    A new formulation of oral viscous budesonide in treating of paediatric eosinophilic oesophagitis: a pilot study

    OBJECTIVES: Oral viscous budesonide is a recent therapeutic option for eosinophilic oesophagitis (EoE) compared with dietary restriction and inhaled steroids. This single-centre, open-label, not blinded study aims to evaluate the efficacy and safety of a new, preprepared oral viscous budesonide suspension (PVB) in children and adolescents with EoE. METHODS: We treated 36 children with PVB (29 boys; median age 12 years) with EoE diagnosed according to European Society for Paediatric Gastroenterology Hepatology and Nutrition guidelines. Patients <150 and >150 cm height received 2 and 4 mg PVB daily, respectively, for 12 weeks. Upper gastrointestinal endoscopy was performed at baseline, after 12 weeks of therapy and 24 weeks after the end of therapy. Baseline and post-treatment scores were calculated for symptoms, endoscopy, and histology. Serum cortisol was performed at baseline, 12, and 36 weeks. RESULTS: At the end of PVB trial, endoscopy showed macroscopic remission in 32 patients (88.9%), whereas at histology median pre- and post-treatment peak eosinophil count/high power field (HPF) markedly decreased from 42.2 (range: 15-100) to 2.9 (range: 0-30); moreover, mean symptom and histology scores impressively improved compared with baseline (Pā€Š<ā€Š0.01). At 24 weeks after the end of PVB therapy, endoscopy showed oesophageal relapse in 21 patients (58.3%), whereas 15 (41.7%) were still in remission. Seven children (19.4%) with positive multichannel intraluminal impedance-pH were treated also with proton pump inhibitors. No significant difference between pre-/post-treatment morning cortisol levels occurred. CONCLUSIONS: The new PVB suspension presented in the present study is effective and safe for treating children with proven EoE. Larger placebo-controlled clinical trials would provide more information about dosing, efficacy, and long-term safety of this formulation, specifically designed for the oesophagus

    Mathematical Structure of Rabi Oscillations in the Strong Coupling Regime

    In this paper we generalize the Jaynes--Cummings Hamiltonian by making use of some operators based on Lie algebras su(1,1) and su(2), and study a mathematical structure of Rabi floppings of these models in the strong coupling regime. We show that Rabi frequencies are given by matrix elements of generalized coherent operators (quant--ph/0202081) under the rotating--wave approximation. In the first half we make a general review of coherent operators and generalized coherent ones based on Lie algebras su(1,1) and su(2). In the latter half we carry out a detailed examination of Frasca (quant--ph/0111134) and generalize his method, and moreover present some related problems. We also apply our results to the construction of controlled unitary gates in Quantum Computation.     Charge dynamics of the Co-doped BaFe2_2As2_2

    We report on a thorough optical investigation over a broad spectral range and as a function of temperature of the charge dynamics in Ba(Cox_xFe1āˆ’x_{1-x})2_2As2_2 compounds for Co-doping ranging between 0 and 18%. For the parent compound as well as for xx=0.025 we observe the opening of a pseudogap, due to the spin-density-wave phase transition and inducing a reshuffling of spectral weight from low to high frequencies. For compounds with 0.051ā‰¤xā‰¤\le x \le 0.11 we detect the superconducting gap, while at xx=0.18 the material stays metallic at all temperatures. We describe the effective metallic contribution to the optical conductivity with two Drude terms, representing the combination of a coherent and incoherent component, and extract the respective scattering rates. We establish that the dcdc transport properties in the normal phase are dominated by the coherent Drude term for 0ā‰¤xā‰¤\le x \le0.051 and by the incoherent one for 0.061ā‰¤xā‰¤\le x \le0.18, respectively. Finally through spectral weight arguments, we give clear-cut evidence for moderate electronic correlations for 0ā‰¤xā‰¤\le x \le0.061, which then crossover to values appropriate for a regime of weak interacting and nearly-free electron metals for xā‰„x\ge0.11

    PIXE and ToF-SIMS analysis of streaker samplers filters

    This paper presents methodological innovations introduced in the characterisation of urban aerosol collected in Italy in a recent campaign. Two complementary ion beam analysis (IBA) techniques were used to analyse Nuclepore filters used in continuous streaker samplers to collect airborne particles in four Italian towns. Na to Pb elemental concentrations were obtained by particle induced X-ray emission (PIXE), while time of flight secondary ion mass spectrometry (ToF-SIMS) produced, on the same samples, time trends for several elements and molecular fragments. In addition, light attenuation measurements were used as a tracer for black carbon. The data produced by these three techniques was merged into a unique data set to address the characterisation of particulate matter sources. Correlations between elemental concentration trends (PIXE) and relative trends for molecular fragments (ToF-SIMS) and black carbon (light attenuation) have been studied by cluster and principal component analysis

