    The Influence of Foot-Strike Technique on the Neuromechanical Function of the Foot

METHODS: Thirteen healthy participants ran barefoot on a force-instrumented treadmill at 2.8 ms with a forefoot (FFS) and rearfoot (RFS; habitual) running technique, whereas kinetic, kinematic, and electromyographic data from the intrinsic foot muscles were collected simultaneously. The longitudinal arch was modeled as a single "midfoot" joint representing motion of the rearfoot (calcaneus) relative to the forefoot (metatarsals). An inverse dynamic analysis was performed to estimate joint moments generated about the midfoot, as well as mechanical work and power. RESULTS: The midfoot was more plantar flexed (higher arch) at foot contact when running with a forefoot running technique (RFS 0.2 Β± 1.8 vs FFS 6.9 Β± 3.0Β°, effect size (ES) = 2.7); however, there was no difference in peak midfoot dorsiflexion in stance (RFS -11.6 Β± 3.0 vs FFS -11.4 Β± 3.4Β°, ES = 0.63). When running with a forefoot technique, participants generated greater moments about the midfoot (27% increase, ES = 1.1) and performed more negative work (240% increase, ES = 2.2) and positive work (42% increase, ES = 1.1) about the midfoot. Average stance-phase muscle activation was greater for flexor digitorum brevis (20% increase, ES = 0.56) and abductor hallucis (17% increase, ES = 0.63) when running with a forefoot technique. CONCLUSIONS: Forefoot running increases loading about the longitudinal arch and also increases the mechanical work performed by the intrinsic foot muscles. These findings have substantial implications in terms of injury prevention and management for runners who transition from a rearfoot to a forefoot running technique.Funding for this study was provided via an industry research grant from Asics Oceania (grant identification number 2014000885

    The effect of cadence on the muscle-tendon mechanics of the gastrocnemius muscle during walking

The aim of this study was to examine the effects of cadence on the medial gastrocnemius (MG) muscle and tendon interaction, and examine how this might influence lower limb energetics. We hypothesized that cadences higher than preferred would increase MG fascicle shortening velocity because of the reduced stride time. Furthermore, we hypothesized that cadences lower than preferred would require greater MG fascicle shortening to achieve increased muscle work requirements. We measured lower limb kinematics and kinetics, surface electromyography of the triceps surae and MG fascicle length, via ultrasonography, during walking at a constant velocity at the participants' preferred cadence and offsets of Β±10%, Β±20%, and Β±30%. There was a significant increase in MG fascicle shortening with decreased cadence. However, there was no increase in the MG fascicle shortening velocity at cadences higher than preferred. Cumulative MG muscle activation per minute was significantly increased at higher cadences. We conclude that low cadence walking requires more MG shortening work, while MG muscle and tendon function changes little for each stride at higher cadences, driving up cumulative activation costs due to the increase in steps per minute.Scott Brennan is supported by an Australian Postgraduate Scholarship.     Shoes alter the spring-like function of the human foot during running

Recent comparisons of shod and barefoot running have led to suggestions that modern running shoes may actually impede leg and foot-spring function by reducing the contributions from the leg and foot musculature. Here we examined the effect of running shoes on foot longitudinal arch (LA) motion and activation of the intrinsic foot muscles. Participants ran on a force-instrumented treadmill with and without running shoes. We recorded foot kinematics and muscle activation of the intrinsic foot muscles using intramuscular electromyography. In contrast to previous assertions, we observed an increase in both the peak (flexor digitorum brevis +60%) and total stance muscle activation (flexor digitorum brevis +70% and abductor hallucis +53%) of the intrinsic foot muscles when running with shoes. Increased intrinsic muscle activation corresponded with a reduction in LA compression (-25%). We confirm that running shoes do indeed influence the mechanical function of the foot. However, our findings suggest that these mechanical adjustments are likely to have occurred as a result of increased neuromuscular output, rather than impaired control as previously speculated.     The role of human ankle plantar flexor muscle-tendon interaction and architecture in maximal vertical jumping examined in vivo

