    A qualitative study exploring the experience and motivations of UK Samaritan volunteers: "Why do we do it?"

    Telephone helplines offer a valued service for those in distress. However, little research has explored the experience of helpline volunteers. Through semi-structured interviews, in this article we explore the volunteering experiences of nine long-term UK Samaritan volunteers. Interviews were analysed using Interpretive Interactionism. The analysis highlighted that this volunteering impacted participants' experience of their sense of self. The decision to volunteer was framed as part of a search for personal meaning, tied to experiences of loss and reparation. They reflected positively on their volunteer identity, but highlighted tensions between a sense of vocation and the experience of care burden. The Samaritan Community also offered a sense of belonging and social support. They experienced involvement as personally meaningful, enabling the construction of a positive self-identity

    Breast cancer worry in higher-risk women offered preventive therapy: a UK multicentre prospective study

    PURPOSE: Women's worry about developing breast cancer may influence their decision to use preventive therapy. However, the direction of this relationship has been questioned. We prospectively investigated the relationship between breast cancer worry and uptake of preventive therapy. The socio-demographic and clinical factors associated with high breast cancer worry were also investigated. METHODS: Women at increased risk of developing breast cancer were recruited from clinics across England (n = 408). Participants completed a survey on their breast cancer worry, socio-demographic and clinical factors. Uptake of tamoxifen was recorded at 3 months (n = 258 women, 63.2%). Both primary and sensitivity analyses were conducted using different classifications of low, medium and high worry. RESULTS: 39.5% of respondents reported medium breast cancer worry at baseline and 21.2% reported high worry. Ethnic minority women were more likely to report high worry than white women (OR = 3.02, 95%CI 1.02, 8.91, p = 0.046). Women educated below degree level were more likely to report high worry than those with higher education (OR = 2.29, 95%CI 1.28, 4.09, p = 0.005). No statistically significant association was observed between worry and uptake. In the primary analysis, fewer respondents with medium worry at baseline initiated tamoxifen (low worry = 15.5%, medium = 13.5%, high = 15.7%). In the sensitivity analysis, participants with medium worry reported the highest uptake of tamoxifen (19.7%). CONCLUSIONS: No association was observed between worry and uptake, although the relationship was affected by the categorisation of worry. Standardised reporting of the classification of worry is warranted to allow transparent comparisons across cohorts

    Expansive components in H II regions

    We study the presence of low intensity high velocity components, which we have termed wing features in the integrated Halpha emission line profiles of the HII region populations of the spiral barred galaxies NGC 1530, NGC 3359 and NGC 6951. We find that more than a third of the HII region line profiles in each galaxy show these components. The highest fraction is obtained in the galaxy whose line profiles show the best S:N, which suggests that wing features of this type may well exist in most, if not all, HII region line profiles. Applying selection criteria to the wing features, we obtain a sample of HII regions with clearly defined high velocity components in their profiles. Deconvolution of a representative sample of the line profiles eliminates any doubt that the wing features could possibly be due to instrumental effects. We present an analysis of the high velocity low intensity features fitting them with Gaussian functions; the emission measures, central velocities and velocity dispersions for the red and blue features take similar values. We interpret the features as signatures of expanding shells inside the HII regions. Up to a shell radius of R(shell)~0.2R(reg), the stellar winds from the central ionizing stars appear to satisfy the energy and momentum requirements for the formation and driving the shell. Several examples of the most luminous HII regions show that the shells appear to have larger radii; in these cases additional mechanisms may well be needed to explain the kinetic energies and momenta of the shells.Comment: 16 pages, 9 figures, accepted for publication in A&

    Artículo de publicación ISIEruptions of basaltic material in small-scale volcanic fields located in intraplate settings display a very diverse range in physical and chemical characteristics. Despite its relevance to the understanding of volcanic hazards, the relationship between physical properties of eruptions (explosivity, volume, location) and chemical composition of erupted products has, to date, not been investigated. Here we present a relationship between mantle heterogeneity and extents of partial melting, and both erupted volumes and eruptive style from the Auckland Volcanic Field (New Zealand), and we suggest that this provides a general model for small-scale "monogenetic" magmatic systems globally. Small volcanic centers consistently take the form of nephelinitic tuff rings and scoria cones, whereas larger centers are produced from effusive eruptions of less alkalic magmas. Nephelinitic melts are generated by melting of a deep, carbonated source, whereas less alkalic melts are the products of melting of a shallower, noncarbonated source. U-Th-Ra isotope data from eruptions closely paired in space and time show that mixing between magmas is extremely limited as a consequence of different ascent mechanisms due to differential segregation of melts from varying sources (early, carbonated melts ascending by higher porosity channels, and later, uncarbonated melts by a more diffusive regime). This suggests that extraction of melt is nearly instantaneous in these environments.     Variable emergence of Autism Spectrum Disorder symptoms from childhood to early adulthood

