    Restoration of impaired intestinal barrier function by the hydrolysed casein diet contributes to the prevention of type 1 diabetes in the diabetes-prone BioBreeding rat

    Aims/hypothesis Impaired intestinal barrier function is observed in type I diabetes patients and animal models of the disease. Exposure to diabetogenic antigens from the intestinal milieu due to a compromised intestinal barrier is considered essential for induction of the autoimmune process leading to type I diabetes. Since a hydrolysed casein (HC) diet prevents autoimmune diabetes onset in diabetes-prone (DP)-BioBreeding (BB) rats, we studied the role of the HC diet on intestinal barrier function and, therefore, prevention of autoimmune diabetes onset in this animal model. Methods DP-BB rats were fed the HC diet from weaning onwards and monitored for autoimmune diabetes development. Intestinal permeability was assessed in vivo by lactulose mannitol test and ex vivo by measuring trans-epithelial electrical resistance (TEER). Levels of serum zonulin, a physiological tight junction modulator, were measured by ELISA. heal mRNA expression of Myo9b, Cldn1, Cldn2 and Ocln (which encode the tight junction-related proteins myosin IXb, claudin-1, claudin-2 and occludin) and Il-10, Tgf-beta (also known as Il10 and Tgfb, respectively, which encode regulatory cytokines) was analysed by quantitative PCR. Results The HC diet reduced autoimmune diabetes by 50% in DP-BB rats. In DP-BB rats, prediabetic gut permeability negatively correlated with the moment of autoimmune diabetes onset. The improved intestinal barrier function that was induced by HC diet in DP-BB rats was visualised by decreasing lactulose:mannitol ratio, decreasing serum zonulin levels and increasing ileal TEER. The HC diet modified ileal mRNA expression of Myo9b, and Cldn1 and Cldn2, but left Ocln expression unaltered. Conclusions/interpretation Improved intestinal barrier function might be an important intermediate in the prevention of autoimmune diabetes by the HC diet in DP-BB rats. Effects on tight junctions, ileal cytokines and zonulin production might be important mechanisms for this effect

    Comparison of Serum CA-125 Level in Ruptured and Unruptured Tubal Pregnancies in Yazd/Iran during 2001-2006

    Abstract: Background & Aims: This study aimed to determine if serial measurement of serum CA-125 level could be used as a useful test in the differential diagnosis of intact and ruptured tubal pregnancies. Methods: In a single-blind prospective controlled clinical study, 59 women with tubal pregnancy of 6–12 weeks (26 women with ruptured tubal pregnancy, 33 women with intact tubal pregnancy and 59women with normal intrauterine pregnancy of the same gestational age as control group) were studied prospectively. Serum CA-125 levels were measured in all women and compared among three groups. Results: Demographic features (age, gestational age, parity and educational level) were not significantly different in three groups. CA-125 level in ruptured tubal pregnancy group was significantly higher than that in control group (P = 0.04). Both these two groups had higher levels of CA-125 comparing to the intact tubal pregnancy group (P = 0.001, P = 0.006, respectively). Conclusion: In intact tubal pregnancies managed with medical treatment, serial measurements of CA-125 could be a supplementary test for an early diagnosis of tubal rupture.     Quantitative Comparison of Un-Stimulated Whole Saliva Flow Rate Among Menopausal Women and Same Aged Men