Anuran and insect species use skeletal structures and/or dynamics in conjunction with similarly compliant structures to amplify muscle power output during jumping. We sought to examine whether human jumpers use similar mechanisms to aid elastic energy usage in the plantar flexor muscles during maximal vertical jumping. Ten male athletes performed maximal vertical squat jumps. Three-dimensional motion capture and a musculoskeletal model were used to determine lower limb kinematics that were combined with ground reaction force data in an inverse dynamics analysis. B-mode ultrasound imaging of the lateral gastrocnemius (GAS) and soleus (SOL) muscles was used to measure muscle fascicle lengths and pennation angles during jumping. Our results highlighted that both GAS and SOL utilised stretch and recoil of their series elastic elements (SEEs) in a catapult-like fashion, which likely serves to maximise ankle joint power. The resistance of supporting of body weight allowed initial stretch of both GAS and SOL SEEs. A proximal-to-distal sequence of joint moments and decreasing effective mechanical advantage early in the extension phase of the jumping movement were observed. This facilitated a further stretch of the SEE of the biarticular GAS and delayed recoil of the SOL SEE.     Simulating the effect of muscle weakness and contracture on neuromuscular control of normal gait in children

    Altered neural control of movement and musculoskeletal deficiencies are common in children with spastic cerebral palsy (SCP), with muscle weakness and contracture commonly experienced. Both neural and musculoskeletal deficiencies are likely to contribute to abnormal gait, such as equinus gait (toe-walking), in children with SCP. However, it is not known whether the musculoskeletal deficiencies prevent normal gait or if neural control could be altered to achieve normal gait. This study examined the effect of simulated muscle weakness and contracture of the major plantarflexor/dorsiflexor muscles on the neuromuscular requirements for achieving normal walking gait in children. Initial muscle-driven simulations of walking with normal musculoskeletal properties by typically developing children were undertaken. Additional simulations with altered musculoskeletal properties were then undertaken; with muscle weakness and contracture simulated by reducing the maximum isometric force and tendon slack length, respectively, of selected muscles. Muscle activations and forces required across all simulations were then compared via waveform analysis. Maintenance of normal gait appeared robust to muscle weakness in isolation, with increased activation of weakened muscles the major compensatory strategy. With muscle contracture, reduced activation of the plantarflexors was required across the mid-portion of stance suggesting a greater contribution from passive forces. Increased activation and force during swing was also required from the tibialis anterior to counteract the increased passive forces from the simulated dorsiflexor muscle contracture. Improvements in plantarflexor and dorsiflexor motor function and muscle strength, concomitant with reductions in plantarflexor muscle stiffness may target the deficits associated with SCP that limit normal gait

    Differential strain patterns of the human Achilles tendon determined in vivo with freehand three-dimensional ultrasound imaging

Available from Company of Biologists via the DOI in this recordThe human Achilles tendon (AT) has often been considered to act as a single elastic structure in series with the muscles of the triceps surae. As such it has been commonly modelled as a Hookean spring of uniform stiffness. However, the free AT and the proximal AT have distinctly different structures that lend themselves to different elastic properties. This study aimed to use three-dimensional freehand ultrasound imaging to determine whether the proximal AT and the free AT exhibit different elastic behaviour during sub-maximal, fixed-end contractions of the triceps surae. Six male and five female participants (mean Β± s.d. age=27 Β± 5 years) performed fixed position contractions of the plantar-flexors on an isokinetic dynamometer at 50% of their maximum voluntary contraction in this position. Freehand three-dimensional ultrasound imaging was used to reconstruct the free-tendon and proximal AT at rest and during contraction. The free-tendon exhibited significantly (P=0.03) greater longitudinal strain (5.2 Β± 1.7%) than the proximal AT (2.6 Β± 2.0%). The lesser longitudinal strain of the proximal AT was linked to the fact that it exhibited considerable transverse (orthogonal to the longitudinal direction) strains (5.0 Β± 4%). The transverse strain of the proximal AT is likely due to the triceps surae muscles bulging upon contraction, and thus the level of bulging may influence the elastic behaviour of the proximal AT.     Musculoskeletal simulations to examine the effects of accentuated eccentric loading (AEL) on jump height

