    Change in abdominal, but not femoral subcutaneous fat CT-radiodensity is associated with improved metabolic profile after bariatric surgery

    Background and aims: Computed tomography (CT)-derived adipose tissue radiodensity represents a potential noninvasive surrogate marker for lipid deposition and obesity-related metabolic disease risk. We studied the effects of bariatric surgery on CT-derived adipose radiodensities in abdominal and femoral areas and their relationships to circulating metabolites in morbidly obese patients. Methods and results: We examined 23 morbidly obese women who underwent CT imaging before and 6 months after bariatric surgery. Fifteen healthy non-obese women served as controls. Radiodensities of the abdominal subcutaneous (SAT) and visceral adipose tissue (VAT), and the femoral SAT, adipose tissue masses were measured in all participants. Circulating metabolites were measured by NMR. At baseline, radiodensities of abdominal fat depots were lower in the obese patients as compared to the controls. Surprisingly, radiodensity of femoral SAT was higher in the obese as compared to the controls. In the abdominal SAT depot, radiodensity strongly correlated with SAT mass (r =-0.72, p < 0.001). After surgery, the radiodensities of abdominal fat increased significantly (both p < 0.01), while femoral SAT radio density remained unchanged. Circulating ApoB/ApoA-I, leucine, valine, and GlycA decreased, while glycine levels significantly increased as compared to pre-surgical values (all p < 0.05). The increase in abdominal fat radiodensity correlated negatively with the decreased levels of ApoB/ApoA-I ratio, leucine and GlycA (all p < 0.05). The increase in abdominal SAT density was significantly correlated with the decrease in the fat depot mass (r =-0.66, p = 0.002).Conclusion: Higher lipid content in abdominal fat depots, and lower content in femoral subcutaneous fat, constitute prominent pathophysiological features in morbid obesity. Further studies are needed to clarify the role of non-abdominal subcutaneous fat in the pathogenesis of obesity. Clinical trial registration number: NCT01373892.     El PAP Alter Código, período Primavera 2023, trabajó con los dos proyectos que se han venido trabajando en semestres anteriores: el videojuego A Orillas del río y el documental Déjennos pescar. Ambos proyectos parten de la metodología interdisciplinaria y colaborativa con miembros de la comunidad Cucapá para crear representaciones audiovisuales no estigmatizantes, que detonen el sentido de comunidad y refuercen su acervo cultural. El videojuego A Orillas del río es del género point and click, de vista isométrica, el cual está inspirado en el cuento oral tradicional El zorro y el coyote, que busca ser una herramienta lúdica de aprendizaje para reforzar el aprendizaje de la lengua Cucapá en los niños. Los resultados obtenidos fueron el demo del nivel uno (escenas uno y dos); colorimetría, arcos de personajes principales; programación de minijuegos. Dentro del documental ‘Déjennos pescar’ los resultados fueron un montaje, una clasificación del material grabado con transcripciones de audio, mientras que en la parte de estrategia de impacto se creó un manual de uso de redes sociales con colorimetría, tipografía, estilo de voz, tipo de contenido según la red social, para los futuros integrantes del equipo.ITESO, A.C

    Science and technology are modifying medicine at a dizzying pace. Although access in our country to the benefits of innovations in the area of devices, data storage and artificial intelligence is still very restricted, the advance of digital medicine offers the opportunity to solve some of the biggest problems faced by medical practice and public health in Mexico. The potential areas where digital medicine can be disruptive are: accessibility to quality medical care, centralization of specialties in large cities, dehumanization of medical treatment, lack of resources to access evidence-supported treatments, among others. This review presents some of the advances that are guiding the new revolution in medicine, discusses the potential and potential barriers to implementation, and suggests crucial elements for the path of incorporation of digital medicine in Mexico

