    In this article the ecological condition of air in Kovel district of Volyn’ region during the years 2009 – 2014 has been studied. It was discovered that during recent years there was a tendency to decreasing the level of emission of pollutants from all the sources of pollution. Decreasing of the level of emission from stationary sources of pollution is caused by decreasing of the level of industrial production and usage of energy-saving technologies. It was discovered that during the period of 2009 to 2014 emission of pollutants in air from all kinds of sources of pollution got lower, to 1.7% or 145.7 t.Вивчено екологічний стан атмосферного повітря Ковельського району Волинської області за 2009–2014 роки. Виявлено, що в останні роки намітилась тенденція до зниження викидів забруднюючих речовин від усіх джерел забруднення. Зменшення викидів від стаціонарних джерел забруднення зумовлене зменшенням обсягів виробництва та переходом на більш сучасні енергозберігаючі технології. Встановлено, що у період з 2009 по 2014 рік викиди шкідливих речовин в атмосферне повітря від усіх джерел забруднення знизились на 1,7% або 145,7 т

    Decreased retraction of blood clots in patients with venous thromboembolic complications

    Haemostatic disorders play an important role in the pathogenesis of acute venous thrombosis. One of the least studied reactions of blood coagulation and thrombogenesis is spontaneous contraction of blood clots, which takes place at the expense of the contractility apparatus of activated blood platelets adhered to fibrin fibres. The work was aimed at studying the parameters of contraction of blood clots, formed in vitro, in blood of 41 patients with acute venous thromboses as compared with the same parameters in apparently healthy donors. We used a new instrumental method making it possible to determine the time from initiation to the beginning of contraction, as well as the degree and velocity of clot contraction. It was revealed that in patients with venous thrombosis the ability of clots to shrink was significantly reduced as compared with the control. We detected a statistically significant retardation of and decrease in of blood clot concentration in patients with venous thrombosis complicated by pulmonary artery thromboembolism as compared with contraction in patients with isolated deep vein thrombosis, witch may be important for early diagnosis and determination of the risk of thromboembolism. Besides, we revealed a statistically significant retardation of contraction in patients with proximal thrombosis as compared with contraction in patients with distal thrombosis, with similar values of the degree of contraction. Contraction was statistically significantly reduced in acute thrombosis (less than 21 days), whereas in subacute thrombosis (more than 21 days) the parameters of contraction were closer to normal values. The obtained findings suggest that reduction of blood clot contraction may be a new, hitherto unstudied pathogenetic mechanism deteriorating the course and outcome of venous thrombosis. The clinical significance of contraction and its impairments, as well as the diagnostic and prognostic value of the laboratory test for blood clot contraction would merit further study

    ДОСВІД ВИЗНАЧЕННЯ ІНТРАТЕКАЛЬНОГО СИНТЕЗУ АНТИТІЛ У ПАЦІЄНТІВ З УРАЖЕННЯМИ ЦЕНТРАЛЬНОЇ НЕРВОВОЇ СИСТЕМИ

    The aim of the work – to study the frequency of intrathecal synthesis of specific antibodies at patients with inflammatory lesions of the central nervous system.Patients and methods. In this work the data of the determination of intrathecal synthesis of specific antibodies (ITSA) in 90 patients are given who were treated at the Lev Hromashevskyi Institute of Epidemiology and Infectious Diseases. The research included quantitative definition of antibodies of the class IgG in serum (S) and cerebrospinal fluid (CSF) to neurotropic pathogens: herpes simplex virus 1/2, cytomegalovirus, Epstein-Barr virus, varicella zoster virus, rubella virus, Borrelies. Calculations of ITSA indicators were carried out according to the method of Reiber H. The condition of a hematoencephalic barrier (HEB) was estimated by means of coefficient of albumine (Qalb) taking into account age norms.Results. ITSA was established in (25.6±4.6) % of the examined patients with damages of the central nervous system. In patients with ITSA most often (in 52.2 %) simultaneously present antibodies to several neurotropic pathogens. Detection of ITSA at the examined patients didn’t depend on concentration of specific antibodies in S and CSF and wasn’t followed by malfunction of HEB. The incidence of HEB dysfunction at patients with ITSA and without ITSA appeared with an identical frequency (13,0 % and 13,6 % respectively).Мета роботи. Вивчити частоту інтратекального синтезу специфічних антитіл у пацієнтів зі запальними ураженнями центральної нервової системи.Пацієнти і методи. В роботі наведені дані визначення інтратекального синтезу специфічних антитіл (ІТСА) у 90 пацієнтів, які проходили лікування в клініці ДУ «Інститут епідеміології та інфекційних хвороб ім. Л.В. Громашевського». Дослідження включало кількісне визначення антитіл класу Ig G в сироватці крові (СК) та спинномозковій рідині (СМР) до нейротропних збудників: вірусу простого герпесу 1/2 типу, цитомегаловірусу, вірусу Епштейна-Барр, вірусу оперізувального лишаю, вірусу кору, вірусу краснухи, бореліям. Розрахунки показників ІТСА проведені за методикою Reiber H. Стан гематоенцефалічного бар’єру (ГЕБ) оцінювали за допомогою коефіцієнту альбуміну (Qalb) з урахуванням вікових норм.Результати. ІТСА встановлений у (25,6±4,6) % обстежених пацієнтів з ураженнями ЦНС. У пацієнтів з ІТСА найчастіше (у 52,2 %) одночасно були присутні антитіла до декількох нейротропних збудників. Виявлення ІТСА в обстежених пацієнтів не залежало від концентрації специфічних антитіл у СК і СМР і не супроводжувалося порушенням функції ГЕБ. Випадки дисфункції ГЕБ у пацієнтів з ІТСА та без ІТСА виявлялися з однаковою частотою (13,0 та 13,6 % відповідно)

