11 informants and 106 respondents. Qualitative data obtained through in-depth interviews with hospital staff were analyzed thematically. Quantitative data obtained through self-administered questionnaire were analyzed using univariate analysis.Results: Informants from the qualitative data collection stated that Sanglah General Hospital has a relatively high level of service and that existing structures to mitigate issues are in place, which act as reinforcing factors. Data from the quantitative survey indicated that clients were satisfied with the quality of service (ServQual), with an overall percentage of 83.82%.Conclusion: Further efforts could be made in order to improve healthcare provision at Sanglah Hospital, particularly from the perspective of hospital facilities, staff support and increased implementation of clinical governance.     The light subunit of mushroom Agaricus bisporus tyrosinase:Its biological characteristics and implications

    The light subunit of mushroom Agaricus bisporus tyrosinase (LSMT) is a protein of unknown function that was discovered serendipitously during the elucidation of the crystal structure of the enzyme. The protein is non-immunogenic and can penetrate the intestinal epithelial cell barrier, and thus, similar to its structural homologue HA-33 from Clostridium botulinum, may be potentially absorbable by the intestine. LSMT also shares high structural homology with the ricin-B-like lectin from the mushroom Clitocybe nebularis (CNL), which has been shown to display biological activity against leukemic cancer cells and dendritic cells. Therefore, we evaluated the biological activity of LSMT. An in vitro assay suggested that LSMT presentation to most of the cancer cell lines studied has a negligible effect on their proliferation. However, inhibition of cell growth and a slight stimulation of cell proliferation were observed with breast cancer and macrophage cells, respectively. LSMT appeared to be relatively resistant against proteolysis by trypsin and papain, but not bromelain. Challenges with gastric and intestinal juice suggested that the protein is resistant to gastrointestinal tract conditions.     Mycobaterium tuberculosis (M.tuberculosis) telah menginfeksi sepertiga penduduk dunia. Salah satu manifestasi klinis yang terinfeksi M.tuberculosis adalah pembesaran kelenjar getah Bening pada regio colli, axilla, inguinal, abdominal yang sering di sebut tuberkulosis kelenjar. Tuberkulosis kelenjar masih sering menimbulkan permasalahan baik dari segi diagnostik, pengobatan dan pemantauan hasil pengobatannya teristimewa di daerah endemis TB, ditambah lagi gejala tuberkulosis pada penderita HIV sering tidak jelas manifestasi yang sering timbul adalah pembesaran kelenjar getah Bening.Telah dilakukan penelitian pada 11 pasien HIV dengan pembesaran kelenjar getah Bening leher yang diduga karena infeksi M.tb serta bersedia secara tertulis mengikuti penelitian ini. Pada semua subjek dilakukan biopsi jarum halus dan biopsi dengan pembedahan. Hasil biopsi tersebut dilakukan pemeriksaan pewarnaan langsung BTA; sitologi dan PCR. Hasil yang didapat adalah preparat BTA langsung dari BJH 36,4%; Sitologi dari BJH positif tuberkulosis 36,4%; PCR tuberkulosis positif 45,5%; Hasil pemeriksaan Patologi Anatomi (PA) yang positif tuberkulosis adalah 72,7%.Berdasarkan penelitian perbandingan pemeriksaan Mycobaterium tuberculosis pada pembesaran KGB pada pasien HIV dianjurkan melakukan pemeriksaan PA dari bahan spesimen ekstirpasi dari kelenjar getah Bening leher, pertimbangankan PCR tuberkulosis yang non invasif

    Mapping ergonomics application to improve SMEs working condition in industrially developing countries: a critical review

    In industrially developing countries (IDC), small and medium enterprises (SMEs) account for the highest proprotion of employment. Unfortunately, the working conditions in SMEs are often very poor and expose employees to a potentially wide range of health and safety risks. This paper presents a comprehensive review of 161 articles related to ergonomics application in SMEs, using Indonesia as a case study. The aim of this paper is to investigate the extent of ergonomics application and identify areas that can be improved to promote effective ergonomics for SMEs in IDC. The most urgent issue found is the need for adopting participatory approach in contrast to the commonly implemented top-down approach. Some good practices in ergonomics application were also revealed from the review, e.g. a multidisciplinary approach, unsophisticated and low-cost solutions, and recognising the importance of productivity. The review also found that more work is still required to achieve appropriate cross-cultural adaptation of ergonomics application. Practitioner Summary: Despite continuous efforts in addressing ergonomics issues in SMEs of IDC, workers are still exposed to poor work conditions.     Maternal mortality in the rural Gambia, a qualitative study on access to emergency obstetric care

    BACKGROUND: Maternal mortality is the vital indicator with the greatest disparity between developed and developing countries. The challenging nature of measuring maternal mortality has made it necessary to perform an action-oriented means of gathering information on where, how and why deaths are occurring; what kinds of action are needed and have been taken. A maternal death review is an in-depth investigation of the causes and circumstances surrounding maternal deaths. The objectives of the present study were to describe the socio-cultural and health service factors associated with maternal deaths in rural Gambia. METHODS: We reviewed the cases of 42 maternal deaths of women who actually tried to reach or have reached health care services. A verbal autopsy technique was applied for 32 of the cases. Key people who had witnessed any stage during the process leading to death were interviewed. Health care staff who participated in the provision of care to the deceased was also interviewed. All interviews were tape recorded and analyzed by using a grounded theory approach. The standard WHO definition of maternal deaths was used. RESULTS: The length of time in delay within each phase of the model was estimated from the moment the woman, her family or health care providers realized that there was a complication until the decision to seeking or implementing care was made. The following items evolved as important: underestimation of the severity of the complication, bad experience with the health care system, delay in reaching an appropriate medical facility, lack of transportation, prolonged transportation, seeking care at more than one medical facility and delay in receiving prompt and appropriate care after reaching the hospital. CONCLUSION: Women do seek access to care for obstetric emergencies, but because of a variety of problems encountered, appropriate care is often delayed. Disorganized health care with lack of prompt response to emergencies is a major factor contributing to a continued high mortality rate

    Existing literature on financial abuse focuses on men’s control over money, goods, assets and over women’s education/work, thereby implicitly constructing economic activity as paid work. This paper responds to this under-recognition of men’s (and in the context of particular communities, their family’s) abuse of and control over women’s unpaid (domestic) labour within a broader conceptualization of economic abuse. Drawing upon life-history interviews with 41 South Asian women from two separate studies in the UK and India, this paper takes an intersectional perspective to explore how gender, migration status, race/ethnicity and class can help understand women’s experiences as a continuum of economic abuse
