    Scénario: Une patiente de 45 ans vous consulte car sa jambe droite est bleue, cyanosée, avec une tension dans le mollet depuis cinq jours, qui s'est étendue progressivement jusqu'au haut de la cuisse. La circonférence du mollet droit est 4 cm plus grande que celle du mollet gauche. La patiente est en surpoids (IMC à 35 kg/m2) et suit depuis quelques années un traitement oestro-progestatif. Elle n'a pas d'anamnèse familiale ou d'antécédent de maladie thromboembolique. Un ultrason montre une thrombose veineuse profonde (TVP) avec un thrombus s'étendant de la veine poplitée à la veine iliaque externe. Question: La patiente pourrait-elle bénéficier d'une thrombolyse? Contexte: Les complications de la TVP comprennent l'embolie pulmonaire et le syndrome post-thrombotique. Un syndrome post-thrombotique léger à modéré peut survenir chez 30-50% des patients malgré un traitement anticoagulant, qui prévient principalement l'embolie pulmonaire et la récidive de TVP. Cette mise à jour de revue systématique compare l'efficacité du traitement anticoagulant à celui combinant les anticoagulants et la thrombolyse (par streptokinase, urokinase ou activateur tissulaire du plasminogène)

    Time resolved optical Kerr effect analysis of urea–water system

    The nuclear dynamics of urea aqueous solution was analyzed by time resolved optical Kerr effect (OKE). The data analysis was achieved in time and in frequency domains. Three relaxation times characterize the time decay of the OKE signal at high mole fractions of urea, while only two relaxation times characterize this decay for the low mole fractions. The observed slowest relaxation time increases with increasing the mole fraction of urea. The comparison between this relaxation time and the ones determined by Raman and nuclear magnetic resonance spectroscopies suggests that the slow relaxation time is related to the reorientation of an axis lying in the plane of the urea molecule. At high mole fractions, the power spectra derived from the Fourier transform of the OKE signal are characterized by one broad peak at around 70 cm−1 and by a shoulder at around 160 cm−1 in the high frequency part of the former peak. This shoulder is related to the hydrogen bond interactions which involve urea molecules. Molecular dynamics simulation results on urea/water system suggest that the power spectra derived from OKE data could be interpreted in terms of translational motions (caging effect) and in terms of rotational motion (libration) of urea molecules

    Glasses doped with rare earth elements possess unique photoluminescence properties. They find application in several devices, such as lasers, optical amplifiers, and sensors. More recently, rare-earth doped glass thin films have been the subject of investigation for the development of frequency-converting layers able to increase the efficiency of silicon solar cells. Another approach to the improvement of the performance of a solar cell is based on the capture of a larger flux of light by the detector, which can be obtained by surface texture, plasmonics, or waveguide structures. Here, the recent advances in this area will be briefly reviewed
