    Seroepidemiology of rubella, measles, HBV, HCV and B19 virus within women in child bearing ages (Saravan City of Sistan and Bloochastan Province)

    Present survey basically focused on women between 15-45 years of age resident in a town of Sistan and Baluchistan province named as Saravan city located in border of Pakistan-Iran in order to find out the seropositivity against the viruses in child bearing ages in the above stated under study community. This descriptive cross-sectional study was carried-out from 2001 up to 2002. Saravan town was divided into 4 geographical areas and each area was further sub-divided into 10 blocks and in each block 10 families were chosen randomly. In the next step by referring to each family from the chosen married women with specified age i.e., 15-45 years, 5 mL blood was collected. Serum was then separated and stored at -20°C before the assay. ELISA kit was employed to detect anti B19, anti rubella, anti measles, anti HBV and anti HCV antibody. Furthermore during samples collection a questionnaire filled for each woman under study. This study showed that 89.6% of women understudy were seropositive against measles, rubella (96.2%), B19 (59.2%), HCV (0.8%) and HBV (19.8%), respectively. According to the results of no serious problem with rubella in this area; But, about measles, the present immunity against measles in this area is insufficient. It seems that incidence of B19 infection in this region is same as other places in Iran. The rate of seropositivity against HBV and HCV indicated of these viruses circulating in the population in this area. © 2007 Academic Journals

    Quantum memory assisted entropic uncertainty and entanglement dynamics: Two qubits coupled with local fields and Ornstein Uhlenbeck noise

    Entropic uncertainty and entanglement are two distinct aspects of quantum mechanical procedures. To estimate entropic uncertainty relations, entropies are used: the greater the entropy bound, the less effective the quantum operations and entanglement are. In this regard, we analyze the entropic uncertainty, entropic uncertainty lower bound, and concurrence dynamics in two non-interacting qubits. The exposure of two qubits is studied in two different qubit-noise configurations, namely, common qubit-noise and independent qubit-noise interactions. To include the noisy effects of the local external fields, a Gaussian Ornstein Uhlenbeck process is considered. We show that the rise in entropic uncertainty gives rise to the disentanglement in the two-qubit Werner type state and both are directly proportional. Depending on the parameters adjustment and the number of environments coupled, different classical environments have varying capacities to induce entropic uncertainty and disentanglement in quantum systems. The entanglement is shown to be vulnerable to current external fields; however, by employing the ideal parameter ranges we provided, prolonged entanglement retention while preventing entropic uncertainty growth can be achieved. Besides, we have also analyzed the intrinsic behavior of the classical fields towards two-qubit entanglement without any imperfection with respect to different parameter

    Theorem on the Distribution of Short-Time Particle Displacements with Physical Applications

    The distribution of the initial short-time displacements of particles is considered for a class of classical systems under rather general conditions on the dynamics and with Gaussian initial velocity distributions, while the positions could have an arbitrary distribution. This class of systems contains canonical equilibrium of a Hamiltonian system as a special case. We prove that for this class of systems the nth order cumulants of the initial short-time displacements behave as the 2n-th power of time for all n>2, rather than exhibiting an nth power scaling. This has direct applications to the initial short-time behavior of the Van Hove self-correlation function, to its non-equilibrium generalizations the Green's functions for mass transport, and to the non-Gaussian parameters used in supercooled liquids and glasses.Comment: A less ambiguous mathematical notation for cumulants was adopted and several passages were reformulated and clarified. 40 pages, 1 figure.     Fluctuations of water near extended hydrophobic and hydrophilic surfaces

    We use molecular dynamics simulations of the SPC-E model of liquid water to derive probability distributions for water density fluctuations in probe volumes of different shapes and sizes, both in the bulk as well as near hydrophobic and hydrophilic surfaces. To obtain our results, we introduce a biased sampling of coarse-grained densities, which in turn biases the actual solvent density. The technique is easily combined with molecular dynamics integration algorithms. Our principal result is that the probability for density fluctuations of water near a hydrophobic surface, with or without surface-water attractions, is akin to density fluctuations at the water-vapor interface. Specifically, the probability of density depletion near the surface is significantly larger than that in bulk. In contrast, we find that the statistics of water density fluctuations near a model hydrophilic surface are similar to that in the bulk

    Implementation strategies to enhance the implementation of eHealth programs for patients with chronic illnesses: realist systematic review

