    One-Year Risk of Stroke after Transient Ischemic Attack or Minor Stroke

    BACKGROUND Previous studies conducted between 1997 and 2003 estimated that the risk of stroke or an acute coronary syndrome was 12 to 20% during the first 3 months after a transient ischemic attack (TIA) or minor stroke. The TIAregistry.org project was designed to describe the contemporary profile, etiologic factors, and outcomes in patients with a TIA or minor ischemic stroke who receive care in health systems that now offer urgent evaluation by stroke specialists. METHODS We recruited patients who had had a TIA or minor stroke within the previous 7 days. Sites were selected if they had systems dedicated to urgent evaluation of patients with TIA. We estimated the 1-year risk of stroke and of the composite outcome of stroke, an acute coronary syndrome, or death from cardiovascular causes. We also examined the association of the ABCD2 score for the risk of stroke (range, 0 [lowest risk] to 7 [highest risk]), findings on brain imaging, and cause of TIA or minor stroke with the risk of recurrent stroke over a period of 1 year. RESULTS From 2009 through 2011, we enrolled 4789 patients at 61 sites in 21 countries. A total of 78.4% of the patients were evaluated by stroke specialists within 24 hours after symptom onset. A total of 33.4% of the patients had an acute brain infarction, 23.2% had at least one extracranial or intracranial stenosis of 50% or more, and 10.4% had atrial fibrillation. The Kaplan–Meier estimate of the 1-year event rate of the composite cardiovascular outcome was 6.2% (95% confidence interval, 5.5 to 7.0). Kaplan–Meier estimates of the stroke rate at days 2, 7, 30, 90, and 365 were 1.5%, 2.1%, 2.8%, 3.7%, and 5.1%, respectively. In multivariable analyses, multiple infarctions on brain imaging, large-artery atherosclerosis, and an ABCD2 score of 6 or 7 were each associated with more than a doubling of the risk of stroke. CONCLUSIONS We observed a lower risk of cardiovascular events after TIA than previously reported. The ABCD2 score, findings on brain imaging, and status with respect to large-artery atherosclerosis helped stratify the risk of recurrent stroke within 1 year after a TIA or minor stroke.     The Alpha Linolenic Acid Content of Flaxseed is Associated with an Induction of Adipose Leptin Expression

    Dietary flaxseed has cardioprotective effects that may be achieved through its rich content of the omega-3 fatty acid, alpha linolenic acid (ALA). Because ALA can be stored in adipose tissue, it is possible that some of its beneficial actions may be due to effects it has on the adipose tissue. We investigated the effects of dietary flaxseed both with and without an atherogenic cholesterol-enriched diet to determine the effects of dietary flaxseed on the expression of the adipose cytokines leptin and adiponectin. Rabbits were fed one of four diets: a regular (RG) diet, or a regular diet with added 0.5% cholesterol (CH), or 10% ground flaxseed (FX), or both (CF) for 8 weeks. Levels of leptin and adiponectin expression were assessed by RT-PCR in visceral adipose tissue. Consumption of flaxseed significantly increased plasma and adipose levels of ALA. Leptin protein and mRNA expression were lower in CH animals and were elevated in CF animals. Changes in leptin expression were strongly and positively correlated with adipose ALA levels and inversely correlated with levels of en face atherosclerosis. Adiponectin expression was not significantly affected by any of the dietary interventions. Our data demonstrate that the type of fat in the diet as well as its caloric content can specifically influence leptin expression. The findings support the hypothesis that the beneficial cardiovascular effects associated with flaxseed consumption may be related to a change in leptin expression

    Patterns and universals of mate poaching across 53 nations : the effects of sex, culture, and personality on romantically attracting another person’s partner

    As part of the International Sexuality Description Project, 16,954 participants from 53 nations were administered an anonymous survey about experiences with romantic attraction. Mate poaching--romantically attracting someone who is already in a relationship--was most common in Southern Europe, South America, Western Europe, and Eastern Europe and was relatively infrequent in Africa, South/Southeast Asia, and East Asia. Evolutionary and social-role hypotheses received empirical support. Men were more likely than women to report having made and succumbed to short-term poaching across all regions, but differences between men and women were often smaller in more gender-egalitarian regions. People who try to steal another's mate possess similar personality traits across all regions, as do those who frequently receive and succumb to the poaching attempts by others. The authors conclude that human mate-poaching experiences are universally linked to sex, culture, and the robust influence of personal dispositions.peer-reviewe

