    Rapid testing may not improve uptake of HIV testing and same day results in a rural South African community: a cohort study of 12,000 women

    <p>Background: Rapid testing of pregnant women aims to increase uptake of HIV testing and results and thus optimize care. We report on the acceptability of HIV counselling and testing, and uptake of results, before and after the introduction of rapid testing in this area.</p> <p>Methods and Principal Findings: HIV counsellors offered counselling and testing to women attending 8 antenatal clinics, prior to enrolment into a study examining infant feeding and postnatal HIV transmission. From August 2001 to April 2003, blood was sent for HIV ELISA testing in line with the Prevention of Mother-to-Child Transmission (PMTCT) programme in the district. From May 2003 to September 2004 women were offered a rapid HIV test as part of the PMTCT programme, but also continued to have ELISA testing for study purposes. Of 12,323 women counselled, 5,879 attended clinic prior to May 2003, and 6,444 after May 2003 when rapid testing was introduced; of whom 4,324 (74.6%) and 4,810 (74.6%) agreed to have an HIV test respectively. Of the 4,810 women who had a rapid HIV test, only 166 (3.4%) requested to receive their results on the same day as testing, the remainder opted to return for results at a later appointment. Women with secondary school education were less likely to agree to testing than those with no education (AOR 0.648, p<0.001), as were women aged 21–35 (AOR 0.762, p<0.001) and >35 years (AOR 0.756, p<0.01) compared to those <20 years.</p> <p>Conclusions: Contrary to other reports, few women who had rapid tests accepted their HIV results the same day. Finding strategies to increase the proportion of pregnant women knowing their HIV results is critical so that appropriate care can be given.</p&gt

    Predators and Prickly Paradigms: Nesting Ecology of Bobwhites and Scaled Quail in West Texas

    Sporadic recruitment appears to be the impetus for the irruptive population patterns observed along the western edge of northern bobwhite (Colinus virginianus) range. We investigated hen survival, nest selection strategies, and reproductive success of sympatric northern bobwhites and scaled quail (Callipepla squamata) in west Texas. Radio-marked bobwhites, along with a smaller sample of sympatric scaled quail, were monitored during the spring and summer of 1994 and 1995. Predation was the major source of mortality, with mammals and raptors responsible for about 60 and 30 percent of the mortalities observed, respectively. We used TrailMaster camera systems to document the presence and behavior of various nest predators. Raccoons (Procyon lotor) appear to be the dominant nest predator in this area, accounting for over 80 percent of the nests destroyed. Photographic surveillance of artificial nests provides some clues for making objective assessments of predators involved in nest depredations. The amount and kinds of egg shell evidence at a nest site varied with egg size (chicken vs quail). We found egg shell evidence at 83 percent of nests using chicken eggs, but only at 3 percent of nests using quail eggs. We predict that snakes are overrated in their importance as a nest predator, when such diagnoses are based solely on the absence of physical evidence (i.e., no egg shells). Our results suggest that pricklypear (Opuntia spp.) was a key nesting microhabitat. Overall, 12 of 21 bobwhite and 8 of 12 scaled quail nests were located in pricklypear. Subsequent investigations revealed that nests situated in pricklypear were afforded higher survival, especially in more arid sites with less traditional nesting cover (e.g., Schizachyrium scoparium). Initial results with intensive, short-term mammal removal suggest that nest survival can be increased for about $0.35 per hectare

    The Relationship Between Professional Burnout and Quality and Safety in Healthcare: A Meta-Analysis

