    Comprehensive study of the benefits of integrating a sharing energy strategy between prosumers

En este nuevo escenario, los prosumidores juegan un papel importante para amortiguar la curva de consumo y, por lo tanto, contribuir a los requisitos de la red. Los conjuntos fotovoltaicos (PV) integrados en edificios residenciales atraen una atención especial, ya que pueden complementarse con sistemas de almacenamiento de energía (ESS), como baterías o ultracondensadores (UC) o incluso una combinación de ambos, con el objetivo de lograr que los clientes puedan administrar su energía. Las razones anteriores justifican la necesidad de un sistema de gestión de energía (EMS) que coordine el funcionamiento de los dispositivos de almacenamiento de energía de manera optimizada, ya que son caros. En este documento, se propone un sistema de control integral para intercambiar energía entre los dispositivos de almacenamiento híbridos (baterías y UC) de dos viviendas con instalaciones fotovoltaicas con el fin de aumentar la disponibilidad de sus ESS y soportar los requisitos de energía durante el mayor tiempo posible, optimizando tamaños y costos, y evitando la saturación.The upgrade of power systems in the last years due to the proliferation of distributed generation with renewable energy sources, active users and electric vehicles has become a challenge. In this new scenario, prosumers play an important role in order to buffer the consumption curve and, therefore, contributing to the grid requirements. Special attention is attracted by photovoltaic (PV) arrays integrated in residential buildings, because they can be complemented with energy storage systems (ESSs), such as batteries or ultracapacitors (UCs) or even a combination of both, with the aim to achieve that customers could manage their energy. The above reasons justify the need of an energy management system (EMS) that coordinates the operation of the energy storage devices in an optimized way, as they are expensive. In this paper, a comprehensive control system is proposed to exchange energy between the hybrid storage devices (batteries and UCs) of two dwells with PV installations in order to increase the availability of their ESSs and support the energy requirements during the most time possible, optimizing sizes and costs and avoiding saturation.peerReviewe

    2-[18F]FDG PET/CT as a Predictor of Microvascular Invasion and High Histological Grade in Patients with Hepatocellular Carcinoma

    Hepatocellular carcinoma (HCC) generally presents a low avidity for 2-deoxy-2-[18F]fluoro-d-glucose (FDG) in PET/CT although an increased FDG uptake seems to relate to more aggressive biological factors. To define the prognostic value of PET/CT with FDG in patients with an HCC scheduled for a tumor resection, forty-one patients were prospectively studied. The histological factors of a poor prognosis were determined and FDG uptake in the HCC lesions was analyzed semi-quantitatively (lean body mass-corrected standardized uptake value (SUL) and tumor-to-liver ratio (TLR) at different time points). The PET metabolic parameters were related to the histological characteristics of the resected tumors and to the evolution of patients. Microvascular invasion (MVI) and a poor grade of differentiation were significantly related to a worse prognosis. The SULpeak of the lesion 60 min post-FDG injection was the best parameter to predict MVI while the SULpeak of the TLR at 60 min was better for a poor differentiation. Moreover, the latter parameter was also the best preoperative variable available to predict any of these two histological factors. Patients with an increased TLRpeak60 presented a significantly higher incidence of poor prognostic factors than the rest (75% vs. 28.6%, p = 0.005) and a significantly higher incidence of recurrence at 12 months (38% vs. 0%, p = 0.014). Therefore, a semi-quantitative analysis of certain metabolic parameters on PET/CT can help identify, preoperatively, patients with histological factors of a poor prognosis, allowing an adjustment of the therapeutic strategy for those patients with a higher risk of an early recurrence

    A retrospective, multicenter study of the efficacy of lapatinib plus trastuzumab in HER2-positive metastatic breast cancer patients previously treated with trastuzumab, lapatinib, or both: the Trastyvere study