    Imaging of cardiac and renal perfusion in a rat model with (13)N-NH 3 micro-PET

    Cardiac dysfunction leads to decreased organ perfusion. We aimed to measure cardiac and renal perfusion simultaneously with the use of (13)N-NH3-microPET in different rat models. Ten male Wistar rats underwent sham surgery (n = 5) or permanent coronary artery ligation to induce myocardial infarction (MI, n = 5). Eleven weeks later (13)N-NH3-microPET scan was performed to study the cardiac and renal perfusion. Cardiac perfusion was significantly reduced in MI group, directly correlated with ejection fraction and inversely correlated with MI size (r = 0.89; p < 0.001 and r = -0.86; p < 0.001 respectively). Renal perfusion showed a notional 17 % non-significant reduction in MI group when compared to sham (3.44 ± 0.40 vs. 4.12 ± 0.48 ml/g/min). There was a trend towards greater reduction of perfusion in cortical than medullar region. Cortex perfusion was negatively correlated with histological changes. (13)N-NH3-microPET may be a potential tool for evaluation of cardiac and renal functional and perfusion changes in presence of cardiac dysfunction in rat models.status: publishe

    Revisión de modelos para el análisis de dilemas éticos

    En la práctica médica pediátrica es frecuente encontrar a pacientes en circunstancias que representan un dilema ético para los profesionales de la salud. Un dilema corresponde a una situación en la que los preceptos morales o las obligaciones de similar obligatoriedad ética se encuentran en conflicto, de forma que cualquier solución posible al dilema es moralmente intolerable. Una revisión de la literatura permitió identificar diferentes modelos que abordan esta clase de dilemas. Se localizaron artículos utilizando las bases de datos Ebsco Host, ProQuest, Ovid e InMex, así como metabuscadores como metacrawler. Algunos de los modelos analizados fueron los siguientes: el Modelo de Anne Davis, el Método de Nijmegen, el Método de Diego Gracia, el Método Integral, el Modelo del Centro de Ética Médica de Bochum, el Modelo de Brody y Payton, el Modelo de Curtin y Flaherty, el Modelo de Thompson y Thompson, la Fórmula SAD, el Modelo de Javier Morata, el Modelo de Elaine Congress, el Modelo IFSW, el Modelo de Loewenberg y Dolgoff, el Modelo de la Ley Social, el Método DOER, el Modelo de Brommer, el Modelo de Corey y Callanan, el Modelo de Pope y Vasquez, el Modelo de Bush, Connell y Denney, el Modelo de Ferrell, Gresham y Fraedrich y el Modelo de Hunt y Vitell. Los criterios compartidos entre los diferentes modelos fueron los siguientes: a) la especificación del dilema ético; b) la descripción de los hechos a considerar; c) la definición de valores, principios y la postura ética que será tomada en consideración; y d) la toma de decisiones con la identificación de alternativas de solución. De acuerdo con la literatura revisada, se explican algunos modelos con el fin de identificar y ejemplificar elementos críticos que pudieran ser utilizados de manera práctica por los Comités de Ética Clínica u Hospitalaria en las instituciones de salud pediátrica en México

    A snapshot of antimicrobial resistance in Mexico. Results from 47 centers from 20 states during a six-month period.

    AIM:We aimed to assess the resistance rates of antimicrobial-resistant, in bacterial pathogens of epidemiological importance in 47 Mexican centers. MATERIAL AND METHODS:In this retrospective study, we included a stratified sample of 47 centers, covering 20 Mexican states. Selected isolates considered as potential causatives of disease collected over a 6-month period were included. Laboratories employed their usual methods to perform microbiological studies. The results were deposited into a database and analyzed with the WHONET 5.6 software. RESULTS:In this 6-month study, a total of 22,943 strains were included. Regarding Gram-negatives, carbapenem resistance was detected in ≤ 3% in Escherichia coli, 12.5% in Klebsiella sp. and Enterobacter sp., and up to 40% in Pseudomonas aeruginosa; in the latter, the resistance rate for piperacillin-tazobactam (TZP) was as high as 19.1%. In Acinetobacter sp., resistance rates for cefepime, ciprofloxacin, meropenem, and TZP were higher than 50%. Regarding Gram-positives, methicillin resistance in Staphylococcus aureus (MRSA) was as high as 21.4%, and vancomycin (VAN) resistance reached up to 21% in Enterococcus faecium. Acinetobacter sp. presented the highest multidrug resistance (53%) followed by Klebsiella sp. (22.6%) and E. coli (19.4%). CONCLUSION:The multidrug resistance of Acinetobacter sp., Klebsiella sp. and E. coli and the carbapenem resistance in specific groups of enterobacteria deserve special attention in Mexico. Vancomycin-resistant enterococci (VRE) and MRSA are common in our hospitals. Our results present valuable information for the implementation of measures to control drug resistance
