Advances toward diagnostic tools for managing zoonotic visceral leishmaniasis
Visceral leishmaniasis (VL) is a life-threatening outcome of Leishmania infantum or Leishmania donovani infection. Dogs are the primary domestic reservoir of L. infantum parasites, and ownership of infected dogs increases the risk of human VL. Controlling infection within dog populations is regarded as critical to VL management in endemic countries, both preventing progression of canine disease and limiting parasite transmission to humans and dogs. Here we discuss various strategies that are used to diagnose canine visceral leishmaniasis (CVL) and the possibilities of adapting these for use within population screening and control programs. In addition, given the variable transmissibility of L. infantum to the sand fly vector, we outline some possibilities for the preferential identification of ‘super-spreader’ dogs among the overall infected population
Improved Thermodynamic Descriptions of Carbides in Ni-Based Superalloys
The Ni-based superalloy René 41 has sparked recent interest for applications in next-generation aircraft engines due to its high-temperature strength that is superior to all similar grades. These desirable properties are achieved by careful control of the microstructure evolution during thermomechanical processing, and this is commonly informed by simulations. In particular, the grain boundary carbides M6C and M23C6 play an essential role in controlling the grain size and strength of the final product. Therefore, a solid understanding of the thermodynamic stability and thermokinetic evolution of these carbides is essential. However, thermokinetic simulations using existing thermodynamic databases have been demonstrated to have discrepancies between thermodynamic stabilities and experimental observations. Here, we collected a new experimental time–temperature–precipitation diagram. In conjunction with improved crystallographic descriptions, these experimental results are used to modify a CALPHAD database for M6C and M23C6. The modified database correctly identifies temperature regions with rapid carbide precipitation kinetics. Further, kinetic simulations and strengthening models successfully predict the hardness increase due to γ′ precipitation. The modified database has been applied to Udimet 700, Waspaloy, and Haynes 282, demonstrating improved results. These updates will facilitate more accurate simulations of the microstructure evolution during thermomechanical processing of advanced Ni-based superalloys for aerospace and other applications
An internet-based self-administered intervention for promoting healthy habits and weight loss in hypertensive people who are overweight or obese: a randomized controlled trial
Background
The prevalence of overweight and obesity is on the rise worldwide with severe physical and psychosocial consequences. One of the most dangerous is hypertension. Lifestyle changes related to eating behaviour and physical activity are the critical components in the prevention and treatment of hypertension and obesity. Data indicates that the usual procedures to promote these healthy habits in health services are either insufficient or not efficient enough. Internet has been shown to be an effective tool for the implementation of lifestyle interventions based on this type of problem. This study aims to assess the efficacy of a totally self-administered online intervention programme versus the usual medical care for obese and overweight participants with hypertension (from the Spanish public health care system) to promote healthy lifestyles (eating behaviour and physical activity).
Method
A randomized controlled trial will be conducted with 100 patients recruited from the hypertension unit of a public hospital. Participants will be randomly assigned to one of two conditions: a) SII: a self-administered Internet-based intervention protocol; and b) MUC-medical usual care. The online intervention is an Internet-delivered, multimedia, interactive, self-administered programme, composed of nine modules designed to promote healthy eating habits and increase physical activity. The first five modules will be activated at a rate of one per week, and access for modules 5 to 9 will open every two weeks. Patients will be assessed at four points: before the intervention, after the intervention (3 months), and at 6 and 12 months (follow-up). The outcome variables will include blood pressure, and Body Mass Index, as primary outcome measures, and quality of life and other lifestyle and anthropometrical variables as secondary outcome measures.
