    Critical Factors of Adopting Enterprise Application Integration Technology: An Empirical Study on Larger Hospitals

    As hospitals extend their service scope, they adopt more information systems. These systems are implemented in different timelines and the interfaces of databases become varied. Frequently, the exchange of information between various systems requires additional coordination or even manual input for unifying data. To embrace automation, the solution is to adopt enterprise application integration (EAI) technology, the middleware, to convert data from among various information systems to enable an efficient flow of data in the hospital. In this paper, we discuss and verify the impact factors on the integration levels of EAI by surveying larger hospitals above the regional level in Taiwan and testing a proposed research model. The findings of this study show that information technology infrastructure, hospital size, external pressure, internal pressure, and external support significantly affect the EAI level

    Hemolytic Uremic Syndrome Caused by Enteroviral Infection

    A 4-year-old boy presented with enteroviral infection complicated with atypical hemolytic uremic syndrome (aHUS). Enterovirus RNA was detected by reverse transcription polymerase chain reaction (RT-PCR) of both blood and kidney biopsy specimens. A survey of the complement system did not reveal a specific complement defect. Supportive therapy with blood components transfusion, plasma therapy, and immunosuppressants was administered, however, renal function did not recover. The results of this report demonstrate that the enterovirus is the cause of aHUS

    利用日、月及年降雨量估算年降雨沖蝕指數

    The Universal Soil Loss Equation (USLE) is presently one of the most widely used models for evaluating soil erosion. The rainfall amount is often used to estimate the R factor for Universal Soil Loss Equation (USLE) modeling. Daily, monthly, and annual rainfall data are commonly available. The purpose of the present study is to establish three models for estimating rainfall erosivity based on daily, monthly, and annual precipitation, respectively, and to identify the most suitable model in the Ailiao River watershed located in southern Taiwan. The data set consists of 2266 storm events, monitored by 6 rainfall stations there. The results show that the 6 stations indicate strong positive relationship (r2 > 0.75) between annual rainfall erosivity and annual rainfall with a 99% confidence level, which means the simplified estimation methods based on annual precipitation are useful for predicting long-term annual rainfall erosivity in most of locations in the Ailiao River watershed. The results also show that annual and monthly precipitation correlate with erosivity better than does daily precipitation based on the analysis results of root mean square error (RMSE) and mean absolute percentage error (MAPE) in the Ailiao River watershed.降雨沖蝕指數是通用土壤流失公式 (universal soil loss equation, USLE) 重要的參數之一,由於計算降雨沖蝕指數需要30 分鐘或更短量測間距的雨量資料,但大部分地區只有日、月或年雨量資料。因此利用何種降雨資料推估年平均降雨沖蝕指數較為接近30 分鐘雨量資料計算的年平均降雨沖蝕指數,是本研究的重點。本研究以隘寮溪集水區為研究區域,蒐集區域內6 個雨量站10 年 (2002-2011 年)10 分鐘等間隔的降雨資料,計算年平均降雨沖蝕指數 (Ryc),另以日、月及年降雨資料,建立三種降雨量與降雨沖蝕指數關係式,利用RMSE、MAPE 及Bias 等誤差分析方法,評估三種降雨沖蝕指數推估關係式之適用性。研究結果顯示,利用年降雨資料推估年降雨沖蝕指數的適用性較好。本研究結果可提供缺乏短量測間距雨量資料地區年降雨沖蝕指數推估之參考依據

    Construction of Fuzzy Map for Autonomous Mobile Robots Based on Fuzzy Confidence Model

    This paper presents the use of fuzzy models to explicitly consider sensor uncertainty and finite resolution in solving the SLAM (simultaneous localization and mapping) problem for autonomous mobile robots. The approach establishes fuzzy confidence models in describing occupied obstacles and available space. The problem is transformed into an optimization task of minimizing the alignment error between newly scanned local fuzzy maps and selected parts of a developing global fuzzy map. In aligning local fuzzy maps into a global fuzzy map, we developed a prediction strategy to crop the most potential part from the sensed local fuzzy maps to be overlapped with the global fuzzy map. A mobile vehicle equipped with a laser range finder, the Hokuyo URG-04LX, is used to demonstrate the procedure of fuzzy map building. Experimental results show that the proposed architecture is effective in generating a comprehensive global fuzzy map, which is suitable for both human comprehension and path design during real-time navigation

    Caffeic acid phenethyl amide ameliorates ischemia/reperfusion injury and cardiac dysfunction in streptozotocin-induced diabetic rats

    BACKGROUND: Caffeic acid phenethyl ester (CAPE) has been shown to protect the heart against ischemia/reperfusion (I/R) injury by various mechanisms including its antioxidant effect. In this study, we evaluated the protective effects of a CAPE analog with more structural stability in plasma, caffeic acid phenethyl amide (CAPA), on I/R injury in streptozotocin (STZ)-induced type 1 diabetic rats. METHODS: Type 1 diabetes mellitus was induced in Sprague–Dawley rats by a single intravenous injection of 60 mg/kg STZ. To produce the I/R injury, the left anterior descending coronary artery was occluded for 45 minutes, followed by 2 hours of reperfusion. CAPA was pretreated intraperitoneally 30 minutes before reperfusion. An analog devoid of the antioxidant property of CAPA, dimethoxyl CAPA (dmCAPA), and a nitric oxide synthase (NOS) inhibitor (Nω-nitro-l-arginine methyl ester [l-NAME]) were used to evaluate the mechanism involved in the reduction of the infarct size following CAPA-treatment. Finally, the cardioprotective effect of chronic treatment of CAPA was analyzed in diabetic rats. RESULTS: Compared to the control group, CAPA administration (3 and 15 mg/kg) significantly reduced the myocardial infarct size after I/R, while dmCAPA (15 mg/kg) had no cardioprotective effect. Interestingly, pretreatment with a NOS inhibitor, (l-NAME, 3 mg/kg) eliminated the effect of CAPA on myocardial infarction. Additionally, a 4-week CAPA treatment (1 mg/kg, orally, once daily) started 4 weeks after STZ-induction could effectively decrease the infarct size and ameliorate the cardiac dysfunction by pressure-volume loop analysis in STZ-induced diabetic animals. CONCLUSIONS: CAPA, which is structurally similar to CAPE, exerts cardioprotective activity in I/R injury through its antioxidant property and by preserving nitric oxide levels. On the other hand, chronic CAPA treatment could also ameliorate cardiac dysfunction in diabetic animals

