    Pattern of skin diseases in children attending the dermatology clinic in Alert referral hospital, Addis Ababa, Ethiopia: a retrospective study

    Background: A Skin disease, which is estimated to affect between 21 and 87% of children, are the reason for up to a third of outpatient visits to pediatricians and dermatologists. It can possibly re-sult in considerable anxiety, parental worry, and embarrassment to the child and lead to loss of confidence, disruption of social relations, and feeling of stigmatization. This study aimed to assess the pattern of skin diseases in children attending at ALERT referral hospital.Methods: The study setting is ALERT referral hospital, Addis Ababa, Ethiopia. A hospital-based, retrospective, cross-sectional descriptive study was carried out between July and August 2020. All children younger than 12 years, who were diagnosed for skin diseases from May 2018 to May 2020, were included. Four hundred twenty-three children were sampled using a random sampling method. SPSS Version 20 software was used for data analysis.Results: The results showed that 385(91%) of patients had one skin disease and the remaining 38(9%) had two or more skin diseases. Fungal infections were present in30.1% of the cases fol-lowed by eczema, which accounted for 27.4%. Among fungal infections, Tinea Capitis (106/116), 91.4% followed by Tinea Corporis and Tinea Pedis were the most common in ALERT dermatology clinic. Among eczema cases, family atopic dermatitis (82/106), 77% was the most common. The result showed seasonal variation in some diseases.Conclusion: Skin fungal infections were the most common followed by eczema, pigmentary dis-order, infestation, viral infection, urticaria, bacterial infection, and others. There was some sea-sonal variation in some diseases

    Head Injury-A Neglected Public Health Problem: A Four-Month Prospective Study at Jimma University Specialized Hospital, Ethiopia

    Background: Trauma, especially head trauma, is an expanding major public health problem and the leading cause of death of the young and productive part of the world’s population. Research is mainly done in high-income countries where only a small proportion of the worldwide fatalities occur. The intention of this study was to analyze head injury in a setting where most patients in low- and middleincome countries receive treatment, a referral hospital with general but no neurosurgical service like Jimma University Specialized Hospital. The study aims to provide surgeons, hospital managers and health planners working in similar set-ups with baseline information for further investigation and prevention programs intending to reduce the burden of head injury.Methods: All head injury patients presented to Jimma University Specialized Hospital between March and June 2010 were included in this prospective research. Epidemiological, clinical and management data were collected for the study.Results: Out of 52 patients, 47 were males. The median age was 20.0 years (SD=13.3). Fights (n=20, 38.5%) and road traffic accidents (n=19, 36.5%) were the most common causes of head injury. Half of the patients sustained mild and 36.5% sustained severe head injury. The initial GCS had a significant correlation with the outcome. The mortality rate was 21.2%. Of all patients 76.9% were managed conservatively.Conclusion: Prevention of road traffic accidents and improvement of conservative care were identified as major methods to reduce the burden of head injury in a set-up similar to Jimma.     One year audit of surgical admissions at Gondar university medical college

    Background: The new University of Gondar enrolled surgeons for post graduate training in November 2003. A new surgical curriculum was designed in partnership with the Leicester Gondar Link. Admissions to the Department of Surgery over twelve months were subject to audit. The objectives of the audit study were to introduce a new concept, to review the surgical experience of the Department, to support organizational change, to integrate audit into post graduate training, and to measure and improve the quality of the service.Method: Social, medical and administrative data on inpatients was recorded on a discharge summary sheet. The data was coded using a six letter descriptive code and transferred to a computer for presentation on a spread sheet.Results: The results list the social and the demographic distribution of patients; the surgical details including diagnosis, operation and outcome; and administrative data of patient numbers and duration of stay.Conclusion: The discussion is related the objectives of the study. The concept of audit was successfully introduced. Although data was incomplete, trends were apparent. The surgical experience of the Department was documented. The results supported the argument for organizational change including a move to sub-specialization. Audit has become an integral part of the postgraduate curriculum underwriting the monthly morbidity and mortality meetings and providing data for clinical research. The data has provided a baseline mortality rate and exposed quality issues for example, demographic restrictions on access to surgery, and the high mortality rate among patients with head injury

Therefore, in this paper, the detour polynomials and detour indices of vertices identified of n-graphs which are connected to themselves and separated from each other with respect to the vertices for n≥3 will be obtained. Also, polynomials detour and detour indices will be found for another graphs which have important applications in Chemistry.

