Retinalis biomarkerek szerepe a vascularis dementia és az Alzheimer-kór korai formáinak diagnosztikájában
A dementiák társadalmunkban egyre nagyobb problémát jelentenek, folyamatosan nő a megbetegedések száma.
A dementia leggyakoribb előfordulási formája az Alzheimer-kór, mely az esetek 60–80%-áért felelős. Kórlefolyása
hosszú, a preklinikai stádiumtól a súlyos Alzheimer-kórig több év, évtized is eltelik. Az Alzheimer-kór egy olyan
progresszív neurodegeneratív betegség, amely súlyos szellemi leépüléshez vezet. A diagnózishoz számos, kognitív
funkciót mérő teszt és képalkotó vizsgálat áll rendelkezésre, a korai diagnózis azonban nagyobb kihívást jelent, így a
betegség gyakran későn kerül felismerésre. Mivel az agyi és a retinalis keringés között bizonyítottan kapcsolat van,
felmerül annak lehetősége, hogy a retina vizsgálata az Alzheimer-kór és a vascularis dementiák korai diagnózisában és
progressziójának követéséhez is segítséget nyújthat. Az utóbbi években a retina képalkotó vizsgálataiban bekövetke-
zett fejlődésnek köszönhetően lehetővé vált a retina strukturális és vérkeringési elváltozásainak vizsgálata a dementiák
különböző típusaiban. Ezek alkalmazása nemcsak a korai diagnózisban, hanem a progresszió megítélésében és a te-
rápia nyomon követésében is segítséget nyújthat a jövőben
Impairment of a model peptide by oxidative stress: Thermodynamic stabilities of asparagine diamide C(alpha)-radical foldamers
Electron structure calculations on N-acetyl asparagine N-methylamide were performed to identify the global minimum from which radicals were formed after H-abstraction by the OH radical. It was found that the radical generated by breaking the C–H bond of the alpha-carbon was thermodynamically the most stable one in the gas- and aqueous phases. The extended ((beta)L and (beta)D) backbone conformations are the most stable, but syn–syn or inverse gamma-turn ((gamma)L) and gamma-turn ((gamma)D) have substantial stability too. The highest energy conformers are the degenerate eL and eD foldamers. Clearly, the most stable beta foldamer is the most likely intermediate for racemization
Terápiás hypothermia alkalmazása az asphyxiás újszülöttek kezelésében – egy hazai neonatológiai centrum eredményei | Hypothermia treatment in asphyxiated neonates – a single center experience in Hungary
Absztrakt:
Bevezetés és célkitűzés: A hypothermiás kezelésben részesült
hypoxiás-ischaemiás encephalopathiás újszülöttek összegyűjtött vizsgálati
adatait elemeztük a Semmelweis Egyetem, I. Gyermekgyógyászati Klinika hármas
szintű Perinatalis Intenzív Centrumában. Módszer: Retrospektív
kohorszvizsgálatunkban a 2013–2015 között született, érett, hypoxiás-ischaemiás
encephalopathia következtében hypothermiás kezelésben részesült 97 beteg adatait
értékeltük, saját fejlesztésű regiszter segítségével.
Eredmények: A gyermekek 59,8%-a császármetszéssel
született, az első vérgázvizsgálat során észlelt pH 7,0 ± 0,2, pCO2
55,9 ± 27,3 Hgmm, bázishiány 16,7 ± 7,2 mmol/l, illetve laktátszint 13,3 ± 4,7
mmol/l (x ± SD) volt. A hypothermiás kezelés 93,7%-ban már a neonatalis
transzport alatt megkezdődött, átlagosan 2,5 ± 0,3 órás életkorban. Az
asphyxiához gyakran társuló sokszervi elégtelenség betegeink 83,2%-ánál volt
jelen. Az intenzív terápiával töltött medián idő 10,8 nap volt. Betegeink
61,3%-a az intenzív terápiát követően hazaadható volt, 32,2%-uk további klinikai
kezelést igényelt, 6,5%-uk meghalt. Következtetés: A saját
fejlesztésű regiszter segítségével az elmúlt évek kezelési eredményeinek gyors
és könnyen kezelhető leíró elemzése vált lehetővé, ami megteremtette a belső
audit és a prospektív betegkövetés feltételeit is. Orv. Hetil., 2017,

Abstract:
Introduction and aim: We aimed to analyze patient
characteristics of term neonates with hypoxic-ischemic encephalopathy treated
with hypothermia at the 3rd level Neonatal Intensive Care Unit of the 1st
Department of Pediatrics, Semmelweis University. Method: We
conducted a retrospective cohort analysis between 2013–2015, including 97
asphyxiated neonates with HIE who received hypothermia treatment, using our
in-house developed novel registry database. Results: 59.8% of
neonates were born with Cesarean section and the first blood gas analysis showed
a pH of 7.0 ± 0.2, pCO2 55.9 ± 27.3 mmHg, base deficit 16.7 ± 7.2
mmol/l, and lactate levels of 13.3 ± 4.7 mmol/l (x ± SD). Hypothermia treatment
was started during neonatal transport in 93.7% of the cases, at 2.5 ± 0.3 hours
of age. Multiorgan failure associated with the perinatal asphyxia was present in
83.2% of the patients. Patients received intensive therapy for a median of 10.8
days, 61.3% of neonates were discharged home directly, 32.2% required further
hospital treatment, and 6.5% died. Conclusion: Our novel
registry database allowed for a quick, user-friendly and time-efficient analysis
of patient characteristics in neonates with HIE. This registry could aid
institutional audit work and prospective clinical data collection. Orv. Hetil.,

Microbiota mitochondria disorders as hubs for early age-related macular degeneration
Age-related macular degeneration (AMD) is a progressive neurodegenerative disease affecting the central area (macula lutea) of the retina. Research on the pathogenic mechanism of AMD showed complex cellular contribution governed by such risk factors as aging, genetic predisposition, diet, and lifestyle. Recent studies suggested that microbiota is a transducer and a modifier of risk factors for neurodegenerative diseases, and mitochondria may be one of the intracellular targets of microbial signaling molecules. This review explores studies supporting a new concept on the contribution of microbiota-mitochondria disorders to AMD. We discuss metabolic, vascular, immune, and neuronal mechanism in AMD as well as key alterations of photoreceptor cells, retinal pigment epithelium (RPE), Bruch's membrane, choriocapillaris endothelial, immune, and neuronal cells. Special attention was paid to alterations of mitochondria contact sites (MCSs), an organelle network of mitochondria, endoplasmic reticulum, lipid droplets (LDs), and peroxisomes being documented based on our own electron microscopic findings from surgically removed human eyes. Morphometry of Bruch's membrane lipids and proteoglycans has also been performed in early AMD and aged controls. Microbial metabolites (short-chain fatty acids, polyphenols, and secondary bile acids) and microbial compounds (lipopolysaccharide, peptidoglycan, and bacterial DNA)-now called postbiotics-in addition to local effects on resident microbiota and mucous membrane, regulate systemic metabolic, vascular, immune, and neuronal mechanisms in normal conditions and in various common diseases. We also discuss their antioxidant, anti-inflammatory, and metabolic effects as well as experimental and clinical observations on regulating the main processes of photoreceptor renewal, mitophagy, and autophagy in early AMD. These findings support an emerging concept that microbiota-mitochondria disorders may be a crucial pathogenic mechanism of early AMD; and similarly, to other age-related neurodegenerative diseases, new treatment approaches should be targeted at these disorders
