    Fall Prevention Abstract Background: According to the CDC, the medical cost for falls was approximately 30billionin2012.Whenapatientfalls,andgetsseriousinjury,thesecostscouldsumupto30 billion in 2012. When a patient falls, and gets serious injury, these costs could sum up to 14,056 per patient. As a result, hospital stays per patient are observed to increase by approximately 6.3 days longer than those patients who haven’t encountered falls. This study focused on identifying high fall risk patients, and developing a plan of care to reduce the occurrence of falls in a hospital setting. Nurses using safe fall interventions techniques have been proven to inspire fall preventions. An assessment of the nursing practices in the microsystem was carried out to identify the barriers that affected the fall prevention and increased falls rates in the unit. This assessment showed an inconsistence in the placement of fall signs by the nursing staff. This discovery was critical to addressing the fall rates in the unit. Purpose: To execute and develop an effective and functional fall prevention plan with a goal to reduce inpatient fall rate by 25%. This should enhance the ability of nurses to comply when implementing fall procedures and using fall prevention equipment. Methods: This study employed the Kurt Lewin’s change model, which included developing a fall prevention committee, creating a new fall signage and continuous education of staff members. Results: The rate falls in the microsystem were observed to have declined significantly. Nursing staff are more compliant with the new fall prevention protocol

    Influence of exercise mode on pregnancy outcomes: ENHANCED by Mom project

    Background: The extent of the benefits of exercise training during pregnancy on maternal, fetal, and neonatal health outcomes has not been sufficiently addressed. While aerobic exercise training has been determined as safe and efficacious throughout pregnancy, the effects of other training modes on fetal health and development as well as any continued benefits for the neonate, especially with regards to cardiovascular development and function, is largely unknown. In the ENHANCED by Mom study we aim to determine the effects of different modes of exercise training (aerobic, circuit, and resistance) throughout pregnancy on childhood health by controlling individual exercise programs and assessing the effects of each on fetal and neonatal health adaptations. Methods/Design: ENHANCED by mom is a cross sectional comparison study utilizing 3 intervention groups in comparison to a control group. Participants will complete three 5 min warmup + 45 min sessions weekly from 16 weeks to 36 weeks gestation of aerobic, resistance, or circuit training, in comparison to non-exercising controls. Maternal physical measurements will occur every 4 weeks throughout the intervention period. Fetal morphometric and heart measurements will occur at 34 weeks gestation. Neonatal measurements will be acquired at birth and at 1 month, 6 months, and 12 months. Discussion: A better understanding on the effects of exercise training during pregnancy on fetal and neonatal health could have a profound impact on the prevention and development of chronic diseases such as obesity, hypertension, and diabetes

    Creating Weyl nodes and controlling their energy by magnetization rotation

    As they do not rely on the presence of any crystal symmetry, Weyl nodes are robust topological features of an electronic structure that can occur at any momentum and energy. Acting as sinks and sources of Berry curvature, Weyl nodes have been predicted to strongly affect the transverse electronic response, like in the anomalous Hall or Nernst effects. However, to observe large anomalous effects the Weyl nodes need to be close to or at the Fermi-level, which implies the band structure must be tuned by an external parameter, e.g. chemical doping or pressure. Here we show that in a ferromagnetic metal tuning of the Weyl node energy and momentum can be achieved by rotation of the magnetization. Taking Co3_3Sn2_2S2_2 as an example, we use electronic structure calculations based on density-functional theory to show that not only new Weyl fermions can be created by canting the magnetization away from the easy axis, but also that the Weyl nodes can be driven exactly to the Fermi surface. We also show that the dynamics in energy and momentum of the Weyl nodes strongly affect the calculated anomalous Hall and Nernst conductivities.Comment: Supp. Material adde

    An exploratory study of the effect of labor pain management on postpartum depression among Chinese women

    Objectives: The purpose of this study was to investigate the effects of pain relief during labor on the occurrence of potential postpartum depression in early postpartum among Chinese women.Material and methods: A quasi-experimental study used, with a convenience sample of 565 women who delivered at the Women’s Hospital, School of Medicine. Three types of pain relief were administered based on the women’s preference (doula, n = 301; transcutaneous electrical nerve stimulation, n = 51; epidural analgesia, n = 213). Pain scores of participants were assessed using a 10-point visual analog scale during labor. The Edinburgh Postnatal Depression Scale was administered in person and by phone at three days and two to four weeks after delivery, respectively. All data were analyzed using SPSS 20.0.Results: Visual analog scale pain scores in the epidural analgesia group decreased significantly during labor compared to those of the other two groups. The occurrence of potential postpartum depression at three days was 6.6% in the epidural analgesia group, 1.3% in the doula group, and 2% in the transcutaneous electrical nerve stimulation group (P = 0.04). Furthermore, potential postpartum depression occurred at two to four weeks after childbirth in 16% (34/213) of the participants in the epidural analgesia group, 7.3% (22/301) of those who received doula support, and in 7.8% (4/51) of those in the transcutaneous electrical nerve stimulation group (P = 0.006).Conclusions: The results indicated that epidural analgesia was an effective pain relief method during labor. However, it did not reduce the occurrence of potential postpartum depression and was associated with higher postnatal depression scores

    Dirac fermions and flat bands in the ideal kagome metal FeSn.

    A kagome lattice of 3d transition metal ions is a versatile platform for correlated topological phases hosting symmetry-protected electronic excitations and magnetic ground states. However, the paradigmatic states of the idealized two-dimensional kagome lattice-Dirac fermions and flat bands-have not been simultaneously observed. Here, we use angle-resolved photoemission spectroscopy and de Haas-van Alphen quantum oscillations to reveal coexisting surface and bulk Dirac fermions as well as flat bands in the antiferromagnetic kagome metal FeSn, which has spatially decoupled kagome planes. Our band structure calculations and matrix element simulations demonstrate that the bulk Dirac bands arise from in-plane localized Fe-3d orbitals, and evidence that the coexisting Dirac surface state realizes a rare example of fully spin-polarized two-dimensional Dirac fermions due to spin-layer locking in FeSn. The prospect to harness these prototypical excitations in a kagome lattice is a frontier of great promise at the confluence of topology, magnetism and strongly correlated physics
