Transferência e espaço potencial : a relação analítica com crianças em estados autísticos e psicóticos

Tese (doutorado)—Universidade de Brasília, Instituto de Psicologia, Programa de Pós-Graduação em Psicologia Clínica e Cultura, 2012.O objetivo do presente trabalho é realizar um estudo teórico e clínico, tomando o conceito de transferência, a obra de Winnicott e a clínica psicanalítica com crianças em estados autísticos e psicóticos, com o intuito de compreender algumas especificidades da transferência e alguns princípios norteadores para essa clínica. Nesta tese, refleti e demonstrei que existe uma diferença na formulação da noção de transferência quando realizamos um estudo comparativo desse conceito nas obras de Freud, de Klein e de Winnicott, e, ainda, que o que difere, nessas conceituações, auxilia a compreensão e o manejo da transferência nessa clínica. Verifiquei que a noção de transferência era insuficiente para compreender os fenômenos clínicos abarcados neste estudo e passei a utilizar o conceito de relação analítica, relacionando-o às noções de transferência, de contratransferência, de experiência e de espaço potencial. Posteriormente, pensei a relação analítica como espaço potencial a partir do qual é possível acolher as experiências dos pacientes em suas dimensões constitutivas, compreendendo o encontro entre analista e analisando como uma possibilidade de constituição psíquica e de retomada do desenvolvimento emocional. Assim, entendi a relação analítica como algo que transcende à reedição de vivências, proporcionando a instauração de experiências novas que favoreçam a constituição do sujeito e da realidade externa. Nesta tese, em vez de pensar na interpretação da relação analítica, coloquei a ênfase na própria relação. A partir da constatação de que a relação analítica com essas crianças não é algo dado, mas uma relação a ser construída, a tese do trabalho é a de que a relação analítica com essas crianças pode ser estabelecida por outros meios que não a interpretação, como o holding, o manejo, os vínculos sensoriais não verbais, o brincar e a imitação. O trabalho foi organizado em cinco capítulos, além da introdução e das considerações finais. O último capítulo contém reflexões clínicas e técnicas, além do relato e da discussão de quatro casos clínicos. Nesse capítulo, trago a imitação enquanto recurso técnico e outros aspectos técnicos como: o uso da contratransferência, a abertura e a disponibilidade emocional do analista, a encenação, a personificação, a reclamação, a linguagem diferenciada, a sustentação do enquadre e o uso da interpretação, dentre outros. ______________________________________________________________________________ ABSTRACTThe objective of this study is to perform a theoretical and clinical study, taking the transference concept, the work of Winnicott and the psychoanalysis with children in autistic and psychotic states, in order to understand some peculiarities of the transference and some guiding principles for this clinic. In this thesis, I reflected and demonstrated that there is a difference between the transference notion when we perform a comparative study of this concept in the works of Freud, Klein and Winnicott, and also that the distinctive traits of these conceptualizations help to understand and to manage the transference in this clinic. I realized that the transference notion was insufficient to understand the clinical phenomena covered in this study, so I started to use the concept of analytic relationship related to the notions of transference, counter transference, experience and potential space. Later, I thought the analytic relationship as a potential space, where the patient’s experiences are welcome in their constitutive dimensions, understanding the bond between analyst and patient as a possibility of psychic constitution and of resuming emotional development. So I understand the analytic relationship as something that transcends the reissue of experiences, providing the establishment of new experiences, encouraging the constitution of the self and of the external reality. In this thesis, instead of thinking in the interpretation of the analytic relationship, I put the emphasis on the relationship itself. From the fact that the analytic relationship with these children is not something given, but a relationship to be built, this paper's thesis is that the analytic relationship with these children may be established by other means besides the interpretation such as holding, handling, nonverbal sensory links, play and imitation. The work was organized into five chapters, plus the introduction and closing remarks. The final chapter contains clinical and technical reflections, and also the discussion of four clinical cases. In this chapter, I bring the imitation as a technical resource and other technical aspects such as: the use of counter transference, the analyst openness and emotional availability, the enactment, the personification, the reclamation, the distinctive language, the setting maintenance and the use of interpretation, among others

Transferential relation with autistic children : a contribution from Winnicott’s approach

O artigo reflete a relação transferencial na clínica psicanalítica com crianças autistas. Verifica-se que, a partir do trabalho de Melanie Klein (1930), a análise da transferência com crianças autistas é realizada por meio de interpretações verbais e o lugar do analista é o de intérprete. A partir do pensamento de Winnicott, o analista ganha outro lugar na situação transferencial com a criança autista para além da função de intérprete. Dentre as ideias de Winnicott, enfatiza-se a questão do holding e da interpretação, o modelo da “mãe suficientemente boa” como um norteador da transferência e a importância dos vínculos sensoriais não-verbais na relação transferencial.This article reflects about the transferential relation in the psychoanalytic clinic with autistic children. It is verified that, from the work of Melanie Klein (1930), the analysis of transference with autistic children is carried out through verbal interpretations and the place of the analyst is that of an interpreter. From the thought of Winnicott, the analyst gains another place in the transferential situation with the autistic child, far beyond the function of an interpreter. Winnicott emphasizes the ideas of holding, interpretation, and the model of a “good enough mother” as a guide for the transference and the importance of non-verbal sensory links in the transferential relation

The look and the body in the transferencial relation with a child in serious psychic suffering

This article is focused on the transference in the psychoanalytic clinic with children in serious psychic suffering. The reflection is on the importance of thinking the transferencial relation far beyond the interpretation and the importance of the non-verbal sensorial bonds in the transference handling. Using Winnicott as a theoretical and clinical reference, an evolution of the analysis of a child who had established a holding set is related, where the transference was marked by the look and the body of the annalist, making possible the resumption of the child psychic constitution process.El foco de este artículo es la transferencia en la clínica psicoanalítica con niños en sufrimiento psíquico grave. Haciendo una reflexión sobre la importancia de pensar en la relación transferencial, más allá de la interpretación y la importancia de los vínculos sensoriales no verbales en el manejo de la transferencia. Utilizando como referencial teórico y clínico las ideas de Winnicott, se relata la evolución del análisis de un niño que tuvo como característica el establecimiento de un ambiente holding, en el cual la transferencia fue marcada por la mirada y por el cuerpo de la analista, posibilitando la reconquista del proceso de constitución psíquica del niño

The transferencial relation beyond the interpretation : reflections from the theory of Winnicott.