    Background: There is growing evidence of the positive effects of electronic health (eHealth) interventions for patients with chronic illness, but implementation of such interventions into practice is challenging. Implementation strategies that potentially impact implementation outcomes and implementation success have been identified. Which strategies are actually used in the implementation of eHealth interventions for patients with chronic illness and which ones are the most effective is unclear. Objective: This systematic realist review aimed to summarize evidence from empirical studies regarding (1) which implementation strategies are used when implementing eHealth interventions for patients with chronic illnesses living at home, (2) implementation outcomes, and (3) the relationship between implementation strategies, implementation outcomes, and degree of implementation success. Methods: A systematic literature search was performed in the electronic databases MEDLINE, Embase, PsycINFO, Scopus, Allied and Complementary Medicine Database, Cumulative Index to Nursing and Allied Health Literature, and Cochrane Library. Studies were included if they described implementation strategies used to support the integration of eHealth interventions into practice. Implementation strategies were categorized according to 9 categories defined by the Expert Recommendations for Implementing Change project: (1) engage consumers, (2) use evaluative and iterative strategies, (3) change infrastructure, (4) adapt and tailor to the context, (5) develop stakeholder interrelationships, (6) use financial strategies, (7) support clinicians, (8) provide interactive assistance, and (9) train and educate stakeholders. Implementation outcomes were extracted according to the implementation outcome framework by Proctor and colleagues: (1) acceptability, (2) adoption, (3) appropriateness, (4) cost, (5) feasibility, (6) fidelity, (7) penetration, and (8) sustainability. Implementation success was extracted according to the study authors’ own evaluation of implementation success in relation to the used implementation strategies. Results: The implementation strategies management support and engagement, internal and external facilitation, training, and audit and feedback were directly related to implementation success in several studies. No clear relationship was found between the number of implementation strategies used and implementation success. Conclusions: This is the first review examining implementation strategies, implementation outcomes, and implementation success of studies reporting the implementation of eHealth programs for patients with chronic illnesses living at home. The review indicates that internal and external facilitation, audit and feedback, management support, and training of clinicians are of importance for eHealth implementation. The review also points to the lack of eHealth studies that report implementation strategies in a comprehensive way and highlights the need to design robust studies focusing on implementation strategies in the future.publishedVersio

    Driving vascular endothelial cell fate of human multipotent Isl1+ heart progenitors with VEGF modified mRNA

    Distinct families of multipotent heart progenitors play a central role in the generation of diverse cardiac, smooth muscle and endothelial cell lineages during mammalian cardiogenesis. The identification of precise paracrine signals that drive the cell-fate decision of these multipotent progenitors, and the development of novel approaches to deliver these signals in vivo, are critical steps towards unlocking their regenerative therapeutic potential. Herein, we have identified a family of human cardiac endothelial intermediates located in outflow tract of the early human fetal hearts (OFT-ECs), characterized by coexpression of Isl1 and CD144/vWF. By comparing angiocrine factors expressed by the human OFT-ECs and non-cardiac ECs, vascular endothelial growth factor (VEGF)-A was identified as the most abundantly expressed factor, and clonal assays documented its ability to drive endothelial specification of human embryonic stem cell (ESC)-derived Isl1+ progenitors in a VEGF receptor-dependent manner. Human Isl1-ECs (endothelial cells differentiated from hESC-derived ISL1+ progenitors) resemble OFT-ECs in terms of expression of the cardiac endothelial progenitor- and endocardial cell-specific genes, confirming their organ specificity. To determine whether VEGF-A might serve as an in vivo cell-fate switch for human ESC-derived Isl1-ECs, we established a novel approach using chemically modified mRNA as a platform for transient, yet highly efficient expression of paracrine factors in cardiovascular progenitors. Overexpression of VEGF-A promotes not only the endothelial specification but also engraftment, proliferation and survival (reduced apoptosis) of the human Isl1+ progenitors in vivo. The large-scale derivation of cardiac-specific human Isl1-ECs from human pluripotent stem cells, coupled with the ability to drive endothelial specification, engraftment, and survival following transplantation, suggest a novel strategy for vascular regeneration in the heart

    Understanding the information needs of women with rheumatoid arthritis concerning pregnancy, post-natal care and early parenting: A mixed-methods study

Background: Although women with rheumatoid arthritis (RA) face a number of challenges in negotiating the journey to parenthood, no studies have explored the information needs of women with RA in relation to their childbearing years. This study aimed to determine the need for (and preferred mode/s of delivery of) information regarding pregnancy, post-natal care and early parenting among women with RA. Methods: Interviews and focus groups were conducted with 27 women with RA who were pregnant in the last 5 years, currently pregnant or planning pregnancy. Verbatim transcripts were analysed using both inductive and deductive approaches. Two validated instruments were used to quantify information needs and preferences: the Educational Needs Assessment Tool (ENAT, range 0-156, higher scores indicate higher educational needs) and the Autonomy Preference Index (API, range 0-100, higher scores indicate stronger preferences). Results: Lack of information about medication safety, access to physical/emotional support services and practical strategies for coping with daily challenges related to parenting were the most prominent of the six key themes identified. Rheumatologists were the primary source for information regarding treatment decisions while arthritis consumer organisations were perceived as critical 'resource hubs'. There was strong preference for information delivered electronically, especially among rural participants. Quantitative outcomes supported the qualitative findings; on average, participants reported high educational needs (mean ENAT score 97.2, SD 30.8) and API scores indicated that desire for information (mean 89.8, SD 5.6) was greater than the need for involvement in treatment decision-making (mean 68.4, SD 8.2). Conclusions: Many women with RA struggle to find adequate information on pregnancy planning, pregnancy and early parenting in relation to their chronic condition, and there is a clear need to develop accessible information that is consumer-focused and evidence-based. Although most participants trusted their rheumatologist as their primary information source, there was consistent demand for more information, particularly regarding the safety of RA medications during pregnancy and breastfeeding, and the importance of learning from other women's personal experiences was strongly emphasised