    AbstractBackground and Objectives: Menopause can be associated with psycho-somatic changes in oro-facial areas like xerostomia and Burning Mouth Syndrome, although these findings are controversial. The present study sought to compare the Un-stimulated Whole Saliva (UWS) flow rate of a group including menopausal & postmenopausal women and same-aged men.Methods: In this cross-sectional analytic-descriptive study 40 menopausal & post-menopausal women (as experimental-group) and 40 same-aged men (as control group) without any systemic diseases and any drug consumption were divided into 2 groups, xerostomia was evaluated by a questionnaire, and their psychological conditions were assessed with HAD scale. UWS flow rate was measured by the spitting method. Data were analyzed by chi-square, Krusscal Walis and Mann-Whitney tests. Results: Mean of UWS flow rates in experimental group was significantly less than that in control group (P=0.006); no significant difference was found between the two groups regarding psychological condition. Also, menopausal women had significantly greater xerostomia than men (45% vs 15%) (P=0.003).Conclusion: Based on this study, xerostomia and reduction in UWS flow rate are sequences of menopause, these findings necessitate the increasing awareness of menopausal & postmenopausal women for controlling the methods of these problems.Keywords: Menopause; Xerostomia; Saliva

    Intravenous paracetamol versus intramuscular pethidine in relief of labour pain in primigravid women

    Background: Intramuscular pethidine is one of most common opioids used for labour analgesia. There are a number of concerns in the literature regarding the use of pethidine. The aim of this study is to compare analgesic efficacy of paracetamol with pethidine for labour pain in normal vaginal delivery. Materials and Methods: In this single-blinded, randomised control trial, 80 primigravid singleton women with full-term pregnancy candidate for normal vaginal delivery, were entered the trial and divided in to pethidine (A) and paracetamol (B) groups. At the time of admission, age and body mass index of mother and gestational age based on last day of period were recorded. In both groups, intravenous promethazine and hyoscine were administered to each patient at the first stage of delivery. From beginning of active phase of delivery, patients in group A received 50 mg intramuscular pethidine injection. At the same time patients in group B, received an intravenous solution infusion containing 1000 mg paracetamol and 300 cc of normal saline. After child birth, average labour pain was assessed using Visual Analogue Scale (VAS) by direct questioning from patient in both groups. Results: After patients’ selection, 19 individual omitted during study due to exclusion criteria and finally 30 patients in paracetamol group and 31 patients in pethidine group remained to enter the trial. There was no significant difference in age and BMI of mothers between both groups (P > 0.05). Maternal age and labour duration in paracetamol group had no meaningful difference with maternal age and labour duration of patients in pethidine group (P > 0.05). The average VAS pain score was significantly lower in paracetamol comparing to that of pethidine group (8.366 out of 10, 9.612 out of 10, respectively, P < 0.001). Conclusion: It is concluded that intravenous paracetamol is more effective than intramuscular pethidine to relief labour pain in normal vaginal delivery.Keywords: Labour pain, intramuscular pethidine, intravenous paracetamol, vaginal deliver

    Effect of Exposure to Pill Contraceptive Low-dose Levels of Homocysteine and Nitric Oxide in Healthy Women

    Introduction: Cardiovascular disease is one of the public health priorities. Consumption of oral contraceptives increase the risk of cardiovascular disease and it still remains a concern. This study aimed to investigate the effect of exposure on pill contraceptive low-dose  levels on homocysteine and nitric oxide. methods: In this cohort ( retrospective+ prospective) study, 100 women with normal menstrual cycle aged betwen 20-35 years old refered to health care centers of Yazd, Iran in 2015.  This study was conducted through face to face interviews by the researcher who asked for demographic and anthropometric characteristics. Anthropometic indices  was measured and the levels of homosysteine and nitric oxide was determined. The data were analyzed using t-test, chi- square test and ANOVA by SPSS 21. Results: The mean and standard deviation of homocysteine levels in the exposed group acompared to non-exposed group were (3/848±2/357 μmol/L) and (3/284±1/616 μmol/L) as well as the mean and standard deviation of nitric oxide in the exposed group were (p-value=0/41) and (181/360±90/44μM) and in the non-exposed group were (162/654±90/913 μM) and (p-value=0/29) , respectively.According to these results, there was not found any statistical significant  difference among these results. Conclusion: Taking low dose oral contraceptives in healthy women did not change any differences in homocysteine and nitric oxide levels as a modifiable risk factors for cardiovascular disease