    Antibiotic consumption and antimicrobial resistance in Poland; findings and implications

    Background: The problem of inappropriate use of antibiotics and the resulting growth in antimicrobial resistance (AMR) has implications for Poland and the world. The objective of this paper was to compare and contrast antibiotic resistance and antibiotic utilisation in Poland in recent years versus other European countries, including agreed quality indicators, alongside current AMR patterns and ongoing policies and initiatives in Poland to influence and improve antibiotic prescribing. Methods: A quantitative ten-year analysis (2007-2016) of the use of antibiotics based on European Centre for Disease Prevention and Control (ECDC) data combined with a literature review on AMR rates and antimicrobial stewardship initiatives. Results: The system of monitoring AMR and appropriate strategies to address AMR rates remain underdeveloped in Poland. The role of microbiological diagnostics and efforts to prevent infections is currently underestimated by physicians. Overall, Poland had one of the highest rates of total consumption of antibiotics in the analysed European countries. Total consumption of antibacterials for systemic use and relative consumption of beta-lactamase sensitive penicillins were characterized by small but statistically significant average annual increases between 2007 and 2016 (from 22.2DIDs to 23.9 DIDs and from 0.8% to 1.3%, respectively). Conclusions: The integrated activities around appropriate antibiotic prescribing in the pre- and post-graduate training of physicians and dentists seem to be particularly important, as well as changes in policies on prescribing antibiotics within ambulatory care. AMR and appropriate prescribing of antibiotics should be the focus of health policy actions in Poland

    Expression of hereditary hemochromatosis C282Y HFE protein in HEK293 cells activates specific endoplasmic reticulum stress responses

    <p>Abstract</p> <p>Background</p> <p>Hereditary Hemochromatosis (HH) is a genetic disease associated with iron overload, in which individuals homozygous for the mutant C282Y <it>HFE </it>associated allele are at risk for the development of a range of disorders particularly liver disease. Conformational diseases are a class of disorders associated with the expression of misfolded protein. HFE C282Y is a mutant protein that does not fold correctly and consequently is retained in the Endoplasmic Reticulum (ER). In this context, we sought to identify ER stress signals associated with mutant C282Y HFE protein expression, which may have a role in the molecular pathogenesis of HH.</p> <p>Results</p> <p>Vector constructs of Wild type HFE and Mutant C282Y HFE were made and transfected into HEK293 cell lines. We have shown that expression of C282Y HFE protein triggers both an unfolded protein response (UPR), as revealed by the increased GRP78, ATF6 and CHOP expression, and an ER overload response (EOR), as indicated by NF-κB activation. Furthermore, C282Y HFE protein induced apoptotic responses associated with activation of ER stress. Inhibition studies demonstrated that tauroursodeoxycholic acid, an endogenous bile acid, downregulates these events. Finally, we found that the co-existence of both C282Y HFE and Z alpha 1-antitrypsin protein (the protein associated with the liver disease of Z alpha 1-antitrypsin deficiency) expression on ER stress responses acted as potential disease modifiers with respect to each other.</p> <p>Conclusion</p> <p>Our novel observations suggest that both the ER overload response (EOR) and the unfolded protein response (UPR) are activated by mutant C282Y HFE protein.</p

    Functional Contribution of Elevated Circulating and Hepatic Non-Classical CD14+CD16+ Monocytes to Inflammation and Human Liver Fibrosis

    BACKGROUND: Monocyte-derived macrophages critically perpetuate inflammatory responses after liver injury as a prerequisite for organ fibrosis. Experimental murine models identified an essential role for the CCR2-dependent infiltration of classical Gr1/Ly6C(+) monocytes in hepatic fibrosis. Moreover, the monocyte-related chemokine receptors CCR1 and CCR5 were recently recognized as important fibrosis modulators in mice. In humans, monocytes consist of classical CD14(+)CD16(-) and non-classical CD14(+)CD16(+) cells. We aimed at investigating the relevance of monocyte subpopulations for human liver fibrosis, and hypothesized that 'non-classical' monocytes critically exert inflammatory as well as profibrogenic functions in patients during liver disease progression. METHODOLOGY/PRINCIPAL FINDINGS: We analyzed circulating monocyte subsets from freshly drawn blood samples of 226 patients with chronic liver disease (CLD) and 184 healthy controls by FACS analysis. Circulating monocytes were significantly expanded in CLD-patients compared to controls with a marked increase of the non-classical CD14(+)CD16(+) subset that showed an activated phenotype in patients and correlated with proinflammatory cytokines and clinical progression. Correspondingly, CD14(+)CD16(+) macrophages massively accumulated in fibrotic/cirrhotic livers, as evidenced by immunofluorescence and FACS. Ligands of monocyte-related chemokine receptors CCR2, CCR1 and CCR5 were expressed at higher levels in fibrotic and cirrhotic livers, while CCL3 and CCL4 were also systemically elevated in CLD-patients. Isolated monocyte/macrophage subpopulations were functionally characterized regarding cytokine/chemokine expression and interactions with primary human hepatic stellate cells (HSC) in vitro. CD14(+)CD16(+) monocytes released abundant proinflammatory cytokines. Furthermore, CD14(+)CD16(+), but not CD14(+)CD16(-) monocytes could directly activate collagen-producing HSC. CONCLUSIONS/SIGNIFICANCE: Our data demonstrate the expansion of CD14(+)CD16(+) monocytes in the circulation and liver of CLD-patients upon disease progression and suggest their functional contribution to the perpetuation of intrahepatic inflammation and profibrogenic HSC activation in liver cirrhosis. The modulation of monocyte-subset recruitment into the liver via chemokines/chemokine receptors and their subsequent differentiation may represent promising approaches for therapeutic interventions in human liver fibrosis