    BLOOM: A 176B-Parameter Open-Access Multilingual Language Model

    Large language models (LLMs) have been shown to be able to perform new tasks based on a few demonstrations or natural language instructions. While these capabilities have led to widespread adoption, most LLMs are developed by resource-rich organizations and are frequently kept from the public. As a step towards democratizing this powerful technology, we present BLOOM, a 176B-parameter open-access language model designed and built thanks to a collaboration of hundreds of researchers. BLOOM is a decoder-only Transformer language model that was trained on the ROOTS corpus, a dataset comprising hundreds of sources in 46 natural and 13 programming languages (59 in total). We find that BLOOM achieves competitive performance on a wide variety of benchmarks, with stronger results after undergoing multitask prompted finetuning. To facilitate future research and applications using LLMs, we publicly release our models and code under the Responsible AI License

    Functional interdependence of BRD4 and DOT1L in MLL leukemia.

    Targeted therapies against disruptor of telomeric silencing 1-like (DOT1L) and bromodomain-containing protein 4 (BRD4) are currently being evaluated in clinical trials. However, the mechanisms by which BRD4 and DOT1L regulate leukemogenic transcription programs remain unclear. Using quantitative proteomics, chemoproteomics and biochemical fractionation, we found that native BRD4 and DOT1L exist in separate protein complexes. Genetic disruption or small-molecule inhibition of BRD4 and DOT1L showed marked synergistic activity against MLL leukemia cell lines, primary human leukemia cells and mouse leukemia models. Mechanistically, we found a previously unrecognized functional collaboration between DOT1L and BRD4 that is especially important at highly transcribed genes in proximity to superenhancers. DOT1L, via dimethylated histone H3 K79, facilitates histone H4 acetylation, which in turn regulates the binding of BRD4 to chromatin. These data provide new insights into the regulation of transcription and specify a molecular framework for therapeutic intervention in this disease with poor prognosis

    A first look at the effects of ionospheric signal bending on a globally processed GPS network

    This study provides a first attempt at quantifying potential signal bending effects on the GPS reference frame, coordinates and zenith tropospheric delays (ZTDs). To do this, we homogeneously reanalysed data from a global network of GPS sites spanning 14 years (1995.0–2009.0). Satellite, Earth orientation, tropospheric and ground station coordinate parameters were all estimated. We tested the effect of geometric bending and dTEC bending corrections, which were modelled at the observation level based, in part, on parameters from the International Reference Ionosphere 2007 model. Combined, the two bending corrections appear to have a minimal effect on site coordinates and ZTDs except for low latitude sites. Considering five days (DOY 301–305, 28 October–1 November 2001) near ionospheric maximum in detail, they affect mean ZTDs by up to ~1.7 mm at low latitudes, reducing to negligible levels at high latitudes. Examining the effect on coordinates in terms of power-spectra revealed the difference to be almost entirely white noise, with noise amplitude ranging from 0.3 mm (high latitudes) to 2.4 mm (low latitudes). The limited effect on station coordinates is probably due to the similarity in the elevation dependence of the bending term with that of tropospheric mapping functions. The smoothed z-translation from the GPS reference frame to ITRF2005 changes by less than 2 mm, though the effect combines positively with that from the second order ionospheric refractive index term. We conclude that, at the present time, and for most practical purposes, the geometric and dTEC bending corrections are probably negligible at current GPS/reference frame precisions

    Geocenter motions from GPS: A unified observation model

    We test a unified observation model for estimating surface-loading-induced geocenter motion using GPS. In principle, this model is more complete than current methods, since both the translation and deformation of the network are modeled in a frame at the center of mass of the entire Earth system. Real and synthetic data for six different GPS analyses over the period 1997.25–2004.25 are used to (1) build a comprehensive appraisal of the errors and (2) compare this unified approach with the alternatives. The network shift approach is found to perform particularly poorly with GPS. Furthermore, erroneously estimating additional scale changes with this approach can suggest an apparently significant seasonal variation which is due to real loading. An alternative to the network shift approach involves modeling degree-1 and possibly higher-degree deformations of the solid Earth in a realization of the center of figure frame. This approach is shown to be more robust for unevenly distributed networks. We find that a unified approach gives the lowest formal error of geocenter motion, smaller differences from the true value when using synthetic data, the best agreement between five different GPS analyses, and the closest (submillimeter) agreement with the geocenter motion predicted from loading models and estimated using satellite laser ranging. For five different GPS analyses, best estimates of annual geocenter motion have a weighted root-mean-square agreement of 0.6, 0.6, and 0.8 mm in amplitude and 21°, 22°, and 22° in phase for x, y, and z, respectively