    BACKGROUND: Healthcare provider burnout is considered a factor in quality of care, yet little is known about the consistency and magnitude of this relationship. This meta-analysis examined relationships between provider burnout (emotional exhaustion, depersonalization, and reduced personal accomplishment) and the quality (perceived quality, patient satisfaction) and safety of healthcare. METHODS: Publications were identified through targeted literature searches in Ovid MEDLINE, PsycINFO, Web of Science, CINAHL, and ProQuest Dissertations & Theses through March of 2015. Two coders extracted data to calculate effect sizes and potential moderators. We calculated Pearson's r for all independent relationships between burnout and quality measures, using a random effects model. Data were assessed for potential impact of study rigor, outliers, and publication bias. RESULTS: Eighty-two studies including 210,669 healthcare providers were included. Statistically significant negative relationships emerged between burnout and quality (r = -0.26, 95 % CI [-0.29, -0.23]) and safety (r = -0.23, 95 % CI [-0.28, -0.17]). In both cases, the negative relationship implied that greater burnout among healthcare providers was associated with poorer-quality healthcare and reduced safety for patients. Moderators for the quality relationship included dimension of burnout, unit of analysis, and quality data source. Moderators for the relationship between burnout and safety were safety indicator type, population, and country. Rigor of the study was not a significant moderator. DISCUSSION: This is the first study to systematically, quantitatively analyze the links between healthcare provider burnout and healthcare quality and safety across disciplines. Provider burnout shows consistent negative relationships with perceived quality (including patient satisfaction), quality indicators, and perceptions of safety. Though the effects are small to medium, the findings highlight the importance of effective burnout interventions for healthcare providers. Moderator analyses suggest contextual factors to consider for future study

    Plasduino: an inexpensive, general purpose data acquisition framework for educational experiments

    Based on the Arduino development platform, Plasduino is an open-source data acquisition framework specifically designed for educational physics experiments. The source code, schematics and documentation are in the public domain under a GPL license and the system, streamlined for low cost and ease of use, can be replicated on the scale of a typical didactic lab with minimal effort.     Complex Faulting and Triggered Rupture During the 2018 MW 7.9 Offshore Kodiak, Alaska, Earthquake

    We combine aftershock relocations, source mechanisms, teleseismic P wave backprojection, and Global Positioning System data inversion to constrain complex faulting geometry of the 2018 MW 7.9 offshore Kodiak earthquake. Relocated aftershocks delineate several N‐S trends including a prominent 110‐km‐long segment, as well as broad NE‐SW trends. Global Positioning System modeling and backprojection indicate that the NE‐SW trending left‐lateral strike‐slip segments released most energy dominating far‐field crustal deformation and radiated wavefield. Backprojection infers fast E‐to‐W rupture propagations superimposed on a slower S‐to‐N migration. We propose a five‐segment model of the rupture that was partially driven by dynamic triggering

    Using digital and hand printing techniques to compensate for loss: re-establishing colour and texture in historic textiles

    Conservators use a range of 'gap filling' techniques to improve the structural stability and presentation of objects. Textile conservators often use fabric supports to provide reinforcement for weak areas of a textile and to provide a visual infill in missing areas. The most common technique is to use dyed fabrics of a single colour but while a plain dyed support provides good reinforcement, it can be visually obtrusive when used with patterned or textured textiles. Two recent postgraduate dissertation projects at the Textile Conservation Centre (TCC) have experimented with hand printing and digital imaging techniques to alter the appearance of support fabrics so that they are less visually obtrusive and blend well with the colour and texture of the textile being supported. Case studies demonstrate the successful use of these techniques on a painted hessian rocking horse and a knitted glove from an archaeological context

    Low energy polarization sensitivity of the Gas Pixel Detector

    An X-ray photoelectric polarimeter based on the Gas Pixel Detector has been proposed to be included in many upcoming space missions to fill the gap of about 30 years from the first (and to date only) positive measurement of polarized X-ray emission from an astrophysical source. The estimated sensitivity of the current prototype peaks at an energy of about 3 keV, but the lack of readily available polarized sources in this energy range has prevented the measurement of detector polarimetric performances. In this paper we present the measurement of the Gas Pixel Detector polarimetric sensitivity at energies of a few keV and the new, light, compact and transportable polarized source that was devised and built to this aim. Polarized photons are produced, from unpolarized radiation generated with an X-ray tube, by means of Bragg diffraction at nearly 45 degrees. The employment of mosaic graphite and flat aluminum crystals allow the production of nearly completely polarized photons at 2.6, 3.7 and 5.2 keV from the diffraction of unpolarized continuum or line emission. The measured modulation factor of the Gas Pixel Detector at these energies is in good agreement with the estimates derived from a Monte Carlo software, which was up to now employed for driving the development of the instrument and for estimating its low energy sensitivity. In this paper we present the excellent polarimetric performance of the Gas Pixel Detector at energies where the peak sensitivity is expected. These measurements not only support our previous claims of high sensitivity but confirm the feasibility of astrophysical X-ray photoelectric polarimetry.Comment: 15 pages, 12 figures.
