Abordagem psicológica da obesidade mórbida: Caracterização e apresentação do protocolo de avaliação psicológica
A obesidade mórbida (OM) é uma versão patológica
de obesidade considerada um grave problema de
saúde para os indivíduos que dela sofrem. O seu tratamento
deve ser baseado numa abordagem biopsicossocial
do indivíduo e do seu processo de doença
que assegure o êxito do tratamento, a curto e longo
prazo. A abordagem psicológica de cariz cognitivocomportamental
estrutura-se ao longo das 3 principais
fases do processo de tratamento: (1) pré-cirurgia, (2)
internamento e (3) pós-cirurgia e follow-up. A primeira
fase desta abordagem corresponde à avaliação psicológica dos doentes candidatos a cirurgia. Neste estudo
apresenta-se a caracterização psicológica da
população com OM, avaliada entre Fevereiro de 2002
e Maio de 2004, duma consulta de especialidade hospitalar.
A metodologia desenvolvida inclui uma entrevista
clínica semi-estruturada e questionários de auto-
-avaliação das dimensões de personalidade (MCMI-
-II), ansiedade e depressão (HADS), qualidade de vida
(MOS-SF/20) e auto-conceito (ICAC). Foram avaliados
212 pacientes com uma média de idades de 41,1
anos e um IMC de 46,6Kg/m2. Apresentam-se os resultados
obtidos e o protocolo de avaliação psicológica
desenvolvido. Salienta-se a importância da avaliação
psicológica para parecer técnico e os seus dados qualitativos
para delineamento dos objectivos psicoterapêuticos
e psico-educativos, como modo imprescindível
ao sucesso deste processo de tratamento.Morbid obesity (MO) is a pathological type of
obesity considered a serious health problem to the
individuals who have it. Its treatment should be based
on a biopsychosocial model of the individual and his/
her illness process for assuring short and long term
treatment success. The psychological approach, cognitive-
behaviour based, is structured along the 3 main
phases of the treatment process: (1) pre-surgery; (2)
inpatient; (3) post-surgery and follow-up. The first
phase of this approach corresponds to the psychological
assessment of the patients that are candidates to
surgery. This study presents the psychological characterization
of the MO population, assessed between
February 2002 and May 2004, of a hospital specialized
consultation. The developed methodology includes a
semi-structured interview and self-assessment questionnaires
of personality (MCMI-II), anxiety and depression
(HADS), quality of life (MOS-SF/20) and
self-concept (ICAC). 212 patients were assessed with
a mean age of 41.1 years old, and a BMI of 46.6Kg/m2.
We present the data and the protocol we developed for
psychological assessment. We emphasize the importance
of the psychological assessment for technical
advice before surgery and its qualitative data for designing
the psycho-educational and psychotherapeutic
objectives, as fundamental to the success of the treatment

Gaps in care and support for patients with advanced breast cancer: a report from the Advanced Breast Cancer Global Alliance
PURPOSE
Although new therapeutic options continue to improve disease-related outcomes in advanced breast cancer (ABC), enhanced focus is needed to improve quality of life for patients currently living with ABC.
METHODS
In November 2019, a multidisciplinary workshop to explore patient perceptions of their information and support needs was held at the ABC Global Alliance Annual Meeting in Lisbon, Portugal. Ninety-two attendees from 27 countries participated in the workshop.
RESULTS
Several key unmet needs were identified and discussed in the workshop, including the following: (1) Significant patient knowledge gaps exist related to the diagnosis and management of ABC, and the availability of patient-focused information to support these gaps in knowledge remains limited. (2) The development of meaningful relationships between patients and health care professionals, and the role of patients in decision making, is often overlooked for patients with ABC. (3) Multidisciplinary care approaches are crucial for patients with ABC; however, these often lack effective coordination. (4) Access to clinical trials for ABC also remains limited. (5) Caregivers, friends, and family members do not receive sufficient guidance to support patients with ABC and manage their own well-being.