Este artigo reflete sobre a importância de pensar a relação transferencial na clínica psicanalítica para além da interpretação. Para isso, utiliza-se a concepção de Winnicott como referencial teórico, sendo ressaltadas a noção de holding, a regressão à dependência e a questão do uso de objetos e sua influência sobre a técnica da interpretação. Winnicott revela que na clínica com alguns pacientes, em especial os autistas e psicóticos, o objetivo da análise, antes de fornecer interpretações, é proporcionar um ambiente suficientemente bom a partir do qual o sujeito pode retomar o processo de constituição de si mesmo e da externalidade do mundo.This article reflects about the importance of thinking the transferencial relation in the psychoanalytic clinic beyond the interpretation. For this, the Winnicott’s concept is used as theoretical reference, highlighting the concept of holding, regression to dependence and the question of the use of objects and its influence on the technique of interpretation. Winnicott shows that in the clinic, with some patients, particularly autistic and psychotic, before providing interpretation, the objective of the analysis is to provide a good environment from which the subject can retake the constitution process of himself/herself and of the externality of the world

A transferência na clínica psicanalítica com crianças em sofrimento psíquico grave

Primeiramente, reflete-se sobre o sofrimento psíquico grave adotando a postura clínica de escutar o sujeito para além da patologia e do sintoma, pensando o sujeito como um ser com singularidades e com particularidades. Em seguida, investiga-se e discute-se sobre as formas de abordar e de analisar a transferência na clínica psicanalítica com crianças em sofrimento psíquico grave e ainda identificam-se algumas das especificidades da relação transferencial nessa clínica. Posteriormente, demonstra-se a importância de pensar o manejo da transferência para além dos limites da interpretação e resgata-se a importância dos vínculos sensoriais não-verbais na relação transferencial. Realiza-se um estudo teórico a partir das obras de Freud, de Klein e de Winnicott, priorizando os textos sobre transferência, e relata-se a evolução do tratamento de duas crianças em sofrimento psíquico grave, enfocando as especificidades e as mudanças na relação transferencial com cada criança. Discute-se a importância de pensar a transferência na clínica com crianças em sofrimento psíquico grave não apenas como uma forma de re-significação de sentido e de significado de algo vivido, mas também como uma forma de constituir o vivido na relação atual com a analista. A relação transferencial é vista como lugar privilegiado para a instauração de experiências inaugurais, do não vivido e do por viver _______________________________________________________________________________________ ABSTRACTThe area of interest in this dissertation is the psychoanalysis with children in serious psychic suffering, aiming the transferencial relationship with them. Firstly, it reflects on the serious psychic suffering, adopting the clinical posture of listen the subject beyond the pathology and the symptoms, thinking the subject as a being with his own singularities and particularities. Next, the ways of thinking and analyzing the transference with children in serious psychic suffering are investigated and discussed, and also its specificities are identified. Subsequently, the importance of thinking the transferencial handling far beyond the limits of the interpretation is demonstrated, bringing back the importance of non-verbal sensorial bonds in the transferencial relationship. A theoretical study on Freud, Klein and Winnicott works is carried out, especially their texts on transference and an evolution of the treatment of two children in serious psychic suffering is related, focusing on the specificities and changing in the transferencial relationship with each child. This dissertation discusses about the importance of thinking clinical transference with children in serious psychic suffering not only as a resignification of the meaning of something experienced, but also as a way to constitute what is experienced in the relationship with the analyst. O sofrimento psíquico grave e a clínica com crianças

O presente artigo tem como foco o sofrimento psíquico grave na clínica com crianças utilizando a psicanálise como referencial teórico. Inicialmente, justifica-se a utilização do termo “sofrimento psíquico grave”, expondo-se algumas dificuldades e problemas das classificações psiquiátricas, realizando-se, então, uma crítica a abordagens reducionistas e organicistas que acabam fazendo uma predição negativa do futuro da criança e dos pais, correndo o risco de paralisar a criança em rótulos. Assim, em vez da avaliação diagnóstica abrir o caminho para um processo terapêutico, ela acaba fazendo fechamentos e conclusões. Na segunda parte desse artigo, é explicitado o paradigma trazido por Melanie Klein sobre a aplicabilidade e a eficácia da utilização do método psicanalítico com crianças em sofrimento psíquico grave e o paradoxo de Kanner referente ao uso inadequado da noção de autismo. Por fim, são citadas algumas contribuições da psicanálise ao estudo do sofrimento psíquico grave onde adota-se a postura clínica de escutar o sujeito para além da patologia e do sintoma, pensando a criança como um ser com singularidades na qual sua forma de ser não se vincula somente à psicopatologia, mas sobretudo à constituição psíquica. Com essa postura, o analista abre espaço para que a experiência clínica seja fundada no acolhimento, na espontaneidade e na criatividade.
