Episiotomy practices in France: epidemiology and risk factors in non-operative vaginal deliveries
Episiotomy use has decreased due to the lack of evidence on its protective effects from maternal
obstetric anal sphincter injuries. Indications for episiotomy vary considerably and there are a great
variety of factors associated with its use. The aim of this article is to describe the episiotomy rate
in France between 2013 and 2017 and the factors associated with its use in non-operative vaginal
deliveries. In this retrospective population-based cohort study, we included vaginal deliveries
performed in French hospitals (N= 584) and for which parity was coded. The variable of interest was
the rate of episiotomy, particularly for non-operative vaginal deliveries. Trends in the episiotomy rates
were studied using the Cochran-Armitage test. Hierarchical logistic regression was used to identify
variables associated with episiotomy according to maternal age and parity. Between 2013 and 2017,
French episiotomy rates fell from 21.6 to 14.3% for all vaginal deliveries (p< 0.01), and from 15.5
to 9.3% (p< 0.01) for all non-operative vaginal deliveries. Among non-operative vaginal deliveries,
epidural analgesia, non-reassuring fetal heart rate, meconium in the amniotic fluid, shoulder dystocia,
and newborn weight (≥4,000 g) were risk factors for episiotomy, both for nulliparous and multiparous
women. On the contrary, prematurity reduced the risk of its use. For nulliparous women, breech
presentation was also a risk factor for episiotomy, and for multiparous women, scarred uterus and
multiple pregnancies were risk factors. In France, despite a reduction in episiotomy use over the last
few years, the factors associated with episiotomy have not changed and are similar to the literature.
This suggests that the decrease in episiotomies in France is an overall tendency which is probably
related to improved care strategies that have been relayed by hospital teams and perinatal networks

Perinatal period and Rorschach: Norms, indicators, mentalisation and theoretical-clinical perspectives
Objective: The perinatal period is a maturation phase characterised by the appearance of a radical psychological and identity crisis, little studied by projective methods, in particular the Rorschach inkblot test. The objective of this article is to offer a list of Rorschach norms and indicators adapted to the perinatal population. This is to comprehend the psychological modifications occurring during pregnancy. Materials and method: The projective Rorschach inkblot test was presented to a representative sample of 54 pregnant primiparous French women. Based on the scoring and analysis methods of the Parisian School, the quantitative norms from the analysis of the psychogram, the chosen cards, and the mentalization indicators were compared to those of the general population. Results: With the exception of F +% and H%, all the normative data for the perinatal population are significantly different from those related to the general population (p .005). On a qualitative level, the interpretation of the data highlights the presence of major inhibition mechanisms, associated with a preconscious functioning stasis. Conclusion: The normative data in Rorschach of the perinatal population should be specifically used for the study and psychological support devoted to this population. It also allows a better understanding of the feminine psychologic modifications during pregnancy

Obésité sévère, quelle prise en charge psychologique ?
Les auteurs s’intéressent aux liens entre la survenue d’une obésité sévère à l’âge adulte et les antécédents d’événements difficiles ou traumatiques, notamment quant à leurs répercussions. Pour commencer, ils recensent les principaux événements de vie difficiles ou traumatiques identifiés dans l’histoire des patients obèses, les définissent, et présentent leurs prévalences. Ils rappellent ensuite les conséquences de tels événements dans le développement somato-psychiques des sujets. Puis, pour étayer et illustrer leurs hypothèses, les auteurs s’appuient sur leur expérience clinique ainsi que sur le test de Rorschach pour présenter deux cas de patients obèses, en demande de chirurgie bariatrique. Enfin, ils concluent sur l’importance du recours à l’entretien clinique ainsi qu’au test de Rorschach au sein de cette population, afin d’initier et faciliter l’expression des affects et ainsi construire et renforcer l’engagement dans une psychothérapie. Ils proposent de nouvelles recommandations pour la prise en charge pré-chirurgicale

Représentations sociales du vieillissement des sujets porteurs de troubles psychiques : impacts et conséquences d’une double stigmatisation
National audienceDans nos sociétés occidentales, l’étude des mécanismes inhérents à la perception sociale du vieillissement a mis en évidence la présence de stigmatisation, de discrimination et d’exclusion opérées à l’encontre des sujets âgés. Ce constat semble d’autant plus marqué lorsqu’il est question de sujets vieillissants porteurs de troubles psychiques. Il revient donc à la recherche de proposer une réflexion concernant les éléments sous-jacents à l’émergence de ces mécanismes dans le but d’identifier et de réduire leurs déterminants. Pour ce faire, les auteurs ont réalisé une revue de littérature portant sur les représentations sociales des individus vieillissants porteurs de troubles psychiques. La méthodologie employée révèle qu’aucune étude portant sur ce point précis n’a été publiée à ce jour. Devant cette absence de données, les auteurs ont recensé les travaux portant sur les représentations sociales inhérentes aux sujets âgés et aux sujets porteurs de troubles psychiques. Ces dernières ont été regroupées, catégorisées et décrites, mettant en exergue la présence d’une double stigmatisation à l’égard des sujets vieillissants vulnérables psychiquement. L’impact discriminatoire de ce phénomène est ensuite discuté en mettant l’accent sur ses conséquences sociétales, institutionnelles et individuelles