    Effect of Acupressure and Trigger Points in Treating Headache: A Randomized Controlled Trial

    Abstract: The efficacy of acupressure in relieving pain has been documented; however, its effectiveness for chronic headache compared to the muscle relaxant medication has not yet been elucidated. To address this, a randomized, controlled clinical trial was conducted in a medical center in Southern Taiwan in 2003. Twenty-eight patients suffering chronic headache were randomly assigned to the acupressure group (n = 14) or the muscle relaxant medication group (n = 14). Outcome measures regarding self-appraised pain scores (measured on a visual analogue scale; VAS) and ratings of how headaches affected life quality were recorded at baseline, 1 month after treatment, and at a 6-month follow-up. Pain areas were recorded in order to establish trigger points. Results showed that mean scores on the VAS at post-treatment assessment were significantly lower in the acupressure group (32.9 ± 26.0) than in the muscle relaxant medication group (55.7 ± 28.7) (p = 0.047). The superiority of acupressure over muscle relaxant medication remained at 6-month follow-up assessments (p = 0.002). The quality of life ratings related to headache showed similar differences between the two groups in the post treatment and at six-month assessments. Trigger points BL2, GV20, GB20, TH21, and GB5 were used most commonly for etiological assessment. In conclusion, our study suggests that 1 month of acupressure treatment is more effective in reducing chronic headache than 1 month of muscle relaxant treatment, and that the effect remains 6 months after treatment. Trigger points help demonstrate the treatment technique recommended if a larger-scale study is conducted in the future

    Makalah ini membahas aspek-aspek “kesederhanaan” (simplicity) sebagai konsep desain bangunan masjid secara berkelanjutan (sustainable) sesuai konteks dengan mengambil studi kasus masjid kawasan Al-Irsyad Satya Kota Baru Parahyangan, Bandung. Masjid sebagai subyek arsitektur dan pusat ibadah menjadi ruang publik yang didesain dari elemen-elemen yang secara ideal mengandung nilai-nilai Islam dan bertujuan mendukung fungsinya. Desain masjid berkonsep simple atau “sederhana” digunakan sebagai alternatif kontemporer untuk mengoptimalisasi fungsi tersebut, meliputi struktur bangunan hingga biaya pemeliharaan (maintenance) sesuai prinsip keberlanjutan. Keterkaitan erat bangunan masjid dengan aktivitas masyarakat berpotensi melibatkan partisipasi masyarakat dan pengelola dalam menerapkan program sustainabilitas sesuai konteks lingkungannya. Metode yang digunakan dalam penelitian ini berbasis pendekatan Grounded Theory secara kualitatif melalui pengumpulan data dari kegiatan observasi, interview dan analisis program keberlanjutan kawasan.     Whole pelvic helical tomotherapy for locally advanced cervical cancer: technical implementation of IMRT with helical tomothearapy

    <p>Abstract</p> <p>Background</p> <p>To review the experience and to evaluate the treatment plan of using helical tomotherapy (HT) for the treatment of cervical cancer.</p> <p>Methods</p> <p>Between November 1st, 2006 and May 31, 2009, 10 cervical cancer patients histologically confirmed were enrolled. All of the patients received definitive concurrent chemoradiation (CCRT) with whole pelvic HT (WPHT) followed by brachytherapy. During WPHT, all patients were treated with cisplatin, 40 mg/m<sup>2 </sup>intravenously weekly. Toxicity of treatment was scored according to the Common Terminology Criteria for Adverse Events v3.0 (CTCAE v3.0).</p> <p>Results</p> <p>The mean survival was 25 months (range, 3 to 27 months). The actuarial overall survival, disease-free survival, locoregional control and distant metastasis-free rates at 2 years were 67%, 77%, 90% and 88%, respectively. The average of uniformity index and conformal index was 1.06 and 1.19, respectively. One grade 3 of acute toxicity for diarrhea, thrombocytopenia and three grade 3 leucopenia were noted during CCRT. Only one grade 3 of subacute toxicity for thrombocytopenia was noted. There were no grade 3 or 4 subacute toxicities of anemia, leucopenia, genitourinary or gastrointestinal effects. Compared with conventional whole pelvic radiation therapy (WPRT), WPHT decreases the mean dose to rectum, bladder and intestines successfully.</p> <p>Conclusion</p> <p>HT provides feasible clinical outcomes in locally advanced cervical cancer patients. Long-term follow-up and enroll more locally advanced cervical carcinoma patients by limiting bone marrow radiation dose with WPHT technique is warranted.</p
