    Dietary Exposure and Risk Assessment of Mycotoxins in Thyme and Thyme-Based Products Marketed in Lebanon

    This study aimed at evaluating the incidence of aflatoxin B1 (AFB1) and ochratoxin A (OTA) in thyme and thyme-based products, related dietary exposure, and cancer risk for regular and high consumption. A total of 160 samples were collected, and 32 composite samples were analyzed. AFB1 and OTA were respectively found in 84% (27/32) and 38% (12/32) of the samples. AFB1 exceeded the limits in 41% (13/32) and 25% (8/32) of the samples according to the Lebanese and European standards, respectively. OTA was unacceptable in only 6% (2/32) and 3% (1/32) of the samples according to the Lebanese and European standards, respectively. AFB1 and OTA daily exposure was shown to be 4.270 and 1.345 ng/kg bw/day, respectively. AFB1 was shown to be associated with 0.41 and 0.35 additional cancer cases per 100,000 persons per year for regular consumption, respectively; while for high consumption, an increase of 0.911 and 0.639 cancer cases per 100,000 person per year was noted, respectively. The margin of exposure (MOE) for OTA was >10,000 for the non-neoplastic effect and >200 for the neoplastic effect, representing no toxicological concerns for consumers

    Genome-wide identification and phenotypic characterization of seizure-associated copy number variations in 741,075 individuals

    Copy number variants (CNV) are established risk factors for neurodevelopmental disorders with seizures or epilepsy. With the hypothesis that seizure disorders share genetic risk factors, we pooled CNV data from 10,590 individuals with seizure disorders, 16,109 individuals with clinically validated epilepsy, and 492,324 population controls and identified 25 genome-wide significant loci, 22 of which are novel for seizure disorders, such as deletions at 1p36.33, 1q44, 2p21-p16.3, 3q29, 8p23.3-p23.2, 9p24.3, 10q26.3, 15q11.2, 15q12-q13.1, 16p12.2, 17q21.31, duplications at 2q13, 9q34.3, 16p13.3, 17q12, 19p13.3, 20q13.33, and reciprocal CNVs at 16p11.2, and 22q11.21. Using genetic data from additional 248,751 individuals with 23 neuropsychiatric phenotypes, we explored the pleiotropy of these 25 loci. Finally, in a subset of individuals with epilepsy and detailed clinical data available, we performed phenome-wide association analyses between individual CNVs and clinical annotations categorized through the Human Phenotype Ontology (HPO). For six CNVs, we identified 19 significant associations with specific HPO terms and generated, for all CNVs, phenotype signatures across 17 clinical categories relevant for epileptologists. This is the most comprehensive investigation of CNVs in epilepsy and related seizure disorders, with potential implications for clinical practice

    Nous avons réalisé des mesures expérimentales de diffusion résonante de rayon X sur des molécules chlorées isolées en phase gazeuse (HCl, CH3Cl). Nous avons utilisé le rayonnement synchrotron dans le domaine des rayons X tendres (1-10 keV) comme source d excitation. L intérêt de cette gamme d énergie réside dans la durée de vie femtoseconde, voir sub-femtoseconde de la lacune créée après absorption d un photon. La spectroscopie de diffusion des rayons X permet de mettre à profit ce temps caractéristique, introduit dans l état intermédiaire, pour sonder dans cette échelle de temps, la dynamique nucléaire. Nous avons mesuré à la fois l émission Ka et Kb au seuil Cl K de ces molécules chlorées. Le comportement de l émission Ka en fonction de l énergie d excitation nous apporte des informations sur la topologie des surfaces d énergie potentielle. La forme de l émission Kb en fonction de l énergie d excitation permet de sonder plus particulièrement la dynamique nucléaire ayant lieu dans l état intermédiaire. Cette émission permet également d accéder à la variation de la section efficace élastique en fonction de l énergie d excitation montrant des profils caractéristique d interférence.     Warburg′s effect on solid tumors

