Eculizumab is a safe and effective treatment in pediatric patients with atypical hemolytic uremic syndrome
Atypical hemolytic uremic syndrome (aHUS) is caused by alternative complement pathway dysregulation, leading to systemic thrombotic microangiopathy (TMA) and severe end-organ damage. Based on 2 prospective studies in mostly adults and retrospective data in children, eculizumab, a terminal complement inhibitor, is approved for aHUS treatment. Here we prospectively evaluated efficacy and safety of weight-based dosing of eculizumab in eligible pediatric patients with aHUS in an open-label phase II study. The primary end point was complete TMA response by 26 weeks. Twenty-two patients (aged 5 months-17 years) were treated; 16 were newly diagnosed, 12 had no prior plasma exchange/infusion during current TMA symptomatology, 11 received baseline dialysis, and 2 had prior renal transplants. By week 26, 14 achieved a complete TMA response, 18 achieved hematologic normalization, and 16 had 25% or better improvement in serum creatinine. Plasma exchange/infusion was discontinued in all, and 9 of the 11 patients who required dialysis at baseline discontinued, whereas none initiated new dialysis. Eculizumab was well tolerated; no deaths or meningococcal infections occurred. Bone marrow failure, wrist fracture, and acute respiratory failure were reported as unrelated severe adverse events. Thus, our findings establish the efficacy and safety of eculizumab for pediatric patients with aHUS and are consistent with proposed immediate eculizumab initiation following diagnosis in children

surfaces, or apparent horizons, in Cauchy data sets for the Einstein equation.
An area estimate for outermost marginally trapped surfaces is proved. The proof
makes use of an existence result for marginal surfaces, in the presence of
barriers, curvature estimates, together with a novel surgery construction for
marginal surfaces. These results are applied to characterize the boundary of

A university system’s approach to enhancing the educational mission of health science schools and institutions: the University of Texas Academy of Health Science Education
Background: The academy movement developed in the United States as an important approach to enhance the educational mission and facilitate the recognition and work of educators at medical schools and health science institutions. Objectives: Academies initially formed at individual medical schools. Educators and leaders in The University of Texas System (the UT System, UTS) recognized the academy movement as a means both to address special challenges and pursue opportunities for advancing the educational mission of academic health sciences institutions. Methods: The UTS academy process was started by the appointment of a Chancellor’s Health Fellow for Education in 2004. Subsequently, the University of Texas Academy of Health Science Education (UTAHSE) was formed by bringing together esteemed faculty educators from the six UTS health science institutions. Results: Currently, the UTAHSE has 132 voting members who were selected through a rigorous, system-wide peer review and who represent multiple professional backgrounds and all six campuses. With support from the UTS, the UTAHSE has developed and sustained an annual Innovations in Health Science Education conference, a small grants program and an Innovations in Health Science Education Award, among other UTS health science educational activities. The UTAHSE represents one university system’s innovative approach to enhancing its educational mission through multi- and interdisciplinary as well as inter-institutional collaboration. Conclusions: The UTAHSE is presented as a model for the development of other consortia-type academies that could involve several components of a university system or coalitions of several institutions
Local and Global Well-Posedness for Aggregation Equations and Patlak-Keller-Segel Models with Degenerate Diffusion
Recently, there has been a wide interest in the study of aggregation
equations and Patlak-Keller-Segel (PKS) models for chemotaxis with degenerate
diffusion. The focus of this paper is the unification and generalization of the
well-posedness theory of these models. We prove local well-posedness on bounded
domains for dimensions and in all of space for , the
uniqueness being a result previously not known for PKS with degenerate
diffusion. We generalize the notion of criticality for PKS and show that
subcritical problems are globally well-posed. For a fairly general class of
problems, we prove the existence of a critical mass which sharply divides the
possibility of finite time blow up and global existence. Moreover, we compute
the critical mass for fully general problems and show that solutions with
smaller mass exists globally. For a class of supercritical problems we prove
finite time blow up is possible for initial data of arbitrary mass.Comment: 31 page
Carotenoids and chlorophyll content in natural soap with addition of vegetative raw material
In the present study, we performed quantitative and qualitative determination of carotenoids and chlorophyll in five samples of natural soap with addition of vegetative raw material: Green tea, Chamerion angustifolium (L.) Holub, Trifolium pratense L., Alchemilla vulgaris L. and Urtica dioica L. There was developed the method of quantitative content of carotenoids and chlorophyll using spectrophotometry with analytical wavelength at 450 nm (carotenoids) and 667 nm (chlorophyll). Qualitative determination was carried out by the comparative TLC analysis. As mobile phases were used in the experiment following a mixed solvent of hexane-acetone (3: 1). Zipf's law on word frequency is observed in English, French, Spanish,
Italian, and so on, yet it does not hold for Chinese, Japanese or Korean
characters. A model for writing process is proposed to explain the above
difference, which takes into account the effects of finite vocabulary size.
Experiments, simulations and analytical solution agree well with each other.
The results show that the frequency distribution follows a power law with
exponent being equal to 1, at which the corresponding Zipf's exponent diverges.
Actually, the distribution obeys exponential form in the Zipf's plot. Deviating
from the Heaps' law, the number of distinct words grows with the text length in
three stages: It grows linearly in the beginning, then turns to a logarithmical
form, and eventually saturates. This work refines previous understanding about
