    Denosumab-associated osteonecrosis of the jaw; A case series and literature review

    Introduction: Medication-related osteonecrosis of the jaw (MRONJ) is a severely debilitating condition of multifactorial pathogenesis. It primarily involves patients receiving intravenous bisphosphonates (BPs) and most recently the new antiresorptive drug, denosumab, for the treatment of skeletal-related malignancies. There is no curative treatment and no consensus exists regarding the clinical management of such patients. This review aims to share our current clinical experience at Sheffield Teaching Hospitals’ Trust and raise awareness of the increase in severity of ONJ in patients receiving denosumab. Patients and Methods: Four new cases with clinical diagnosis of MRONJ were presented to Sheffield Teaching Hospitals’ Trust. MRONJ was attributed to denosumab therapy, as all patients were treated solely with denosumab for skeletal-related malignancies. Results: All cases appear to have a more aggressive mode of ONJ compared to that seen with IV and/or oral BPs so far. The cause of MRONJ was observed in the presence of periodontal disease alone and following dental extractions. Progression of the disease occurred considerably faster with the development of widespread suppuration and tooth mobility within weeks. Imaging revealed rather extensive areas of bony destruction, sometimes with associated periosteal reaction in keeping with a chronic bony infection. Conclusion: It is imperative for all dental and medical teams involved in treating these patients to understand the side effects of RANKL inhibitors on bone metabolism and how it affects treatment. Helping patients to understand the chronicity and potential progression of the disease is essential to a satisfactory outcome

    Evolution of public-private partnership: the UK perspective through a case study approach

    Purpose Since the 1990s, the National Health Service (NHS)advisory officers have developed considerable expertise in managing the process of specifying, procuring, contracting and running public–private partnership (PPP) projects. However, there has been a relatively consistent trajectory in the findings of studies and evaluation of PPP from its initial introduction in the health sector in 1992 to the present time. Therefore, the purpose of this study is to critically evaluate the PPP experience in the UK context using a case study in the NHS. Design/methodology/approach The partnership literature is primarily focussed on process issues, and the impact of partnerships on improving outcomes cannot be assumed. By conducting a critical review on most updated research studies and innovative approaches in this area, the literature as to the place of PPPs in health in the context of the UK is critically explored and whether they have a role in system resilience is examined. A case study has be used as well to describing the processes of a PPP arrangement. Findings Health-care PPP is one of the options relating to health system resilience. However, their contribution in the NHS has been mixed, with success noted in short-term clinical and services contracts while in the long-term the value for money argument has not been proven. In theory, the role of PPPs in bringing together ingredients supporting system resilience such as finance, management and innovation in the UK has not always been successful, and NHS providers have taken the approach to exit such arrangements. Research limitations/implications More research work is needed to capture the 21st-century challenges and critical success factors during its implementation. Practical implications The creation of strong partnerships is moving service delivery away from a project-by-project approach to one that includes strategic and policy developments for long-term results. Originality/value This is a fresh discussion in the role of PPP in system resilience in the UK perspective through a case study describing an exit from a PPP arrangement

    The Impact of interviewer working hours on police interviews with children

    Fatigue resulting from unpredictable or extended working conditions is a factor that negatively impacts the performance of police officers. In this study, we considered how investigative interviewing of children is influenced by interviewer working conditions. We examined two working conditions concerning when interviews were conducted: (a) during early duty shift and (b) an hour before the end of an interviewer’s duty shift and after the end of a shift. We analysed 102 police interviews with children and identified clues that interviews which commenced during early duty shift had more appropriate approaches than interviews in the other condition. Inappropriate approaches were not significantly affected by interviewer working conditions. These outcomes suggest considering new knowledge specific to the behaviour of interviewers according to working conditions and provide promising foundations for further research

    Cell-type-specific signaling networks in heterocellular organoids

    Despite the widespread adoption of organoids as biomimetic tissue models, methods to comprehensively analyze cell-type-specific post-translational modification (PTM) signaling networks in organoids are absent. Here, we report multivariate single-cell analysis of such networks in organoids and organoid cocultures. Simultaneous analysis by mass cytometry of 28 PTMs in >1 million single cells derived from small intestinal organoids reveals cell-type- and cell-state-specific signaling networks in stem, Paneth, enteroendocrine, tuft and goblet cells, as well as enterocytes. Integrating single-cell PTM analysis with thiol-reactive organoid barcoding in situ (TOBis) enables high-throughput comparison of signaling networks between organoid cultures. Cell-type-specific PTM analysis of colorectal cancer organoid cocultures reveals that shApc, KrasG12D and Trp53R172H cell-autonomously mimic signaling states normally induced by stromal fibroblasts and macrophages. These results demonstrate how standard mass cytometry workflows can be modified to perform high-throughput multivariate cell-type-specific signaling analysis of healthy and cancerous organoids

    The ‘new’ migration for work phenomenon: The pursuit of emancipation and recognition in the context of work

    This article examines the ‘new’ migration for work phenomenon gripping Southern Europe since the Global Financial Crisis struck in 2008, by focusing on the case of skilled Greeks migrating to Germany for work purposes. In applying Honneth’s concept of emancipation to the domain of work, the article frames emancipation as a phenomenon which emerges from an individual’s search for meaningful work and as a form of resistance to deteriorating institutions and social injustice. Informed by this is an assessment of the new migration for work phenomenon from Greece to Germany by employing survey data on the perceptions of skilled emigrants. Following analysis of the findings, it is concluded that migration is a form of emancipation that allows individuals to regain recognition and self-respect while also to protest the erosion of social and human rights in their home country

    Light4Health eLearning Course: health research for interior lighting design. Re-thinking design approaches based on science

    This paper presents the results of \u27Light4Health\u27 (L4H), a three-year EU Erasmus+ Strategic Partnership grant project (2019-2021), which investigated, systematized and taught health-related research on the impact of natural and artificial light on human health and well-being relevant to indoor lighting design. The objective was to re-think evidence-based lighting design approaches for residential, working/educational, and healthcare spaces, in order to develop a novel cross-disciplinary eLearning platform, that intersects lighting design and current peer-reviewed health research through a select combination of the most relevant research, methods, and tools. The content was developed through teaching workshops with international researchers, teachers, and students. Participating students were introduced to the application of relevant research and new metrics, in order to produce creative lighting design proposals. Students were able to inform lighting design approaches that support health and well-being without compromising creativity via the tools and methods developed through the health sciences. With this project, participants and consortium members have narrowed the gap that exists between research and the practice/application of lighting, by translating research from complex scientific jargon into various tools for designers to use. The knowledge gained, was consolidated into an open-access online curriculum for international lighting design students, educators and professionals via the free eLearning Moodle platform (https://course.light4health.net/)