CONCLUSION
The variety of unmet needs explored in the workshop demonstrates that patients with ABC still face considerable challenges related to quality of care and support, which will not be resolved until tangible action is taken. Issues highlighted in the workshop should be prioritized by working groups to shape the development of community-based solutions. There is a need for the global community to act proactively to maximize awareness of these ongoing unmet needs and existing resources, while socializing and building new initiatives and resources that will help to close these gaps for patients
Psychosocial morbidity and its correlates in cancer patients of the Mediterranean area: findings from the Southern European Psycho-Oncology Study
Addressing disparities and challenges in underserved patient populations with metastatic breast cancer in Europe
People with metastatic breast cancer face many challenges and disparities in obtaining optimal cancer care. These challenges are accentuated in underserved patient populations across Europe, who are less likely to receive quality healthcare for reasons including socioeconomic inequalities, educational or cultural status, or geographic location. While there are many local and national initiatives targeted to address these challenges, there remains a need to reduce disparities and improve access to healthcare to improve outcomes, with a focus on multidisciplinary stakeholder engagement. In October 2019, a range of experts in metastatic breast cancer, including healthcare professionals, patient representatives, policymakers and politicians, met to discuss and prioritize the critical needs of underserved patient populations with metastatic breast cancer in Europe. Six key challenges faced by these communities were identified: the need for amplification of the metastatic breast cancer patient voice, better and wider implementation of high-quality guidelines for metastatic breast cancer, more collaboration between stakeholders, tailored support for patients from different cultural and ethnic backgrounds, improved data sharing, and work-related issues. The Expert Panel then conceived and discussed potential actionable goals to address each key challenge. Their conclusions present a set of interrelated approaches to address the different challenges and could serve as the basis for concerted improvement of the lives of patients with metastatic breast cancer in Europe
VALIDATION OF THE CALM MODEL, A BRIEF PSYCHOTHERAPEUTIC INTERVENTION, FOR ABC PATIENTS IN THE PORTUGUESE CONTEXT: A SPARC MBC CHALLENGE PROJECT
Caracterização desenvolvimentista de significações de doença e de confronto em mulheres com cancro da mama
Tese de mestrado em Psicologia (Psicoterapia e Psicologia da Saúde) apresentada à Universidade de Lisboa através da Faculdade de Psicologia e de Ciências da Educação, 2002INTRODUÇÃO: Os estudos científicos sobre as consequências psicológicas do
cancro da mama nas mulheres permitem retirar quatro conclusões genéricas: (a) as
significações pessoais sobre a doença ou a forma como a pessoa avalia o seu
processo de doença e de tratamento estão intimamente ligadas ao tipo e à
intensidade das reacções emocionais; (b) as reacções emocionais típicas das
pacientes com cancro da mama são as reacções de ansiedade, ligadas a
significações de ameaça e de incerteza e as reacções de tristeza ou depressão,
associadas a significações de perda percebida; (c) os processos de confronto
psicológico influenciam não apenas a qualidade da adaptação ao processo de
doença, podendo aumentar ou diminuirá intensidade das reacções; emocionais, mas
também parecem ter alguma influência no tempo de sobrevivência'; (d) a qualidade
de vida é uma dimensão importante e deve ser valorizada e promovida durante o
tempo de vida das doentes. Contudo, estes estudos podem criticar-se a dois níveis:
(1) apresentam uma grande diversidade de conceitos para se referirem às
significações e aos processos de confronto das mulheres, tornando difícil fazer
estudos comparativos e, (2) em geral carecem de um enquadramento teórico que
permita compreender melhor o que está em jogo, no processo de vivência da
doença, do ponto de vista cognitivo. Assim, neste trabalho será apresentada uma
conceptualização cognitivista e desenvolvimentista das significações de doença e
dos processos de confronto concomitantes. Procura-se, fazer uma integração destas
significações em modelos cognitivos e desenvolvimentistas, que emergiram no
âmbito da Psicologia da Saúde, da Psicoterapia Cognitiva e da Psicologia do
Desenvolvimento sócio-cognitivo.