Discussion
The literature highlights the need for more studies on the benefits of using the Internet to promote lifestyle interventions. Fractura triplana epifisaria distal de tibia: a propósito de un caso con interposición de periostio
Se presenta un caso de fractura triplana en 2 fragmentos en un paciente varón
de 13 años tratado quirúrgicamente, observando la interposición perióstica en el plano axial a
nivel del fragmento antero-externo, que impedÃa la reducción anatómica de la zona epifisaria
articular. Se analiza la anatomÃa quirúrgica de las fracturas triplanas considerando que esta lesión
puede ser más frecuente de lo que se ha informado, y sólo valorada en los casos en que se
emplee un abordaje antero-externo.The case of a 13-year-old boy with a two-fragment triplane fracture of the distal
epiphysis of the tibia is presented. At surgery, an interposition of the periosteum in the axial
plane behind the antero-lateral fragment, preventing the anatomical reduction of the epiphyseal
joint surface was found. The surgical anatomy of triplane fractures is review taking into account
that this lesion can be more frequent that it has been reported. The true incidence
should be evaluated at surgery by antero-lateral approach
Fracturas del macizo trocantéreo. Estudio comparativo Ender-DHS: revision de 114 casos
Se presenta un estudio retrospectivo de 114 fracturas pertrocantéreas tratadas
con enclavado elástico de Ender (71) y tornillo placa deslizante, DHS (43). Se efectuó un estudio
estadÃstico descriptivo-comparativo entre los 2 grupos de pacientes según la técnica quirúrgica
empleada. Se constató un mayor Ãndice de estabilidad usando el DHS (81%), frente al
6 1 % del otro grupo. La tasa de complicaciones fue del 16% y 55% respectivamente. Los resultados
clÃnicos fueron mejores en los enfermos tratados con DHS, fundamentalmente en
cuanto a la deambulación que se consiguió en el 98% (Ender 75%), ausencia de dolor y movilidad
conservada. Los autores piensan que la estabilidad preoperatoria no influye en la solidez
del montaje usando el DHS, pero con la técnica de Ender ésta disminuye en las fracturas inestables;
por ello, la técnica de Ender estarÃa solamente indicada en el tratamiento de fracturas
estables en pacientes de edad avanzada o con mal estado general, aunque en clara competencia
con el DHS.A retrospective study of 114 intertrochanteric fractures operated on either with
Ender's rods (71) or dynamic hip screws, DHS (43) is presented. A statistical descriptive-comparative
study between the 2 groups was attempted. There was a greater index of stability using the
DHS (82%) than Ender's rods (61%). The rate of complications was 16% and 55% respectively.
The clinical outcome was better in cases treated with DHS, specially for walking ability,
which was regained in the 98% of cases (Ender 75%), painless and preserved mobility. The authors
think that the stability before operation do not influence the solidity of the assembly using
the DHS device. On the contrary, with the Ender's technique the fixation is no satisfactory in
unstables fractures. Ender's rods would only be suitable for treatment of stable fractures in older
patient or with serious associated diseases, although in clear competition with the DHS
Attachment-specific speech patterns induce dysphoric mood changes in the listener as a function of individual differences in attachment characteristics and psychopathology
Objectives Early childhood experiences influence cognitive-emotional development, with insecure attachment predisposing to potential psychopathologies. We investigated whether narratives containing attachment-specific speech patterns shape listeners’ emotional responses and social intentions. Design First, 149 healthy participants listened to three narratives characteristic for secure, insecure-preoccupied, and insecure-dismissing attachment. Following each narrative, the wellbeing and interpersonal reactivity as a particular aspect of emotional reactivity of the listener were assessed. Likewise, psychopathological aspects of personality were evaluated. A follow-up study compared 10 psychosomatic patients with a current depressive episode and/or personality disorder with distinct depressive symptoms and 10 age- and gender-matched healthy controls. Methods Effects of narratives on listeners’ mental state were tested with repeated-measures AN(C)OVA. Mediating effects in the listener (attachment characteristics in the context of personality traits) were explored. Narrative effects were compared between patients and controls. Results Listening to insecure attachment narratives reduced wellbeing in controls. Nevertheless, tendency for social interaction was highest following the insecure-preoccupied narrative. Importantly, listeners’ individual attachment characteristics mediated the relationship between wellbeing/interpersonal reactivity following the insecure-preoccupied narrative and levels of psychopathology. Furthermore, compared with healthy participants, patients showed higher emotional reactivity following exposure to the insecure-preoccupied narrative, represented by lower wellbeing and lower estimation of friendliness towards the narrator. Conclusions Exposure to attachment-specific speech patterns can result in dysphoric mood changes. Specifically, the insecure-preoccupied narrative influenced the listeners’ emotional state, which was further mediated by the individual attachment patterns and psychopathological personality characteristics. This deepens the understanding of interpersonal processes, especially in psychotherapeutic settings
Opto-mechanical measurement of micro-trap via nonlinear cavity enhanced Raman scattering spectrum
High-gain resonant nonlinear Raman scattering on trapped cold atoms within a
high-fineness ring optical cavity is simply explained under a nonlinear
opto-mechanical mechanism, and a proposal using it to detect frequency of
micro-trap on atom chip is presented. The enhancement of scattering spectrum is
due to a coherent Raman conversion between two different cavity modes mediated
by collective vibrations of atoms through nonlinear opto-mechanical couplings.
