Design of the Quality of Life in Motion (QLIM) study: a randomized controlled trial to evaluate the effectiveness and cost-effectiveness of a combined physical exercise and psychosocial training program to improve physical fitness in children with cancer
<p>Abstract</p> <p>Background</p> <p>Childhood cancer and its treatment have considerable impact on a child's physical and mental wellbeing. Especially long-term administration of chemotherapy and/or radiotherapy impairs physical fitness both during and after therapy, when children often present with muscle weakness and/or low cardiorespiratory fitness. Physical exercise can improve these two elements of physical fitness, but the positive effects of physical exercise might be further increased when a child's wellbeing is simultaneously enhanced by psychosocial training. Feeling better may increase the willingness and motivation to engage in sports activities. Therefore, this multi-centre study evaluates the short and long-term changes in physical fitness of a child with a childhood malignancy, using a combined physical exercise and psychosocial intervention program, implemented during or shortly after treatment. Also examined is whether positive effects on physical fitness reduce inactivity-related adverse health problems, improve quality of life, and are cost-effective.</p> <p>Methods</p> <p>This multi-centre randomized controlled trial compares a combined physical and psychosocial intervention program for children with cancer, with care as usual (controls). Children with cancer (aged 8-18 years) treated with chemotherapy and/or radiotherapy, and who are no longer than 1 year post-treatment, are eligible for participation. A total of 100 children are being recruited from the paediatric oncology/haematology departments of three Dutch university medical centres. Patients are stratified according to pubertal stage (girls: age ≤10 or >10 years; boys: ≤11 or >11 years), type of malignancy (haematological or solid tumour), and moment of inclusion into the study (during or after treatment), and are randomly assigned to the intervention or control group.</p> <p>Discussion</p> <p>Childhood cancer patients undergoing long-term cancer therapy may benefit from a combined physical exercise and psychosocial intervention program since it may maintain or enhance their physical fitness and increase their quality of life. However, the feasibility, patient need, and effectiveness of such a program should be established before the program can be implemented as part of standard care.</p> <p>Trial registration number</p> <p>NTR1531 (The Netherlands National Trial Register)</p
Transfusion-transmitted infections
Although the risk of transfusion-transmitted infections today is lower than ever, the supply of safe blood products remains subject to contamination with known and yet to be identified human pathogens. Only continuous improvement and implementation of donor selection, sensitive screening tests and effective inactivation procedures can ensure the elimination, or at least reduction, of the risk of acquiring transfusion transmitted infections. In addition, ongoing education and up-to-date information regarding infectious agents that are potentially transmitted via blood components is necessary to promote the reporting of adverse events, an important component of transfusion transmitted disease surveillance. Thus, the collaboration of all parties involved in transfusion medicine, including national haemovigilance systems, is crucial for protecting a secure blood product supply from known and emerging blood-borne pathogens
Dendritic cells in cancer immunology and immunotherapy
Dendritic cells (DCs) are a diverse group of specialized antigen-presenting cells with key roles in the initiation and regulation of innate and adaptive immune responses. As such, there is currently much interest in modulating DC function to improve cancer immunotherapy. Many strategies have been developed to target DCs in cancer, such as the administration of antigens with immunomodulators that mobilize and activate endogenous DCs, as well as the generation of DC-based vaccines. A better understanding of the diversity and functions of DC subsets and of how these are shaped by the tumour microenvironment could lead to improved therapies for cancer. The role and potential of umbilical cord blood in an era of new therapies: a review
In light of pioneering findings in the 1980s and an estimation of more than 130 million global annual births, umbilical cord blood (UCB) is considered to be the most plentiful reservoir of cells and to have regenerative potential for many clinical applications. Although UCB is used mainly against blood disorders, the spectrum of diseases for which it provides effective therapy has been expanded to include non-hematopoietic conditions; UCB has also been used as source for regenerative cell therapy and immune modulation. Thus, collection and banking of UCB-derived cells have become a popular option. However, there are questions regarding the cost versus the benefits of UCB banking, and it also raises complex ethical and legal issues. This review discusses many issues surrounding the conservation of UCB-derived cells and the great potential and current clinical applications of UCB in an era of new therapies. In particular, we describe the practical issues inherent in UCB collection, processing, and long-term storage as well as the different types of ‘stem’ or progenitor cells circulating in UCB and their uses in multiple clinical settings. Given these considerations, the trend toward UCB will continue to provide growing assistance to health care worldwide
ESTRO recommendations on preoperative radiation therapy in breast cancer: current and future perspectives - Endorsed by ASTRO.
BACKGROUND AND PURPOSE: Preoperative radiation therapy (RT) for breast cancer is not a novel concept, though available data are insufficient to translate current knowledge into clinical practice. Nonetheless, potential advantages of this approach are emerging in multiple scenarios, incorporating increasing treatment personalization and technological improvements in RT. This paper aims to synthesize and summarize the literature on preoperative RT in distinct breast cancer treatment settings, providing perspectives based on existing evidence and gaps in knowledge. METHODS: The ESTRO Breast subgroup proposal for elaborating perspectives on preoperative RT was approved by the ESTRO Guidelines Committee, and a panel of experts in the field was identified. Four working groups were created, focusing on the different clinical settings where preoperative RT has been investigated: patients with early-stage breast cancer at low risk of recurrence, patients with breast cancer at high risk of recurrence, and patients with an indication for mastectomy. The fourth group focused its search on cross cutting themes, such as preclinical and translational aspects, radiobiology, RT techniques and quality assurance. After a literature search including the identification of key points and gaps in the literature, the four working groups presented their findings and perspectives were formulated, discussed and approved by the panel. RESULTS: Overall, 27 phase I and phase II studies enrolling patients from the year 2000 onward were considered, collecting data such as RT dose and fractionation, clinical outcomes, and complications rates. The expert panel stated perspectives for the different clinical scenarios based on available evidence and current gaps in knowledge, to be addressed by future clinical research. CONCLUSION: Given the current lack of clinical data to support the development of formal guidelines, we present our perspectives, which can be useful for implementing new clinical trials and research projects, overcoming current limitations, and potentially generating high-quality practice-changing data, introducing preoperative RT in specific breast cancer treatment settings in the future