    Spectral soil analysis for fertilizer recommendations by coupling with QUEFTS for maize in East Africa: A sensitivity analysis

    Laboratory analysis of soil properties is prohibitively expensive and difficult to scale across the soils in sub Saharan Africa. This results in a lack of soil-specific fertilizer recommendations, where recommendation can only be provided at a regional scale. This study aims to assess the feasibility of using spectral soil analysis to provide soil-specific fertilizer recommendations. Using a range of spectrometers [NeoSpectra Saucer (NIR), FieldSpec 4 (vis-NIR) with contact probe or mug light interface, FTIR Bruker Tensor 27 (MIR)], 346 archived soil samples (0–20 cm) with known soil chemical properties collected from Ethiopia, Kenya and Tanzania were scanned. Partial least square regression (PLSR) was used to develop prediction models for selected soil properties including pH, soil organic carbon (SOC), total nitrogen, Olsen P, and exchangeable K. These predicted properties, and associated uncertainty, were used to derive fertilizer recommendations for maize using the Quantitative Evaluation of the Fertility of Tropical Soils (QUEFTS) model parameters for sub-Saharan Africa. Most soil properties (pH, SOC, total nitrogen, and exchangeable K) were well predicted (Concordance Correlation Coefficient values between 0.88 and 0.96 and Ratio of Performance to Interquartile values between 1.4 and 5.9) by all the spectrometers but there were performance variations between soil properties and spec- trometers. Use of the predicted soil data for the development of fertilizer recommendations gave promising results when compared to the recommendations obtained with the conventional soil analysis. For example, the least performing NeoSpectra Saucer over/under-estimated up to 8 and 24 kg ha-1N and P, respectively, though there was insignificant variation in estimation of P fertilizer among spectrometers. We conclude that spectral technology can be used to determine major soil properties with satisfactory precision, sufficient for specific fertilizer decision making in East Africa, possibly even with portable equipment in the fiel

    Eliapixant (BAY 1817080), a P2X3 receptor antagonist, in refractory chronic cough: a randomised, placebo-controlled, crossover phase 2a study

    Background ATP acting via P2X3 receptors is an important mediator of refractory chronic cough (RCC). This phase 2a double-blinded crossover study assessed the safety, tolerability and efficacy of eliapixant (BAY 1817080), a selective P2X3 receptor antagonist, in adults with RCC attending specialist centres.Methods In period A, patients received placebo for 2 weeks then eliapixant 10 mg for 1 week. In period B, patients received eliapixant 50, 200 and 750 mg twice daily for 1 week per dose level. Patients were randomised 1:1 to period A–B (n=20) or B–A (n=20). The primary efficacy end-point was change in cough frequency assessed over 24 h. The primary safety end-point was frequency and severity of adverse events (AEs).Results 37 patients completed randomised therapy. Mean cough frequency fell by 17.4% versus baseline with placebo. Eliapixant reduced cough frequency at doses ≥50 mg (reduction versus placebo at 750 mg: 25% (90% CI 11.5–36.5%); p=0.002). Doses ≥50 mg also significantly reduced cough severity. AEs, mostly mild or moderate, were reported in 65% of patients with placebo and 41–49% receiving eliapixant. Cumulative rates of taste-related AEs were 3% with placebo and 5–21% with eliapixant; all were mild.Conclusions Selective P2X3 antagonism with eliapixant significantly reduced cough frequency and severity, confirming this as a viable therapeutic pathway for RCC. Taste-related side-effects were lower at therapeutic doses than with the less selective P2X3 antagonist gefapixant. Selective P2X3 antagonism appears to be a novel therapeutic approach for RCC

    Mixed-species allometric equations and estimation of aboveground biomass and carbon stocks in restoring degraded landscape in northern Ethiopia