OBJECTIVOS: 1) aceder às significações de doença, avaliadas de acordo com os
atributos de representação de doença de Leventhal (i.e., identidade, causas, curso
temporal, consequências e controlabilidade); 2) analisar os conteúdos ou temas de
significação, relacionados com reacções emocionais de ansiedade e de depressão, e
sua distribuição na amostra; 3) ordenar as significações de doença de acordo com
critérios do desenvolvimento sócio-cognitivo e propor sequências hierárquicas de
significação; 4) aceder às significações de confronto ou operações dialécticas
utilizadas pelas mulheres com cancro da mama e analisar correspondências entre
aquelas e os níveis de significação da doença; 5) analisar correlações entre
operações dialécticas, reacções emocionais, e dimensões de qualidade de vida,
auto-avaliadas em questionário nos dois momentos de avaliação (no momento da
entrevista e 6 meses depois); 6) analisar correlações entre níveis de significação da doença, reacções emocionais, e dimensões de qualidade de vida, auto-avaliadas em
questionário nos dois momentos de avaliação.
MÉTODO: Foram seleccionadas aleatoriamente 52 pacientes com cancro da mama
da Consulta Multidisciplinar de Cirurgia da Mama do Hospital S. José em Lisboa.
Para aceder às significações das pacientes, foi efectuada uma entrevista individual
semi-estruturada e avaliada a ansiedade, depressão e qualidade de vida através de
escalas clínicas (Self Anxiety Scale, Self Rating Depression Scale, e MOS-Short
Form). As transcrições das entrevistas foram submetidas a uma análise de conteúdo
temática e desenvolvimentista das significações de doença, e os dados qualitativos
e quantitativos foram submetidos a uma análise estatística para testar as hipóteses
colocadas.
CONCLUSÕES: Ao nível da representação da doença, foi possível, por um lado,
caracterizar um perfil típico do cancro da mama de acordo com os atributos
definidos por Leventhal e, por outro, identificar os temas de significação
relacionados com as reacções emocionais de ansiedade e de depressão. Do ponto de
vista do desenvolvimento sócio-cognitivo foi possível ordenar as significações de
doença das pacientes em diferentes níveis de desenvolvimento, verificando-se que
estas funcionam preferencialmente em níveis inferiores e intermédios de
significação da doença. Foi proposta uma nova hierarquia desenvolvimentista de
significação para o atributo das consequências da doença. Em termos da análise
estatística entre as variáveis propostas nos objectivos, constatou-se uma tendência
para uma correspondência entre níveis superiores de significação da doença e
operações dialécticas de restruturação compensatória, e uma correspondência entre
níveis inferiores e operações dialécticas de destruturação. Em termos da relação
entre níveis de significação e intensidade das reacções emocionais e qualidade de
vida, verificou-se que as mulheres que funcionam em níveis superiores significação
da doença têm tendência a apresentar valores mais baixos de depressão e melhor
qualidade de vida do que as dos níveis inferiores. Finalmente, em termos da
relação entre o tipo de confronto (operação dialéctica) e a intensidade das reacções
emocionais e da qualidade de vida, verificou-se que algumas operações dialécticas
apresentam correspondências com a intensidade das reacções emocionais e a
qualidade de vida.INTRODUCTION: The scientific studies on the psychological consequences of
breast cancer on women allow four general conclusions: a) illness subjective
meanings or the way in which the person appraises his/her illness process and
treatments are closely tied to the type and intensity of emotional reactions; b) the
typical emotional reactions are anxiety reactions, linked to subjective meanings of
threat and uncertainty, and depression reactions linked to subjective meanings of
perceived loss; c) the psychological coping processes influence not only the quality
of adaptation to the illness process, increasing or decreasing the emotional reactions
intensity, but also seem to have some influence on survival; d) quality of life is an
important dimension and should be valued and promoted during patients lives.
However these studies can be criticized at two levels: 1) they use an array of
different concepts to refer to the women's subjective meanings and their coping
process making it difficult to make comparison studies; 2) in general they are not
sustained by a theoretical framework that would enable, a better understanding of
what is at stake from a cognitive point of view in the women's experience of facing
and dealing with their illness. In this study a cognitive-developmental framework of
illness subjective meanings and coping processes is presented. An integration of
these subjective meanings into a cognitive-developmental framework that emerged
from Health Psychology, Cognitive Psychotherapy and Socio-Cognitive
Developmental Psychology it is proposed.