    Lactic acidosis is the result of imbalance between the systemic formation of lactate and its hepatic metabolism. In cancer patients, lactic acidosis is mainly associated with hematologic malignancies (leukemia and lymphomas) and the mechanism is known as Warburg′s effect. We report a 76-year-old male known to have hypertension and coronary artery disease, who presented with abdominal distension and lactic acidosis. His initial evaluation showed multiple liver masses that were biopsied and the patient was diagnosed with undifferentiated carcinoma of unknown primary, involving the liver. The patient had progression of lactic acidosis leading to his death on day-15. As the lactic acidosis was not in the setting of hypoxia or hemodynamic instability, we made the diagnosis of malignancy-associated type B lactic acidosis, also known as the Warburg′s effect. Warburg′s effect can occur in solid cancer if the tumor involves the liver. It has bad prognostic implications. The use of intravenous bicarbonate as a temporary measure is of controversial benefit, as it can potentially worsen the metabolic acidosis and its use should be limited to patients with very low pH. In cancer patients, the use of lactatebased intravenous fluids can be potentially harmful and can increase the risk of tumor metastasis, at least in animal malignancy models

    Cutaneous Necrotizing Vasculitis and Leukopenia in a Cocaine User: Is Levamisole the Culprit?

    Levamisole is an antihelminthic drug banned by the US Food and Drug Administration (FDA) in 2000 because of its dangerous side effects. Over the past few years, it has been identified as an adulterant in cocaine and reported to cause cutaneous vasculitis in cocaine users. The health burden of levamisole is serious since it is estimated that over 5 million Americans use cocaine and that 70% of the cocaine used in the USA contains levamisole. In this paper we report the case of a 23-year-old female cocaine user that presented with purpuric rash and skin necrosis, found to have positive c-ANCA and anti-proteinase 3 antibodies. Her skin biopsy showed fibroconnective tissue with signs of necrosis, acute and chronic inflammation, and thrombus formation. She was diagnosed with levamisole-induced vasculitis and successfully treated with withdrawal of cocaine use and local wound care

    Tongue necrosis secondary to giant cell arteritis, successfully treated with tocilizumab: a case report

    Abstracts Background Giant Cell Arteritis (GCA) is a large vessel vasculitis that most commonly presents with headache, scalp tenderness, jaw claudication, and vision changes. Various other, less common, manifestations have been reported in the literature such as scalp and tongue necrosis. Though most patients respond to corticosteroids, some cases of GCA are refractory to the high doses of corticosteroids. Case presentation We present a 73-year-old female with GCA refractory to corticosteroids presenting with tongue necrosis. This patient significantly improved with a dose of tocilizumab, an IL-6 inhibitor. Conclusion To the best of our knowledge, this is the first case report of a patient with refractory GCA presenting with tongue necrosis that had rapid improvement with tocilizumab. Prompt diagnosis and treatment can prevent severe outcomes such as tongue amputation in GCA patients with tongue necrosis, and tocilizumab may be effective for corticosteroid-refractory cases

    A proposed core model of the new public health for a healthier collectivity: how to sustain transdisciplinary and intersectoral partnerships

    International audienceAt present, there is no conceptual model by which public health could be represented as intersectoral governance collaborating with society and the state, and acting as a collective on the determinants of health. In this article, our interdisciplinary group, representing core competencies in public health, suggest two complementary conceptual models as frameworks for a diverse public concerned with public health and its core functions. The first conceptual ‘core model’ roots from the Ottawa Charter for Health Promotion. It represents the interrelationships of the three main poles united at the biopower level: the collectivity (entire population), the contemporary state and public health. In the second conceptual model, we present the various components in the meta-network of public health governance. We also present the roles of heterogeneous actors and how they can collaborate within a prominent process of capacity building and development of practice in public health. Thus, we emphasize the importance of intersectoral partnerships the contemporary state can make with public health without inducing any rupture with the social fabric. Our two complementary models can help actors from all sectors better understand the most frequent questions in public health governance (functions, roles, ingredients) and the challenges that intersectoral actors may very likely encounter in the implementation of these frameworks. The sustainability of well-balanced transdisciplinary and intersectoral partnerships contribute to a successful implementation of public health governance, and most importantly to a good health status for the collectivity