The physical conditions of this technique are roughly estimated on Rubidium
atoms, and a simple quantum analysis as well as a multi-body semiclassical
simulation on this nonlinear Raman process is conducted.Comment: 7 pages, 2 figure
Dysregulated Proinflammatory and Fibrogenic Phenotype of Fibroblasts in Cystic Fibrosis
Morbi-mortality in cystic fibrosis (CF) is mainly related to chronic lung infection and inflammation, uncontrolled tissue rearrangements and fibrosis, and yet the underlying mechanisms remain largely unknown. We evaluated inflammatory and fibrosis responses to bleomycin in F508del homozygous and wild-type mice, and phenotype of fibroblasts explanted from mouse lungs and skin. The effect of vardenafil, a cGMP-specific phosphodiesterase type 5 inhibitor, was tested in vivo and in culture. Responses of proinflammatory and fibrotic markers to bleomycin were enhanced in lungs and skin of CF mice and were prevented by treatment with vardenafil. Purified lung and skin fibroblasts from CF mice proliferated and differentiated into myofibroblasts more prominently and displayed higher sensitivity to growth factors than those recovered from wild-type littermates. Under inflammatory stimulation, mRNA and protein expression of proinflammatory mediators were higher in CF than in wild-type fibroblasts, in which CFTR expression reached similar levels to those observed in other non-epithelial cells, such as macrophages. Increased proinflammatory responses in CF fibroblasts were reduced by half with submicromolar concentrations of vardenafil. Proinflammatory and fibrogenic functions of fibroblasts are upregulated in CF and are reduced by vardenafil. This study provides compelling new support for targeting cGMP signaling pathway in CF pharmacotherapy
Hemostatic Status of Pre and Post Intracoronary Injection of Peripheral Blood Stem Cells in Patients with Recent Myocardial Infarction
Aim: to investigate hemostatic parameter changes, such as platelet aggregation, blood and plasma viscosity, prothrombin time, APTT, CRP and fibrinogen, before and after administration of stem cell therapy.
Methods: a total of 24 patients were enrolled. Peripheral blood stem cells (PBSCs) were harvested and injected into the infarct-related artery after 5 consecutive days of G-CSF administration. Recombinant human erythropoietin was administered at the time of intracoronary PBSCs injection. Results: we were able to evaluate 11 from 24 of patients regarding hemostatic status pre–post stem cell injection. There were no significant difference between baseline vs 3 months in spontaneous aggregation (p=0.350), PT (p=0.793), aPTT (p=0.255) and TT (p=0.254). There were also no significant difference between baseline vs 3 months in plasma viscosity (p=0.442) and blood viscosity (p=0.843). Nevertheless the patient who had their blood and plasma viscosity above or below normal laboratory range return to normal level after the treatment. Both PT and APTT also show normalization value. Both Fibrinogen and CRP level show significant decrease between baseline and 3 months after treatment (p=0.009) and (p=0.04) respectively. Conclusion: combined G-CSF and EPO based-intracoronary infusion of PBSCs may open new perspective in the treatment of hypercoagulable state post AMI
