Short and long term follow-up of patients with transient left ventricular apical ballooning
Background: Transient left ventricular apical ballooning (TLVAB) was described in Japan
as the syndrome characterized by balloon-like left ventricular wall abnormalities without
accompanying coronary atherosclerosis. The aim of the study was to evaluate the short and
long-term follow-up of our patients with TLVAB.
Methods: The population of 1869 patients with the preliminary diagnosis of acute coronary
syndrome was screened. We collected data regarding clinical course, physical or emotional
trauma, ECG abnormalities, presence of balloon-like left ventricular abnormality. During
follow-up, the data on cardiac events (death, recurrence of chest pain, re-hospitalization) were
collected. Serial echocardiographic scans were performed to follow the TLVAB regression.
Results: Thirty (1.6%) patients (only women, mean age 72 ± 12 y) showed characteristic left
ventricle abnormalities. There was an evidence of significant trauma (40% physical and 60%
emotional) in every patient. ECG analysis showed negative T waves in anterior leads in 93%
and ST elevation in 7% of the patients. Maximal MB creatine kinase and troponin I levels
were 38 ± 12 U/l and 1.12 ± 0.75 ng/dl, respectively. The clinical course during hospitalization
was benign in 96% and only in one patient (4%) the pulmonary oedema occurred. During
follow-up (6-24 months, mean 12 months), there were two cases of chest pain recurrence, but
without clinical features of TLVAB (examined by echo or angiography). Mean time for left
ventricular abnormalities regression was 3 ± 1 months.
Conclusions: The transient left ventricular apical ballooning is a benign syndrome mimicking
acute coronary syndrome with good long term outcome
Probing the role of a non-thermal plasma (NTP) in the hybrid NTP catalytic oxidation of methane
Three recurring hypotheses are often used to explain the effect of non-thermal plasmas (NTPs) on NTP catalytic hybrid reactions; namely, modification or heating of the catalyst or creation of new reaction pathways by plasma-produced species. NTP-assisted methane (CH4) oxidation over Pd/Al2O3 was investigated by direct monitoring of the X-ray absorption fine structure of the catalyst, coupled with end-of-pipe mass spectrometry. This in situ study revealed that the catalyst did not undergo any significant structural changes under NTP conditions. However, the NTP did lead to an increase in the temperature of the Pd nanoparticles; although this temperature rise was insufficient to activate the thermal CH4 oxidation reaction. The contribution of a lower activation barrier alternative reaction pathway involving the formation of CH3(g) from electron impact reactions is proposed
Organic chemistry of NH<sub>3</sub> and HCN induced by an atmospheric abnormal glow discharge in N<sub>2</sub>-CH<sub>4</sub> mixtures
The formation of the chemical products produced in an atmospheric glow discharge fed by a N2-CH4 gas mixture has been studied using Fourier Transform InfraRed (FTIR) and Optical Emission Spectrometry (OES). The measurements were carried out in a flowing regime at ambient temperature and pressure with CH4 concentrations ranging from 0.5% to 2%. In the recorded emission spectra the lines of the second positive system CN system and the first negative system of N2 were found to be the most intensive but atomic Hα, Hβ, and C (247 nm) lines were also observed. FTIR-measurements revealed HCN
and NH3 to be the major products of the plasma with traces of C2H2. These same molecules have been detected in Titan's atmosphere and the present experiments may provide some novel insights into the
chemical and physical mechanisms prevalent in Titan's atmosphere with these smaller species believed to be the precursors of heavier organic species in Titan's atmosphere and on its surface
Bezpośrednia oraz odległa ocena chorych z przemijającym balotowaniem koniuszka lewej komory
Wstęp: Przemijające balotowanie koniuszka lewej komory (TLVAB) opisano po raz pierwszy
w Japonii. W zespole tym występują zaburzenia kurczliwości ściany lewej komory serca współistniejące
z prawidłowym obrazem tętnic wieńcowych
Metody: Badana populacja składała się z 1869 osób (leczonych w latach 2001-2004),
u których rozpoznano początkowo ostry zespół wieńcowy. Zgromadzono dane dotyczące urazów
fizycznych i emocjonalnych, nieprawidłowości w badaniach EKG oraz obecności zaburzeń
kurczliwości lewej komory o charakterze balotowania (wentrykulografia lub echokardiografia).
W czasie obserwacji oceniano występowanie incydentów sercowych (zgon, nawrót bólu
w klatce piersiowej, ponowna hospitalizacja) oraz wykonywano badania echokardiograficzne
w celu obserwowania regresji TLVAB.