    Accurate biomass estimation is critical to quantify the changes in biomass and carbon stocks following the restoration of degraded landscapes. However, there is lack of site-specific allometric equations for the estimation of aboveground biomass (AGB), which consequently limits our understanding of the contributions of restoration efforts in mitigating climate change. This study was conducted in northwestern Ethiopia to develop a multi-species allometric equation and investigate the spatial and temporal variation of C-stocks following the restoration of degraded landscapes. We harvested and weighed 84 trees from eleven dominant species from six grazing exclosures and adjacent communal grazing land. We observed that AGB correlates significantly with diameter at stump height D 30 (R 2 = 0.78; P < 0.01), and tree height H (R 2 = 0.41, P < 0.05). Our best model, which includes D 30 and H as predictors explained 82% of the variations in AGB. This model produced the lowest bias with narrow ranges of errors across different diameter classes. Estimated C-stock showed a significant positive correlation with stem density (R 2 = 0.80, P < 0.01) and basal area (R 2 = 0.84, P < 0.01). At the watershed level, the mean C-stock was 3.8 (±0.5) Mg C ha−1. Plot-level C-stocks varied between 0.1 and 13.7 Mg C ha−1. Estimated C-stocks in three- and seven-year-old exclosures exceeded estimated C-stock in the communal grazing land by 50%. The species that contribute most to C-stocks were Leucaena sp. (28%), Calpurnia aurea (21%), Euclea racemosa (20.9%), and Dodonaea angustifolia (15.8%). The equations developed in this study allow monitoring changes in C-stocks and C-sequestration following the implementation of restoration practices in northern Ethiopia over space and time. The estimated C-stocks can be used as a reference against which future changes in C-stocks can be compared

    The P2X3 receptor antagonist filapixant in patients with refractory chronic cough: a randomized controlled trial

    BACKGROUND: P2X3 receptor antagonists seem to have a promising potential for treating patients with refractory chronic cough. In this double-blind, randomized, placebo-controlled study, we investigated the efficacy, safety, and tolerability of the novel selective P2X3 receptor antagonist filapixant (BAY1902607) in patients with refractory chronic cough. METHODS: Following a crossover design, 23 patients with refractory chronic cough (age: 60.4 ± 9.1 years) received ascending doses of filapixant in one period (20, 80, 150, and 250 mg, twice daily, 4-days-on/3-days-off) and placebo in the other. The primary efficacy endpoint was the 24-h cough frequency on Day 4 of each dosing step. Further, subjective cough severity and health-related quality of life were assessed. RESULTS: Filapixant at doses ≥ 80 mg significantly reduced cough frequency and severity and improved cough health-related quality of life. Reductions in 24-h cough frequency over placebo ranged from 17% (80 mg dose) to 37% (250 mg dose), reductions over baseline from 23% (80 mg) to 41% (250 mg) (placebo: 6%). Reductions in cough severity ratings on a 100-mm visual analog scale ranged from 8 mm (80 mg) to 21 mm (250 mg). No serious or severe adverse events or adverse events leading to discontinuation of treatment were reported. Taste-related adverse events occurred in 4%, 13%, 43%, and 57% of patients treated with filapixant 20, 80, 150, and 250 mg, respectively, and in 12% treated with placebo. CONCLUSIONS: Filapixant proved to be efficacious, safe, and-apart from the occurrence of taste disturbances, especially at higher dosages-well tolerated during the short therapeutic intervention.     Metabolomic profiling of amines in sepsis predicts changes in NOS canonical pathways

    Rationale Nitric oxide synthase (NOS) is a biomarker/target in sepsis. NOS activity is driven by amino acids, which cycle to regulate the substrate L-arginine in parallel with cycles which regulate the endogenous inhibitors ADMA and L-NMMA. The relationship between amines and the consequence of plasma changes on iNOS activity in early sepsis is not known. Objective Our objective was to apply a metabolomics approach to determine the influence of sepsis on a full array of amines and what consequence these changes may have on predicted iNOS activity. Methods and measurements 34 amino acids were measured using ultra purification mass spectrometry in the plasma of septic patients (n = 38) taken at the time of diagnosis and 24–72 hours post diagnosis and of healthy volunteers (n = 21). L-arginine and methylarginines were measured using liquid-chromatography mass spectrometry and ELISA. A top down approach was also taken to examine the most changed metabolic pathways by Ingenuity Pathway Analysis. The iNOS supporting capacity of plasma was determined using a mouse macrophage cell-based bioassay. Main results Of all the amines measured 22, including L-arginine and ADMA, displayed significant differences in samples from patients with sepsis. The functional consequence of increased ADMA and decreased L-arginine in context of all cumulative metabolic changes in plasma resulted in reduced iNOS supporting activity associated with sepsis. Conclusions In early sepsis profound changes in amine levels were defined by dominant changes in the iNOS canonical pathway resulting in functionally meaningful changes in the ability of plasma to regulate iNOS activity ex vivo
