    Protocol for the development of a CONSORT-equity guideline to improve reporting of health equity in randomized trials.

    BACKGROUND: Health equity concerns the absence of avoidable and unfair differences in health. Randomized controlled trials (RCTs) can provide evidence about the impact of an intervention on health equity for specific disadvantaged populations or in general populations; this is important for equity-focused decision-making. Previous work has identified a lack of adequate reporting guidelines for assessing health equity in RCTs. The objective of this study is to develop guidelines to improve the reporting of health equity considerations in RCTs, as an extension of the Consolidated Standards of Reporting Trials (CONSORT). METHODS/DESIGN: A six-phase study using integrated knowledge translation governed by a study executive and advisory board will assemble empirical evidence to inform the CONSORT-equity extension. To create the guideline, the following steps are proposed: (1) develop a conceptual framework for identifying "equity-relevant trials," (2) assess empirical evidence regarding reporting of equity-relevant trials, (3) consult with global methods and content experts on how to improve reporting of health equity in RCTs, (4) collect broad feedback and prioritize items needed to improve reporting of health equity in RCTs, (5) establish consensus on the CONSORT-equity extension: the guideline for equity-relevant trials, and (6) broadly disseminate and implement the CONSORT-equity extension. DISCUSSION: This work will be relevant to a broad range of RCTs addressing questions of effectiveness for strategies to improve practice and policy in the areas of social determinants of health, clinical care, health systems, public health, and international development, where health and/or access to health care is a primary outcome. The outcomes include a reporting guideline (CONSORT-equity extension) for equity-relevant RCTs and a knowledge translation strategy to broadly encourage its uptake and use by journal editors, authors, and funding agencies

Retrieved from ://WOS:000356451400049Sugarcane stem borers of the genus of Diatraea (Lepidoptera: Crambidae) form a species complex that causes serious economic losses to sugarcane production in the Cauca River Valley and other regions of Colombia. Two primary species, Diatraea saccharalis (F.) and D. indigenella Dyar and Heinrich, have been effectively managed for more than 4 decades through augmentative releases of the tachinid flies Lydella minense (Townsend) and Billaea claripalpis (Wulp) (Diptera: Tachinidae) and the egg parasitoid Trichogramma exiguum Pinto & Platner (Hymenoptera: Trichogrammatidae). Here we review the current pest status of Diatraea species, damage assessment protocols, management tactics, and the environmental factors and cultural practices that can affect biological control outcomes. Recent changes in the cultivars grown have the potential to increase pest populations and diminish biological control efficacy. Additionally, recent outbreaks of new Diatraea species may further increase overall pest pressure. Thus, there is a need to develop supplementary tactics for the management of these pests that will be compatible with biological control, as well as more reliable protocols for assessing host plant resistance against the increase in infestation intensity

    Tissue MicroArray (TMA) analysis of normal and persistent Chlamydophila pneumoniae infection

    BACKGROUND: Chlamydophila pneumoniae infection has been implicated as a potential risk factor for atherosclerosis, however the mechanism leading to persistent infection and its role in the disease process remains to be elucidated. METHODS: We validated the use of tissue microarray (TMA) technology, in combination with immunohistochemistry (IHC), to test antibodies (GroEL, GroES, GspD, Ndk and Pyk) raised against differentially expressed proteins under an interferon-gamma (IFN-γ) induced model of chlamydial persistence. RESULTS: In the cell pellet array, we were able to identify differences in protein expression patterns between untreated and IFN-γ treated samples. Typical, large chlamydial inclusions could be observed in the untreated samples with all antibodies, whereas the number of inclusions were decreased and were smaller and atypical in shape in the IFN-γ treated samples. The staining results obtained with the TMA method were generally similar to the changes observed between normal and IFN-γ persistence using proteomic analysis. Subsequently, it was shown in a second TMA including archival atheromatous heart tissues from 12 patients undergoing heart transplantation, that GroEL, GroES, GspD and Pyk were expressed in atheromatous heart tissue specimens as well, and were detectable morphologically within lesions by IHC. CONCLUSION: TMA technology proved useful in documenting functional proteomics data with the morphologic distribution of GroEL, GroES, GspD, Ndk and Pyk within formalin-fixed, paraffin-embedded cell pellets and tissues from patients with severe coronary atherosclerosis. The antibodies GroEL and GroES, which were upregulated under persistence in proteomic analysis, displayed positive reaction in atheromatous heart tissue from 10 out of 12 patients. These may be useful markers for the detection of persistent infection in vitro and in vivo

    The Role of Indoleamine 2,3-Dioxygenase in LP-BPM5 Murine Retroviral Disease Progression