Wyniki: W badanej populacji zidentyfikowano 30 (1,6%) osób (wyłącznie kobiety, śr. wieku
72 ± 12 lat), u których wykryto opisywane zaburzenia kurczliwości lewej komory. U wszystkich
chorych z badanej grupy wystąpił przed hospitalizacją istotny uraz (u 40% fizyczny,
u 60% emocjonalny). W EKG u 93% pacjentów stwierdzono ujemne załamki T w odprowadzeniach
przedsercowych, u 7% uniesienia odcinka ST. Najwyższe stężenia izoenzymu MB kinazy
kreatynowej oraz troponiny I wynosiły odpowiednio 38 ± 12 j./l oraz 1,12 ± 0,75 ng/dl. Niepowikłany
przebieg hospitalizacji charakteryzował 96% osób, jedynie u 1 chorej (4%) rozwinął się
obrzęk płuc. W czasie obserwacji (6-24 miesięcy, śr. 12 miesięcy) w 2 przypadkach wystąpił nawrót
bólu w klatce piersiowej, jednak bez wykładników klinicznych TLVAB (echokardiografia lub
angiografia). Średni czas do ustąpienia zaburzeń lewej komory wynosił 3 ± 1 miesiące.
Wnioski: Przemijające balotowanie koniuszka lewej komory jest przebiegającym łagodnie
zespołem chorobowym, z korzystnym długoterminowym rokowaniem, naśladującym ostry
zespół wieńcowy. Większość przypadków choroby wystąpiła jesienią lub zimą, co pozwala
nazwać TLVAB chorobą "sezonową"

W pracy przedstawiono analizę obrazowania Paryża – jednego z najważniejszych europejskich ośrodków kulturowych międzywojnia, które często pojawia się w literaturze modernistycznej – w dwóch powieściach: Źródle samotności (1928) autorstwa Radclyffe Hall i Good Morning, Midnight (1939) Jean Rhys. Rozdział pierwszy, który skupia się na rosnącym znaczeniu metropolii – a przede wszystkim Paryża – w pierwszej połowie dwudziestego wieku, porusza zagadnienie rozwoju miast oraz przytacza historię stolicy Francji i jej zasadniczą rolę w sztuce nowoczesnej. Omawia on także różne podejścia do obrazowania miasta w prozie i poezji modernistycznej oraz kluczowe reprezentacje Paryża w międzywojennej literaturze anglojęzycznej. Rozdział drugi poświęcony jest queerowemu Paryżowi w powieści Hall, który, mimo że pozwala jej bohaterce eksplorować swoją tożsamość płciową i seksualność, ostatecznie karze Stephen za odstępstwo od normy. Podkreślone zostają tu także podziały klasowe, które zmuszają ją do wyboru między swoją pozycją społeczną a seksualnością. Rozdział trzeci dotyczy powieści Good Morning, Midnight Rhys i skupia się na podziale wśród paryskich przyjezdnych na światowych turystów i obcych na wygnaniu. Śledząc doświadczenia głównej bohaterki, analiza powieści kładzie nacisk na to, jak niepewna pozycja społeczna Sashy, a także jej płeć, dystansują ją obydwu grup i sprawiają, że miasto staje się dla niej niebezpieczne. Celem pracy jest pokazanie, że obydwie powieści traktują mit „miasta świateł” sceptycznie: zamiast zgłębiać życie kulturowe stolicy Francji, Hall i Rhys podkreślają przede wszystkim napięcia społeczne, które dzielą Paryż i alienują bohaterki omawianych powieści.The thesis analyses the portrayals of Paris – one of the key European cultural centres of the interwar period, frequently featured in modernist literature – in two novels: Radclyffe Hall’s The Well of Loneliness (1928) and Jean Rhys’s Good Morning, Midnight (1939). Its first chapter discusses the growing significance of the metropolis in general and Paris in particular in the first half of the twentieth century, exploring the development of urban centres, the history of the French capital and its centrality to modernist movements. The chapter also provides an overview of the various approaches to the representation of the cityscape in modernist writing and the portrayals of Paris in interwar literature written in English. The second chapter is devoted to Hall’s portrayal of Paris as a queer city, which, though it enables the protagonist Stephen to explore her gender and sexuality, ultimately penalises her deviation from the norm. The analysis also emphasises the city’s class divisions, examining Stephen’s attempts to negotiate between her social status and sexuality. The third chapter, which focuses on Jean Rhys’s Good Morning, Midnight, explores two modes of being a foreigner in Paris, as either a cosmopolitan tourist or a stranger in exile. Following the experiences of the novel’s protagonist, it highlights how Sasha’s uncertain social position, as well as her gender, distances her from both groups, making Paris dangerous for her. The thesis argues that both novels approach the myth of “the city of lights” with a dose of scepticism: rather than explore the cultural life of the French capital, Hall and Rhys highlight the social tensions that run through the city, alienating their protagonists