    Protein kinase C isoenzymes in human neuroblasts involvement of PKCĪµ in cell differentiation

    AbstractAlthough neuronal cells are a major target of phorbol ester action, the activity of the various protein kinase C (PKC) isoenzymes have not been studied in detail in human neuroblasts. Differentiation of the LAN-5 human neuroblastoma cell line by interferon-Ī³ (IFN-Ī³) is accompanied by a twofold increase in PKC activity. Since PKC is a multigene family, we investigated which isoforms were expressed in control and differentiated cells, and which of these isoenzymes is involved in neuronal differentiation. We found that: (1) PKC activity is higher in differentiated than in undifferentiated cells; (2) RT-PCR analysis showed the expression of mRNA for PKCĪ±, -Ī³, -Ī“ -Īµ and-Ī¶ and the absence of mRNA for Ī² in untreated LAN-5 cells; (3) Western blot evaluation with PKC isoform-specific antibodies showed the same pattern of PKC expression in non-differentiated cells; (4) Expression of PKCĪµ mRNA was significantly enhanced by IFN-Ī³-induced differentiation, while the other isoforms were not affected; (5) Differentiation of LAN-5 cells with IFN-Ī³ or retinoic acid induced overexpression of the PKCĪµ protein, while inhibition of cell proliferation by fetal calf serum starvation was without effect. These findings suggest that expression of PKCĪµ isoform is tightly coupled with neuronal differentiation and may play a role in the maintenance of the differentiated state

    THE MAXIMIZATION OF THE 4TH DIMENSION OF THE BUILDING SITE

    Abstract. The aim of the contribution is to elaborate the digitalization of the construction process and of the management of the construction for both the organization of work activity and safety on the building site. The BIM methodology requires a digital implementation of construction through the creation of virtual models pertaining to the various professions involved. The CoSIM, acronym for Construction Site Information Modelling, is the application of BIM in the building site and where the federated model of the design phase materializes. The goal of the methodology developed is to obtain an elevated quality standard of the project, while trying to maximise costs and time as much as possible to favour an increase in the level of safety and security on the building site. The BSS, Building Site Safety, will allow a maximization of the safety design, not only by graphically depicting work activity but also forming a digital system for the documentation of the building site. To support this, the model is integrated with a simple plug-in which contains a library of objects to use for the designing and planning of safety on site, from general information to state of emergency warning. Thanks to the combination of the CoSIM and the BSS we can have a reduction of the variant, a reduction of time and costs due to an improved planning of work activity and allows the worker to be aware of all the modalities of procedure before entering the building site

    High prevalence of anti-hepatitis e virus antibodies among blood donors in central Italy, february to march 2014

    Prevalence of anti-hepatitis E virus (HEV) antibodies is highly variable in developed countries, which seems partly due to differences in assay sensitivity. Using validated sensitive assays, we tested 313 blood donors attending a hospital transfusion unit in central Italy in January and February 2014 for anti-HEV IgG and IgM and HEV RNA. Data on HEV exposure were collected from all donors. Overall anti-HEV IgG prevalence was 49% (153/313). Eating raw dried pig-liver sausage was the only independent predictor of HEV infection (adjusted prevalence rate ratio = 2.14; 95% confidence interval: 1.23ā€“3.74). Three donors were positive for either anti-HEV IgM (n = 2; 0.6%) or HEV RNA (n = 2; 0.6%); they were completely asymptomatic, without alanine aminotransferase (ALT) abnormalities. Of the two HEV RNA-positive donors (both harbouring genotype 3), one was anti-HEV IgG- and IgM-positive, the other was anti-HEV IgG- and IgM-negative. The third donor was positive for anti-HEV IgG and IgM but HEV RNA-negative. HEV infection is therefore hyperendemic among blood donors (80% men 18ā€“64 years-old) from central Italy and associated with local dietary habits. Nearly 1% of donors have acute or recent infection, implying potential transmission to blood recipients. Neither ALT nor anti-HEV IgM testing seems useful to prevent transfusion-transmitted HEV infection. Ā© 2016, European Centre for Disease Prevention and Control