    [Purpose]: To evaluate the efficacy and safety of lapatinib (L) and trastuzumab (T) combination in HER2-positive metastatic breast cancer (MBC) patients previously treated with T and/or L.[Materials and methods]: We conducted a retrospective, post-authorized, multicenter study including patients with HER2-positive MBC or locally advanced breast cancer (ABC) treated with the combination of L–T. Concomitant endocrine therapy, as well as brain metastasis and/or prior exposure to L, were allowed.[Results]: One hundred and fifteen patients from 14 institutions were included. The median age was 59.8 years. The median number of prior T regimens in the advanced setting was 3 and 73 patients had received a prior L regimen. The clinical benefit rate (CBR) was 34.8% (95% CI 26.1–43.5). Among other efficacy endpoints, the overall response rate was 21.7%, and median progression-free survival (PFS) and overall survival were 3.9 and 21.6 months, respectively. Heavily pretreated and ≥ 3 metastatic organ patients showed lower CBR and PFS than patients with a low number of previous regimens and < 3 metastatic organs. Moreover, CBR did not significantly change in L-pretreated compared with L-naïve patients (31.5% versus 40.5% for L-pretreated versus L-naïve). Grade 3/4 adverse events were reported in 19 patients (16.5%).[Conclusion]: The combination of L–T is an effective and well-tolerated regimen in heavily pretreated patients and remains active among patients progressing on prior L-based therapy.     Ibrutinib in Combination With Rituximab for Indolent Clinical Forms of Mantle Cell Lymphoma (IMCL-2015): A Multicenter, Open-Label, Single-Arm, Phase II Trial

    PURPOSE The need for an individualized management of indolent clinical forms in mantle cell lymphoma (MCL) is increasingly recognized. We hypothesized that a tailored treatment with ibrutinib in combination with rituximab (IR) could obtain significant responses in these patients. METHODS This is a multicenter single-arm, open-label, phase II study with a two-stage design conducted in 12 Spanish GELTAMO sites (ClinicalTrials.gov identifier: NCT02682641). Previously untreated MCL patients with indolent clinical forms defined by the following criteria were eligible: no disease-related symptoms, nonblastoid variants, Ki-67 < 30%, and largest tumor diameter <= 3 cm. Both leukemic non-nodal and nodal subtypes were recruited. Patients received ibrutinib 560 mg once daily and a total of eight doses of rituximab 375 mg/m(2). Ibrutinib could be discontinued after 2 years in the case of sustained undetectable minimal residual disease (MRD). The primary end point was the complete response (CR) rate achieved after 12 cycles according to Lugano criteria. RESULTS Fifty patients with MCL (male 66%; median age 65 years) were enrolled. After 12 cycles of treatment, 42 (84%; 95% CI, 74 to 94) patients had an overall response, including 40 (80%; 95% CI, 69 to 91) with CR. Moreover, undetectable MRD in peripheral blood was achieved in 87% (95% CI, 77 to 97) of cases. At 2 years, 24 of 35 evaluable patients (69%) could discontinue ibrutinib because of undetectable MRD. Four patients had disease progression; three were non-nodal MCL and carried high genomic complexity and TP53 mutations at enrollment. No unexpected toxicity was seen except one patient with severe aplastic anemia. CONCLUSION Frontline IR combination achieves a high rate of CRs and undetectable MRD in indolent clinical forms of MCL. Discontinuation seems appropriate in cases with undetectable MRD, except for TP53-mutated cases

    1 Introducir y potenciar la prensa escrita diaria en las escuelas rurales como objeto e instrumento de estudio. 2 Facilitar los medios y fomentar la realización de periódicos escolares por los alumnos. 3 Ayudar a comprender los contenidos de Ciencias Sociales y Lenguaje en los libros. 4 Estimular el interés por la lectura activa y la ampliación del vocabulario. 5 Desarrollar el espíritu crítico frente a los mensajes de los medios de comunicación. 6 Fomentar la intercomunicación cultural en el colegio y en el medio local. 7 Comprender el significado de una prensa plural de una sociedad. Colegios estatales de Aragón, alumnos de dos a catorce años en la segunda etapa de EGB. 1 Recopilación de datos y experiencias. 2 Análisis de varios periódicos regionales y nacionales. 3 Realización de un documento de apoyo para entregar a los profesores. 4 Confección de encuestas (nivel de lectura de los padres de los alumnos, ortografía y comprensión). 5 Contacto con los profesores de los centros públicos que deseaban trabajar la experiencia. 6 Asistencia al Simposium sobre prensa y escuela.