OBJECTIVES: 1) To get patients' illness subjective meanings, assessed through
Leventhal major attributes of illness representations (i.e., identity, causes, duration,
consequences and controllability or cure); 2) analyse the content or themes of
subjective meanings concerning emotional reactions of anxiety and depression and
its distribution on our sample; 3) to order illness subjective meanings accordingly to
socio-cognitive developmental criteria and to propose new hierarchical sequences of
subjective meanings; 4) to get coping processes or dialectical actions used by
breast cancer women and analyse correspondences between these and the levels of
illness subjective meanings; 5) analyse correspondences between dialectical actions,
emotional reactions and quality of life dimensions self-assessed by questionnaires
on two moments of assessment (on the interview moment and 6 months later); 6)
analyse correspondences between levels of illness subjective meanings and
emotional reactions and quality of life dimensions.
METHOD: Fifty-two breast cancer outpatient women from Breast Surgery
Multidisciplinary Team Consultation at Hospital S. José, Lisboa, were randomly
selected and submitted to a semi-structured interview to obtain their illness subjective meanings and coping processes and also filled out clinical scales to
evaluate anxiety (Zung Self Anxiety Scale), depression (Zung Self Rating
Depression Scale) and quality of life (MOS-Short Form). The interview material was
submitted to a thematic and developmental content analysis, and the qualitatitive
and quantitative data went through a statistical analysis to test the hypothesis.
CONCLUSIONS: It was possible to characterize a typical profile of breast cancer
representation, following Leventhal major attributes of illness representation, and to
identify the themes of the subjective meanings linked with the emotional reactions
of anxiety and depression. From a socio-cognitive developmental perspective, it was
possible to order patients' illness subjective meanings into different developmental
levels, verifying that the majority tend to use mainly low and intermediate levels. It
was proposed a new hierarchical developmental sequence for the attribute
consequences of illness. In the statistical analysis between the variables we found a
tendency for a correspondence between superior levels of illness subjective
meanings and restructuring dialectical actions, and a correspondence between lower
levels and destructuring dialectical actions. We also found that women that use
higher levels of illness subjective meanings tend to have lower levels of depression
and better quality of life than the ones that use lower levels. Finally, we found that
there are some dialectical actions that have a significant correspondence with the
intensity of emotional reactions and quality of life
ONCORELIEF: a H2020 project supporting the development of an App and AI system for assisting cancer patients' improvement of wellbeing and quality of life
Nowadays technology can assist professionals and patients by using Big Data and Artificial Intelligence in monitoring in real time patients’ health condition. The ONCORELIEF project (www.oncorelief.eu) is a H2020 funded project, with a consortium of 13 partners from 8 European countries, aiming to validate in real-world settings the development of a patient-centered AI system which inputs, analyses and integrates big datasets from the biopsychosocial and environmental dimensions of patients with cancer, to improve post-treatment health status, increase their wellbeing and QoL (quality of life), and assist the follow-up care of cancer patients

Emotional distress, brain functioning, and biobehavioral processes in cancer patients: a neuroimaging review and future directions
Despite emerging evidence that distress and adversity can contribute to negative health outcomes in cancer, little is known about the brain networks, regions, or circuits that can contribute to individual differences in affect/distress states and health outcomes in treated cancer patients. To understand the state-of-the-science in this regard, we reviewed neuroimaging studies with cancer patients that examined the associations between negative affect (distress) and changes in the metabolism or structure of brain regions. Cancer patients showed changes in function and/or structure of key brain regions such as the prefrontal cortex, thalamus, amygdala, hippocampus, cingulate cortex (mainly subgenual area), hypothalamus, basal ganglia (striatum and caudate), and insula, which are associated with greater anxiety, depression, posttraumatic stress disorder (PTSD) symptoms, and distress. These results provide insights for understanding the effects of these psychological and emotional factors on peripheral stress-related biobehavioral pathways known to contribute to cancer progression and long-term health outcomes. This line of work provides leads for understanding the brain-mediated mechanisms that may explain the health effects of psychosocial interventions in cancer patients and survivors. A multilevel and integrated model for distress management intervention effects on psychological adaptation, biobehavioral processes, cancer pathogenesis, and clinical outcomes is proposed for future research
