CRITERIA OF THE CLINICAL OUTCOME OF THE ACUTE CORONARY SYNDROME INTO THE PROGRESSIVE STENOCARDIA AT THE HOSPITAL STAGE
Aim. The aim of this study is to develop a prognostic flowchart for the clinical outcome of acute coronary syndrome into progressive stenocardia at the hospital stage.Materials and methods. The studies were carried out basing on the results of the survey of 68 patients admitted to the infarction department of the Republican Clinical Hospital of the Center for Special Emergency Medical Care in Makhachkala in 2015 which correspond to the basic principles of the Declaration of Helsinki - Ethical Principles for Medical Research Involving Human Subjects as amended in 2000. The severity of acute heart failure by Killip, electrocardiography (ECG) and EchoCG studies were conducted according to generally accepted standards.Findings. The research allowed developing the model of theΒ prognostic block diagram. All indicators of relative risk (RR) for the studied indicators were reliable. Calculations of the values of RR were carried out according to the most frequent ranges of concentration of indicators of the number of patients. The greatest statistical power was possessed by concentration ranges for cardiac markers - TP-I from 0.6 to 1.1 ng / ml and BNP-32 from 60 to 110 pg / ml; For inflammation markers - IL-1Ξ² from 1.4 to 2.4 pg / ml and TNF-Ξ± from 2.6 to 3.6 pg / ml; For endothelial dysfunction markers - NO from 10 to 15 ΞΌmol / l and ET from 3.3 to 4.3 fmol / ml; For immunomarkers, cardiolipin antibodies (CA) from 1 to 3 U / ml and NP from 17 to 26 nmol / l. Measurement of these parameters at the initial stage of admission of patients with acute coronary syndrome (ACS) to the hospital (the starting point of reference) and the statement of positive results (the point of separation) made it possible to predict the outcome of ACS into the progressive stenocardia (end point) with high probability.Conclusion. In the calculation of relative risk, were taken into account the concentration ranges that were most frequently encountered among the number of patients. This methodological approach allowed to significantly increase the reliability of the prognosis, thus we reasonably recommend the use of the proposed flowchart in the clinical practice
Economic damage caused by primary invalidity of the population of Dagestan Republic
The research of age peculiarities in Dagestan showed that in the structure prevail disabled individuals of young age. The invalidity level among urban population is higher, there are more disabled people of the second group of invalidity, it is equal to 48%. Divided by types of diseases the first place with 36.5% take individuals recognized as disabled due to the disease of blood circulatory system. Total economic damage caused by the disease of blood circulatory system is equal to 223 000 000 rub. per year
CLINICAL EVALUATION OF VOICE IN CHILDREN WITH DYSPHONIA
Objective: make a comparative analysis of the voice of the index (VHI) and endoscopic studies in the pathology of the voice of children.Material and Methods: The study involved 87 children, from 6 months to 14 years. The clinical study included fibrorinolaryngoscopy with video fixation (C-MAC, K. Storz, Γ-2,7 mm) and a survey of parents (or guardians) of children at the Uzbek version of the voice handicap index (pVHI), with sub- sections (functional - F, the physical - P and emotional - E).Results: endoscopic diagnosed laryngitis 20,6% (n = 18), functional 16,1% (n = 14) and mutational 9,2% (n = 8), dysphonia, benign larynx (respiratory papillamatosis) - 18.4 % (n = 16), the vocal cords paresis - 5.7% (n = 5), the vocal folds nodules 26.4% (n = 23), throat structure anomalies (acquired) 3,4% (n = 3). The survey revealed pVHI average points total pVHI and its sub-group of patients were as follows: F - 13,94, P - 15.48, E - 12.15 and T - 41.58, which is significantly higher than the group of healthy children and children with functional disorders of voice.Conclusions: Fibrolaryngoscopy with the smallest diameter of the lumen of the laryngoscope with video fixation allowed to quickly ascertain the diagnosis of organic disease of the larynx in children. The objective of the study: to investigate the changes in preferable coping-strategies during treatment of the school-age children in a TB sanatorium. 77 patients (children and adolescents) were enrolled into the study, they all had rehabilitation due to respiratory tuberculosis. All patients were divided into two groups: 8-12 years old (17 boys and 13 girls) and 13-17 years old (24 boys and 23 girls). Coping strategies in the school-age children were investigated twice: at admission and discharge from the sanatorium, using a questionnaire adapted by N.A. Sirota and V.N. Yaltonsky and modified by R. M. Granovskaya and I.M. Nikolskaya. Analysis of the structure of the responses identified a group of patients who were oriented in the direction from the problem. This group presented a lower number of scores of the problem solution scale and a higher number of scores in the scales of avoidance, denial, fantasy formation. The other group of patients, focused in the direction towards the problem, characterized by a higher number of scores of the scales of problem solution and communication and a lower number of scores for the scales of denial and fantasy formation. Statistically significant changes were observed in the scale assessing communication (p = 0.03); there was a statistically significant reduction in the scores reflecting avoidance and denial (p < 0.001). Microparticle movements in optical funnels and pods
Three-dimensional microparticle movements induced by laser beams with a funnel-and tubular pod-like structure, in the neighbourhood of the focal plane of an optical trapping setup, are experimentally studied. The funnel and pod beams constructed as coherent superpositions of helical Laguerre-Gaussian modes are synthesized by a computer generated hologram using a phase-only spatial light modulator. Particle tracking is achieved by in-line holography method which allows an accurate position measurement. It is experimentally demonstrated that the trapped particle follows different trajectories depending on the orbital angular momentum density of the beam. In particular applying the proposed pod beam the particle rotates in opposite directions during its movement in the optical trap. Possible applications of these single-beam traps for volumetric optical particle manipulation are discussed. SYNTHESIS AND STUDY OF BIMETALLIC CATALYTIC SYSTEMS FORMED IN SITU BY ALUMINUM, 1, 2-DICHLOROETHANE AND Fe (III), Ni (II), Mn (II) CHLORIDES
Novel bimetallic catalytic systems based on the catalytic complex (CTC) are synthesized from the reaction of Al metal and 1, 2-dichloroethane in the mode of in situ and modified with the metal chlorides of Mn (II), Fe (III), and Ni (II) in paraffin medium. The structures of synthesized catalytic samples were confirmed by bulk and surface characterization with FTIR, X-ray diffraction (XRD), thermal analyses (TG/DTG/DTA), dynamic light scattering spectroscopy (DLS) for determination the particle size and X-ray fluorescence microscopy (XRFM)