    OBJECTIVE: Autism spectrum disorder (ASD) is currently considered an early-onset, neurodevelopmental condition. Follow-up studies of clinic-ascertained autism suggest that autistic symptoms typically decline with age, although symptom improvement is limited for some. To date there have been no population-based prospective studies investigating the natural history of autistic symptoms from childhood to adulthood. This study aimed to characterize the development and heterogeneity of autistic symptoms in a UK population-based cohort from childhood to age 25 years. METHOD: Data were analyzed in a prospective UK population-based cohort (ALSPAC). Trajectories were derived using five-assessments of parentrated Social Communication Disorders Checklist (SCDC) spanning ages 7-25 years. Additional measures were used to validate symptom trajectories. RESULTS: We identified three distinct SCDC trajectory classes: low (88.5%), declining (5.0%) and late‐emerging (6.5%). Both the declining and late-emerging classes were associated with child and adult ASD measures, low IQ, communication problems, peer problems and worse adult functioning, compared to the low class. Male sex was associated with an increased likelihood of being in the declining trajectory class (OR=2.84, 95% CI=2.19-3.69). This sex difference was not observed in the late-emerging group (OR=1.00, 95% CI=0.80-1.24) compared to the low class. CONCLUSIONS: ASD symptom levels emerged early and tended to decline across development although impairment was still present in adulthood for some. For others, autistic symptoms emerged across adolescence and adulthood. This challenges our current understanding that ASD symptoms inevitably first manifest early in development

    Effect of national guidance on survival for babies born at 22 weeks’ gestation in England and Wales: population based cohort study

    Objectives To explore the effect of changes in national clinical recommendations in 2019 that extended provision of survival focused care to babies born at 22 weeks’ gestation in England and Wales. Design Population based cohort study. Setting England and Wales, comprising routine data for births and hospital records. Participants Babies alive at the onset of care in labour at 22 weeks+0 days to 22 weeks+6 days and at 23 weeks+0 days to 24 weeks+6 days for comparison purposes between 1 January 2018 and 31 December 2021. Main outcome measures Percentage of babies given survival focused care (active respiratory support after birth), admitted to neonatal care, and surviving to discharge in 2018-19 and 2020-21. Results For the 1001 babies alive at the onset of labour at 22 weeks' gestation, a threefold increase was noted in: survival focused care provision from 11.3% to 38.4% (risk ratio 3.41 (95% confidence interval 2.61 to 4.45)); admissions to neonatal units from 7.4% to 28.1% (3.77 (2.70 to 5.27)), and survival to discharge from neonatal care from 2.5% to 8.2% (3.29 (1.78 to 6.09)). More babies of lower birth weight and early gestational age received survival focused care in 2020-21 than 2018-19 (46% to 64% at <500g weight; 19% to 31% at 22 weeks+0 days to 22 weeks+3 days). Conclusions A change in national guidance to recommend a risk based approach was associated with a threefold increase in 22 weeks’ gestation babies receiving survival focused care. The number of babies being admitted to neonatal units and those surviving to discharge increased

    "There's not enough knowledge out there": examining older adults' perceptions of digital technology use and digital inclusion classes

    Older adults' definitions of digital technology, and experiences of digital inclusion sessions, were examined using qualitative approaches. Seventeen older adults (aged between 54 and 85) participated in two focus groups that each lasted approximately 90 minutes to explore how older adults understood technology within their lived experience. Interpretative Phenomenological Analysis yielded two main themes: Thirst for knowledge and a wish list for digital technology sessions. A separate content analysis was performed to identify what technology older adults identified as digital technology. This analysis revealed that the older adults most frequently defined digital technology as computers and telephones. The findings support the conclusions that this group of older adults, some of whom were 'successful users', have a wide knowledge of digital technology, are interested in gaining more skills, and desire knowledge acquisition through personalised one-to-one learning sessions

    Teacher and youth priorities for education for environmental sustainability: A co-created manifesto

    What would it mean to put environmental sustainability at the heart of education? This article describes a process of inclusive, participatory manifesto-making to identify young people's (aged 16–18 years) and teachers' priorities for education for environmental sustainability across the UK. Drawing on analysis of qualitative data from over 200 teachers and young people who participated in futures and visualisation workshops, we identify key educational priorities at the levels of classroom, school, community and policy, based on consensus between teacher and youth perspectives. Whilst consensus-seeking comes with a risk of favouring ‘soft’ actions which reinforce unsustainable practices and systems, the process of identifying more desirable futures and immediate barriers that must be negotiated to reach them has the potential to create spaces for more critical pedagogies and practices. There is a need for policy makers and school leaders to recognise the interests of teachers and young people to enable greater participation in decision making at different scales, and to ensure that those with the greatest capacity bear their burden of responsibility for education for environmental sustainability