    Indoleamine 2,3-dioxygenase (IDO) is an immunomodulatory intracellular enzyme involved in tryptophan degradation. IDO is induced during cancer and microbial infections by cytokines, ligation of co-stimulatory molecules and/or activation of pattern recognition receptors, ultimately leading to modulation of the immune response. LP-BM5 murine retroviral infection induces murine AIDS (MAIDS), which is characterized by profound and broad immunosuppression of T- and B-cell responses. Our lab has previously described multiple mechanisms regulating the development of immunodeficiency of LP-BM5-induced disease, including Programmed Death 1 (PD-1), IL-10, and T-regulatory (Treg) cells. Immunosuppressive roles of IDO have been demonstrated in other retroviral models, suggesting a possible role for IDO during LP-BM5-induced retroviral disease progression and/or development of viral load

Sixty (20/diameter) CP-Ti casted dumbbell rods with diameters of 1.5, 2.0, and 3.5 mm were sectioned and welded using two joint openings (0.0 (00) and 0.6 mm (06)). Six groups were formed, amounting to a total of 9 (n=10) with inclusion of intact groups. Welding was executed using 360 V/8 ms (1.5 and 2.0 mm) and 380 V/9 ins (3.5 mm). Joints were finished, polished, and submitted to radiographic examination to visually analyze presence of porosity (PP). Specimens were submitted to cyclic tests, and the number of cycles until failure (NC) was recorded. Fractured surfaces were examined by SEM. Kruskal-Wallis and Dunn (alpha=0.05) tests demonstrated that NC was lower for all diameters with 06, and for 3.5 mm/00. NC and PP were found to have a negative correlation (Spearman Coefficient).     Effects of laser-weld joint opening size on fatigue strength of Ti-6Al-4V structures with several diameters

Sixty dumbbell rods were machined in Ti-6Al-4V alloy with central diameters of 1 center dot 5, 2 center dot 0 and 3 center dot 5 mm. The specimens were sectioned and then welded using two joint openings (0 center dot 0 and 0 center dot 6 mm). The combination of variables created six groups, which when added to the intact groups made a total of nine groups (n = 10). Laser welding was executed as follows: 360 V per 8 ms (1 center dot 5 and 2 center dot 0 mm) and 380 V per 9 ms (3 center dot 5 mm) with focus and frequency regulated to zero. The joints were finished, polished and submitted to radiographic examination to be analysed visually for the presence of porosity. The specimens were then subjected to a mechanical cyclic test, and the number of cycles until failure was recorded. The fracture surface was examined with a scanning electron microscope (SEM). The Kruskal-Wallis test and Dunn test (alpha = 0 center dot 05) indicated that the number of cycles required for fracture was lower for all specimens with joint openings of 0 center dot 6 mm, and for 3 center dot 5-mm-diameter specimens with joint openings of 0 center dot 0 mm. The Spearman correlation coefficient (alpha = 0 center dot 05) indicated that there was a negative correlation between the number of cycles and the presence of porosity. So, laser welding of Ti-6Al-4V structures with a thin diameter provides the best conditions for the juxtaposition of parts.     Modulated photoactivation methods - Effect on marginal and internal gap formation of restorations using different restorative composites

    This study evaluated the effect of modulated photoactivation methods on gap formation of restorations using different composites. Methods: Sixty human third molars were selected, sectioned (two fragments), and ground to expose a flat enamel area. A cavity (5 mm long X 2 mm wide X 2 mm deep-outer margins in enamel and inner margins in dentin) was prepared on the central area of flattened surface. Single bond adhesive system was applied according to manufacturer's instructions. The specimens were assigned into 12 groups (n 10), according to the restorative composite (Filtek Z250, Herculite XRV, and Heliomolar) and the photoactivation method [continuous light (CL); soft-start (SS); pulse delay (PD); and intermittent light (IL)]. Outer margins were stained using Caries Detector, observed under stereomicroscope, and images were transferred to a computer measurement program. Then, the specimens were sectioned in slices and the internal gaps were assigned using the same method. The length of gaps was expressed as a percentage of total length of the margins. Data (internal adaptation) were submitted to ANOVA and Tukey's test. Results: All restorations showed perfect seal of the enamel outer margins. Modulated photoactivation methods (SS, PD, and IQ showed a significant reduction on internal gap formation when compared with CL, regardless of the composite. Filtek Z250 showed the best internal adaptation, regardless of the photoactivation method. Conclusion: Modulated photoactivation methods decrease the internal gap formation of composite restorations, and should be encouraged on clinical practice. (C) 2007 Wiley Periodicals, Inc.82B234635