The level of evidence for the positive effects of AEL remains weak, with conflicting evidence over the effectiveness in enhancing performance. Therefore, we proposed to theoretically explore the influence of implementing AEL during constrained vertical jumping using computer modelling and simulation and examined whether the proposed mechanism of enhanced power, increased elastic energy storage and return, could enhance work and power. Methods We used a simplified model, consisting of a ball-shaped body (head, arm, and trunk), two lower limb segments (thigh and shank), and four muscles, to simulate the mechanisms of AEL. We adjusted the key activation parameters of the muscles to influence the performance outcome of the model. Numerical optimization was applied to search the optimal solution for the model. We implemented AEL and non-AEL conditions in the model to compare the simulated data between conditions. Results Our model predicted that the optimal jumping performance was achieved when the model utilized the whole joint range. However, there was no difference in jumping performance in AEL and non-AEL conditions because the model began its push-off at the similar state (posture, fiber length, fiber velocity, fiber force, tendon length, and the same activation level). Therefore, the optimal solution predicted by the model was primarily driven by intrinsic muscle dynamics (force-length-velocity relationship), and this coupled with the similar model state at the start of the push-off, resulting in similar push-off performance across all conditions. There was also no evidence of additional tendon-loading effect in AEL conditions compared to non-AEL condition. Discussion Our simplified simulations did not show improved jump performance with AEL, contrasting with experimental studies. The reduced model demonstrates that increased energy storage from the additional mass alone is not sufficient to induce increased performance and that other factors like differences in activation strategies or movement paths are more likely to contribute to enhanced performance.UQ Graduate School ScholarshipAustralian Research Council (ARC

    Human Leg Model Predicts Ankle Muscle-Tendon Morphology, State, Roles and Energetics in Walking

    A common feature in biological neuromuscular systems is the redundancy in joint actuation. Understanding how these redundancies are resolved in typical joint movements has been a long-standing problem in biomechanics, neuroscience and prosthetics. Many empirical studies have uncovered neural, mechanical and energetic aspects of how humans resolve these degrees of freedom to actuate leg joints for common tasks like walking. However, a unifying theoretical framework that explains the many independent empirical observations and predicts individual muscle and tendon contributions to joint actuation is yet to be established. Here we develop a computational framework to address how the ankle joint actuation problem is resolved by the neuromuscular system in walking. Our framework is founded upon the proposal that a consideration of both neural control and leg muscle-tendon morphology is critical to obtain predictive, mechanistic insight into individual muscle and tendon contributions to joint actuation. We examine kinetic, kinematic and electromyographic data from healthy walking subjects to find that human leg muscle-tendon morphology and neural activations enable a metabolically optimal realization of biological ankle mechanics in walking. This optimal realization (a) corresponds to independent empirical observations of operation and performance of the soleus and gastrocnemius muscles, (b) gives rise to an efficient load-sharing amongst ankle muscle-tendon units and (c) causes soleus and gastrocnemius muscle fibers to take on distinct mechanical roles of force generation and power production at the end of stance phase in walking.     Mechanical properties during healing of Achilles tendon ruptures to predict final outcome: A pilot Roentgen stereophotogrammetric analysis in 10 patients

    <p>Abstract</p> <p>Background</p> <p>There are presently few methods described for in vivo monitoring of the mechanics of healing human tendon ruptures, and no methods for prediction of clinical outcome. We tested if Roentgen stereophotogrammetric analysis (RSA) can be used to follow the restoration of mechanical properties during healing of ruptured Achilles tendons, and if early measurements can predict clinical results.</p> <p>Methods</p> <p>Achilles tendon repair was studied with RSA in 10 patients with a total rupture. Tantalum beads were implanted in conjunction with surgical repair. The patients were evaluated at 6, 12 and 18 weeks, and after 1 year. RSA was performed with two different mechanical loadings, and the strain induced by increasing load was measured. The transverse area was determined by ultrasound. CT scan at 12 weeks confirmed that the tantalum beads were located within the tendons. Functional testing was done after 1 year. A heel raise index was chosen as primary clinical outcome variable.</p> <p>Results</p> <p>The strain was median 0.90, 0.32 and 0.14 percent per 100 N tendon force at 6 weeks, 18 weeks and one year respectively. The error of measurement was 0.04 percent units at 18 weeks. There was a large variation between patients, which appears to reflect biological variation. From 6 to 18 weeks, there was a negative correlation between increase in transverse area and increase in material properties, suggesting that healing is regulated at the organ level, to maximize stiffness. Modulus of elasticity during this time correlated with a heel raise index at one year (Rho = 0.76; p = 0.02).</p> <p>Conclusion</p> <p>We conclude that the RSA method might have potential for comparing different treatments of Achilles tendon ruptures.</p
