    ANALYSE LEXICALE DE LA QUALITÉ DE VIE AU TRAVAIL DES PSYCHIATRES À L'HÔPITAL

    International audiencePurpose:A qualitative study was conducted in 2013 with hospital psychiatrists working in two psychiatric hospitals of different regions in order to document the quality of work life of public hospital psychiatrists. Method: Semi-guided interviews were conducted with psychiatrists using an interview guide. Physicians were randomized according to health institution, age, gender and adult or paediatric psychiatry speciality. Data were analysed with the Alceste software and interpreted. Results: Sixteen psychiatrists were interviewed. Four themes, composed of sub-themes, were highlighted. The first them concerned medical and social patient care, with the child's social and family environment, structural constraints and interactions with the care network The second them concerned quality of care with the training of healthcare workers and the relational dimension. The third them concerned working in the public hospital system with the role of the "physician-administrator", loss of the meaning of work and job satisfaction. The last theme refers to the specificity of working in psychiatry with forensic aspects and violence issues. Conclusion: This study suggests that psychiatrists are globally satisfied with their jobs. The main factors to be improved are working time organization to allow more time to listen to the patient, interprofessional exchanges and paramedical staff training. These data should be assessed by means of a quantitative study

    Optimizing urothelial cell preparation for the human urinary micronucleus assay

    Biological monitoring of early genotoxic effects in urothelial cells using the urinary micronucleus (MNu) assay is promising for early detection of cancer, such as bladder carcinoma. But many problems are encountered, the major being the poorly differential staining of cells, particularly in women having an important amount of squamous cells. We have optimized the protocol and obtained a differential staining of the cell types present in urine on 10 subjects. Following Carnoy I fixation and Papanicolaou staining, urothelial cells were blue while most squamous cells were pink. This differential staining allowed for optimization of the MNu assay on a single urine void, for both females and males. Even if our MNu means were comparable to the literature, the great variation in reported MNu results could reside in the ability of scorers to distinguish correctly between urothelial and squamous cells. When monitoring exposed populations, this erroneous distinction could largely influence the results, even more in women’s urine samples. Given a situation where exposure would not increase micronuclei frequency in vaginal squamous cells, their erroneous analysis in the MNu assay could mask an early genotoxic effect. Therefore, as transitional cell carcinoma of the bladder originates from transformed urothelial cells, restricting micronuclei analysis to urothelial cells could yield a more precise estimate of cancer risk in exposed populations. Moreover, it is hoped that the improvements proposed in this paper will allow for an easier implementation of the MNu assay in various set-ups and enhance its specificity, since MNu are considered a suitable biomarker

    Contraintes psycho-organisationnelles et mentales au travail d'agents hospitaliers des services d'urgences et réanimation

    INTRODUCTION: Les services d'urgences et de réanimation sont probablement les plus stressants. L'objectif de cette étude a été de mesurer les contraintes psycho-organisationelles et mentales au travail auxquelles sont exposés les personnels paramédicaux des services d'urgence et de réanimation en comparaison avec les agents des services médicaux et chirurgicaux du même hôpital. METHODE: Un cahier d'auto-questionnaire, adressé au domicile de 598 infirmier(e)s et aide-soignant(e)s, a exploré les caractéristiques démographiques et professionnelles des agents, leur appréciation des conditions de travail, leur satisfaction au travail et le retentissement perçu du travail sur leur santé. Le Nursing Work Index Extended Organization a évalué les contraintes psycho-organisationnelles et le Job Content Questionnaire a mesuré les contraintes mentales. RESULTATS: Un total de 223 agents, 122 infirmiers et 101 aides-soignantes ont participé à l'étude, soit un taux de réponse de 37,3%. La plupart des agents des deux populations sont satisfaites de leur travail et ont du plaisir à s'y rendre. Ils ont la même appréciation de leurs conditions de travail et du retentissement du travail sur leur santé. Après ajustement pour l'âge, le sexe, le travail à temps complet et le travail le week-end, les agents des services d'urgences et de réanimation enregistrent davantage de contraintes pour l'organisation favorisant la communication et le respect des congés. Ils ont également le score de demande psychologique et la prévalence de job strain et d'iso strain les plus élevés. DISCUSSION: La présence d'une forte satisfaction au travail malgré d'importantes contraintes psycho-organisationnelles et mentales souligne toute la complexité de la notion de bien-être au travail.
